  • In diabetes mellitus, renal disease is a common complication, characterized by increased urinary albumin excretion and reduced eGFR. According to KDIGO CKD stage classification, Korean characteristics were analyzed according to urinary albumin and eGFR using the National Health and Nutrition Examination Survey VI raw data. According to KDIGO classification, diabetic patients were classified as Low risk 72.0%, Moderate risk 19.3%, High risk 5.6% and Very high risk 3.0%. Low risk decreased from 74.7% to 52.2%, and moderate to very high risk increased from 25.4% to 47.8% as the duration of diabetes mellitus was prolonged. The risk factors were CKD stage 1 (HR 2.064) to stage 4 (HR 11.049), the highest risk of hypertension. The incidence of renal disease was elevated according to duration of hypertension and HR 0.42 of kidney disease was decreased in the group maintaining proper blood pressure. In the hypertensive patients, the group administered with target blood pressure had a reduction of the kidney disease by 42% than the group with the hypertension. Therefore, controlling and managing hypertension to target blood pressure is important for the prevention of kidney disease.

  • The purpose of this study was to analyze the management behavior in patients with hypertension and diabetes through the total inspection of a specific rural area. Data was collected through self-administered questionnaires from May 19th to September 21st in 2014. As the result, Among 355 (36.6%) patients who were diagnosed with hypertension, 337 (94.9%) were currently undergoing treatment, and among 109 (11.2%) patients who were diagnosed with diabetes mellitus, 96 (80.1%) were currently undergoing treatment. Among the total of 211 people in hypertension potential risk group, 89 (42.2%) were diagnosed with hypertension, and among the total of 356 people in hypertension potential risk group, 78 (22.0%) were diagnosed with diabetes mellitus. In conclusion, from the results of the analysis, we suggest that effective national strategies for prevention and management for patients with hypertension and diabetes should be established and implemented.

  • As the convergence technology is developing rapidly and the portable mobile devices are spreading in ubiquitous smart healthcare, users were able to get medical information easily through the smart health platform. According to such rapid industrialization, wellness care, aging society, information society, changes in disease aspects and life style, user-centered healthcare, and health promotion contents are being offered. In this study, we proposed the mining based mental health and blood pressure management service for the smart health. The proposed method provides the mental health management service and the blood pressure management service for chronic disease patients within the mining based smart health platform. Users receive optimized healthcare services regardless of time and place in the PHR based smart health platform. For the performance evaluation of the proposed mining based mental health and blood pressure management service, F-measure verification are conducted.

  • Chronic diseases such as hypertension require a differentiated approach according to age and life cycle. Chronic diseases such as hypertension require differentiated management according to the life cycle. It is also known that the cause of hypertension is a combination of various factors. This study uses machine learning prediction techniques to analyze various factors affecting hypertension by life cycle. To this end, a total of 35 variables were used through preprocessing and variable selection processes for the National Health and Nutrition Survey data of the Korea Centers for Disease Control and Prevention. As a result of the study, among the tree-based machine learning models, XGBoost was found to have high predictive performance in both middle and old age. Looking at the risk factors for hypertension by life cycle, individual characteristic factors, genetic factors, and nutritional intake factors were found to be risk factors for hypertension in the middle age, and nutritional intake factors, dietary factors, and lifestyle factors were derived as risk factors for hypertension. The results of this study are expected to be used as basic data useful for hypertension management by life cycle.

  • This study was conducted to identify the factors of hypertension treatment in Gwangju and to establish a hypertension cure rate management plan by using local community health surveys to provide the hypertension cure rate management plan centering around the wellness local community. The research collected 13,714 Gwangju research data among a total of 685,820 local community health surveys of KDCA (Korea Disease Control and Prevention Agency) from 2017 to 2019. Among the data, 2,941 subjects, those with diagnosed hypertension aged over 30, were selected and analyzed through SAS 9.4, SAS Enterprise Miner 15.1. The results are as follows. The differences in hypertension diagnosis cure rate in Gwangju based on the subjects' socioeconomic characteristics were shown in gender, age, marital status, level of educational attainment, economic activity status, and monthly income. The significant differences in hypertension cure rate based on health behavior characteristics were shown in current smoking, monthly alcohol consumption, high-risk drinking, breakfast, recognition of good health level, diabetes and treatment, annual unmet medical needs, and annual health center use. As a result of the logistic regression analysis and interactive decision tree analysis to identify the factors affecting hypertension treatment, the research found that the factors that appear are age, marital status, diabetes and treatment, and annual unmet medical needs. Accordingly, to increase the recognition of the importance of hypertension treatment to people of young ages and not to develop complications, public health-educational effort in Gwangju is needed with an effective preparation plan.

  • The purpose of this study is to investigate risk factors of pre-hypertension and hypertension in rural residents. Nine hundred and ninety four subjects aged 40-70 yrs in Chungnam-do participated in this study. The subjects (n = 824) were classified into three groups of hypertensive, pre-hypertensive, and normotensive according to the Joint National Committee (JNC)-7 criteria. The weight, body mass index (BMI), waist-hip ratio (WHR), and serum total protein, albumin, BUN, and triglyceride (TG) were positively correlated with SBP and DBP. After adjusted by age, sex and BMI, the total protein, albumin and TG were significantly correlated with SBP and DBP (p < 0.01). There was no significant difference in eating habits according to the level of blood pressure. The serum albumin, creatinine, Glu-FBS, Glu-PP l20, and triglyceride were higher in both prehypertensive and hypertensive group than in the normotensive group. However, mean serum cholesterol was not different among three blood pressure groups. In this study, the common risk factors of pre-hypertension and hyper-tension were male, age of fifties, lower education level, ex-smoking, higher drinking frequency, higher BMI, body fat %, waist circumference, WHR, serum albumin and diabetes, even though the degree of risks in these variables were higher in the hypertensive group. The higher BUN was a risk factor of prehypertension, while the family history, prediabetes, serum total protein, Glu-PP l20 and higher alcohol drinking amount were the risk factors of hypertension. This result suggests that maintaining good health habit and normal range of blood parameters as well as controlling body weight have to be paid attention in order to prevent hypertention, and further reseasch on the relationship of blood pressure and BUN are needed.

As a result, the participants' blood pressure level in the hypertensive patients case management improved just after the case management compare to just before one. but it had a tendency to increase once more after 6 months and 12 months case management. In addition, it was confirmed to maintain the effect by the case management in Group II compare to Group I.
