The Relationship among the degrees of pain coping strategies, Pain and Depression of patients with Rheumatoid Arthritis (류마티스 관절염 환자의 통증대처, 통증 및 우울정도의 관계)

  • This study has done for the purpose of investigation among the degrees of pain coping strategies, pain, and depression of patients with rheumatoid arthritis. The subjects of this study were 214 patients diagnosed with rheumatoid arthritis. The data were collected from september 4 to 16, 1995 by questionnaires. The data were analysed by the use of frequency, t-test, Pearson Correlation Coefficients, ANOVA, and Duncan test. The results Were as follows ; 1. The mean score of pain was 213.5(range : 4-390) and that of depression was 21.4 (range : 1-50). The mean score of passive pain coping strategies was 30.2(range : 11-48) and that of active pain coping strategies was 18.6(range : 7-33) 2. The passive pain coping degree showed a positive correlation with the pain degree (r=.475, p=.0001) and the active pain coping degree showed a inversive correlation with that(r= -.296, p=.0001). The passive pain coping degree showed a positive correlation with the depression degree (r=.425, p=.0001) and the active pain coping degree showed a inversive correlation with that(r=-.299, p=.0001). The pain degree showed a positive correlation with the depression degree(r=.374, p=.0001).

The Effect of 8 Constitution Acupuncture on degenerative arthritis of knee joint (퇴행성(退行性) 슬관절염(膝關節炎)에 체질침요법(體質鍼療法)이 미치는 영향)

  • Objective: This study was designed to compared the effect of general acupuncture on degenerative arthritis of knee joint with 8 constitution acupuncture by Visual Analogue Scale(VAS). Methods: VAS was used for this research, with the pain indicator of 10 and 0 which represents a pain of first visit and a status of no pain respectively. Two groups, consisting of 40 patients, was randomly allocated for this research. SPSS(Statistic Program for Social Science) 10.0 for window was used for a statistical analysis. Independent T-test and Mann Whitney U test was performed to evaluate the improvement of each group by the treatment number and to compare that of two groups. Results: As a result of the comparison analysis between two groups after 20th treatment, the improvement of subjective symptoms in 8 constitution acupuncture group was shown more effective than that in general acupuncture group, while, After 10th treatment, there was no significant difference between two groups in VAS. Conclusion: It is suggested that the effect of 8 constitution acupuncture should be significantly different from that of general acupuncture on degenerative arthritis of knee joint. Further study is needed to confirm the effectiveness of 8 constitution acupuncture.

Radiological Analysis of the Degenerative Arthritis of the Ankle (족관절 퇴행성 관절염의 방사선학적 분석)

  • Purpose: This study was performed to investigate the radiological characteristics of the degenerative arthritis of the ankle using the standing radiographs. Materials and Methods: From June 2001 to May 2005, 36 patients (56 ankles) who were treated for osteoarthritis of ankle were analysed. Angle of tibial shaft and tibial joint surface on AP view (TSA), angle of tibial joint surface on lateral view (TLS), tibial and medial malleolus angle (TMM) and talo-1st metatarsal angle were checked on standing radiograph. The patients with medial joint and total joint involvement were categorized into three stages according to the location of involvement. The degree of joint space narrowing was categorized into two groups. Results: There were no significant differences in TSA, TLS, TMM and talo-1st metatarsal angle with regard to the stage of arthritis. However, the difference between the less severe group and the severe group existed. Conclusion: Angular deformity was not correlated with stage, but correlated with severity. The deformity of distal tibial articular surface does not seem to be a cause of primary osteoartiritis, but rather a result from it.

Subtalar Arthrodesis using Cannulated Screws and Morselized Bone Graft (유관 나사와 분쇄 골이식을 사용한 거골하 관절 유합술)

  • Purpose: To analyze the clinical and radiological outcome of subtalar arthrodesis using cannulated screws and morselized bone graft. Materials and Methods: Twenty one patients with follow-up of more than 1 year after subtalar arthrodesis were included in this study. Mean age was 40.8 years, and mean follow-up duration was 38 months. Underlying diseases were 19 cases of posttraumatic arthritis (18 calcaneal fractures and 1 talar fracture) and 2 cases of tarsal coalition. Clinically AOFAS ankle-hindfoot score, operation time, complication and satisfaction of patients were analyzed. Radiologically time to union, arthritis of surrounding joints, preoperative and postoperative talar declination angle were analyzed. Results: AOFAS ankle-hindfoot score was improved from preoperative 33 points to postooperative 79 points. Eighteen patients (86%) were satisfied with the results. Mean operation time was 91 minutes. All cases were fixed with 1-2 cannulated screws and morselized bone graft. Mean time to radiologic union was 12.1 weeks. There was 1 case of delayed union. There was no significant perioperative changes in talar declination angles. Conclusion: Subtalar arthrodesis using cannulated screws and morselized bone graft seems to be relatively simple and effective treatment method for subtalar arthritis.

  • Rheumatoid arthritis is a severe autoimmune disorder, related to joints. It is associated with serious cartilage destruction. This causes disability and reduces the excellence of life. Numerous treatments are existed to combat this disease, however, they are not very efficient and possess severe side effects, higher doses, and frequent administration. Therefore, newer therapies are developed to overcome all these limitations. These include different monoclonal antibodies, immunoglobulins, small molecules used for immunotherapy and transgenes for gene therapy. One of the main goals of these new generation therapeutics is to address the underlying distressing biological processes by specifically targeting the causative agents with fewer systemic side effects and greater patient console. It is very fortuitous that loads of progressive investigations are going on in this field and many of them have entered into the successful clinical trial. But till date, a limited molecule has got FDA clearance and entered the market for treating this devastating disease. This review highlights the overview of conventional therapy and advancements in newer therapeutics including immunotherapy and gene therapy for rheumatoid arthritis. Further, different novel techniques for the delivery of these therapeutics of active and passive targeting are also described.

Studies on the Anti-inflammatory and Analgesic Activities of Ohjuksan (오적산(五積散)의 소염(消炎) 진통작용(鎭痛作用)에 관한 연구(硏究))

  • Ohjuksan has been used for the treatment of cold and pantalgia in traditional medicine. The anti-inflammatory activity of the aqueous extract from Ohjuksan (OJSE) was investigated utilizing acetic acid-induced edema and adjuvant arthritis in rats. The effects of this agent on acute toxicity and acetic acid-induced writhing syndrome in mice were also examined. OJSE did not showed acute toxicity at 2400mg/kg (p.o.) and 1200mg/kg(i.p.) body weight. It was also showed to have significant analgesic action on the writhing syndrome in mice induced by 0.7% acetic acid at 300,600mg/kg body weight. It showed anti-inflammatory activity in 5% acetic acid-induced edema and adjuvant arthritis with oral administration in rats and exhibited significant preventive effect on edema at 300 and 600mg/kg(p<0.01). In the method of adjuvant arthritis, orally administered for 19 days, it inhibited the hind paw edema in rats with 300 and 600 mg/kg body weight from 5 th day to 19th day. These results suggest that OJSE had analgesic and anti-inflammatory action.

The Therapeutic Effect of Natural Honeybee (Apis mellifera) Venom in Adjuvant-induced Arthritic Rat (관절염 유발 랫드에 대한 생봉독의 치료 효과)

  • This study was performed to assess that clinco-therapeutic effect of natural Italian honeybee (Apis mellifera) venom in adjuvant-induced arthritic rat. Ninety Sprague- Dawley rats of male were injected with complete Freund's adjuvant (CFA). Adjuvant arthritis was produced by a single subcutaneous injection of 1 mg Mycobacterium butyricum suspended in 0.1 ml paraffin oil into the right hindpaw. Righting reflex was uniformly lost and considered to be the point of arthritis development on day 14 after CFA injection. Experimental groups were divided into three groups. When arthritis was developed in the rat hind-paw, tested groups were administrated with prednisolone (10 mg/kg, p.o) and honeybee venom (one bee, s.c) at an interval of two days. Control group was subcutaneously injected with 0.1 ml of physiological saline solution in the rat at an interval of two days. Clinical findings, hematological values and histopathological findings were observed during or after the drugs administration. In tested groups, the development of inflammatory edema and polyarthritis on day 14 after treatment was suppressed. No significant differences of hindpaw edema volume and lameness score between prednisolone and honeybee venom groups were observed during or after therapeutic drugs treatment. WBC counts of prednisolone and honeybee venom treatment groups as compared with the control group were getting remarkably decreased during or after the therapeutic drugs administration(p<0.01). Erosions of articular cartilage and inflammatory cell infiltrations during or after the therapeutic drugs treatment was effectively suppressed in natural honey venom.

Inhibitory Effect of Cudraniae Tricuspidatae Radix on Collagen-induced Arthritis in Mice (천파석(穿破石)이 콜라겐유도 생쥐관절염에 미치는 억제효과)

  • Objectives : The present study was conducted to examine the effect of Cudraniae Tricuspidatae Radix(CTR), which is the radix part of Cudrania tricuspidata Bureau, on cytokine secretion from the joint cells and spleen cells of mice with arthritis induced by collagen and verify its efficacy against rheumatoid arthritis. Methods : Three kinds of extract were prepared from CTR through extraction with hot-water and ethanol. The levels of cytokine secreted from the cells were measured, after the mice knee joint cells were cultured with each extract. Results : The three kinds of extracts from CTR decreased the growth rate and levels of inflammatory cytokines such as IL-$1{\beta}$, IL-6, and TNF-${\alpha}$ from knee joint cells of mice. All of the organic-soluble fraction, such as hexane, ethyl ether, ethyl acetate, butanol, and residual water-soluble fraction decreased the levels of IL-$1{\beta}$, TNF-${\alpha}$, IL-6, and IL-10. Conclusion : These results suggest that CTR had some effects on rheumatitis, with fat and water-solubles in organic solvent considered as the effective element.

Effects of Herba ChelidoniiㆍClematis Florida ThunbㆍChaenomelis Fructus Complex Herbal Acupuncture on Anti-Inflamation and Liver in Rats with Arthritis induced Freund's Complete Adjuvant (백굴채ㆍ목과ㆍ위영선 혼합 약침액이 관절염 백선의 염증과 간에 미치는 영향)

  • Objective : In order to study the effects of Herba ChelidoniiㆍClematis Florida ThunbㆍChaenomelis Fructus complex herbal acupuncture on anti-inflammation and liver in rats with arthritis induced Freund's complete adjuvant. Method : We performed several experimental items : that is edema value, numbers of WBC. total protein, total bilirrubin, SGOT, SGPT. Result: The change in the right plantar edema shows that Control group was reduced by 0% but, ST group ,Sample A group and Sample B group were reduced by 287.2%, 343.0%, 272.5% respectively. In the WBC count, ST group, Sample A group, Sample B group showed a decrease with statistical significance as compared with control group. In the total protein, Sample B group showed a decrease with statistical significance as compared with control group. In the total bilirrubin, SGOT, SGPT, None showed noxious variation with statistical significance as compared with control group. Conclusion : As a result of this experiment, it is concluded that Herba ChelidoniiㆍClematis Florida ThunbㆍChaenomelis Fructus complex herbal acupuncture showed that therapeutic effect of anti-inflammation in adjuvant arthritis and non-toxic effect of liver.

The Effects of a Self-Help Empowerment Strategy Program on the Empowerment and Health Status of Rheumatoid Arthritis Patients (능력증강전략을 활용한 자조관리과정이 류마티스관절염 환자의 능력증강과 신체적, 심리적 기능에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of a six week self-help empowerment strategy program on the empowerment status, pain intensity, physical function, joint flexibility and depression status in rheumatoid arthritis patients. Method: The subjects of the study consisted of a convenience sample thirteen rheumatoid arthritis patients. Data was collected from July to November, 2001. All patient received a self-help empowerment strategy program, once a week for six weeks. Result: After receiving the self-help empowerment strategy program, the empowerment status increased significantly, the pain intensity and the depression score decreased significantly and the level of physical function significantly improved but the flexibilities of shoulders, knees and ankles were not significantly changed. Conclusion: Further studies need to be done to determine the feasibility of implementing empowerment strategy and its possibility as an effective intervention program for decreasing pain intensity and depression score and improving the physical function.
