Daily Dose of Apricot Kernel in Treatise on Cold Damage Diseases (상한론(傷寒論) 탕제에서 행인(杏仁) 1 일 복용량)

  • Objectives : Daily Dose of Apricot Kernel in Treatise on Cold Damage Diseases is usually written in the number, sometimes in the volume. The seed coat and acute end of Apricot Kernel must be removed, so author want to know its daily dose and proportion of seed coat and acute end. Methods : Assuming dosage by editions of Treatise on Cold Damage Diseases. And comparing it with measured weight of Apricot Kernel distributed in market. Results : The number of prescriptions including Apricot Kernel is ten, and eight of that are made to decoction, two of that are made to pill prescription. And two of decoction are made by reducing and uniting prescriptions. The daily dose of six decoction are 70, 47 or 35 in numbers. The 70 Apricot Kernel except seed coat and acute end are 1/2 Sheong ($33m{\ell}$) in volume, 3 Ryang (19.5 g) in weight. Weight of Apricot Kernel the most common in market is 0.28 ~ 0.38 g. 70 Apricot Kernel are 23.10 g, seed coats of that are 1.15 g, acute ends of that are 2.43 g, 70 Apricot Kernel except seed coat and acute end is 19.5 g. So, seed coat is 5%, acute end is 10% by proportion, which is the same with it assumed based on writings. Conclusions : 70 Apricot Kernel except seed coat and acute end are 1/2 Sheong, 3 Ryang, and it is 33 mL and 19.5 g respectively. It also correspond with current market goods.

Molecular characterization of Japanese indigenous grape cultivar 'Koshu' (Vitis vinifera) leaf and berry skin during grape development

  • We investigated the transcriptional profiles of Japanese indigenous grape cultivar 'Koshu' (Vitis vinifera) leaf and berry skin during ripening. In leaf, 64 genes were abundantly transcribed at the end of $v{\acute{e}}raison$ (14 weeks post-flowering), whereas the expression of 61 genes was upregulated at the end of ripening (20 weeks post-flowering). In berry skin, 67 genes were abundantly transcribed at the end of $v{\acute{e}}raison$, whereas the expression of 86 genes was upregulated at the end of ripening. Gene expression associated with biological processes was activated in both tissues at the end of ripening. The expression of genes associated with photosynthesis, sugar synthesis, anthocyanin synthesis, cinnamic acid synthesis, and amino acid metabolism was observed in leaf and berry skin during ripening, together with the accumulation of sugars, anthocyanins, cinnamic acids, and amino acids. Transcripts of AUX/IAA family proteins that repress the activities of auxin-induced proteins were expressed in berry skin at the end of $v{\acute{e}}raison$. Transcripts of genes related to the ubiquitin-proteasome system that degrades AUX/IAA family proteins were abundantly expressed in berry skin at the end of ripening, suggesting that the expansion of skin cells at $v{\acute{e}}raison$ is suppressed by AUX/IAA family proteins, and that the ubiquitin-proteasome system induces the expansion of skin cells during ripening by degrading AUX/IAA family proteins. These transcriptional profiles, which provide new information on the characteristics of 'Koshu' grapevine during ripening, may explain the unique characteristics of 'Koshu' grape in comparison with those of European grapes used for winemaking, and may contribute to the improvement of 'Koshu' grape quality.

  • In this thesis, we consider isoparametric curves of quadratic F-B$\acute{e}$zier curves. F-B$\acute{e}$zier curves unify C-B$\acute{e}$zier curves whose basis is {sint, cos t, t, 1} and H-B$\acute{e}$zier curves whose basis is {sinht, cosh t, t,1}. Thus F-B$\acute{e}$zier curves are more useful in Geometric Modeling or CAGD(Computer Aided Geometric Design). We derive the relation between the quadratic F-B$\acute{e}$zier curves and the quadratic rational B$\acute{e}$zier curves. We also obtain the geometric properties of isoparametric curve of the quadratic F-B$\acute{e}$zier curves at both end points and prove the continuity of the isoparametric curve.

The Fate of Neglected Vascular Injury of the Hand in Acute Hand Injuries (급성 수부 손상에서 간과된 혈관성 손상을 가진 수부의 운명)

  • In acute hand injury, there are sometimes hattened neglected or overlooked vascular injury by primary operators. The authors evaluated the final results and prognosis after secondary revascularization. In eight cases, the authors performed secondary revascularization after prolonged warm ischemia. Five fingers in five cases among them were successfully survived and three cases finally necrosed. The mean warm ischemic time was 56.1 hours. In revascularization procedures, end-to-end artery anastomosis was possible in six cases. In two cases, vein graft was needed to anastomose digital artery, which resulted in complete survival of the fingers. In all three cases, revision amputation of the fingers was done. In acute complex hand injury, the importance of evaluation of the vascular injury can not be overemphasized. The necessity of the early secondary revascularization as well as serious consequence caused by misdiagnosis of vascular injury should be aware.

DIFFUSIVE SHOCK ACCELERATION WITH MAGNETIC FIELD AMPLIFICATION AND ALFVÉNIC DRIFT

  • We explore how wave-particle interactions affect diffusive shock acceleration (DSA) at astrophysical shocks by performing time-dependent kinetic simulations, in which phenomenological models for magnetic field amplification (MFA), Alfv$\acute{e}$nic drift, thermal leakage injection, Bohm-like diffusion, and a free escape boundary are implemented. If the injection fraction of cosmic-ray (CR) particles is ${\xi}$ > $2{\times}10^{-4}$, for the shock parameters relevant for young supernova remnants, DSA is efficient enough to develop a significant shock precursor due to CR feedback, and magnetic field can be amplified up to a factor of 20 via CR streaming instability in the upstream region. If scattering centers drift with Alfv$\acute{e}$n speed in the amplified magnetic field, the CR energy spectrum can be steepened significantly and the acceleration efficiency is reduced. Nonlinear DSA with self-consistent MFA and Alfv$\acute{e}$nic drift predicts that the postshock CR pressure saturates roughly at ~10 % of the shock ram pressure for strong shocks with a sonic Mach number ranging $20{\leq}M_s{\leq}100$. Since the amplified magnetic field follows the flow modification in the precursor, the low energy end of the particle spectrum is softened much more than the high energy end. As a result, the concave curvature in the energy spectra does not disappear entirely even with the help of Alfv$\acute{e}$nic drift. For shocks with a moderate Alfv$\acute{e}$n Mach number ($M_A$ < 10), the accelerated CR spectrum can become as steep as $E^{-2.1}$ - $E^{-2.3}$, which is more consistent with the observed CR spectrum and gamma-ray photon spectrum of several young supernova remnants.

Comparison of the Effectiveness of Sling Exercise and McKenzie Exercise in Patients with Acute Cervical Whiplash Associated Disorder Following Rear-end Collision

  • Purpose: This study compared the effectiveness of sling exercise and McKenzie exercise in patients with acute cervical whiplash associated disorder (WAD) caused by rear-end collision. Methods: Thirty WAD patients were randomly assigned to one of three groups; a sling exercise (SE) group, a McKenzie exercise (McE) group, and a control group. Members of the SE and McE exercised three times a week for four weeks under the researcher's guidance. Three groups performed TENS treatment three times a week for four weeks. Diagnostic radiological equipment was used to measure whiplash injury. Visual analog scale (VAS), neck disability index (NDI), and range of motion (ROM) were used in this study. Results: The three groups showed a significant pre-post treatment difference in measures of VAS and NDI (p<0.05). The SE group showed a significant pre-post treatment difference in measures of flexion and extension changes compared to the McE and control group (p<0.05). The SE group also showed significantly greater improvement in the VAS, NDI, and ROM changes than the control group (p<0.05). Conclusion: These findings indicate that sling exercise is a stronger and more aggressive intervention for treatment of acute WAD patients.

Providing End-of-Life Care in the Community: What Are the Challenges in Malaysia?

  • A 72-year-old woman with metastatic lung cancer to bone and brain and with left external iliac vein thrombosis was under the care of a community palliative care provider. She experienced an acute pain crisis due to acute limb ischemia of the left lower limb. Goals-of-care discussions were held with the patient and her family; she prioritized symptom control and end-of-life care at home. The family and patient were aware of her short prognosis. Her complex pain was managed by the community palliative team, and her family was empowered to give subcutaneous injections. We illustrate a case showing the importance of community health services with palliative care support in providing symptom management and support to patient and family caregivers throughout the course of a life-limiting illness. It also highlights family caregivers' potential psychological distress in delivering subcutaneous injections in terminal care for a patient at home.

Acute Rotator Cuff Tears due to Low Voltage Electrical Injury: A Case Report

  • Since shoulder have a higher proportion of muscle which would have low electrical resistance, there could be more electrical damage to the rotator cuff muscles. We present a patient with acute rotator cuff tear by sudden uncontrolled jerking contractions caused by an electrical shock. A case of 42-year-old man with acute rotator cuff tear due to electrical injury to the shoulder was presented. Magnetic resonance imaging showed a full thickness tear and an undulating appearance of the peripheral end of the torn supraspinatus and infraspinatus muscle, suggesting an acute complete rupture. By arthroscopic surgery, the torn rotator cuff tendons were repaired with a suture bridge technique. At the final follow-up, the patient had a full, pain-free range of motion and had fully recovered shoulder muscle power.

  • Acute toxicity study was conducted on a Hepatitis B vaccine (Hepaccine-B-inj.) with mice, guinea pigs, and rabbits, in accordance with the norms suggested by the F.D.A. in U.S.A. Dose ranges were 2 doses/mouse, 5 doses/guinea pig, 10 doses/rabbit. They received the vaccine subcutaneously and intraperitoneally. Thereafter, all animals injected were observed of general signsdaily, and of body weight for two weeks. At the end of the observation period (or at the time of death), all animals received the highest dose group were autopsied and gross observation was made on various organs and tissues. No significant toxicity was noted.

The Effect of the diameter and anastomotic angles on the compliance and the stress distribution of the end-to-side anastomosis (직경 및 문합각도가 단측 문합의 컴플라이언스 및 응력분포에 미치는 영향)

  • Von Mises stress and compliance distribution was evaluated using a finite element analysis on the end-to-side anastomosis of an artery with length of $20\sim24mm$, inner diameter of 4mm, thickness of 0.5mm and a PTFE graft with length of 10mm, inner diameter of 2mm, thickness of 0.2mm when the anastomotic angle was taken from $30^{\circ}\sim90^{\circ}$ in every $10^{\circ}$ and the diameter ratio from $0.1\sim1$ in every 0.1. The inner pressure of $1330dyne/mm^2$ was applied inside the 2 conduits. It was found that the compliance whose magnitude is larger on the acute angle anastomotic side than on the acute angle side became larger as the anastomotic angle became smaller and the diameter ratio larger and that the equivalent stress on the acute angle anastomotic side was larger than that on the abtuse angle side and became larger as the anastomotic angle and the diameter ratio became larger.
