혈액투석 환자의 수면장애 영향요인 (Factors Influencing Sleep Disorders in Patients on Hemodialysis)

  • Purpose: This study was conducted to identify the factors contributing to sleep disorders in patients on hemodialysis. Methods: A descriptive correlational study design was used. The participants were 135 patients on hemodialysis in hemodialysis clinics. Data were collected from March to May 2007 using structured questionnaires and hematologic sample. Stepwise multiple regression was used to identify factors influencing sleep disorders among the demographic-clinical factors, depression and fatigue. Results: About 69% of the patients had a sleep disorders. There were significant differences in sleep disorders according to marital status, caregiver, religion, economic level, insurance, erythropoietin, somnifacient, and antihypertensive agents. But there were no differences according to age, gender, dialysis period, or antidepressants. Their sleep disorders had significant correlations with depression, and fatigue, and a significant negative correlation with Protein, Albumin, Phosphate and BUN. But there were no correlations with hematocrit, hemoglobin, creatinine, sodium, potassium, or calcium. Depression and fatigue were factors influencing sleep disorders. They accounted for 43.8% of the variance in sleep disorders in these patients. Conclusion: Findings provide an understanding of sleep disorders and the factors that are an influence in patients on hemodialysis. To promote sleep in these patients, nursing interventions to manage depression and fatigue are needed.

  • Purpose: The purpose of this study was to identify the effects of emotional labor, compassion fatigue and occupational stress on the somatization of nurses in hemodialysis units. Methods: The sample consisted of 139 nurses in hemodialysis units from a tertiary hospital, a general hospital, a dialysis clinic, and a care hospital in G province. Data were analyzed using frequencies, percentages, means and standard deviations, t-test, ANOVA, Duncan test, Pearson's correlation and hierarchical multiple regression. Results: There were significant correlations of the experience of emotional labor, compassion fatigue and occupational stress with the somatization of nurses in hemodialysis units. Factors influencing somatization intention were 'emotional labor' (${\beta}=.37$, p<.001), which explained 28% of the variance (F=10.00, p<.001). Conclusion: The results of this study indicate that the factor influencing the somatization of nurses in hemodialysis units was emotional labor. Therefore, strategies to decrease emotional labor of nurses in hemodialysis units are required.

  • Purpose: This study was attempted to provide the foundations for the nursing intervention program that could improve hemodialysis patient's quality of life by examining influences of family support, self-efficacy and fatigue on quality of life among hemodialysis patients. Methods: We have surveyed 200 patients that aged 18 or older being diagnosed with end-stage renal disease (ESRD) and are receiving hemodialysis treatment in three general hospitals equipped more than 400 beds, one specialized dialysis clinics located in city M. Using the SPSS 17.0 windows program, gathered data were analyzed by number, percentage, mean, standard deviation, t-test, ANOVA, Duncan, Pearson's correlation coefficients and multiple regression analysis. Results: We found the factors that had great influences on patient's quality of life were family support (${\beta}=.155$, t=2.18, p=.031), fatigue (${\beta}=-.487$, t=-7.73, p<.001), and monthly income (${\beta}=.147$, t=2.36, p=.019). Explanatory power of this model was 49.6%. Conclusion: The quality of life of the hemodialysis patients on hemodialysis is affected by family support, fatigue, and monthly income. The higher monthly income, family support and the less fatigue, the more likely they can improve their quality of life.

혈액투석 환자의 환자역할행위 이행에 대한 예측변인 (Factors Predicting Sick Role Behavioral Compliance in Hemodialysis Patients)

  • Purpose: This study was done to identify factors predicting sick-role behavioral compliance in hemodialysis patients. Methods: The subjects were 133 patients from outpatient dialysis clinics at seven major general hospitals located in D and P cities. Data were collected by self-report questionnaires. For data analysis, SPSS/WIN 14.0 program was utilized including one-way ANOVA, independent t-test, Pearson correlation coefficients, and stepwise multiple regression. Results: This study showed a positive correlation between sick-role behavioral compliance and social support (r=.29 p<.001), knowledge (r=.31 p<.001), self-efficacy (r=.52, p<.001) and age (r=.21, p=.015). The factors predicting sick-role behavioral compliance were self-efficacy (${\beta}$=.46), knowledge (${\beta}$=.25), and age (${\beta}$=.18). These factors accounted for 36% of the variance of sick-role behavioral compliance in hemodialysis patients. Conclusion: The results indicate that it is necessary to increase self-efficacy and knowledge in order to improve sick-role behavioral compliance in hemodialysis patients.

경혈지압이 혈액투석 환자의 소양증 및 수면에 미치는 효과 (Effects of Acupressure on Pruritus and Sleep in Patients on Hemodialysis)

  • Purpose: This study was done to investigate the effects of acupressure on pruritus and sleep among patients undergoing hemodialysis. Methods: The study design was a nonequivalent control group repeat measures quasiexperimental study. Data were collected from June to September 2015 in the kidney center at a hospital located in B city. Participants were 42 patients; 20 in the experimental group and 22 in the control group. For the experimental group, acupressure on the spots of Sameumgyo (SP6), Zoksamli (ST36), Hyolhae (SP10), and Gokji (LI11) was provided at the time of hemodialysis three times a week for 12 weeks. The numeric rating scale (NRS) was used to measure pruritus, and sleep was measured with the sleep scale developed by Jinju Oh et al. Results: There was a significant decrease in pruritus (F=7.80, p=.008) and increase in sleep (F=80.90, p<.001) in the experimental group compared to the control group. Conclusion: The findings confirmed in the study show that acupressure is an effective nursing intervention to decrease pruritus felt by patients on hemodialysis and to increase the quality of their sleep.

  • Purpose: The aim of this study was to evaluate the nutritional status and to identify factors affecting malnutrition in hemodialysis patients. Methods: Data were collected from a convenience sample of 125 hemodialysis patients who agreed to participate in the study, between August 1 and October 4, 2014. Five structured questionnaires were used: Patient-generated Subjective Global Assessment, Simplified Nutritional Appetite Questionnaire, Self-rating Depression Scale, Fatigue Scale, and Family Support Scale. Data were analyzed by descriptive statistics, t-test, Mann-Whitney U test, ${\chi}^2$-test, and stepwise multiple regression analysis by using SPSS/WIN 22.0 program. Results: Of 125 patients, 30.4% were found to be malnourished. Malnutrition was related to age, family household income, depression, fatigue, social support, appetite, and levels of C-reactive protein and serum albumin. Stepwise multiple regression analysis showed that appetite, C-reactive protein level, fatigue, and albumin level were significant factors affecting malnutrition in hemodialysis patients. Conclusion: Regular and consistent nutritional assessment is essential in hemodialysis patients. Nurses who care for hemodialysis patients need to consider the factors identified from these findings when assessing their patients' nutritional status and needs.

  • Purpose: The purpose of this study was to identify the effect of auricular acupressure on pruritus and fatigue in hemodialysis patients. Methods: The study design was a randomized control group pre-post test. Initially, forty-four patients were randomly assigned to one of two groups. There was a loss of three participants assigned to the treatment group. The period of data collection was from December 2014 to March 2015. Both groups completed a pre-test. The treatment group received auricular acupressure once a week for ten weeks. Data were collected from the treatment group at two time periods: five weeks and ten weeks following initiation of the treatment protocol. Data were collected from the control group at week 5 and week 10. Data analysis was performed using IBM SPSS Statistics 21.0 program, specifically with the independent t-test and the Repeated Measures of ANOVA. Results: Auricular acupressure was effective in reducing pruritus (F=13.93, p<.001) and fatigue (F=18.33, p<.001). Conclusion: Auricular acupressure is a non-invasive simple method that can be used for the relief of symptoms reported by hemodialysis patients. This treatment modality could be used in several clinical areas.

  • Purpose: The purpose of this study was to determine the time and content of dietary education by analyzing the physiological indicators of hemodialysis patients during their first year and to present baseline data related to dietary education for patients on hemodialysis. Methods: For this retrospective study physiological indicators for 73 patients on hemodialysis were analyzed. Data included levels of potassium, phosphorus, albumin and the IDWG rate, and Kt/V during the first year after starting hemodialysis. Repeated-measure ANOVA was used for statistical analysis. Results: Serum levels of phosphorus (p<.006), albumin (p<.001) and the IDWG rate (p<.001) increased significantly in the 7 months from the start of hemodialysis. Conclusion: The results indicate that the period in which diet related educational intervention is necessary is before physiological changes, that is, after 6 months of dialysis. The contents for effective education should include phosphorus-limited and weight control dietary restrictions at 6 months.

  • Purpose: This study was done to gain understanding of what career and related experience mean to individuals undergoing hemodialysis. Methods: Ten male patients receiving hemodialysis participated in the study. Data collection took place between November 18, 2008 and February 10, 2010, via unstructured interviews. Data collection and analysis were conducted simultaneously, and Colaizzi's phenomenological method (1978) was used for the analysis. Results: The significance the participants found in their "dual" life as worker and dialysis patients was classified into five categories: 'Recognition of self-existence value', 'My health comes before my work', 'Being afraid of stigma', 'Limitation of restricted work', and 'Difficulty with time management.' Conclusion: It was found that the dialysis patients showed ambivalent feelings towards their careers, hoping they will be able to continue to work yet fearing that the continued work might break balance the between their livelihood and healing. Therefore, it is recommended that hours for hemodialysis be more flexible to ensure that patients can keep their jobs and better manage their time while undergoing treatment.

  • Purpose: The aim of this study was to investigate the relationships between the hemodialysis unit nurses' experience of verbal abuse, job stress, burnout, and turnover intention, and to identify the explained variances for turnover intention. Methods: The research design was a descriptive survey using a probability sampling. The data were measured using self-report questionnaires from 231 hemodialysis unit nurses in seven cities in Korea and were analyzed with SPSS 20.0. Results: The mean scores were $2.45{\pm}0.62$ (out of 5) for experience of verbal abuse, $2.40{\pm}0.26$ (out of 4) for job stress, $2.31{\pm}0.48$ (out of 4) for burnout, and $3.16{\pm}0.05$ (out of 5) for turnover intention. There were significant correlations among the experience of verbal abuse, job stress, burnout, and turnover intention within the hemodialysis unit nurses. Factors influencing turnover intention were 'burnout' (${\beta}=.42$, p<.001) and 'job stress' (${\beta}=.17$, p=.015)which explained 42% of the variance (F=15.98, p<.001). Conclusion: Results suggest that in order to reduce nurses' turnover intention, nurse managers should explore strategies to reduce nurses' burnout and job stress in nurses in hemodialysis units.
