하지 인공관절 치환술 환자의 건강 관련 삶의 질 영향요인 (Factors Influencing Health-related Quality of Life among Knee or Hip Arthroplasty Patients)

  • Purpose: The purpose of this research was to identify factors that influence health-related quality of life of patients who have had knee or hip arthroplasty. Factors included sleep disorders, pain, activities of daily living, and depression. Methods: The participants in this research were 97 patients within one year after receiving knee or hip arthroplasty surgery and who regularly visited a hospital outpatient department. Data were analyzed using descriptive statistics, t-test, ANOVA, and stepwise regression analysis with the SPSS program. Results: Sleep disorders, pain, activities of daily living, and depression had significant correlations with health related quality of life. Factors influencing health-related quality of life among arthroplasty patients were the length of time since surgery, sleep disorders, activities of daily living, and depression. These factors explained 58% of the participants' health-related quality of life. Conclusion: The results show that various factors can affect patients' health-related quality of life following joint replacement surgery. The findings indicate that sleep disorders and depression should receive continued attention after the arthroplasty. It is also necessary to develop appropriate nursing intervention programs to lower depression and increase patient participation in daily living activities.

Effects of Active Vibration Exercise on Trunk Muscle Activity, Balance, and Activities of Daily Living in Patients with Chronic Stroke

  • Purpose: This study aimed to investigate the effect of active vibration exercise on trunk muscle activity, balance, and activities of daily living in patients with chronic stroke. Methods: Twenty-four patients with chronic stroke were randomly assigned to an experimental (n=12) or a control (n=12) group. The experimental group performed an active vibration exercise workout for 20 minutes a day five times a week for 4 weeks using a flexi-bar. Trunk muscle activity was measured using surface electromyography. Balance was measured using the Berg balance scale. Activities of daily living were measured using the functional independence measure. Results: The intragroup comparison showed significant differences in the rectus abdominis, internal oblique, and external oblique muscle activity values, Berg balance scale score, and functional independence measurement values in the experimental group. The intergroup comparison showed that differences in the rectus abdominis and internal oblique external oblique muscle values, Berg balance scale score, and functional independence measurement value for the experimental group were significantly related to those in the control group. Conclusion: Based on these results, this study proved that the active vibration exercise effectively improved trunk muscle activity, balance, and activities of daily living in patients with chronic stroke.

저소득층 노인이 지각한 건강상태와 우울 및 일상생활수행능력 간의 관계 (A Study on the Perceived Health Status, Depression and Activities of Daily Living for the Low-income Elderly in Urban Areas)

  • Purpose: This study was designed to identify the correlation among perceived health, depression and activities of daily living in low-income community dwelling elderly. Methods: The subjects consisted of 254 basic livelihood security person aged over 65, who live in house located in Buk-gu, Daegu city. The data was collected through interview with questionnaire from July 1 to August 20, 2009. Results: The average number of the perceived status in study subjects was 2.37. The average scores of activities of daily living (ADL) were 5.55. There was a significant relationship between perceived health status & ADL (r=.270, p=.01) and between depression & ADL (r=-.163, p=.01). The general characteristics which significantly affected depression was sex (t=-1.986, p=.49), education (F=8.968, p=.000), marital status (F=2.588, p=.037), occupation (t=33.258, p=.000). The general characteristics which significantly affected ADL was occupation (t=-7.677, p=.000). Conclusion: The finding of this study give useful information for constructing an intervention program and care for low-income elderly.

원예 프로그램이 시설 내 치매환자의 인지기능, 일상생활 수행능력 및 악력에 미치는 효과 (The Effect of Horticultural Program on Cognitive Function, Activities of Daily Living(ADL) and Hand Grip Strength of Institutionalized Dementia Patients)

  • Purpose: The purpose of this study was to examine the effect of use of a horticultural program on the cognitive function, activities of daily living, and hand grip strength of institutionalized dementia patients. Method: Twenty three study participants completed a five week protocol comprised of one week of baseline and four weeks of treatment. The study design was a one group repeated measurement study. For the first and fifth week of the study period, data was collected with 3 instruments: Korean Version of Hasegawa Dementia Scale (HDS-K), Katz's ADL index, hand grip strength. Data analysis was conducted by SPSS version 10.0. Result: Total HDS-K score(p=.000), Activities of daily living(p=.013) and hand grip strength(p=.001) was significantly improved after treatment compared to the baseline. Conclusion: The horticultural program was effective for improving cognitive function, ADL and hand grip strength of the dementia patients. Repeated studies utilizing a control group are needed, using various durations, plants and processes, to confirm these results and to standardize the horticultural program protocol.

뇌졸중으로 요양병원에 입원한 환자의 낙상에 대한 두려움 영향요인 (Factors Affecting Fear of Falling in Stroke Patients)

  • Purpose: This study aims to investigate the fear of falling of stroke patients and identify factors affecting the fear of falling. Method: For basic data, structured questionnaires were used for 137 stroke patients in hospitals in B city sampled for this study from June to August, 2010. Results: A multiple regression analysis used to identify variables affecting the fear of falling of stroke patients indicates that the most affecting variable was activities of daily living (${\beta}$=-.558), followed by self-efficacy of falling (${\beta}$=-.223), primary caregiver (${\beta}$=-.118), perception of health (${\beta}$=-.128) and drunken status (${\beta}$=-.108). Total variance explained by activities of daily living, self-efficacy of falling, the primary caregiver, perception of health and drunken status was 81.2% (F=70.845, $R^2$=.812). Conclusion: As discussed above, the factors significantly affecting the fear of falling in stroke patients were activities of daily living and self-efficacy of falling. Therefore, in order to reduce fear of falling of stroke patients and prevent falling, it is suggested that programs should be developed that will improve self-efficacy of falling and daily living activities.

여성노인의 일상생활 수행능력, 우울, 생활만족도와의 관련 영향요인 (Factors Affecting Activities of Daily Living, Depression and Life Satisfaction among Aged Women)

  • Purpose: The purpose of this study was to identify the impact of activities of daily living, instrumental activities of daily living and depression on life satisfaction among aged women. Methods: The sample consisted of 309 aged women of over 60. Data were analyzed using t-test, ANOVA, Pearson correlation coefficient and multiple regression analysis. Results: Significant factors affecting the life satisfaction were spouse, religion, perceived health status, age, and depression. The most significant variables among them were depression and perceived health status. The explanatory power on life satisfaction was 57% in regression model. Conclusion: Depression and perceived health status are important variables to influence on life satisfaction. Therefore, it is suggested that nursing intervention to reduce or prevent depression and enhance health status should be developed to promote quality of life in aged women.

  • Purpose: The purpose of this study was to identify the effect of Tai Chi exercise on loneliness, sleep pattern, and instrumental activities of daily living in elderly women. Methods: A quasi-experimental design, a nonequivalent control group pretest-posttest was used. Elderly women who agreed to participated in the study were assigned into an experimental group (n=20) or a control group (n=20). The experimental group participated in Tai Chi exercise program (Sun-style 12 forms) for 50 minutes per session, 2 times a week for 8 weeks. Tai Chi exercise program consisted of 10 minutes for warm-up, 35 minutes for main session, and 5 minutes for cooling down. Study outcomes were measured through study questionnaires. Data were analyzed using Chi-square test, t-test and Mann-Whitney test by SPSS WIN 14.0 program. Results: Compared with the control group, the experimental group showed significant improvement in loneliness (t=-2.733, p=.009), sleep pattern (t=2.552, p=.015), and instrumental activities of daily living (z=-2.171, p=.030). Conclusion: Tai Chi exercise program may be an effective nursing intervention for elderly women.

  • Purpose. The purpose of this study was to determine the relationship between sleep quality and cognitive function, depression, and institutionalized elderly activity. The goal is to improve the quality of life through early intervention in the sleeping problems of the elderly hospitalized in the nursing home. Method Twentythree patients who were diagnosed with CDR (clinical dementia grade) 0.5 ~ 2 stages by a psychiatrist from September 3 to 30, 2017 were measured for sleep status, depression, cognitive function and activities of daily living at the same time once a day. The collected data were analyzed using SPSS (v.21, IBM, USA) program. Results The correlation between sleep status and cognitive function, depression, and activities of daily living was not statistically significant, and there was a moderate correlation between cognitive function and depression. Conclusion The results of this study suggest that the development and application of a program that activates the cognitive function that is institutionalized with a positive correlation between cognitive function and depression will be effective in decreasing depression in the elderly patients hospitalized in the nursing home.

Effects of Functional Electrical Stimulation (FES) on the Temporal-spatial Gait Parameters and Activities of Daily Living in Hemiplegic Stroke Patients

  • PURPOSE: This study examined the effects of functional electrical stimulation (FES) on temporal-spatial gait and the activities of daily living in hemiplegic stroke patients. METHODS: The subjects were 29 hemiplegic stroke patients (57.7 ± 10.3). The patients walked at a self-controlled speed in four states: (1) walking without FES (non-FES), (2) walking with FES on the gluteus medius in the stance phase (GM), (3) walking with FES on the common peroneal nerve and tibialis anterior in the swing phase (PT), (4) walking with both GM and PT. A GAITRite system, Timed-Functional Movements battery, and Timed UP and Go test were used to measure the variables. RESULTS: Significant improvements were observed in all variables of the GM+PT, GM, and PT states compared to the non-FES state (p < .05). There were significant improvements in the GM+PT state compared to GM and PT states (p < .05). Moreover, significant improvements were noted in the single support time on the affected side, backward walking 10ft, and side stepping 10ft on the affected side of the GM state compared to the PT state (p < .05). There were significant improvements in the stride length on the affected side and side stepping 10ft on the unaffected side of the PT state compared to the GM state (p < .05). CONCLUSION: FES is effective in improving the temporal-spatial gait and activities of daily living in hemiplegic stroke patients.

뇌졸중 환자의 사회재적응 구조모형: Roy의 적응모형에 기반하여 (A Structural Equation Model on Social Re-Adjustment of Stroke Patients: Based on Roy's Adaptation Model)

  • Purpose: This study aimed to develop and test a structural equation model on social re-adjustment of individuals with stroke based on a literature review and Roy's adaptation model. Methods: This study involved 321 participants who had a stroke and visited the outpatient department after discharge. The hypothetical model was developed based on Roy's adaptation model and a comprehensive review of previous literature on the topic. The model comprised four exogenous variables (neurological damage, gender [man], age, and social support) and five endogenous variables (activities of daily living, acceptance of disability, depression, rehabilitation motivation, and social re-adjustment). The data were analyzed using SPSS Windows software version 22.0 and AMOS 23.0. Results: Out of 28 research hypotheses, 18 were supported, and they indicated approximately 64% probability of social re-adjustment. Social re-adjustment is directly and significantly affected by age, social support, activities of daily living, and depression. Social re-adjustment is indirectly affected by neurological impairment, gender (men), age, social support, and rehabilitation motivation. Conclusion: Continuous assistance and care should be provided for individuals with disabilities caused by sudden neurological damage to facilitate gradual improvement in their social re-adjustment. To enhance social re-adjustment, especially among older adults, newly developed interventions should focus on improving their activities of daily living, preventing depression, and enhancing support from family and healthcare personnel.
