3D 동영상획득을 위한 Cone-Beam 형 X-Ray CT 시스템 구현에 관한 연구 (The Study on the Implementation of the X-Ray CT System Using the Cone-Beam for the 3D Dynamic Image Acquisition)

  • 본 논문에서는 3차원 동영상을 구현하기 위하여 인체에 유해한 방사능 피폭량이 부챗살 형태의 Fan Beam 보다 상대적으로 적은 원추형 Cone Beam CT 시스템을 설계하였다. 설계된 시스템은 X-ray가 조사된 각도에서 획득한 데이터를 영상처리부로 전송하고 영상을 구성하는 계산 속도를 빠르게 하기 위하여 초월함수가 보다 적은 3단계 회전 행렬을 이용하여 3차원 영상을 구현하는 회전 기반법을 사용하였다. 본 연구에서는 3차원 영상을 구성하는 시간단축을 통해 초당 3~5장의 영상을 얻음으로써 3차원 동영상을 실시간으로 구현하였다.

Cone beam CT와 일반 CT의 흡수선량 및 유효선량 비교평가 (Comparison of cone beam CT and conventional CT in absorbed and effective dose)

  • Purpose: This study provides comparative measurements of absorbed and effective doses for newly developed cone beam computed tomography (CT) in comparison with these doses for conventional CT. Materials and Methods: Thermoluminescent dosimeter rods (TLD rod: GR-200, Thermo Fisher Scientific Inc., Waltham, MA, USA) were placed at 25 sites throughout the layers of Male ART Head and Neck Phantom (Radiology Support Devices Inc., Long Beach, USA) for dosimetry. Implagraphy, DCT Pro (Vatech Co., Hwasung, Korea) units, SCT-6800TXL (Shimadzu Corp., Kyoto, Japan), and Crane x 3+(Soredex Orion Corp., Helsinki, Finland) were used for radiation exposures. Absorption doses were measured with Harshaw 3500TLD reader (Thermo Fisher Scientific Inc., Waltham, MA, USA). Radiation weighted doses and effective doses were measured and calculated by 2005 ICRP tissue weighting factors. Results: Absorbed doses in Rt. submandibular gland were 110.57 mGy for SCT 6800TXL (Implant), 24.56 mGy for SCT 6800TXL (3D), 22.39 mGy for Implagraphy 3, 7.19 mGy for DCT Pro, 5.96 mGy for Implagraphy 1, 0.70 mGy for Cranex 3+. Effective doses $(E_{2005draft)$ were 2.551 mSv for SCT 6800TXL (Implant), 1.272 mSv for SCT 6800TXL (3D), 0.598 mSv for Implagraphy 3, 0.428 mSv for DCT Pro and 0.146 mSv for Implagraphy 1. These are 108.6, 54.1, 25.5, 18.2 and 6.2 times greater than panoramic examination (Cranex 3+) doses (0.023mSv). Conclusion: Cone beam CT machines recently developed in Korea, showed lower effective doses than conventional CT. Cone beam CT provides a lower dose and cost alternative to conventional CT, promising to revolutionize the practice of oral and maxillofacial radiology.

치과용 콘빔 CT를 이용한 상악 정중과잉치의 3차원 분석 (Three dimensional evaluation of impacted mesiodens using dental cone beam CT)

  • Purpose : This study was performed to analyze the position, pattern of impacted mesiodens, and their relationship to the adjacent teeth using Dental cone-beam CT. Materials and Methods : Sixty-two dental cone-beam CT images with 81 impacted mesiodenses were selected from about 2,298 cone-beam CT images at Chonnam National University Dental Hospital from June 2006 to March 2009. The position, pattern, shape of impacted mesiodenses and their complications were analyzed in cone-beam CT including 3D images. Results : The sex ratio (M : F) was 2.9 : 1. Most of the mesiodenses (87.7%) were located at palatal side to the incisors. 79% of the mesiodenses were conical in shape. 60.5% of the mesiodenses were inverted, 21% normal erupting direction, and 18.5% transverse direction. The complications due to the presence of mesiodenses were none in 43.5%, diastema in 19.4%, tooth displacement in 17.7%, delayed eruption or impaction in 12.9%, tooth rotation in 4.8%, and dentigerous cyst in 1.7%. Conclusions : Dental cone-beam CT images with 3D provided 3-dimensional perception of mesiodens to the neighboring teeth. This results would be helpful for management of the impacted mesiodens.

3D CT 동영상 구성을 위한 속도 개선 알고리즘 (The Algorithm Improved the Speed for the 3-Dimensional CT Video Composition)

  • 본 논문에서는 인체에 유해한 방사능 피폭량이 부챗살 형태의 Fan Beam 보다 상대적으로 적은 원추형 Cone Beam CT 시스템을 사용하여 3차원 영상을 빠르게 구성하기 위한 회전 기반법 알고리즘을 제안하였다. 그리고 계산 속도를 빠르게 하기 위하여 초월함수가 더 적은 3단계 회전 행렬을 이용하여 3차원 영상을 구현하였다. 또한 본 연구에서 구성한 영상을 일반적으로 사용되는 라돈 변환 알고리즘으로 구현된 3차원 영상과 비교하여 영상의 질은 평균적으로 단면 영상 당 10% 정도의 오차를 보이며 미약하게 저하되었으나 영상 구성 속도 면에서는 35배 개선됨을 보였다. 이는 대략 초당 $4{\sim}5$ 프레임을 얻을 수 있는 수치이며 간단한 구조를 가진 피사체라면 보다 많은 프레임을 얻을 수 있어 3차원 동영상을 실시간으로 구현할 수 있는 가능성을 보였다.

Cone-beam CT와 multi-detector CT영상에서 측정된 CT number에 대한 비교연구 (Comparison of CT numbers between cone-beam CT and multi-detector CT)

  • Purpose : To compare the CT numbers on 3 cone-beam CT (CBCT) images with those on multi-detector CT (MDCT) image using CT phantom and to develop linear regressive equations using CT numbers to material density for all the CT scanner each. Materials and Methods : Mini CT phantom comprised of five 1 inch thick cylindrical models with 1.125 inches diameter of materials with different densities (polyethylene, polystyrene, plastic water, nylon and acrylic) was used. It was scanned in 3 CBCTs (i-CAT, Alphard VEGA, Implagraphy SC) and 1 MDCT (Somatom Emotion). The images were saved as DICOM format and CT numbers were measured using OnDemand 3D. CT numbers obtained from CBCTs and MDCT images were compared and linear regression analysis was performed for the density, $\rho$ ($g/cm^3$), as the dependent variable in terms of the CT numbers obtained from CBCTs and MDCT images. Results : CT numbers on i-CAT and Implagraphy CBCT images were smaller than those on Somatom Emotion MDCT image (p<0.05). Linear relationship on a range of materials used for this study were $\rho$=0.001H+1.07 with $R^2$ value of 0.999 for Somatom Emotion, $\rho$=0.002H+1.09 with $R^2$ value of 0.991 for Alphard VEGA, $\rho$=0.001H+1.43 with $R^2$ value of 0.980 for i-CAT and $\rho$=0.001H+1.30 with $R^2$ value of 0.975 for Implagraphy. Conclusion: CT numbers on i-CAT and Implagraphy CBCT images were not same as those on Somatom Emotion MDCT image. The linear regressive equations to determine the density from the CT numbers with very high correlation coefficient were obtained on three CBCT and MDCT scan.

A method for mandibular dental arch superimposition using 3D cone beam CT and orthodontic 3D digital model

  • Objective: The purpose of this study was to develop superimposition method on the lower arch using 3-dimensional (3D) cone beam computed tomography (CBCT) images and orthodontic 3D digital modeling. Methods: Integrated 3D CBCT images were acquired by substituting the dental portion of 3D CBCT images with precise dental images of an orthodontic 3D digital model. Images were acquired before and after treatment. For the superimposition, 2 superimposition methods were designed. Surface superimposition was based on the basal bone structure of the mandible by surface-to-surface matching (best-fit method). Plane superimposition was based on anatomical structures (mental and lingual foramen). For the evaluation, 10 landmarks including teeth and anatomic structures were assigned, and 30 times of superimpositions and measurements were performed to determine the more reproducible and reliable method. Results: All landmarks demonstrated that the surface superimposition method produced relatively more consistent coordinate values. The mean distances of measured landmarks values from the means were statistically significantly lower with the surface superimpositions method. Conclusions: Between the 2 superimposition methods designed for the evaluation of 3D changes in the lower arch, surface superimposition was the simpler, more reproducible, reliable method.

한국인에서 3차원 conebeam CT를 이용한 부이공의 해부학적인 평가 (ANATOMICAL ASSESSMENT OF ACCESSORY MENTAL FORAMEN USING 3D CONE BEAM COMPUTED TOMOGRAPHY IN KOREAN)

  • Purpose: The mental foramen (MF) is an important anatomical structure during local anesthesia and surgical procedures in terms of achieving effective mental nerve blocks and avoiding injuries to the neurovascular bundles. Thus, understanding the anatomic features of the mandibular canal and accessory mental foramen in Korean could contribute to the surgical anatomic assessment. This study was to elucidate frequency, position and course of AMF (accessory mental foramen) in Korean using 3D cone beam computed tomography. Materials and Methods: The CBCT (Conbeam computed tomography) DICOM data (Alphard, Asahi, Japan) from 540 patients in korean were analyzed. We investigated images of 3D CBCT using Ondemand (CyberMed, Korea) software program on the incidence and anatomical characteristics of accessory foramen. Results: The accessory mental foramina were found in 17 patients. Accessory mental foramina exist predominantly in the apical area of the second premolar and posteroinferior area of the mental foramen. The accessory branches of the mandibular canal showed common characteristics in the course of gently sloping posterosuperior direction in the buccal surface area. The size of most AMF was obviously smaller than that of MF. Conclusion: We could identify frequency, position and course of AMF (accessory mental foramen) by the anatomical study of the accessory mental foramen using 3D cone beam CT in Korean.

콘빔CT 촬영 시 mAs의 변화에 따른 피부선량과 영상 품질에 관한 평가 (Evaluation of Skin Dose and Image Quality on Cone Beam Computed Tomography)

  • 목 적: 선형가속기에 부착된 온보드영상장치(On-Board Imager)를 이용한 콘빔CT (Cone Beam Computerized Tomography)는 환자의 셋업 오차 확인 및 보정, 장기 및 표적의 움직임 확인이 용이한 장점이 있는 영상유도방사선 치료 장비이다. 하지만 촬영 시 받게 되는 imaging dose는 2차 암 발생위험의 원인이 된다. 이에 본 저자는 촬영조건(mAs)을 변화시킨 4가지 촬영 mode로 피부선량과 영상 품질을 비교 평가하여 적정한 촬영 mode를 제시하고자 한다. 대상 및 방법: 인체 모형 팬톰(RANDO phantom)을 사용하여 열형광선량계(TLD-100, Harshaw)를 두부, 흉부, 복부로 나누어 각 부위별로 8개씩 위치시킨 후 4가지의 촬영 mode (A: 125 kvp 80 mA 25 ms, B: 125 kvp 40 mA, 25 ms, C: 125 kvp 80 mA 10 ms, D: 125 kvp 40 mA, 10 ms)로 피부선량(skin dose)을 각각 3회씩 측정한 후 그 평균값을 얻어 평가하였고 catphan 504 phantom을 이용하여 장비 제조사의 영상 품질 정도관리 protocol에 따라서 각 촬영 mode 별 영상품질(image quality)을 비교 분석하였다. 결 과: 피부선량을 측정한 결과 두부에서는 A mode: 8.96 cGy, B mode: 4.59 cGy, C mode: 3.46 cGy, D mode: 1.76 cGy였고, 흉부는 A mode: 9.42 cGy, B mode: 4.58 cGy, C mode: 3.65 cGy, D mode: 1.85 cGy가 복부에서는 A mode: 9.97 cGy, B mode: 5.12 cGy, C mode: 4.03 cGy, D mode: 2.21 cGy의 값으로 측정이 되었다. 이는 A mode를 기준으로 약 B mode는 50%, C mode 60%, D mode는 80%의 선량 감소를 나타내었다. 영상품질 평가 항목인 HU reproducibility, Low contrast resolution, Spatial resolution (high contrast resolution), HU uniformity를 분석한 결과 모든 촬영mode에서 장비제조사에서 제시하는 tolerance 이내의 값으로 평가되었다. 결 론: 콘빔CT에 있어서 좋은 영상품질을 유지하면서 imaging dose를 줄이는 것은 중요하다. 이에 본 실험결과를 바탕으로 하여 soft tissue가 관심영역일 경우는 A mode로 두부 촬영 시 bone이 관심영역일 경우 D mode가 일반적인 경우에는 B, C mode가 적용 가능하리라 생각된다. 또한 콘빔CT촬영으로 인해 증가되는 2차 암 발생위험은 낮은 mAs의 촬영조건을 선택함으로써 줄일 수 있을 것이다.

오픈-소스 소프트웨어를 이용한 콘빔 전산화단층영상의 중첩 및 시각화 (Cone-beam CT superimposition and visualization using open-source softwares)

  • ITK-SNAP (http://www.itksnap.org) and 3D Slicer (CMFreg extension module; www.slicer.org) are open-source softwares which can be used for superimposition of cone-beam CT images. For superimposition, segmentation of bone is done with ITK-SNAP, and then voxel based superimposition of CBCT images can be performed using 3D Slicer. 3D Slicer has various visualization modules which are not provided in common commercial programs. 'Models' module is used to visualize two overlapping three-dimensional images, and this allows various visualizations by changing view mode and color of the model. In addition, differences between two CBCT images can be represented in a color map using 'ShapePopulationViewer' module. This report introduces how to superimpose and visualize CBCT images using ITK-SNAP and 3D Slicer, and the usefulness and limitations of both softwares will be discussed in comparison with commercial softwares.

Utility of the computed tomography indices on cone beam computed tomography images in the diagnosis of osteoporosis in women

  • Purpose : This study evaluated the potential use of the computed tomography indices (CTI) on cone beam CT (CBCT) images for an assessment of the bone mineral density (BMD) in postmenopausal osteoporotic women. Materials and Methods : Twenty-one postmenopausal osteoporotic women and 21 postmenopausal healthy women were enrolled as the subjects. The BMD of the lumbar vertebrae and femur were calculated by dual energy X-ray absorptiometry (DXA) using a DXA scanner. The CBCT images were obtained from the unilateral mental foramen region using a PSR-$9000N^{TM}$ Dental CT system. The axial, sagittal, and coronal images were reconstructed from the block images using $OnDemend3D^{TM}$. The new term "CTI" on CBCT images was proposed. The relationship between the CT measurements and BMDs were assessed and the intra-observer agreement was determined. Results : There were significant differences between the normal and osteoporotic groups in the computed tomography mandibular index superior (CTI(S)), computed tomography mandibular index inferior (CTI(I)), and computed tomography cortical index (CTCI). On the other hand, there was no difference between the groups in the computed tomography mental index (CTMI: inferior cortical width). Conclusion : CTI(S), CTI(I), and CTCI on the CBCT images can be used to assess the osteoporotic women.
