Clinical Study of Acupotomy Treatment for Tarsal Tunnel Syndrome

  • Background: Compared with conventional closed therapy, acupotomy has the advantages of lower cost, convenient application, and better single and long-term effects. The aim of this study was to explore the clinical effect of acupotomy in the treatment of Tarsal Tunnel syndrome. Methods: There were 80 patients enrolled into the study who were randomly assigned to either the acupotomy or closed therapy group, with 40 patients in each group. The acupotomy group was treated once every 6 days, on 3 occasions for the duration of treatment, and the closed therapy group was treated twice a week, for 3 weeks as the course of treatment. The effects of treatment were analyzed and evaluated according to the standard of curative effect. Results: The "cure rate" of the number of patients in the acupotomy group whose symptoms had completely disappeared (13 patient out of 39) was higher than the closed therapy group (1 patient out of 39), and this difference was significant (p < 0.01). The number of patients in the acupotomy group who did not benefit from the therapy (5 patients) was significantly lower than the closed therapy group (15 patients; p < 0.05). The total number of patients in the acupotomy group who benefitted from the therapy (34 patients) resulted in an effective rate of 87.18%, which was higher than the closed therapy group (24 patients; 61.53%). The difference was statistically significant (p < 0.05). Conclusion: Acupotomy is effective in the treatment of Tarsal Tunnel syndrome, was superior to traditional closed therapy, and is worthy of clinical application.

The Comparison of Effectiveness between Bee Venom and Sweet Bee Venom Therapy on Low back pain with Radiating pain. (요각통 환자에 대한 Bee Venom과 Sweet Bee Venom의 효능 비교 연구)

  • Objective The aim of this study is to investigate if Sweet Bee Venom therapy has the equal effect in comparison with Bee Venom Therapy on Low back pain with Radiation pain. Methods Clinical studies were done 24 patients who were treated low back pain with radiation pain to Dept. of Acupuncture & Moxibusition, of Oriental Medicine Se-Myung University from April 1, 2007 to September 30, 2007. Subjects were randomly divided into two groups ; Bee Venom treated group(Group A, n=10), Sweet Bee Venom treatred group(Group B, n=14). In Bee Venom treated group(Group A), we treated patients with dry needle acupuncture and Bee Venom therapy. In Sweet Bee Venom treatred group(Group B), we treated patients with dry needle acupuncture and Sweet Bee Venom therapy. All process of treatment were performed by double blinding method. To estimate the efficacy of controlling pain. we checked Visual Analog Scale(VAS). For evaluating functional change of patients, Straight Leg Raising Test(S.L.R.T) was measured. Results 1. In controlling pain, Sweet Bee Venom treatred group(Group B) had similar ability in comparison with Bee Venom treated group(Group A). 2. In promoting function, Sweet Bee Venom treatred group(Group B) had similar ability in comparison with Bee Venom treated group(Group A). Conclusions It may be equal effects as compared with using Bee Venom to treat low back pain with radiation pain using Sweet Bee Venom. We can try to treat other disease known to have effect with Bee Venom.

Anti-apoptotic and Neuroprotective Effects of Acupuncture at $LR_3$ on Focal Brain Ischemic Injury Induced by Intraluminal Filament Insertion in Rats (다종(多種)의 태충(太衝)($LR_3$) 침척요법(鍼刺療法)이 Intraluminal Filament 삽입술로 유발된 백서(白鼠)의 허혈성 국소 뇌손상에 미치는 영향)

  • Objectives: This study was performed to investigate the effects of acupuncture therapy(AT, AT-9), electro-acupuncture therapy(EAT) and low level laser acupuncture therapy(LAT) at LRJ on the focal ischemia-induced by intraluminal filament insertion in rats. Methods : In the present syudy, the focal ischemia was induced by Intraluminal filament insertion into left middle cerebral artery. The subjects were divided into five groups after focal brain ischemia. (n=15, in each group) : Control with no treatment, AT with acupuncture at $LR_3$, AT-9 with acupuncture at $LR_3$ and rotating 9 times in a clockwise direction, EAT with electro-acupuncture at $LR_3$ and LAT with invasive laser acupuncture at $LR_3$. Anti-apoptotic and neuroprotective effects of acupuncture were observed by mGluR5 mRNA, Bax mRNA, Bcl-2 mRNA, Cytochrome C protein, Cresyl violet-stain and Choline acetyltransferase (ChAT)-stain in the hippocampus. Results: 1. In LAT, mGluR5, Cresyl violet-stain and ChAT-stain were increased. 2. In LAT, Cytochrome C protein was decreased. 3. In AT-9, Bax, Cytochrome C protein and the Bax/Bcl-2 ratio were decreased. 4. In AT-9, Bcl-2, Cresyl violet-stain and ChAT-stain were increased. 5. In EAT, Bcl-2 and Cresyl violet-stain were increased. Conclusions: These results suggests that LAT and AT-9 show anti-apoptotic and neuro-protective effects and that LAT and AT-9 may be useful for managing stroke by focal brain ischemia.

  • Objective : To study the effects and merits of the Yanzhen Therapy(眼針療法), Method : We researched through studying 28 kinds of literature. Result : 1. The Division of Eye Region(眼區分劃), the Position of Yanxue(眼穴定位) and the mutual connection with Internal Organ(臟腑) during the Yanzhen Therapy(眼鍼療法) were made according to Huata's Thesis, and were made by combining Neijing(內經) Theory about eyes, Yinyang and Five Elements(陰陽五行) Theory, Meridian & Zang-Xiang Theory(經絡臟象學說), the Five Circles & Eight Regions(五輪八廓學說) Theory, and the Zhouyi Eight Diagrams(周易八卦) Theory. 2. The assignment between the Internal Organ and Eye-Division(眼區分劃) was formed on the basis of the Five-Circles and Eight-Regions Theory in Ophthalmology(眼科五輪八廓學說) of Zhengzhizhunsheng(證治準繩). 3. The Assignment(排列) & Position(方位) of the Congenital Eight Diagram(先天八卦) and the Acquired Eight Diagram(後天八卦) have influenced the Division of Eight Eye Region(眼鍼八區) and the Position of Yanxue(眼穴定位) significantly. 4. It showed no difference with the Map of Eye Position of Eight-Division(八廓眼位圖) like as Shenshiyaohan(審視瑤函), Yinhaizhinan(銀海指南) and Sanzijing of Ophthalmology(眼科三字經). Conclusion : While combining organically with other methods of acupuncture, the Yanzhen Therapy(眼鍼療法) may have better clinical effectiveness, and is also expect to expand the clinical indication.

A Case of Calcific Tendinitis (견관절 석회화 건염으로 진단된 견비통환자(肩臂痛患者) 치험(治驗) 1례(例) 보고(報告) - 단순 방사선 영상의 변화를 중심으로 -)

  • Objective : The purpose of this study is to report the patient diagnosed as calcific tendinitis located at supraspinatus, who improved by Oriental medical treatment, through analysis of radiograph. Methods : The patient was managed by acupuncture, moxibustion and cupping without medication and physical therapy. We measured the shoulder state by radiograph and evaluated with UCLA shoulder rating scale. Results : After 5 week treatment, the pain of patient and The UCLA functional average score was improved. Also we found that the deposits of calcific tendinitis was almost disappeared in radiograph. Conclusions : The results suggest that acupuncture, moxibustion and cupping can be effective in reducing the pain and the deposits of calcific tendinitis. We think that it need the further study and clinical trial for calcific tendinitis.

Treatment of Cervical Myelopathy with Acupotomy Combined with Korean Medicine Treatments: Two Clinical Cases

  • The degenerative spinal cord disease cervical spondylotic myelopathy (CSM), and cervical myelopathy caused by trauma, can result in debilitating symptoms affecting quality of life. This study used acupotomy and other Korean medicine treatments (acupuncture, herbal medicine, and physical therapy) to improve the symptoms of CSM and cervical myelopathy. The visual analog scale, the modified Japanese Orthopaedic Association scale (mJOA scale), the Nurick grading system, and the American Spinal Injury Association impairment scale were used as the evaluation criteria to determine the effectiveness of treatment. The functional status of both patients improved from mild to moderate, with improved gait, local sensation, and level of pain. The degree of spinal cord injury remained the same. The findings of this study suggest that combined Korean medicine treatments including acupotomy may be helpful in the treatment of CSM and cervical myelopathy.

  • Fat pad syndrome is a knee joint condition/disease where acute or chronic inflammation occurs in the fat pad of the knee joint, and it is a cause of anterior knee pain. Fat pad syndrome usually occurs concurrently with other conditions/diseases to collectively cause anterior knee pain. No study to date has reported the treatment of anterior knee pain solely due to fat pad syndrome. Here, we report a case of fat pad syndrome of the knee joint as the sole cause of anterior knee pain in a 49-year-old woman who received integrated Korean medicine treatments (pharmacopuncture, acupuncture, herbal medicine, deep fascial meridian therapy, and chuna). Using patient-reported pain scale scores, the level of the patient's pain was relieved, and her mobility improved. Integrated Korean medicine treatments could be effective for patients who have fat pad syndrome of the knee joint.

Effect of Scratching Method Complex Therapy on the Patients with Positional Vertigo (두위 현훈에 대한 괄법(刮法) 복합치료 효과)

  • Objectives : This study was done for reporting the effect of scratching method on patients with positional vertigo. Methods : We investigated 22 cases of patients with dizziness, and devided patients into two groups : We treated one group by complex oriental medical treatment with scratching method therapy, and did the other group by complex oriental medical treatment without scratching method therapy. Korean version of dizziness handicap inventory at baseline and final were used for evaluating the effect of the treatment. Results : 1. In scratching method group and non-scratching method group, compared with baseline, at final, Korean version of dizziness handicap inventory was significantly decreased. 2. At final, scratching method group showed significant decrease on Korean version of dizziness handicap inventory compared with non-scratching method group. Conclusions : Scratching method showed significant improvement in dizziness.

  • Objectives : This study was done for reporting the effect of scratching method on patients with peripheral facial palsy. Methods : We investigated 34 cases of patients with peripheral facial paralysis, and devided patients into two groups : We treated one group by complex oriental medical treatment with scratching method therapy, and did the other group by complex oriental medical treatment without scratching method therapy. gross grading system of House-Brackmann(HB score) and Yanagihara's unweighed grading system(Y score) at baseline and final were used for evaluating the effect of the treatment. Results : 1. In scratching method group and non-scratching method group, compared with baseline, at final, HB score was significantly decreased and Y score was significantly increased. 2. At final, scratching method group showed significant decrease on HB score and significant increase on Y score compared with non-scratching method group. Conclusions : scratching method showed significant improvement in peripheral facial palsy.

Comparative Clinical Study of Jung-an Acupuncture and General Acupuncture on Bell's Palsy Patients (Bell's Palsy에 대한 침치료와 침치료 및 정안침요법 병행치료의 비교연구)

  • Objectives : This study was designed to evaluate the effect of Jung-an acupuncture on Bell's palsy(peripheral facial paralysis) patients. Methods : We investigated 24 cases of patients with peripheral facial paralysis. The patients were divided into two groups. Both Group A, B were treated with general acupuncture and Group A was added to treatment with Jung-an acupuncture. We evaluated the treatment effect of each group three times by using Yanagihara's unweighted grading system. Results : As a result of evaluation by using Yanagihara score, they were not significant scores between two groups after treatment. Conclusions : Jung-an acupuncture and general Acupuncture has significant effect on acute(before 4 weeeks at onset) Bell's palsy. But there is no statistical significance in Jung-an acupuncture on acute phase(within 4 weeks of onset).
