A Survey on the Complications Associated with Acupotomy in a Single Korean Medicine Clinic (일개한의의료기관에서 시행한 침도치료 시술 후 이상반응에 대한 설문조사)

  • Objectives : This study aimed to evaluate adverse outcome associated with acupotomy and compare it with that of acupuncture. Methods : We retrospectively analyzed the chart records of 71 patients who received acupotomy at Daemyung Korean medical clinic from January 7, 2020 to March 6, 2020. We divided the acupotomy treatment area into 10, including the head, hand, chest, knees, shoulders, low back, neck, upper extremities, thighs, and feet. Furthermore, we investigated the adverse effect of acupotomy on those areas after treatment. Data were analyzed using descriptive statistics, and frequency analysis. Results : 'Bruise of specific region' accounted for the largest portion with 29 cases, followed by 24 cases of 'feeling tired and sleepy' and 17 cases of 'itch'. There were 16 cases of 'sustained pain' at the area of treatment, 11 cases of 'fatigue', 5 cases of 'swelling', 5 cases of 'dizziness', 4 cases of 'hematoma'. There were 3 cases of 'vertigo' and 2 cases of 'parathesia' and 'wide bruises', 'headache', 'gastric discomfort', 'bleeding', and 'skin rash' each. There was one case each that experienced 'unclear pronunciation', 'nausea', 'abnormal sweating', 'vomiting' and 'emotional/psychological reactions'. Conclusions : Adverse outcomes associated with acupotomy were mostly 'bruise of specific region', 'feeling tired and sleepy', 'itch', 'sustatained pain', and 'fatigue'. However, no adverse outcome or irreversible damage that have a serious effect on the body were observed.

  • The two major problems related to the blood flow in replaced prosthetic heart valve are thrombus formation and hemolysis. Reliability of prosthetic valve is very important because its failure means the death of patient. There are many factors affecting the valvular failures and their representatives are mechanical failure and thrombosis, so early noninvasive detection is essentially required. The purpose of this study is to detect the various thromboses formation by using acoustic signal acquisition and its spectral analysis on the frequency domain. We made the thrombosis models using Polydimethylsiloxane (PDMS) and they are thrombosis model on the disc, around the sewing ring and fibrous tissue growth across the orifice of valve. Using microphone and amplifier, we measured the acoustic signal from the prosthetic valve, which is attached to the pulsatile mock circulation system. A/D converter sampled the acoustic signal and the spectral analysis is the main algorithm for obtaining spectrum. Then the spectrum of normal and 5 different kinds of abnormal valve were obtained. Each spectrum waveform shows a primary and secondary peak. The secondary peak changes according to the thrombus model. To quantitatively distinguish the frequency peak of the normal valve from that of the thrombosed valves, analysis using a neural network was employed. Acoustic measurement has been used as a noninvasive diagnostic tool and is thought to be a good method for detecting possible mechanical failure or thrombus.

  • Peritoneopericardial diaphragmatic hernia(PPDH) is uncommon and congenital disease in dogs and cats. In PPDH, the peritoneal organs such as liver, small intestine, stomach and omentum are displaced into the congenital defect between pericardial sac and diaphragm and cause the abnormal round and enlarged cardiac silhouette. Abnormal cardiac silhouette contacts with the cranial diaphragmatic border consistently and soft tissue- and/or gas- density structures are summated over the cardiac density in radiography. The contrast medium flows from peritoneal cavity into the pericardial sac and demonstrates the herniated abdominal organs and the abnormal defect in positive peritoneography. In this study, 4 dogs was diagnosed as PPDH using radiography, peritoneography and thoracic ultrasonography and showed various clinical signs according to the kind, amount and clinical state of herniated abdominal organs.

Notomelia with polydactyly in Korean native cow (과다발가락을 가진 한우의 등사지증)

  • An abnormal Korean native cow of five years old with an extra leg was observed macroscopically and radiographically. The results were summarized as follows. 1. External features included two normal forelimbs and hindlimbs, and an extra abnormal forelimb which was underdeveloped. The extra forelimb attached to the regions of back on right scapula. 2. The extra forelimb had antebrachial meromelia. It consisted of undeveloped and severely deformed scapula and humerus, fused carpal bones, imperfectly duplicated metacarpal bones, and polydactyly. The polydactyly was consisted of seven rows of digits with seven hooves, and a rudimentary hoof of dewclaw. 3. The ectopic limp was devoid of muscular tissue.

  • Carpal tunnel syndrome can be produced by abnormal mass effect due to trauma, ganglion cysts, various soft tissue tumors, musculotendinous variants, and aberrant vascular structures. Persistent median artery is one of the causes of the carpal tunnel syndrome. Thrombosed persistent median artery usually accompanies the anomaly of the median nerve and causes a sudden onset of severe pain and paresthesia. In contrast to previous literature, we report the rare case of gradual onset and mild symptom of a 53-year-old man with a thrombosed persistent median artery but without anomaly of the median nerve and abnormal finding of electrophysiologic study.

  • Accessory mitral valve tissue is a very rare congenital cardiac malformation and it is an uncommon cause of left Ventricular outflow tract obstruction(LVOTO). The pathogenetic mechanism of subaortic obstruction is thought to be systolic ballooning of the abnormal valve tissue into LVOT. We are reporting a case of an accessory mitral valve tissue that was associated with LVOTO, and this was completely relieved after trans-aortic surgical excision of the accessory tissue.

Safety Evaluation of IGEs Separated and Refined from Colostrum (초유로부터 분리.정제된 IGFs의 안전성 평가에 관한 연구)

  • This study was carried out to investigate safety evaluation of IGEs separated and refined from bovine milk and commercial recombinant human IGFs. In order to evaluate toxicity of these samples, acute toxicity test and short term toxicity test were investigated with IGF-I separated and refined from colostrum and commercial recombinant human IGF-I from R&D systems company. for acute toxicity test, we selected recombinant human IGF-I from R&D systems company and establish one control group and three dose-level groups(0, 10, 20 and 50 $\mu\textrm{g}$ per rat). We have intravenously injected tail of rats with selected sample once. After 20 days, pathological cellular tissue analyses were investigated with liver, kidney and spleen of 12 rats in all test groups. However, Morbid tissue and abnormal statistical results were not discovered in all cellular tissues. For short term toxicity test, we selected IGF-I separated and refined from colostrum and establish one control group and three dose-level groups(0, 5, 10 and 15 $\mu\textrm{g}$/day per rat). Rats were orally injected with selected sample once a day during two weeks. After short term toxicity test period, Pathological cellular tissue analyses were investigate with liver, kidney and spleen of 12 rats in all test groups. However, Morbid tissue and abnormal statistical results were not discovered in all cellular tissues. These results suggest that IGF-I treated groups show no significant toxicological findings with changes of body weight, food consumption, water consumption, and pathological findings compared with control groups.

Ex Vivo MR Diffusion Coefficient Measurement of Human Gastric Tissue (인체의 위 조직 시료에서 자기공명영상장치를 이용한 확산계수 측정에 대한 기초 연구)

  • The aim of this study is to investigate the feasibility of ex vivo MR diffusion tensor imaging technique in order to observe the diffusion-contrast characteristics of human gastric tissues. On normal and pathologic gastric tissues, which have been fixed in a polycarbonate plastic tube filled with 10% formalin solution, laboratory made 3D diffusion tensor Turbo FLASH pulse sequence was used to obtain high resolution MR images with voxel size of $0.5{\times}0.5{\times}0.5mm^3\;using\;64{\times}32{\times}32mm^3$ field of view in conjunction with an acquisition matrix of $128{\times}64{\times}64$. Diffusion weighted- gradient pulses were employed with b values of 0 and $600s/mm^2$ in 6 orientations. The sequence was implemented on a clinical 3.0-T MRI scanner(Siemens, Erlangen, Germany) with a home-made quadrature-typed birdcage Tx/Rx rf coil for small specimen. Diffusion tensor values in each pixel were calculated using linear algebra and singular value decomposition(SVD) algorithm. Apparent diffusion coefficient(ADC) and fractional anisotropy(FA) map were also obtained from diffusion tensor data to compare pixel intensities between normal and abnormal gastric tissues. The processing software was developed by authors using Visual C++(Microsoft, WA, U.S.A.) and mathematical/statistical library of GNUwin32(Free Software Foundation). This study shows that 3D diffusion tensor Turbo FLASH sequence is useful to resolve fine micro-structures of gastric tissue and both ADC and FA values in normal gastric tissue are higher than those in abnormal tissue. Authors expect that this study also represents another possibility of gastric carcinoma detection by visualizing diffusion characteristics of proton spins in the gastric tissues.

Multiple Symmetric Lipomatosis: Characteristics and Treatment in Korean Patients (다발성 대칭성 지방종증: 한국인에서의 특징 및 치료)

  • Purpose: Multiple symmetric lipomatosis (MSL) is a relatively rare disorder characterized by presence of multiple, symmetric, nonencapsulated fat masses in face, neck, shoulder and other areas. There has been only a few cases reported in Korea. The main purpose of this research is to examine the Korean patients to see what kinds of special characteristics occurred due to this disease and to decide the proper treatment.Methods: A total of 16 patients were evaluated retrospectively. 5 patients were treated at our hospital. The other patients were reviewed from literature. We analyzed the biological characters of patients, location of fat deposit, morphologic characters of patients, clinical evidence of neuropathy, associated metabolic disorders and treatment modality.Results: All cases were male patient. The mean age of onset was 47.43 years. All patients were moderate to heavy alcoholics. The most common location of fat deposition was posterior neck and abdomen. In neurologic exam of 9 patients, 5 patients showed muscle weakness, tremor, pain and autonomic nerve dysfunction. In metabolic studies of 9 patients, total cholesterol values were higher in 1 patient. A glucose tolerance test was abnormal in 1 patient. In treatment modality, 14 patients were treated with surgical resection, 1 patient was treated with liposuction and surgical excision, 1 patient was treated only with liposuction. Conclusion: To treat MSL patients successfully, we should concentrate not only on the removal of the fatty tissue but also on neurologic abnormities, metabolic disorders and associated diseases.

Two Cases of Choristoma in Base of the Tongue (설기저부에 발생한 골성 분리종 2예)

  • A choristoma is a tumor like mass of normal tissue in an abnormal location. Lingual osseous choristoma, previously known as osteoma of the tongue base, is rare entity. That is defined as a normal bone mass occuring in the soft tissue of either the skin or the mucosa of oral cavity. The cause of osseous choristoma of the tongue is still unknown, but only several theories have been suggested. Because of the rarity of this entity, our purpose is to add our case to the literature, and to discuss the nature of the disease.
