  • Purpose : The prevalence of hepatitis A virus (HAV) in a certain community reflects that community's living standards and hygienic conditions. And the pattern of HAV infection differs over time and geography. Recently, a shift in prevalence has been observed in cases from chilhood to adulthood. We studied the HAV antibody prevalence in the general population in Busan. Methods : From October 2004 to March 2005, total 472 subjects were tested for HAV antibodies. All samples were collected from patients in Maryknol Hospital. Results : The overall seropositive rate was 22.8% (108/472). The seropositive rates were 1.7% in subjects aged 2-5 years, 1.7% in 6-10 years, 0% in 11-20 years, 40.5% in 21-30 years, 82.1% in 31-40 years, 94.7% in 41-50 years, and 100% in subjects aged over 50 years. There was no significant gap between gender groups. Conclusion : As the socioeconomic conditions in Korea have improved, the HAV seropositive rate in school-aged children has dramatically decreased in the last 20 years. But, the seropositive rate of HAV didn't differ according to gender. The seropositive rate of HAV in the pediatric group was very low, which suggests the increasing possibility of clinical HAV infection in adults in the near future. Therefore, we should actively prevent the spread of hepatits A virus. In order to do that, we need to reorganize our lifestyle and personel hygiene and carry out active and passive immunization to high risk groups.

  • In order to assess the physical growth pattern of Korean Youth, the authors measured the body height, body weight, chestgirth and sitting height of 40967 persons (24832 males and 16135 females) from primary schools, middle and high schools and colleges of metropolitan (urban) and rural areas, and calculated the mene, standard deviation, standard error and coefficient variance of the body weight chestgirth, sitting height and various pertinent index by body height to demonstrate the standard physical growth and development by body height of sex and age. The following conclusions were obtained. 1. Physical growth and development: Rapid growth of physical growth in terms of body height has been observed among males in the age $7{\sim}15$ and among female $7{\sim}13$. Growth in terms of body height turned out to be slower among students of higher age by both sexes. The age of cross over between to sexes is between 10 to 13 years where upon girls out grows boys. Maximum annual growth were upon girls out grows boys. Maximum annual growth were both of 6.16cm from 8 to 9 years old and 12 to 13 years old for boys and 7.2cm from 8 to 9 and 6.1cm from 9 to 10 for girls. This indicates that girls enter a rapidly growing stage 2 years earlier than boys. Meanwhile, prominent improvement in body height of national students over period of ten year was noticed. 2. The distribution status of body height by age: The distribution status of body height by age were as follows; 7 year of age: boys-30cm range of body height from 104.0cm to 133.9cm, girls-27cm from 104.0cm to 130.9cm 8 year of age: boys-30cm from 116.0 to 145.9cm girls-33cm from 113.0 to 145.9cm 9 year of age: boys-30cm from 116.0 to 145.9cm girls-33cm from 113.0 to 145.9cm 10 year of age: boys-39cm from 116.0 to 154.9cm girls-39cm from 119.0 to 157.9cm 11 year of age: boys-45cm from 119.0 to 163.9cm girls-39cm from 122.0 to 160.9cm 12 year of age: boys-45cm from 125.0 to 169.9cm girls-42cm from 125.0 to 166.9cm 13 year of age: boys-45cm from 128.0 to 172.9cm girls-42cm from 128.0 to 169.9cm 14 year of age: boys-48cm from 131.0 to 178.9cm girls-36cm from 134.0 to 169.9cm 15 year of age: boys-42cm from 137.0 to 181.9cm girls-33cm from 137.0 to 169.9cm 16 year of age: boys-39cm from 146.0 to 184.9cm girls-30cm from 143.0 to 172.9cm 17 year of age: boys-39cm from 146.0 to 184.9cm girls-27cm from 143.0 to 169.9cm 18 year of age: boys-36cm from 152.0 to 187.9cm girls-27cm from 146.0 to 172.9cm 19 year of age: boys-30cm from 155.0 to 184.9cm girls-24cm from 146.0 to 169.9cm 20 year of age: boys-24cm from 158.0 to 181.9cm girls-l8cm from 149.0 to 166.9cm 3. Standard values of body weight, chest-girth and sitting height by body height of age were found all age groups from 7 to 20 years old and listed in tables from3-a to 16-a. 4. • The objective of the study is to figure out the status of reproductive health and general characteristics related to maternal health for Korean-Americans living in Los Angeles. We collected data from the married women who wanted no more additional child birth and were attending the Family Planning Clinic of Koryo Health Foundation in Los Angeles during 1988. There were 494 women met the eligibility requirement for this study. The results are summarized below. 1. In the age distribution of the women who desired no more additional child birth, women 30-34 age group constituted the largest proportion at 36.6 percent ; the mean age of women was 35.1915.55. The mean number of child birth was 1.77, and the proportion of the women by number of child birth were 35.2 percent for one children, 50.1 percent for two children 10.5 percent for three children, and 2.6 percent for four children. All of the women experienced pregnancy at least once, and mean number of pregnancy was 3.42. The mean number of total experience of induced abortion was 1.56. and 76.7 percent of these women had experience with induced abortions. To prevent further pregnancies, 90.1 percent of the women were utilizing the contraceptive methods, and the highest proportion by the contraceptive methods was condoms(53.7%), 9.3 percent in spermicides, 8.7 percent in IUDs, 8.7 percent in rhythm method, and 6.9 percent in oral pills. 2. The mean age of women at each stage of reproductive life cycle were 14.74 years at time of menarche, 24.55 years at time of marriage, 26.60 years at time of the first child birth, and 28.75 years at time of the last child birth. In age distribution of the women by birth cohort (Group I : birth cohort 1940-1954, Group H : birth cohort 1955-1970), the mean menar-cheal age of the women was 14.96 years in group I , and 14.53 years in group H . Mean age at time of marriage was 25.01 years in group I and 24.08 years in group H . Mean child birth age of the women by birth cohort was 27.19 years In group I and 26.01 years in Group II for the first child birth and 30.07 years in group I and 27.45 years in group II for the last child birth. The total length of reproductive life cycle from menarche to menopause (presumed to be at 49 of age years) was 34.26 years. The len-gth of phase I (from menarche to marriage) was 9.81 years, while phase H (marriage to first birth) was 2.05 years, and phase Ill (first birth to last birth) was 2. 15 years, and the last phase of reproductive life cycle, phase IV (last birth to menopause) was 20.25 years. The proportion of each phase 10 total length of reproductive life cycle was 28.6 percent, 6.0 percent 6.3 percent, and 59.t percent respectively. In the tendency of each phase in reproductive life cycle by birth cohort (group I , U ), the length of phase I, II , III of birth cohort group II was diminished in comparison with those of birth cohort group I , but the length of phase IV was extended by 2.38 years. 3. Among the women, the mean number of total pregnancy by birth cohort group was 2.01 in group I and 1.10 in Group II, and mean number of child birth was 1.97 in group I and 1.58 in group II. In terms of pregnancy was-tage rate by birth cohort group, among the total pregnancy of birth cohort group I , 51.8 percent of the cases resulted in induced abortions or spontaneous abortions whils 48.2 percent resulted in live births, and 42.2 percent or total pregnancy in group II resulted in pregnancy wastage and 57.8 percent of the cases resulted in live births.

  • Purpose: This study was conducted to assess the nutritional status of female marriage immigrants attending Korean language class in Gwangju, Korea by analyzing daily food and nutrient intake. Methods: Eighty-three female immigrants completed a survey. Anthropometric measurements were obtained, and dietary food intake was assessed using a 1-day 24 hour recall method. Results: The average length of residence in Korea was 5.3 years, and mean age of subjects was 31.0 years old. The home countries of subjects were Vietnam (50.6%), China (24.1%), Philippines (13.3%), and others (12%). Due to the length of residence, there were significant differences in body weight (p < 0.05), BMI (p < 0.05), percent body fat (p < 0.05), and diastolic blood pressure (p < 0.05). The subjects who were 30~49 years old consumed more vegetables and less seaweed than the subjects who were 20~29 years old. The other kinds of consumed foods were similar among groups in different age groups or lengths of residence in Korea. Average energy intake of subjects was 1,641.0 Kcal. The group with less than 5 years of residence showed higher cholesterol intake than the group with 5 or more years of residence in Korea (p < 0.05). There was no significant difference in nutrient intake between the groups of different age or length of residence. There was a positive association among dietary cholesterol intake and consumption of eggs, milk. and dairy products, and blood pressure. Conclusion: The study shows that length of residence affects rate of obesity and nutritional status. Further extensive research is needed to understand the effect of dietary changes and nutritional status of female marriage immigrants as well as for their successful adaptation to develop a more active and long-term nutrition education program.

  • Purpose : To evaluate the effectiveness and tolerance of the postoperative radiation therapy for patients with Stage III thymoma and to define the optimal radiotherapeutic regimen Materials and Methods : We retrospectively analyzed the records of 24 patients with Stage III thymoma who were referred for postoperative radiation therapy in our institution from June, 1987 to May, 1999. Surgical therapy consisted of total resection in one patient, subtotal resection in seventeen, and biopsy alone in six patients. Age of the patients was ranged from 20 to 62 years with mean age of 47 years. Male to female ratio was 14 to 10. Radiation therapy was delivered with linear accelerator producing either 6 MeV or 10 MeV photons. The irradiated volume included anterior mediastinum and known residual disease. The supraclavicular fossae were not irradiated. The delivered total dose was ranged from 30 to 56 Gy. One patient received 30 Gy and eighteen patients received minimum of 50 Gy. Follow up period was ranged from 12 months to 8 years with median follow up of 40 months. Results : The overall local control rate for entire group of patients was $67\%$ at 5 years. The cumulative local failure rates at one, three and five year were $18\%,\;28\%\;and\;33\%$, respectively. In patients treated with subtotal resection and biopsy alone, local control rate was $76\%\;and\;33\%$, respectively. The actuarial observed survival rate at 5 years was $57\%$, and actuarial adjusted survival at 5 years was $72\%$. The difference between 5 year survival rates for patients treated with subtotal resection and biopsy alone was not statistically significant $(62\%\;vs\;30\%)$. Conclusion : We might conclude that postoperative radiation therapy was safe and effective treatment for patients with Stage III thymoma. Postoperative radiation therapy is recommended in cases where tumor margin is close or incomplete resection is accomplished.

  • This study examined the contribution of the risk factors that cause cardiovascular disease in Koreans. The subjects were 20,012 adults aged 30~74 years who were enrolled from the 2005~2013 Korean National Health and Nutrition Examination Survey, had filled the risk factors data, and had no cardiovascular disease history. Data analysis was conducted using SAS 9.3, and a weighted composite sample design analysis was performed. The contribution was obtained by applying a squared semi-partial correlation. As a result, they were higher in the order of age (63.9%), smoking (17.3%), systolic blood pressure (7.9%), diabetes (6.9%), antihypertensive medication use (3.0%), and BMI (0.9%) in men. In women, the contributions were in the order of age (54.1%), systolic blood pressure (23.5%), diabetes (13.6%), antihypertensive medication use (5.1%), smoking (3.1%), and BMI (0.6%%). According to age bracket analysis, smoking (51.8~33.7%) was the biggest risk factor, followed in order by age (26.6%) in their 30s and systolic blood pressure (17.3~21.0%) in those older than 40 years in men. In women, the systolic blood pressure (56.2~40.5%) was the main contributor to the risk factors, followed by age (24.0%) in their 30s and diabetes (13.6~32.6%) in those older than 40 years. The contribution of the income level was similar. Therefore, to prevent cardiovascular disease, various programs that reflect the contributions of the risk factors should be developed. Moreover, it is recommended to use the results shown in this study as basic data.

  • In order to find out the effect of sowing time on establishment, survival and yield of pasture species when hoof and tooth cultivation was applied, this experiment was conducted from 1982 to 1984 at the comfound of Alpine Experiment Station. The results obtained are as follows: 1. Grasses establishment of 15 April, 15 May, 1 August and 30 August plots was 11.1%, 9.8%, 12.4% and 3.1%, resulting in 10% establishment in all plots except 30 August. The establishment of legumes in spring sowing was about 30%, showing significant difference (P < 0.05) than that of autumn sowing. 2. Botanical composition of sown pasture in all plots 2 years after sowing ranged from 66% to 80%. There was no significant difference between treatment in terms of DM Production 3 years after sowing although there was the difference in establishment and survival of sown pasture at the beginning of grassland establishment. It indicates that appropriate fertilization and grazing management after sowing play an important role in grassland improvement when hoof and tooth cultivation is applied. 3. The utilization of forages in plot 2 years after seeding was from 45 to 53%.

  • The purpose of this study is to investigate the characteristics of the vegetation structure and the location environment of Albizzia kalkora (AK) growing in Mt. Yudal located in Mokpo city and in the nearby islands. The AK community in Mt. Yudal in Mokpo city (Community I) is located in a region which is relatively high above the sea level. The average age of the major kinds of trees found in the region is about 30 years. The vegetation structure in the community shows an early stage of vegetation development due to continued disturbance. In Community IV, on the sandy soil in the flatland near the seashores, the average age of the major kinds of trees is about 9 years. In this community, a pure forest is presumed to have been formed in a poor environment which is artificially disturbed in relatively recent times even as AK with its strong adoptability was introduced into the region. In other communities (II, III), the vegetation state shows a competition between AK and deciduous oak trees, and the average age of the major kinds of trees is about 13 to 30 years. AK communities with a better developed vegetation structure are located on the higher steep slopes near the seashore. In the early stage of vegetation development, the forest floor received more effective light for photosynthesis, and thus more seedlings of AK emerged and grew. The probability of AK appearing in the damaged or sterile soil near the seashore was high because of its strong adaptability. However, as the vegetation structure developed further and the soil fertility increased, the domination of AK in the vegetation structure decreased as deciduous oak trees won the competition with AK.

  • The Ionescu-Shiley bovine pericardial xenograft valve was the most common cardiac substitute valve at Seoul National University Hospital. Since the follow-up extended for longer than 10 years, a total of and consecutive 107 patients with double mitral and aortic valve replacement using this valve from May 1979 to June 1984 were studied for the long-term clinical results. Their ages were 34.0$\pm$11.9 years at surgery, and eight patients died within 30 days of surgery with operative mortality rate of 7.5%. Ninety-nine early survivors were followed up for a total of 488.1 patient-years[Mean$\pm$SD, 4.9$\pm$2.7 years], and nine died with the linearlized late mortality rate of 1.84% /patient-year[pt-yr]. They experienced major complications: thromboembolism, 0.615% /pt-yr bleeding, 0.205% /pt-yr; endocarditis, l. 639%/pt-yr; overall valve failure, 6.146% /pt-yr; and primary tissue failure, 1.639%/ pt-yr. The actuarial survival rates were 91.4$\pm$2.9% and 89.6$\pm$3.4% at postoperative 5 and 10 years, and the probability of freedom from thromboembolism was 95.8$\pm$2.5% at 10 years. The primary tissue failure began to occur from postoperative 6 years and the probabilities of freedom from structural valve failure were 80.2$\pm$7.9% and 62.3$\pm$12.7% at 8 and 10 years after surgery respectively. Although there was increasing number of patients with valve tissue failure after 6 years, the evidence of expected premature and accelerated valve degeneration among young population was not clear on the age-related analysis. And, no definite cumulative patient groups beyond the various age limits could be suggested for or against the use of this valve.

  • A total and consecutive 163 patients underwent cardiac valve replacement using the Hancock porcine xenograft cardiac valves from 1 976 to 1984. Of 198 substitute valves, 177 were the Hancock valves. One hundred twenty-nine patients[79.1%] had single valve replacement: MVR 118, AVR 8 and TVR 3; 33[20.3%] had double valve replacement: MVR+AVR 27 and MVR+TVR 6; and a single case had triple valve replacement. Other surgical procedures were added in 34 patients. The operative mortality rate within 30 days of surgery was 6.1%, and it was, however, 4.2%, with single MVR. Late mortality rate was 6.7% or 1.95%/patient-year of a linealized mortality rate. Early survivors of 153 patients were followed up for a total of 565.1 patient-years [a mean of 44.3*27.1 months]. The linealized annual complication rates were: 1.95% emboli/patient-year, 0.89% bleeding/patient-year 1.24% endocarditis/patient-year, and 4.25% overall failure/patient-year. Primary tissue failure occurred at a rate of 1.59%/patient-year. The actuarial survival rates including operative mortality were 87.0*4.1% and 77.3*6.6% at 5 and 11 years after surgery respectively. The probability of freedom from thromboembolic complication of 89.2*3.4% at 5 years after surgery lasted unchanged upto 11 years. The probability of freedom from overall valve failure was 81.3*4.5% at postoperative 5 years, and it dropped down to 26.2*19.4% at 11 years, although the latter was statistically insignificant because of a small number of patients entering into the years approaching the follow-up end. However, the probability of freedom from the primary tissue failure was 81.3*10.6% at postoperative 9 years, which coincides closely with the speculated rate of tissue degeneration of about 20% in 10 years. These clinical results confirm the low thrombogenicity of the Hancock porcine valve and the reasonable failure rate of tissue degeneration.
