Case Study of Treating Acne Scars Using Reconstruction of Skin Scars and Auto Microneedle Therapy System (흉터복원술과 자동 미세침(Auto Microneedle Therapy System, AMTS)을 이용한 여드름 흉터의 치험례)

  • Objective : Acne is very common disease, it can leave permanent scars. However, there are few reports about it in oriental medicine academia. This study performed to evaluate the effect of reconstruction of skin scars and auto microneedle therapy system on acne scars. Methods : Six patients were treated by using reconstruction of skin scars and auto microneedle therapy system. We compared the degree of treatment of acne scars on pre and post case photos and Qualitative Global Acne Scarring Grading System(QGASC). Satisfaction degree were asked from the patients. Results & Conclusion : After comparing case photos and QGASC, observing patients satisfaction, it can be considered that reconstruction of skin scars and auto microneedle therapy system to be valid acne scars treatment.

Comparative Study on Treatment of Acne Scar (여드름 흉터의 치료방법에 대한 비교연구)

  • Objective : This study performed to evaluate that duration of acne scars effects on Qualitative Global Acne Scarring Grading System(QGASC) grade changing and treatment duration. Also this study performed to evaluate the relationship between dysmenorrhea and acne scars, and the effects of each treatment on treatment duration and Korean Acne Grading System(KAGS) and QGASC grade.Methods : 95 patients with acne scars were understood by medical records and case photos, and we evaluated pre and post acne scar grade based on KAGS and QGASC. We analyzed data by SPSS 22 for windows program, and figured out the relation between duration of acne and QGASC grade and treatment duration by Spearman correlation analysis, and performed Mann-Whitney U-test to figure out the relation between dysmenorrhea and QGASC grade. We performed Wilcoxon test to evaluate that treatment of acne scars effects on KAGS grade and QGASC grade, and performed Kruskal-Wallis test to figure out the difference of each treatment which effects on KAGS grade and QGASC grade, and performed Post-hoc analysis to figure out the group comparison.Results & Conclusions : QGASC grade depending on duration of acne was statistically significance. Subcision and Microneedle Therapy System(MTS) and pharmacopuncture were significantly improved acne scars. Other treatments were significantly more effective than subcision and pharmacopuncture combined treatment. Treatment duration of Subcision and MTS combined treatment was significantly most shortest than other treatment.

Clinical Study of Twenty-seven Case of Treating Acne scar Using Microneedle Therapy and Subcision Combined with Beevenom Pharmacopuncture Therapy (미세피부침, 절개침법과 봉독 약침 복합 요법의 여드름 흉터 치료 27례 임상 보고)

  • Objectives : Microneedle therapy and Subcision are commonly used for therapeutic modalities of acne scars in Korean medicine. Beevenom could promote the expression of various cytokines, enzymes and receptors related to increasing collagen content of connective tissues. Through this mechanism, we considered that Beevenom pharmacopuncture therapy is applicable to the treatment of acne scars. We investigated twenty-seven patients with acne scars treated by Microneedle therapy and Subcision combined with Beevenom pharmacopuncture therapy and evaluated the clinical effect.Methods : Twenty-seven patients with acne scars were treated by Microneedle therapy and Subcision combined with Beevenom pharmacopuncture therapy and evaluated the clinical effect. for twelve weeks. The severity of acne scars and treatment effect were evaluate by standardized photography, GASC, ECCA scale and patient`s satisfaction. And side effects was also checked.Results : After twelve weeks for treatment, there was an improvement in GASC, ECCA scale. 77.8% of patient replied excellent or good on patient satisfaction survey. One patient sufferied from pruritus, papules and comedones as side effect of treatment.Conclusion : It can be considered that Microneedle, Subcision, Beevenom pharmacopuncture combining therapy are effective method for treating acne scars.

  • Background and Objective : Atrophic scars are the most common complication of acne. Many modalities are proposed but each does not yield satisfactory clinical outcomes. Among therapeutic modalities of acne scars, subcision is a simple, safe procedure with a different and basic mechanism for correcting atrophic and depressed scars. However, there are few reports about such procedure. This study performed to evaluate the effect of subcision treatment on atrophic acne scar. Methods : Five patients with atrophic acne scars of various types(rolling, superficial and deep boxcar, pitted and icepick) were treated by superficial dermal undermining with 25~27 gauge needles. We used GASC(Global Acne Scarring Classification). Satisfaction degree was asked from the patients. Results and Conclusions : After observing patients satisfaction, comparing case photos and GASC, it can be considered that subcision treatment brings great results when performed on atrophic post acne scars.

Clinical Reports on Atrophic Acne Scar Treatment with Sae-ssack Therapy (새싹테라피 3단계를 이용한 위축성 여드름 흉터 치료 20례)

  • Objectives : The purpose of this study was to evaluate the therapeutic efficacy of Sae-ssack(sprout) therapy on atrophic acne scars. Methods : Twenty patients had atrophic acne scars on the face were treated with Sae-ssack therapy. Sae-ssack therapy consists of a total of three treatments, a first-stage Sae-ssack acupuncture, a second-stage texture treatment, and a third-stage anti-pigmentation therapy. The evaluation of the treatment effect was made by the Korean medical doctor who did not participate in the procedure, and the PGA(Physician's Global Assessment), QGASC(Qualitative Global Acne Scarring Grade System) and patient satisfaction were used as the evaluation index. Results and Conclusions : According to the above indicates, the Sae-ssack therapy was evaluated to be effective in the repair of atrophic acne scars. There were no visible or consistent side effects and patient satisfaction was also very high.

  • Objective : The aim of this study is to report the effect of Hani-maehwa laser(fractional mode) treatment on acne scar. Methods : The patient with atrophic facial acne scars was treated with 5 sessions of laser therapy at 4-week intervals. The therapeutic response to treatment was assessed at 3 months after last laser session. The treatment effect was evaluated by standardized photography, anti aging, skin brightness and flatness. That was measured by micro fluorescence measurement camera(ECO SKIN, Cuvitz Inc.) in 16(left zygoma) and 26(right zygoma) spot. And side effects were also checked. Results : Atrophic facial acne scars have improved markedly. Anti aging, skin brightness and flatness were improved. Especially anti aging and flatness were noticeably improved. Adverse effects were not reported.Conclusions : Korean medicine cautery method applies to high level laser(CO2 Hani-maehwa laser). These are different designs but same principle. It can be considered that laser therapy is effective method for treating acne scars.

Case Study of Treating Acne Scar Using Scar Regeneration Acupuncture Therapy and Micro-needle Therapy (흉터재생침과 미세피부침을 이용한 여드름 흉터의 치험례)

  • Objectives : There are numerous methods in treating acne scar. On the contrary there are hardly any cases on treating acne scars through Korean medicine. By observing we compared how much acne scar was treated by scar regeneration acupuncture therapy and micro-needle therapy treatment through observing pre and post case photos, KAGS, KCADI and patient satisfaction. Results : By comparing pre and post procedure through acne scar grade criteria, total of 5 patients showed improvement and could see distinct change in skin comparing case photos. After comparing pre and post procedures, total of 5 patients showed improvements in figures of Korean acne index. 4 patients replied very satisfied and 1 replies satisfied on patient satisfaction survey. Conclusions : After observing patients satisfaction, and comparing case photos, acne scar grade criteria, korean acne index, it can be considered that scar regeneration acupuncture therapy and micro-needle therapy done in our clinic to be valid treatment.

  • Objective: Acne scarring is the most common sequela after dermal inflammation caused by acne. Many methods are currently used to treat acne scarring; doctors using traditional Korean medicine have also attempted to treat acne scarring in a new way that was developed from traditional methods. This study examines the effectiveness of re-cell therapy.Method: Eleven patients with various types of acne scars were treated by re-cell therapy. We used the global acne scarring classification (GASC) and ECCA (from the French “echelle d’evaluation clinique des cicatrices d’acne”), both of which are generally used to measure and grade the degree of acne scarring.Results: As a result of the treatment, the conditions of all patients were conspicuously improved in both scales. The GASC scores of the patients changed from grade 19.6±10.4 to grade 4.9±3.3 (p=0.001), and the ECCA scores changed from grade 120.9±28.7 to grade 41.8±18.3.Conclusion: Re-cell therapy showed effectiveness in treating acne scarring and could be applied in clinical cases.

A Clinical Report on Facial Atrophic Scars treatment with Acupotomy (도침을 이용한 안면부 위축성 흉터의 치험례)

  • Objectives: The purpose of this study is to evaluate the clinical effectiveness of acupotomy in the treatment of various facial atrophic scars except acne scars. Methods: Four patients with facial atrophic scars were treated by superficial dermal undermining with acupotomy. They were treated six times, and evaluated by comparing before and after photos, PGA(Physician's Global assessment), SESES(The Stony Brook Scar Evaluation Score) and patient satisfaction. Results and Conclusions: After observing case photos, PGA, SBSES, patient's satisfaction, it can be considered that acupotomy is an effective treatment modality for facial atrophic scars and it has no significant side effects.

Effectiveness of a Fractional Picosecond 1,064-nm Laser in Improving Traumatic Scars with Depression

  • The picosecond laser has been widely used to remove tattoos, and improve wrinkles, acne scarring, and pigmented scars. This study was performed to investigate the efficacy and safety of a 1,064-nm fractional picosecond laser treatment for depressed traumatic scars. A total of twenty Korean patients with depressed scars were treated with a 1,064-nm fractional picosecond laser at a two-week interval with a spot size of 3 mm, fluence of 3.5 to 5.5 J/cm2, and frequency of 2 Hz with a combination of microlens arrays. At 4 weeks after the final treatment, there was a statistically significant improvement in the modified Vancouver Scar Scale (mVSS) and the patient satisfaction score without any significant complication. A 1,064-nm fractional picosecond laser treatment is a safe and effective method to improve traumatic wound scars with depression.
