Synovial Chondromatosis of the Temporomandibular Joint :A Case Report (악관절에 발생한 연골종증 증례)

  • the primary diagnosis of synovial chondromatosis of temporomandibular joint is extremely rare. often many months, or even years pass before the diagnosis is confirmed. synovial chondromatosis is an uncommon disease of cartilage transformation of synovial membrane with formation of loose bodies within the joint space. a 28-year-old women had experienced pain of the left TMJ area at mastication. Radiographs of the left TMJ revealed calcified loose bodies in superior temporomandibular joint space. treatment consisted of removal of calcified loose bodies and synovail membrane. after surgery, pain of Lt temporomandibular area at mastication disappeared. until present after surgery there have been no recuurence of symptoms.

Inhibitory Effect of Deer Antler Aqua-acupunture (DAA) on Cathepsin S Activity and Rheumatoid Arthritis in Rats (생쥐에서 녹용약침액이 자가항원제시형 Cathepsin S 활성의 저해와 류마티스 관절염 억제에 미치는 효과)

  • 목적 : 시스테인 단백분해 효소인 cathepsin는 인간과 생쥐의 항원제시세포에서 II형 주적합항원 불변사슬(MHC class II invariant chain)의 분해에 관여한다. 본 연구는 녹용 약침액이 류마티스 관절염 생쥐 모델의 골조직(연골과 활액) 유래 cathepsin 활성에 미치는 영향을 검정하였다. 방법 : 관절염 동물모델은 BALb/c계 생쥐를 생후 3일에 흉선 적출(3d-Tx)을 하여 만들었다. 동물모델의 골조직, 임파절세포, 비장 등을 녹용처치군과 대조군으로 나누어 cathepsin의 활성도 및 자가항원 특이(C-II-specific) T-세포의 활성도를 비교 분석하였다. 결과 : 각 장기에서 cathepsin S의 활성은 녹용약침 처치군에서 농도 의존적으로 유의성 있게 억제되었고, T-세포 특이 자가항원반응은 녹용약침 처치군의 임파절 세포에서 유의성있게 억제되었다. 그리고 T-세포 특이 자가항원 반응의 불활성화에는 녹용 10~20ug/ml의 용량으로 충분하였다. 결론 : 이러한 실험결과는 녹용 약침액이 cathepsin S를 선택적으로 억제시켜 류마티스 관절염과 같은 자가면역 질환에 유효한 치료약물로 사용될 수 있음을 시사한다.

  • Toinsul consists of breathing method and exercising method by stimulation of acupuncture point and meridian. It activates the circulation of energy and blood and helps excretion of noxious vapor and it improves elasticity and vitality of muscles, tendon md ligament. It sharpens both secretion of synovia .and circulation around joints and also controls the whole body through meridian. This report introduces some about the exercise methods for treatment of lumbago and shoulder joint diseases among medical, surgical and gynecological diseases. It can be used for health care, treatment, relaxation muscles and recovery from fatigue in an aspect of prevention. It has been widely used for treatment of diseases and promotion of health so far even though it is not yet demonstrated scientifically. Therefore, it should be used mere efficiently as an important part of the oriental physical therapy.

Studies on Normal Synovial Fluid from Tibiotarsar Joints of Korean Native Cattle II. Synovial Protein Values (한우(韓牛)의 관절활액(關節滑液)에 관한 연구(硏究) 2. 활액단백성상(滑液蛋白性狀))

  • The mean values of total protein, albumin, globulins and A/G ratio of synovial fluid from the normal tibiotarsal joints of 55 healthy Korean native cattle were investigated. The results obtained were summarized as followings. 1. The mean values of synovial total protein for the entire group were $0.98{\pm}0.05g/dl$, with a range of 0.43 to 1.83g/dl for individual samples, and $1.00{\pm}0.07g/dl$ in slaughtering cattle and $0.92{\pm}0.06g/dl$ in living group, respectively. Compared with serum, synovial fluid contained far less total protein(p<0.01). 2. The mean values for the group were; albumin, $0.42{\pm}0.02g/dl$, globulins, $0.56{\pm}0.04g/dl$, and A/G ratio, $0.99{\pm}0.10$, with a range of 0.17-0.82g/dl, 0.03-1.32g/dl, and 0.15-3.15 for individual sample, respectively. 3. No statistical significant differences in the mean values of total protein, albumin, globulins, and A/G ratio have been observed between the synovial fluids of slaughtering and living animals. 4. Significant correlations existed between the globulin levels of synovial fluid and serum(r=0.3939), but the other values were not established.

  • Cystic hygroma, a cystic subtype of lymphangioma, is relatively rare tumor of lymphatic origin. The lesion is a benign, painless, soft, compressible malformation of the lymphatic system. They can arise anywhere along the lymphatic system, however they are usually located in the head and neck regions and in most cases appear by the age of 2 years. The cases in the adult is rarely occurred and a few cases are described in the literature. Surgical excision remains the treatment of choice. But complete extirpation of these lesion is often impossible, because the tumor tends to spread along vital structures. Therefore recurrence rates are accordingly high. This is a case report about 19 year old male patient with cystic hygro a on right retromandibular area. We obtained the successful, functional and esthetic result by surgical excision of the mass. Therefore, we report the case with a review of literatures.

The Effect of Bee Venom & Melittin Solution on Cell Death in Synovial Cell Line (봉독(蜂毒) 및 Melittin 약침액(藥鍼液)이 관절염(關節炎) 활액세포(滑液細胞)에 미치는 영향(影響))

  • Objectives : This study is aimed to investigate the effects of bee venom and melittin on cell death in synovial cell line. Methods : It was evaluated by using MTT assay, morphologic method, DNA fragmenation, NO generation, flow cytometry, immunocytochemistry analysis, RT-PCR, Western blot. Results : The obtained results are summarized as follows: 1. The MTT assay demonstrated that synovial cell viability was significantly inhibitted dose-dependently by treatment with bee venom and melittin in comparison with control. 2. The morphologic study demonstrated that synovial cell showed apoptosis after treatment with bee venom and melittin for 6 hours using microscope. 3. In case of NO generation bee venom group and melittin group showed significant inhibition in comparison with control. 4. The Flow cytometry demonstrated that apoptosis of synovial cell treated with bee venom and melittin was related with stop of cell cycle in stage of $G_0/G_1$. 5. DNA fragmenation demonstrated that synovial cell treated with bee venom and melittin showed DNA ladder below l Kbp. 6. Immunocytochemistry assay demonstrated that COX-II and PLA2 were strongly down-regulated by treatment with bee venom and melittin whereas iNOS was almostly not expressed by bee venom treatment and slightly expressed by melittin treatment. 7. RT-PCR analysis demonstrated that iNOS were strongly down-regulated by treatment with bee venom and melittin whereas COX-II was almostly not expressed by bee venom treatment and slightly expressed by melittin treatment. 8. Western blot demonstrated that iNOS were strongly down-regulated by treatment with $15{\mu}g/ml$ bee venom whereas COX-II was strongly down-regulated from $5{\mu}g/ml$ bee venom. Conclusions : These results suggest that bee venom and melittin have significant effect on cell death in synovial cell line and further study is needed in vivo.

Effect of Microcurrent Therapy on Interleukin-6 Expression in Adjuvant Induced Rheumatoid Arthritis Rat Model (미세전류치료가 아주반트 유도 류마티스관절염 유발 흰쥐의 인터루킨-6 발현에 미치는 영향)

  • 연구목적 : 미세전류 자극이 아주반트로 류마티스 관절염을 유발한 실험동물의 염증반응과 통증에 미치는 효과를 알아보기 위하여 실험동물의 발허리발가락관절내 염증반응 정도를 나타내는 인터루킨-6(interleukin-6)의 발현과 핫플레이트(hot plate)를 이용한 발도피지연시를 측정하여 미세전류의 효과에 대하여 알아보고자 하는데 목적이 있다. 연구방법 : 실험동물은 무작위로 대조군(n=18)과 미세전류를 적용한 실험군(n=18)으로 구분하였고, 각 군당 6마리씩 1일군, 7일군, 14일군으로 배정하였다. 류마티스 관절염 유발후 1일, 7일, 14일에 모든 실험동물의 열통각 역치를 나타내는 발도피지연시와 발허리발가락관절내 인터루킨-6의 발현정도를 측정하였다. 각 집단 내의 기간에 따른 발도피지연시와 인터루킨-6의 면역반응성은 일원배치 분산분석을 실시하였고, 사후분석으로는 Duncan의 다중범위검정을 실시하였다. 실험군과 대조군을 비교하기 위하여 독립표본 t-test를 실시하였다. 연구결과 : 실험결과는 다음과 같다. 1) 아주반트 주사 1일후, 실험군과 대조군에서 인터루킨-6 면역반응성과 발도피지연시는 비슷한 양상을 보였다. 2) 인터루킨-6 면역반응성은 아주반트 주사 7일, 14일 후 대조군이 실험군보다 유의하게 증가되었다(p<.05). 3) 발도피지연시는 아주반트 주사 7일, 14일 후 실험군이 대조군보다 유의하게 증가되었다(p<.05). 결론 : 이상의 결과로부터, 미세전류 자극이 아주반트로 유발된 류마티스관절염 모델에서 활액 조직내 염증반응을 감소시키고 열통각역치는 증가되는 것을 알 수 있었다.

ANALYSIS OF TMJ STATUS IN THE PATIENTS WITH MANDIBULAR FRACTURES: PRELIMINARY STUDY ARTHROSCOPIC EXAMINATION, HISTOMORPHOLOGY AND JOINT FLUID ANALYSIS (하악골 골절 환자들의 악관절 상태 평가: 일차보고 관절내시경적 검사, 조직형태학적 및 관절활액 분석)

  • The purpose of this study is to validate the potential etiologic factors for temporomandibular disorder(TMD). TMJ arthroscopic examination was performed in upper joint compartment of 32 joints from 20 patients with mandibular fractures. Synovial fluid was collected from the upper joint space during pumping manipulation with normal saline. Cytologic smearing and histomorphologic exam of synovial fluid were performed in 15 joints. Prostaglandin $E_2(PGE_2)$ concentration was measured in 11 joints. Leukotriene $B_4(LTB_4)$ concentration was measured in 8 joints. There were several arthroscopic variables such as ecchymosis, fibrillation, and adhesion. Histomorphologic exam showed a variety of findings such as bloody smears, cellular cluster, degenerated cells and cartilage, undifferentiated crystal. Mean $PGE_2$ concentrations were 316.5 pg/ml. Mean LTB4 concentrations were 45.9pg/ml. This study demonstrated a variety of findings on inflammatory and degenerative changes of TMJ. Because acute trauma such as mandibular fracture is a major etiologic factor in cartilage degradation and biochemical and intraarticular pathology, clinicians must identify and address TMJ signs and symptoms during follow-up periods in the long term.

  • Tophaceous gout, which is usually presented in the synovial fluid, bursal lining, cartilage or other soft tissues, may cause a nonoutlet impingement in the rotator cuff and bursa. In chronic tophaceous gout patient with the shoulder pain, a surgeon should consider the possibility of the tophaceous gout of the rotator cuff. We report a surgical experience of a 50-year-old man with tophaceous gout of the rotator cuff causing impingement syndrome. The patient was treated successfully with arthroscopic debridement and subacromial decompression.

  • Synovial chondroma is an uncommon benign lesion characterized by metaplastic cartilage formation within the synovial connective tissue, usually intraarticular, commonly affects the knee, hip and elbow. We would like to present the case of a 65-year-old man suffering from synovial chondroma of the right elbow responsible for radial nerve entrapment neuropathy. This is a case of synovial chondroma of the right elbow in an 65-year-old man presenting with pain and restricted joint movement of the right elbow, loss of extension and sensation of the right thumb and wrist. Plain radiographs showed narrowing of elbow joint space, bony spur on the edge of the joint, and radio-opaque sclerotic change of subchondral area. MRI revealed $16{\times}12$ mm sized round mass on the radial head, homogenous low signal on T1WI, heterogenous high and low signal on T2WI. The patient underwent marginal excision of the mass, compressing the radial nerve. Diagnosis was confirmed by histologic examination.
