Crown-type implant-assisted removable partial dentures (CIRPDs) can be a feasible treatment option for partially edentulous patients. Here we report a case with remaining unilateral mandibular teeth. Two implants were placed in the posterior portion of the mandible using a surgical guide, and a distal-extension removable partial denture with implant-supported surveyed crowns was fabricated. After 12 months, both the abutment teeth and implants were in good condition. The treatment outcomes were satisfactory in terms of masticatory function and esthetics. The advantages of CIRPDs and considerations for obtaining successful clinical outcomes with these dentures are also discussed.

PURPOSE. To compare the fracture resistance and the mode of failure of endodontically treated teeth restored with different post-core systems. MATERIALS AND METHODS. Root canal treatment was performed on 40 maxillary incisors and the samples were divided into four groups of 10 each. For three experimental groups post space preparation was done and teeth were restored with cast post-core (Group B), stainless steel post with composite core (Group C) and glass fiber post with composite core using adhesive resin cement (Group D). Control group (A) samples were selected with intact coronal structure. All the samples were prepared for ideal abutment preparation. All the samples were subjected to a load of 0.5 mm/min at $130^{circ}$.until fracture occurred using the universal testing machine. The fracture resistance was measured and the data were analyzed statistically. The fracture above the embedded resin was considered to be favorable and the fracture below the level was considered as unfavorable. The statistical analysis of fracture resistance between different groups was carried out with t-test. For the mode of failure the statistical analysis was carried out by Kruskal-Wallis test and Chi-Square test. RESULTS. For experimental group Vs control group the fracture resistance values showed significant differences (P<.05). For the mode of failure the chi-square value is 16.1610, which means highly significant (P=.0009) statistically. CONCLUSION. Endodontically treated teeth without post core system showed the least fracture resistance demonstrating the need to reinforce the tooth. Stainless steel post with composite core showed the highest fracture resistance among all the experimental groups. Teeth restored with the Glass fiber post showed the most favorable fractures making them more amenable to the re-treatment.
PURPOSE. The aim of this in vitro study was to evaluate the effect of sintering procedures on marginal discrepancies of fixed partial metal frameworks fabricated using different sintering-based computer-aided design and computer/aided manufacturing (CAD/CAM) techniques. MATERIALS AND METHODS. Forty resin die models of prepared premolar and molar abutment teeth were fabricated using a three-dimensional (3D) printer and divided into four groups (n = 10) according to the fabrication method of metal frameworks used: HM (via hard milling), SM (via soft metal milling), L25 (via direct metal laser melting [DMLM] with a 25 ㎛ layer thickness), and L50 (via direct DMLM with a 50 ㎛ layer thickness). After the metal frameworks were fabricated and cemented, five vertical marginal discrepancy measurements were recorded in each site (i.e., buccal, facing the pontic, lingual, and facing away from the pontic) of both abutment teeth under a stereomicroscope (×40). Data were statistically analyzed at a significance level of 0.05. RESULTS. No statistically significant differences (P>.05) were found among the four axial sites of metal frameworks fabricated by sintering-based CAD/CAM techniques. The HM and L25 groups showed significantly (P<.001) lower marginal discrepancy values than the SM and L50 groups. CONCLUSION. Marginal discrepancy in the sites facing the pontic was not influenced by the type of sintering procedure. All fabrication methods exhibited clinically acceptable results in terms of marginal discrepancies.
Background: In this study, zirconia copings were fabricated by setting clinically acceptable inner values for prostheses using computer-aided design/computer-aided manufacturing (CAD/CAM). The processed copings were evaluated for the marginal and internal fit of each abutment shape with a CAD program using the silicone replica technique. Methods A total of 20 copings was produced by selecting models commonly used in clinical practice. After injecting the sample, the minimum thickness, internal adhesion interval, and distance to the margin line were set to 0.5, 0.05, and 1.00 mm using a dental CAD program, respectively. It was measured using a 2D section function in a three-way program of the silicon replication technology. Although the positions and number of measurements of the anterior and posterior regions differed, nine parts of each pre-tube were designated and measured by referring to a previous study to compare the two samples. Results As a result, the average margin of the mesial, distal, and buccal (labial) surfaces was 59.90 ㎛ in the anterior region and 60.40 ㎛ in the posterior region. The mean axial wall margin was 67.25 ㎛ in the anterior region and 69.25 ㎛ in the posterior region. In occlusion, the anterior teeth (77.70 ㎛), posterior teeth (77.60 ㎛), and both anterior and posterior regions were within the clinically acceptable range. Conclusion The edge and inner fit of zirconia coping manufactured using the CAD/CAM system showed clinically applicable results. To reduce errors and increase accuracy, materials and machine errors that affect the manufacture of prosthetics should be investigated. Based on our results, the completeness of prosthetics could increase if the inner value and characteristics of the material are adjusted when applied in clinical practice.
연구 목적: IPS Empress Esthetic$^{(R)}$ system의 ingot은 다양한 반투명도를 가지고 있지만, 실제 다양한 색조의 자연치를 배경으로 각각의 ingot을 laminate veneer로 제작했을 때 어느 정도의 차폐능력을 나타내는지 알 수 없다. 본 연구의 목적은 반투명도가 서로 다른 ingot으로 제작한 IPS Empress Esthetic$^{(R)}$ laminate veneer 수복 시 다른 색상의 지대치상에서 최종 색조의 차이(color difference, ${\Delta}E$)를측정, 분석해서 그 차폐효과를 평가하고, 최종 색조에 미치는 영향을 알아보고자 하였다. 연구 재료 및 방법: Laminate veneer 수복을 위해 삭제된 6가지 다른 색조(A1, A3, A4, B2, B3, C3)의 자연치아 위에 반투명도가 서로 다른 6가지 IPS Empress Esthetic$^{(R)}$ ingot (E 01, E 03, E OC1, E TC1, E TC2, E TC3)으로 제작된 총 두께 0.6 mm의 도재 시편들을 translucent shade의 시적용 합착제로 시적한 뒤 각 시편 조합의 색조를 색채계(colorimeter)를 이용하여 측정하였다. 서로 다른 색조의 지대치아 위에서 측정된 도재시편들의 색차를 비교, 분석하였다. 결과: 지대치 색조 및 coping의 반투명도에 따라서 laminate veneer 수복물의 최종 색조가 다르게 나타났다(P<.05). 모든 도재 시편에서 A3, B3, 그리고 C3 shade를 가진 지대치들 사이의 평균 색차값(${\Delta}E$)이 2.7 이하로 나타났고, 지대치들 간의 색조가 A3 와 A4, B3 와 A4, 그리고 C3 와 A4 인 경우 평균 색차값은 몇 가지 도재 시편에서 2.7 이하를 나타내어 laminate veneer 수복물에 의한 지대치 색조의 차폐효과를 보여 주었다. 반면, A1 과 B2 shade의 지대치는 다른 지대치들과 비교하여 높은 색조 차이를 보였으며, 특히A4와 B2, A3 와 B2, 그리고 A1 과 A4 색조의 지대치들 사이에는 그 색조 차이가 컸다. 결론: IPS Empress Esthetic$^{(R)}$ coping으로 제작된 laminate veneer의 최종 색조는 지대치 색조 및 coping의 반투명도에 의해 영향을 받았다. 서로 다른 $a^*$와 $b^*$ 값을 나타내는 지대치들의 색상 차이는 어느 정도 차폐 되는 효과를 보여주고 있으나, 명도($L^*$)의 차이가 많이 나타나는 지대치 색조들을 차폐하는 것은 한계가 있었다.
가철성 국소의치에서 지대치는 국소의치의 지지, 유지, 안정에 필수적으로, 기능적인 의치 제작의 중요한 요소이다. 치주적으로 건전한 지대치가 충분하고 양측으로 위치할 경우 기능적인 의치 제작이 용이 하겠지만, 지대치가 부족하거나 편측에 위치하는 경우에는 생역학적으로 불리하여 기능적인 의치를 만들기가 상대적으로 어렵다. 한편, 최근에는 임플란트 서베이드 크라운을 이용해 지지, 유지, 안정 세 요소를 보강하는 임플란트 지지 국소의치가 치료 계획으로 많이 선택되고 있다. 특히, 소수 잔존치만 남은 경우에 임플란트 서베이드 크라운을 추가함으로써 저작 능률, 심미, 환자의 편안감, 발음의 개선 등을 얻을 수 있다. 본 증례는 하악에 편측 소수 잔존 지대치를 갖는 환자에서 반대편 소구치부에 임플란트 서베이드 크라운을 추가하여 양측성 후방연장 국소의치를 제작하였다. 지대치의 비대칭성을 해소하였고, 환자와 술자 모두 만족하는 결과를 얻었기에 본 증례를 보고하는 바이다.
치주적으로 지지가 불리한 소수의 잔존치가 남은 환자에게 있어 하이브리드 텔레스코픽형 이중관의치는 장기적으로 안정적인 예후를 보이는 치료가 된다. 편측 악궁에 소수 잔존치가 남고 반대측 악궁에 다수의 자연치가 존재 하는 경우 임플란트를 식립하여 자연치와 혼재된 형태의 이중관 의치를 제작 할 시 의치의 지지 및 안정요소의 향상과 더불어 저작력의 균형을 맞추어 줄 수 있게 된다. 본 증례는 55세 여자 환자로 만성 치주염으로 인해 치주 지지가 감소한 소수 잔존치 상태에서 자연치만으로 하이브리드 텔레스코픽형 이중관 의치 치료를 통해 9.5년간 의치를 사용하였다. 9.5년 후 지대치 파절로 내원 하여 임플란트의 식립을 통해 자연치와 임플란트가 혼재된 하이브리드 텔레스코픽형 이중관 의치로 재수 복 하였으며, 이를 통해 재 치료에서도 장기적 예후의 안정성을 기대 할 수 있다.
Purpose: Pink gingival esthetic especially on the anterior teeth has been an important success criterion in implant-supported restoration. Inter-implant papillae are a critical factor for implant esthetics, and various techniques for inter-implant papilla reconstruction have been introduced. The aim of this study is to suggest and evaluate a surgical technique for reconstructing inter-implant papillae. Methods: A 28-year-old man had an implant placed on the #13 and #14 area. Four months after implant placement, a second stage surgery was planned for inter-implant papilla reconstruction. At the time of the abutment connection, I-type incisions were performed on the #13i & #14i area followed by full-thickness flap elevation and connection of a healing abutment on underlying fixtures without suture. Results: Two weeks after the second stage implant surgery, soft tissue augmentation between the two implants was achieved. Conclusions: I-shaped incisions for papilla reconstruction performed during the second stage implant surgery were useful for inter-implant papilla reconstruction and showed a good esthetic result.

Purpose: This retrospective study was performed to analyze the relationship between complications of the posterior mandibular single crowns and distance from the adjacent teeth to the implant. Subjects and Methods: Of the patients who presented Ewha Womans University Mokdong Hospital & Yonsei University Dental Hospital with missing of the posterior mandibular molar and restored with implant-supported 18 Single crowns between 1996 thru 2007, 115 patients had been followed after crown delivery. The subjects were divided into complication-followed group and a control without any problems. The distance from the most distal tooth to the implant were measured. The prosthetic & biologic complications were reviewed by the cantilever distance and analyzed by abutment type, age & gender statistically using SAS version 9.1 (SAS Inc., USA). Results and Conclusion The results were as follows; 1) The posterior mandibular single crown with cantilever showed higher incidence of follow-up complications upon logistic analysis (p<0.05). 2) The prosthetic and biologic complications are related with the cantilever distance with 2.1 odds ratio and 3.39 cut-off value of specificity & sensitivity by SPSS 12.0. 3) The complications are neither significant in abutment types nor age & gender.
The purpose of the present study was to evaluate the marginal discrepancy and topography of artificial crown on teeth extracted due to severe periodontal disease. Twenty specimens were invested into metamethylacrylate resin and cutted into vertical slices along with the long axis of tooth. The selected marginal discrepancy between the outer edge of the crown and the finishing line of abutment was examined by stereo- microscope(Olympus, PM-VSP-3, Japan) at magnification of up to 10, and the topography of finishing margin on crown was observed by stereomicroscopeat magnification of up to $70{\times}$. The results were as follows. (1) The mean marginal discrepancy between extracted tooth and artificial crown were $50.82{\mu}m$. (2) There was a considerable difference in the microstructure of finishing margins among specimens. Microscopic Structure on finishing margin showed indefinite line, poor fit (open, underextended and overextended), distorted margin, and surface roughness. This study suggested that there could be necessary to consider the response of periodontium to the emergence profile of natural tooth and precision of marginal geometry while establishing treatment planning for the reconsruction of the artificial crown.
