하악골 이단술을 시행한 환자에서 중립대를 활용한 상하악 총의치 수복 증례 (Neutral zone approach for rehabilitation in a patient who underwent mandibulotomy: A case report)

  • 본 증례는 65세 남환으로, 10년 전 편도암으로 하악골이단술을 시행한 환자에서 중립대를 활용한 상하악 총의치 수복 증례이다. 본 환자는 무치악 기간이 짧아 치초제의 흡수가 거의 없으며, 혀의 운동기능에도 이상이 없었지만, 수술 이후의 변화로 인해 하악의 후구치 삼각융기가 상악의 구상절흔보다 안쪽에 위치해 있으며 하악의 구치부 치조제는 상대적으로 설측으로 돌아간 구조를 보였다. 만약 통상적으로 해부학적 지표를 참고하여 치아를 배열한다면, 혀의 기능 공간을 침범하게 되며, 이는 의치의 안정성을 저하시킬 것이다. 이런 경우 중립대 개념을 적용한 의치의 제작은 좋은 참고점이 될 수 있을 것이다. 연성 이장재를 사용하여 중립대를 인기하였으며, 이를 인덱스로 제작하여 치아배열을 시행하였다. 중립대의 가장 큰 장점은 의치의 안정을 얻는 것에 있다. 본 환자의 경우 하악 좌측 구치부가 혀 공간을 침범하지 않도록 구치 치아의 설측배열 한계를 얻기 위해 중립대 기법을 적용하였다. 특히, 치조제의 흡수로 의치의 안정성을 얻기 어려운 난증례 환자의 경우 이러한 방법을 사용한다면 의치의 안정성 면에서 더욱 좋은 결과를 얻을 수 있을 것이라 보기에 본 증례를 보고하는 바이다.

경동맥 내중막 두께에 따른 죽상경화반의 위험요인과의 상관관계 (Correlation between Carotid Intima-media Thickness and Risk Factors for Atherosclerosis)

  • 본 연구는 건강검진을 목적으로 내원하여 경동맥초음파를 실시한 113명을 대상으로 하였다. 경동맥의 내중막 두께는 심혈관 질환 및 뇌혈관 질환과 유의한 상관관계를 보이는 것으로 알려져 있다. 임상에서 선별 검사로 많이 이용되고 있는 경동맥 초음파를 이용하여 경동맥 내중막 두께와 체질량지수, 허리둘레, 혈청지질수치, 공복 혈당, 당화혈색소, 혈압등이 갖는 연관 관계에 대하여 알아보고자 하였다. 경동맥초음파상 IMT두께가 0.8 mm 이상을 비정상으로 간주하고 죽상경화증(atherosclerosis)의 유무를 평가하였으며, 혈청검사를 통하여 지질학적 수치 및 공복혈당수치, 당화혈색소를 분류하여 상관관계를 알아보았다. 그 결과 허리둘레(p=.022), 저밀도 콜레스테롤(p=.004), 공복혈당수치(p=.019), 당화혈색소 수치(p=.002)가 죽상경화증(atherosclerosis을 일으키는 예측인자로 분석되었다. ROC 곡선 분석에서 허리둘레에서 민감도 87.80%(95% CI : 73.8-95.9), 특이도 41.67%(95% CI : 30.2-53.9), 저밀도 콜레스테롤에서 민감도 78.05%(95% CI : 62.4-89.4), 특이도 50.00%(95% CI : 38.0-62.0 공복혈당에서 민감도 73.11%(95% CI : 57.1-85.8), 특이도 61.11 (95% CI : 48.9-72.4)), 당화혈색소에서 민감도 82.93%(67.9-91.8), 특이도 43.06%(31.4-55.3)를 나타내었다. 로지스틱 회귀분석에서 죽상경화증(atherosclerosis) 발생위험성은 허리둘레(WC)>76 cm에서 0.248배, 저밀도 콜레스테롤(LDL-C)${\geq}124mg/dL$에서 3.475배, 당화혈색소(HbA1C)>5.4%에서 0.618배로 나타났다. 향후 심뇌혈관질환의 효과적인 일차예방 역할을 위해 다수의 대상자를 대상으로 혈액검사수치를 고려하여 혈관의 추적관찰이 용이한 경동맥초음파를 통한 전향적연구가 필요할 것으로 사료 된다.

B-type Natriuretic Peptide 수치와 이완기 심초음파 파라미터와의 연관성 연구 (Studies for B-type Natriuretic Peptide Values and Its Association with Diastolic Echocardiographic Parameters)

  • B-type natriuretic peptide (BNP)와 심초음파는 심부전 진단과 예후를 판단하는 지표로써 수축 기능이상에 많은 연구를 했으나, 본 연구에서는 BNP 결과와 심초음파 이완 기능을 나타내는 항목의 상관관계를 알아보고자 한다. BNP 검사와 심초음파를 실시한 188명을 대상으로 BNP 결과에 따라 5그룹으로 분류하고, 심초음파의 좌심실 구혈률, 좌심방 크기, E velocity, A velocity, Deceleration time, E/A ratio, E', A', S', E/E'와의 상관관계를 조사하고, 그 중 심부전을 진단 받은 67명을 Relaxation abnormality (Grade I), Pseudonormalization (Grade II), Restrictive physiology (Grade III)로 등급을 분류하고 BNP 결과와 심초음파 항목을 비교한 결과 BNP 수치가 상승할수록 심초음파 항목 중 좌심실 구혈률, 좌심방 크기, E/A ratio, Deceleration time, A', S', E/E'와 유의한 상관관계가 있었다. 그룹간의 비교에서 E/E' 항목이 가장 좋은 유의성을 보여주었다. 심부전 환자의 등급 분류에 따른 BNP 결과는 $623.0{\pm}459.7pg/mL$, $1013.2{\pm}1155.1pg/mL$, $1693.4{\pm}1544.0pg/mL$로 유의성이 있었으며(p<0.01), 등급이 올라갈수록 심초음파 항목 중 좌심실 구혈률, 좌심방 크기, E velocity, A velocity, E/A ratio, Deceleration time, A', E/E' 와 상관관계가 있었다(p<0.001). BNP 결과가 상승할수록 심초음파 이완기 항목과 유의성이 있었고, 심초음파에서 Restrictive physiology 그룹의 BNP 결과가 가장 높았다.

감성돔(Acanthopagrus schlegeli) 치어의 골격발달 및 생리적 조건에 미치는 외인성 갑상선호르몬$(T_3)$의 영향 (Effects of Exogenous Thyroid Hormone $(T_3)$ on Skeletal Development and Physiological Conditions of Juvenile Black Seabream (Acanthopagrus schlegeli))

  • 감성돔, A. schlegeli 치어의 골격발달 및 생리 변화에 미치는 3,5,3'-triiodo-L-thyronine $(T_3)$ 의 효과를 파악하기 위해 $T_3$를 0, 10, 20, 50 및 100ppm으로 사료에 섞어 경구 투여하였다. 실험종료시 일간섭식량은 대조구와 10ppm 및 20ppm 처리구와는 차이가 없었지만, 50ppm구 및 100ppm구는 오히려 대조구에 비해 낮아지는 것을 알 수 있었다. 체장 성장의 경우는 10ppm 처리구만이 대조구에 비해 빨랐고, 그 외 처리구는 대조구와 차이가 없었다. 체장에 대한 아가미 뚜껑, 두부, 가슴지느러미 및 꼬리지느러미 길이의 비율은 $T_3$ 처리 농도가 증가할수록 높아지는 경향이었다. 한편, $T_3$ 처리에 따른 비정상적 골격발달을 나타내는 개체는 $20\~100ppm$ 처리구에서 관찰되었으며, 50ppm과 100ppm 처리구에서 유의하게 많았다. HSI는 $T_3$ 처리 농도가 높아질수록 낮아지는 경향을 보였다. $T_3$ 처리 종료시인 사육 40일째의 TCH는 처리 농도가 높아질 수록 낮아지는 경향을 나타냈으나, 사육 50일째의 TCH는 100ppm 처리구에서만 유의하게 낮았다. 실험 종료시의 어체성분을 분석한 결과, 수분과 단백질은 10ppm 처리구에서만 대조구에 비해 유의하게 많았다. 지질은 모든 $T_3$ 처리구에서 대조구보다 적게 나타났으나, 회분은 $T_3$ 처리량이 많아질수록 높아지는 경향이었다.

동위원소 이뇨 요관그람을 이용한 소아 요관폐쇄의 평가 (Assessment of Hydroureteronephrosis in Children Using Diuretic Radionuclide Ureterography)

  • 수신증 또는 요관수류수신증으로 이뇨 신장스캔을 시행한 80명의 환아중 이노신장스캔영화영상에서 10분 이상의 뚜렷한 지속적인 요관 영상을 보이는 12명의 16예의 신장요관계를 대상으로 요관 관심영역에 의한 이뇨요관그람 요관곡선과 신장 관심 영역에 의한 이뇨 레노그람 신장곡선의 배뇨 반감시간을 포함한 각각의 정량적 지표를 비교 분석하여 다음의 결과를 얻었다. 1) 이뇨 신장스캔영화영상에서 10분 이상의 뚜렷한 지속적 요관 영상을 보이는 경우인 스캔상 요관 이상(요관확장)의 진단률을 방사선학적 검사에 의한 결과와 비교하였을 때, 예민도와 특이도는 각각 88%와 99%이었다. 2) 이뇨 요관그람 요관곡선상 이뇨 요관배뇨 반감시간의 비폐쇄의 폐쇄에 대한 경계치를 10분 이하로 하였을 때 요관 폐쇄 여부를 판별한 예민도와 특이도는 각각 86% (7/8) 이었다. 3) 이뇨 요관그람 요관곡선상 이뇨 요관배뇨 반감시간(Ut1/2)과 이뇨 레노그람 신장곡선상 이뇨 신장배노 반감시간(Kt1/2)간에 유의한 상관 관계를 보였다(r=0,70: p<0.01). 결론적으로 소아의 폐쇄성 요로질환에서 이뇨 신장스캔상 10분 이상의 지속적 요관 영상을 보이는 경우 이뇨요관그람 요관곡선을 이뇨 레노그람 신장곡선과 함께 분석하여 요관 폐쇄의 유무와 정확한 부위를 평가하여 치료 방침을 결정하는 것이 필수적일 것으로 사료된다.

인공호의 부영양화에 따른 피라미(Zacco platypus) 개체군의 전장-체중 관계 및 비만도 지수 (Length-Weight Relations and Condition Factor (K) of Zacco platypus Along Trophic Gradients in Reservoir Ecosystems)

  • 본 연구는 인공호의 영양 단계에 따른 피라미 개체군의 전장-체중 관계 및 비만도 지수 (K)의 차이를 알아보기 위해 2008년~2010년까지 부안호, 양양호, 진양호, 금풍지, 예당지 및 마산지 등의 6개 인공호를 대상으로 실시하였으며, 광범위한 분포 특성과 섭식 특성을 보이는 피라미(Zacco platypus)를 본 연구의 대상 어종으로 선정하였다. 총인 (TP)과 엽록소-a (Chl-a)를 이용한 영양상태지수(TSI) 분석 결과 양양호와 부안호는 빈영양(30~40), 진양호와 금풍지는 중영양 (40~50), 예당지와 마산지는 부영양 (50~70)상태로 나타났다. 6개 호수에서 총 47종 26,226개체의 어류가 채집되었으며, 빈영양호 (양양호, 부안호)에서는 민감종이 우세한 현상을 보였다. 한편, 중영양호 (진양호, 금풍지)와 부영양호 (예당지, 마산지)에서는 내성종이 우세한 것으로 분석되어 호수의 영양 상태에 따라 우세종의 내성특성에 차이를 보이는 것으로 나타났다. 전장-체중 관계 분석 결과에 따르면, 회귀계수 b값은 중 영양호(3.07~3.17)와 부영양호(3.15~3.21)에 비해 빈영양호 (2.77~2.79)에서 낮은 값을 보였다. 이러한 결과는 호수의 영양상태가 피라미 개체군의 성장도에 영향을 주는 것으로 사료된다. 전장에 따른 비만도 지수 (K) 관계 분석 결과 중영양호와 부영양호에서는 양의 기울기를 보인 반면, 빈영양호에서는 음의 기울기를 나타내 호수의 영양상태에 따라 차이를 보였다. TSI (TP), TSI (Chl-a)와 회귀계수 b값의 상관관계를 분석한 결과 b=0.012*TSI (TP)+2.395 ($R^2$=0.797, p=0.017), b=0.013*TSI (Chl-a)+2.367 ($R^2$=0.822, p=0.013)으로 나타나 높은 상관관계를 보였다. 한편, 부영양화가 진행될수록, 비정상개체의 비율이 증가하는 것으로 나타났다. 결론적으로, 전장-체중 관계와 비만도 지수 분석에 의한 피라미 개체군의 성장도는 영양염류나 식물플랑크톤의 생체량과 같은 영양상태를 반영하는 것으로 사료되며, 과도한 부영양화는 수환경의 오염을 초래하여 피라미 개체군의 성장과 건강도에 부정적인 영향을 미치는 것으로 사료된다.

Juxtaductal stenosis가 동반된 PA/VSD환자에서 체폐단락술 부위에 따른 폐동맥 크기의 변화 (Pulmonary Arterial Growth Pattern after Shunt Operation in Patients of Pulmonary Atresia with Ventricular Septal Defect Associated with Juxtaductal Stenosis)

  • 심실중격결손이 동반된 폐동맥폐쇄(PA/VSD)는 폐동맥의 형태 및 공급원이 매우 다양하고, 폐동맥의 발육부전 과 협착 및 폐동맥지 연결이상 유무가 교정수술시의 문제점으로 대두되고 있으므로, 저자들은 동맥관인접협 착(juxtaductal stenosis)이 동반되어 있는 환자들을 대상으로 체폐단락술전후 폐동맥 크기의 변화를 비교하 여, 수술방법 및 시기를 결정하는데 도움이 되고자 본 연구를 하였다. 1991년 7월부터 1996년 7월까지 연세대 학교 심장혈관센터에서 심실중격결손이 있는 폐동맥폐쇄환자중 동맥관인접협착이 동반되어 체폐단락술을 시행 한 59례가 있었으며, 수술전후의 심도자술 및 심혈관조영술을 시행한 29례를 대상으로 하였다. 우측(10례, Group I) 및 좌측단락술(19례, Group II)에서 단락술전후의 하행대동맥, 양측폐동맥 및 동맥관인접협착 부위의 직경을 측정하여, 수술부위에 따른 수술전후 변화를 비교하였다. 두 군에서, 하행대동맥직경에 대한 동측 폐동 맥직경의 비(ratio)가 수술전 0.78$\pm$0.31에서 수술후 1.01$\pm$0.26로, 또한 수술전 0.67$\pm$0.18에서 수술후 0.84$\pm$ 0.27로 각각 유의하게 증가하였으며, 편측 폐동맥직경의 비는 수술전 0.92$\pm$0.28에서 수술후 1.05$\pm$0.15로, 또한 수술전 0.94$\pm$0.27에서 수술후 1.08$\pm$0.37로 각각 증가하였으나 통계학적으로 유의하지는 않았다. 동맥관 인접협착 부위의 변화는 수술전 0.43$\pm$0.27에서 수술후 0.39$\pm$0.25로, 또한 수술전 0.32$\pm$0.10에서 수술후 0.30$\pm$0.16로 감소하는 경향을 보였으나 통계학적으로 유의하지 않았으며, 2례의 경우에서는 수술후 단절된 소견을 보였다. 체폐단락술을 통한 폐동맥혈류의 확보는 폐혈관 성장에 좋은 효과가 있으므로 폐혈류 감소 및 폐동맥 발육부전환자에서 추천되는 치료법이나 심실중격결손을 동반한 폐동맥폐쇄의 경우에는 동맥관인접협착의 변화가 중요한 것으로 생각된다. 본 연구 결과에서는 체폐단락술을 시행한 동측의 폐동맥이 성장함을 확인할 수 있었고, 동맥관인접협착은 악화될 가능성이 있는 것으로 생각되었다. 또한, 좌측 체폐단락술을 시행하여, 2례의 좌측 폐동맥의 단절을 경험하였다. 그러므로, 체폐단락술을 시행한 경우에는, 보다 철저한 술후 추적 및 검사가 요구되며, 동맥관인접협착이 동반된 측의 폐동맥에 단락술을 시행한 경우에는 조기에 완전 교정술을 고려하여야 할 것으로 사료된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms.
