The Research about the Class Structure of the Wooden Chamber Tombs in Gyeongju Region (경주지역 목곽묘의 위계구조 검토)

  • The study is for tracing the mutational process, considering the transformational process of wooden chamber tombs in Gyoungju region during 2~6 century. In addition, class features and the meaning of tombs were studied. First of all, the wooden chamber tombs were classified into 10 different styles(1A~IVC), based on the filling materials, the ways of structure, and the structure of burial chamber. There were only one or two tombs that belongs to the highest classes 'Ga'~'Da' in each period, which has the numeral features. Furthermore, the styles of the central wooden chamber tombs according to the period and the region have distinctive features and have changed so far. It shows that there are the rank among the tombs in the regions. In other words, regarding the ranks of the class 'Ga'~'Cha', there were strict rules and the limitations on the size of the tombs and the number and the quality of the burial relics. It means that the inner-outer size or the structure of the tombs and the number, quality of the burial goods depended on the buried person's birth or social classes, or achievements. Based on Shila social classes, the class 'Ga' tombs were for the highest classes when considered the size, burial relics, and the quality. The class 'Na' and 'Da' tombs were for the higher classes, 'Ma' for middle classes, 'Ba', 'Sa' for lower classes. In addition, 'Ah', 'Ja' tombs were for higher commoner, 'Cha' for lower commoner. Thus, class 'Ga'~'Sa' is the leading class of Shila.

Concentrations and Risk Assessment of Heavy Metal in Shellfish and Crustacean Collected from Vladivostok Area in Russia (러시아 블라디보스토크산 패류 및 갑각류의 중금속 함량 및 위해도 평가)

  • Exposure to heavy metals through the consumption of contaminated seafood poses a health risk to humans. In Korea, seafood imports are increasing with consumption, with the largest increase in imported seafood coming from Russia. Peter the Great Bay and the Razdolnaya River are both major fisheries and protected areas under the Northwest Pacific Action Plan located near Vladivostok, Russia. This study analyzed heavy metal [cadmium (Cd), lead (Pb), total mercury (tHg), and total (tAs), and inorganic (iAs) arsenic] concentrations in shellfish and crustaceans collected from these areas. Except for iAs, the major toxic heavy metal concentrations of the samples met the national standards (Cd, 0.024-0.982 and 0.003-0.379 mg/kg; Pb, 0.021-1.533 and 0.002 mg/kg; tHg, 0.006-0.015 and 0.036-0.097 mg/kg). The tAs concentrations of three samples exceeded the Russian standard (5 mg/kg, wet weight), whereas the iAs concentrations were extremely low (ND-0.033 mg/kg). Compared with the provisional tolerable weekly intake (PTWI) of the Ministry of Food and Drug Safety and Joint FAO/WHO Expert Committee on Food Additives, the percentages of PTWI of Cd, Pb, and tHg were 0.239%, 0.001-0.049%, and 0.013-0.302%, respectively. These findings reveal that there is no potential health risk by heavy metals through the consumption of Russian seafood obtained in the surveyed areas.

Determinations of Toltrazuril and Toltrazuril Sulfone Levels in Olive Flounder Paralichthys olivaceus Samples Using Liquid Chromatography-Electrospray Ionization Tandem Mass Spectrometry (LC-MS/MS를 이용한 넙치(Paralichthys olivaceus)시료의 톨트라주릴 및 톨트라주릴 설폰 분석)

  • Several studies investigating the prevention and treatment of external parasites in farmed olive flounder Paralichthys olivaceus have found that the anticoccidial agent toltrazuril sulfone is an effective antiparasitic. Prior to undertaking a full-scale study, we developed analytical methods to detect the levels of toltrazuril and toltrazuril sulfone in farmed flounder samples using liquid chromatography-electrospray ionization tandem mass spectrometry (LC-MS/MS). This analysis showed that LC-MS/MS changed the mobile phase and collision energy of toltrazuril and toltrazuril sulfone. This was validated using established conditions. Sample pre-treatment for this process involved extraction with dichloromethane and purification by liquid-liquid extraction in formic acid, acetonitrile, and h-hexane, followed by determination of all compounds by LC-MS/MS. Separation was achieved within 10 min by gradient elution using a Capcell Pak C18 ($3.0{\mu}m$, $100{\times}2.0mm$) analytical column (Shiseido UG 120V) with a mixture of 0.1% (v/v) formic acid and acetonitrile. Multiple reaction monitoring was used for selective detection of toltrazuril and toltrazuril sulfone. This method yields satisfactory results for linearity, precision, and limits of quantification. Therefore, the method established in our study will serve as a basis for further research on parasite control by toltrazuril and toltrazuril sulfone.

Difficulties in End-of-Life Care and Educational Needs of Intensive Care Unit Nurses: A Mixed Methods Study (중환자실 간호사의 임종간호 어려움과 임종간호 교육요구 조사: 혼합연구방법)

  • Purpose: This study aimed to identify the difficulties with end-of-life care (EOLC) experienced by intensive care unit (ICU) nurses and to investigate their educational needs for EOLC. Methods: Mixed methods were used to survey ICU nurses at a university hospital. Quantitative data (N=106) were collected through a questionnaire and analyzed using an independent samples t-test, ANOVA, Mann-Whitney U test and $Scheff{\acute{e}}$ test. Qualitative data (N=19) were collected through focus group interviews and analyzed through qualitative content analysis. Results: The mean score on the difficulty of EOLC was 3.41 out of 5. The education needs derived from the qualitative analysis was categorized into four themes: 1) guidelines on professional EOLC, 2) spiritual care, 3) a program to take care of feelings of patients, families and nurses, and 4) activities to think about death. Conclusion: This study confirmed that ICU nurses were experiencing an extreme difficulty in providing EOLC. In addition, a qualitative analysis confirmed that they needed an EOL nursing program. To mitigate the difficulties experienced by nurses involved in EOLC, there is an urgent need to develop an education program for EOLC tailored to nurses' needs.

Secondary Fermented Extract of Chaga-Cheonggukjang Attenuates the Effects of Obesity and Suppresses Inflammatory Response in the Liver and Spleen of High-Fat Diet-Induced Obese Mice

  • Cheonggukjang and chaga mushrooms have numerous health benefits, and have been used in alternative medicine. Therefore, a powder mixture of 98: Cheonggukjang and 2: Chaga extracts was fermented with Lactobacillus acidophilus KCTC3925 (FCC) and its anti-obesity effects in high-fat diet (HFD)-induced obese mice were determined. Five-week-old male ICR mice were fed a normal diet or HFD in the presence or absence of 3% and 5% FCC by weight (n = 10 per group). After 12 weeks, the mice were sacrificed, and the serum and tissue samples were collected for analysis. Body weight and epididymal fat pad weight were significantly lowered in the 3% and 5% FCC groups compared with those in the HFD control group (p < 0.01). FCC supplementation suppressed serum triglyceride and increased serum HDL-C levels (p < 0.01). Serum GOT, GPT, and leptin levels, hepatic COX-2 mRNA expression, and splenic COX-2 and IL-4 mRNA expression were significantly higher in the HFD groups than in the control group (p > 0.05); however, except for splenic IL-4 levels, the increases were significantly attenuated by FCC supplementation. Expression of ICAM-1, an aortic inflammatory marker, was significantly increased in the HFD group; this effect was suppressed in the 3% FCC group (p < 0.01) but not in the 5% FCC group. FCC suppressed the body weight and epididymal fat pad weight gain, as well as inflammatory responses in the liver and spleen of HFD-fed mice. Thus, FCC supplementation will be beneficial for the treatment of obesity-related effects.

Correlation between Surface Electromyography and Conventional Electromyography in Facial Nerve Palsy (안면마비 환자에서 표면 근전도 검사와 통상적 근전도 검사간 상관관계)

  • Objective: To assess the correlation between surface electromyography (SEMG) and conventional EMG in patients with facial nerve palsy. Additionally, compare the discomfort and the time required by the patients in two methods. Method: 36 patients with facial palsy were given nerve conduction studies (NCS) via conventional EMG. Then, the peak root mean square (RMS) values were obtained from the SEMG. We also recorded visual analogue scale (VAS), House-Brackmann scale, and the time required for the examination. Results: Pearson's correlation coefficient between the amplitude loss ratio of the RMS values obtained by SEMG compared to the unaffected side (RSEMG) and the amplitude loss ratio of CMAP amplitudes compared to the unaffected side (RCMAP) was 0.567 at the frontalis, 0.456 at the orbicularis oculi, 0.393 at the nasalis, and 0.437 at the orbicularis oris. An increase in RSEMG is positively correlated with an increase in RCMAP. The mean VAS score with conventional EMG was $3.55{\pm}1.42$, whereas that experienced when using SEMG was $0.11{\pm}0.52$ and the mean time required for conventional EMG was $610{\pm}103.84$ seconds, while that required for SEMG was $420{\pm}86.32$ seconds. Conclusion: This study demonstrated a significant positive correlation between facial muscle activities as measured by SEMG and conventional EMG in patients with facial nerve palsy. SEMG has the benefits of being more comfortable and faster when diagnosing facial palsy.

Effects of the aquatic exercise program on diabetes self efficacy and self care behaviors in diabetic patients in community (수중운동 프로그램이 지역사회 당뇨병 환자의 자기효능감과 자가간호행위에 미치는 효과)

  • This study is the quasi-experimental research of nonequivalent groups and is designed to investigate the effects of an aquatic exercise program with diabetes self management education on the self care efficacy, self care behaviors in diabetic patients in community. The subjects of this study were 26 patients in the experimental group who participated in the aquatic exercise program and 25 patients in the control group who did not paricipated in. And they were registered at the W Health Clinic in D city. The aquatic exercise program was conducted twice a week for 80 minutes, and consisted of a 30 minutes diabetes self management education and a 50 minutes aquatic exercise. After this program, there were significant differences in FBS(p=.047), HbA1c(p<.001), diabetes self efficacy(p<.001), and self care behaviors(p<.001). In order to reduce the prevalence of diabetes and prevent complications in the community, it is believed that the constant operation of aquatic exercise programs including self management education is necessary.

Characterization of Green Mold Contamination caused by Penicillium brevicompactum in Hypsizygus marmoreus (느티만가닥버섯에서 Penicillium brevicompactum에 의해 유발된 곰팡이 오염 특성)

  • Beech mushroom (Hypsizygus marmoreus) is one of the most popular mushrooms in Japan, China, and Korea because of its delicious taste, nutritious value, and unique crunchy texture. In 2018-2019, unusual signs on the mushroom were observed in mushroom farms in Gyeongnam Province. The main signs were the inhibition of mycelial growth and primordial formation on the surface of mushroom media. When green mold caused contamination at early stage, the rate of pinhead formation by the mushroom greatly decreased to 46.5-71.5%. Conidia of the causal green mold were ellipsoidal, dark green, and measured 2.0-3.1㎛ long. The optimum temperature for mycelial growth of the green mold was 25℃. The phylogenetic tree obtained from the internal transcribed spacer (ITS) rDNA sequences showed that the isolated green mold corresponded to Penicillium brevicompactum (100.0%). This is the first report of green mold contamination caused by P. brevicompactum on H. marmoreus.

The Determinants of Public Attitudes toward Welfare Spending in Korea: Focusing on Welfare Perceptions and the Quality of Government (복지태도의 결정요인 분석: 복지수준과 '정부의 질'에 대한 인식의 효과를 중심으로)

  • The purpose of this paper is to examine the determinants of public attitudes toward welfare spending in relation with the perception of welfare level. Data from the national survey of 'the SSK Good Government Research Unit' are used to perform a logistic regression analysis of the association between welfare attitudes and government perceptions, political orientations, and socio-economic backgrounds. The most prominent finding is that the determinants of welfare attitudes differ markedly between the two different groups of welfare perception. The empirical results say that persons living in a corrupt government tend to prefer welfare spendings despite having low level of welfare, and persons living in a unjust government tend to prefer welfare expansion because welfare level must be low. Persons living in a government of great capacity tend to have positive attitudes to welfare spending despite having a high level of welfare, whereas persons living in a government of less capacity tend to have negative attitudes to welfare expansion because the level of welfare is too high beyond government capacity. This distinction between the two perceptions of welfare level provides a systemic understanding of public attitudes toward welfare spending.

Factors Affecting Postoperative Complications and Outcomes of Cervical Spondylotic Myelopathy with Cerebral Palsy : A Retrospective Analysis

  • Objective : Cervical surgery in patients with cervical spondylotic myelopathy (CSM) and cerebral palsy (CP) is challenging owing to the complexities of the deformity. We assessed factors affecting postoperative complications and outcomes after CSM surgery in patients with CP. Methods : Thirty-five consecutive patients with CP and CSM who underwent cervical operations between January 2006 and January 2014 were matched to 35 non-cerebral palsy (NCP) control patients. Postoperative complications and radiologic outcomes were compared between the groups. In the CP group, the Japanese Orthopaedic Association score; Oswestry neck disability index; modified Barthel index; and values for the grip and pinch, Box and Block, and Jebsen-Taylor hand function tests were obtained pre- and postoperatively and compared between those with and without postoperative complications. Results : Sixteen patients (16/35%) in the CP group and seven (7/35%) in the NCP group (p=0.021) had postoperative complications. Adjacent segment degeneration (p=0.021), postoperative motor weakness (p=0.037), and revisions (p=0.003) were significantly more frequent in the CP group than in the NCP group; however, instrument-related complications were not significantly higher in the CP group (7/35 vs. 5/35, p=0.280). The number of preoperative fixed cervical deformities were significantly higher in CP with postoperative complications (5/16 vs. 1/19, p=0.037). In the CP group, clinical outcomes were almost similar between those with and without postoperative complications. Conclusion : The occurrence of complications during the follow-up period was high in patients with CP. However, postoperative complications did not significantly affect clinical outcomes.
