Palliative and end-of-life care for heart failure patients in an aging society

  • The populations of Asian countries are expected to age rapidly in the near future, with a dramatic increase in the number of heart failure (HF) patients also anticipated. The need for palliative and end-of-life care for elderly patients with advanced HF is currently recognized in aging societies. However, palliative care and active treatment for HF are not mutually exclusive, and palliative care should be provided to reduce suffering occurring at any stage of symptomatic HF after the point of diagnosis. HF patients are at high risk of sudden cardiac death from the early stages of the disease onwards. The decision of whether to perform cardiopulmonary resuscitation in the event of an emergency is challenging, especially in elderly HF patients, because of the difficulty in accurately predicting the prognosis of the condition. Furthermore, advanced HF patients are often fitted with a device, and device deactivation at the end of life is a complicated process. Treatment strategies should thus be discussed by multi-disciplinary teams, including palliative experts, and should consider patient directives to address the problems discussed above. Open communication with the HF patient regarding the expected prognosis, course, and treatment options will serve to support the patient and aid in future planning.

  • Almost 18,000 children were protected in 285 residential facilities all over the country in 2008 in Korea. The recent trends of the residential care include downsizing, and emphasizing independent living programs for aging out youths. Since the 1990s when the active research on the children in residential facilities has begun, the study focus has extended to the issues of developmental status and outcomes of the children, and recently, to the independency of the youths discharged from the facilities. The practical and policy issues are development of therapeutic programs for the children in care, increase of counseling staff, extension of programs for restoring children's relationship with their families, and planning of various policies to support the independence of discharged youths.

  • The HFPD measurement method is a technique to analyze aging state of high voltage insulation materials and detect higher frequency signals than conventional PD measurement method therefore it takes less noise effect and could execute active line measurement. It is possible to analyze main discharge phenomena and obtain access to aging progress occurred in insulation materials through accumulation of HFPD signals during determined interval and expression of fractal dimension using statistical process of accumulated signals. In this study, the statistical parameters (skewness & kurtosis) and fractal dimensions are changed by discharge patterns that is shown up different characteristics with applied voltages and times.

  • Recently, the HFPD measurement methods that detect high frequency bandwidth of frequency components of partial discharge pulses are proposed and many studies are carried out. The HFPD measurement method is able to detect partial discharge signal in active line, therefore this method has a merit that can does condition monitoring of power equipments and measure growth state of partial discharge erosion with on-line and real time. In this study, the radiated HFPD signals are detected by antenna and aging diagnosis is executed with a neural network algorithm. As a result of this study, the aging diagnosis of insulation material using neural networks is possible.

  • These days, the aging population is becoming a critical social issue; the activation of the silver industry for the Y-O generation, meaning the young-old group of aged 55-64, is accelerating now. Pants are very important item to this generation, since they pursue active life with an exercise and leisure activities. Therefore, the physical, psychological, and social characters as well as the changed life style of women in the Y-O generation were reviewed, based on former studies and books. The consumer's patterns and actual wearing conditions about slacks were researched and analyzed through the survey toward 300 women aged 55-65 living in Seoul. Based on this research, information was provided for the development of slacks pattern which is suitable to the deformed body and aesthetic consciousness of the Y-O generation.

  • Japan is experiencing a most rapidly aging demographic. Despite this, the overall morbidity has been increasing due to the proportion of aging population that has increased rapidly along with the proportion of lifestyle related diseases, such as: all types of cancer, heart disease, stroke, diabetes, etc. As a result, the number of people requiring special care has become a serious problem. In response to these emerging health issues, the Strategic Planning force proposed the National Health Promotion Actualization Initiative in the 21st Century (“Healthy Japan 21”) so that it could be achieved by the year 2010. The policies should be conducted by providing adequately as well as with active participation and collaboration, effectively networking with the various organizations dealing with health issue. The Japanese Health Promotion Act passed by the National Assembly, 2001. As well as the many individuals that contributed to the development of this national health plan.

Effect of Process Variables and exisisting Ions on Highly Active Nano-sized ITO Powders Prepared by Precipitation Method (고활성 ITO (Indium-Tin Oxide) 나노 분말을 침전법으로 합성시의 공정 변수 및 존재하는 이온의 영향)

  • The objectives of this study were the development of a synthesis technique for highly active nanosized ITO powder and the understanding of the reaction mechanisms of the ITO precursors. The precipitation and agglomeration phenomena in ITO and $In_{2}O_{3}$ precursors are very sensitive to reaction temperature, pH, and coexisting ion species. Excessive $Cl^-$ ion and $Sn^{+4}$ ions had a negative effect an synthesizing highly active powders. However, with a relevant stabilizing treatment the shape and size of ITO and $In_{2}O_{3}$ precursors could be controlled and high density sintered products of ITO were obtained. By applying the reprecipitation process (or stabilization technique), highly active ITO and $In_{2}O_{3}$ powders were synthesized. Sintering these powders at $1500^{\circ}C$ for 5 hours produced 97% dense ITO bodies.

A Study on Purchase and Wearing Condition of Outdoor Wear among Active Senior Males (액티브 시니어 남성의 아웃도어 웨어 구매 및 착용 실태)

  • This study was carried out to offer basic data that can be used for clothing industry for active senior males emerging as a new consumer bracket in the rapid aging age. To this end, this study conducted a questionnaire survey on consumers' wearing condition targeting active senior males. As a result of the questionnaire survey targeting active senior males, the outdoor wear items that they had the most were windbreaker jackets, long pants, and they said the most important reason for their preferred brand was wearing sensation. The matters to consider in purchasing outdoor wear were also wearing sensation and size. Consequently, the active senior males regard wearing sensation as very important. The measurements that they felt uncomfortable by body part were the abdominal extension circumference, sleeve length and jacket length of a jacket, and the rise and lengths of pants. To design the outdoor wear patterns suitable for the active senior male's body type, the pattern measurements of these body parts need to be modified. From the fact-finding survey result on outdoor wear companies, their measurement indicating method was the same for jackets, but each brand used different methods for pants. Based on the results of this survey, outdoor wear needs to be developed to meet active senior needs.

The influence of expectations regarding aging on health-promoting behaviors (노화기대가 성인의 건강증진행위에 미치는 영향)

  • The purpose of the current study was to identify expectations regarding aging and health-promoting behaviors and to examine whether expectation regarding aging was associated with health-promoting behaviors among community-residing Korean adults. Data were collected from 233 adults dwelling in the community of Daegu and Kyungpook province. The influence of expectations regarding on health-promoting behaviors was analyzed through hierarchical multiple regression controlling for sociodemographic variables. As a result, the mean score of expectations regarding aging was significantly lower in 40s and 50s than 20s and 30s. The participants had the lowest expectations regarding aging in terms of physical health domain and the highest expectations in terms of mental health domain. No significant differences were found in health-promoting behaviors among different age groups. After controlling for sociodemographic variables, expectations regarding aging were independently associated with health-promoting behaviors in adults in 20s~30s but not in those in 40s~50s. The findings suggest the need for encouraging Korean adults to strive for having positive and active perspective on aging and for getting higher expectations regarding aging, in particular, for 20s and 30s, as a health-promoting strategy.

A Study on the Applicability of Wood Preservatives to Wooden Cultural Properties by Aging Treatment (열화 처리에 의한 목재 보존제의 목조문화재 적용성 평가 연구)

  • Wooden cultural heritage are exposed to the external environment as they and there are many difficulties in conservation due to their location and size. Among them, biological damage caused by termites or mold consumes a lot of money and time. Select and use wood preservatives to prevent biological damage: Wood preservatives were selected and the worst environmental conditions, temperature 60±3℃, humidity 55±5%, and light intensity of 0.35 W/m2, were subjected to aging treatment to analyze chemical changes. Through the deterioration process, it was confirmed that the change in color difference decreased in the wood preservative treatment compared to the Control group. As a result of measuring the content of the active ingredient contained in the deterioration process of the wood preservative, it was confirmed that the active ingredient content of Gori22 and Bondex Preserve III was higher than that of the comparative Wood Keeper A. Through experiments, the shelf life and treatment period can be predicted by measuring the extent to which wood preservatives affect the change of wood specimens during the deterioration process and the content of active ingredients. In conclusion, various wood preservatives were prepared, and the possibility of selectively selecting wood preservatives according to the environment, topography and period was presented as a major evaluation factor.
