A Case Report on a Patient with Cervical Vertigo Who Improved with a Combination of Korean Medicine Treatments (한방 복합치료로 호전된 경추성 현훈 환자의 증례보고 1례)

  • Objective: To report the case of a patient suffering from cervical vertigo who was treated with a combination of Korean medicine treatments (including acupuncture, pharmaco-acupuncture, and Chuna manual therapy). Method: An outpatient with cervical vertigo was given a combination of Korean medicine treatments five times, from December 5 to 16, 2016. All of the treatments, acupuncture with electric stimulation and pharmaco-acupuncture, were applied on the trigger points (TP) of the cervical muscles and the upper trapezoid muscles. Chuna manual therapy was then applied on the cervical area. The Numeric Rating Scale (NRS) - according to outpatient's statement on vertigo, neck pain and headache - and the Korean Dizziness Handicap Inventory (DHI), which is a self-evaluating scale of dizziness in daily life, were used to evaluate the effects of this treatment. Results: At the end of the treatment, the vertigo and the headache disappeared. The NRS decreased from 9 to 0 for vertigo and from 5 to 0 for headache. Although the neck pain also decreased, it did not disappear; this NRS decreased from 7 to 1. The DHI score also decreased significantly, from 64 to 4. Conclusion: Since the symptoms of the patient with cervical vertigo improved after the very first treatment and the improvement of the symptoms was observed as the treatment progressed, this case report is worth supporting the efficacy of combined Korean medical treatments. However, because there was only one subject, it is hard to say that this efficacy can be generalized. Moreover, this report cannot prove the individual efficacy since various treatments were combined. While there are many researches related to cervical vertigo in Western medicine, the studies in Korean medicine are comparably insufficient; therefore, follow-up research on more subjects is needed.

A Study on the Characteristics of Patients Treated for Dizziness with Jeoreongchajeonja-tang: A Retrospective Analysis of 63 Cases (저령차전자탕을 이용해 현훈을 치료한 환자의 특징 고찰 : 63례의 후향적 분석)

  • Purpose: The purpose of this study was to conduct a clinical analysis of patients treated for dizziness with Jeoreongchajeonja-tang, evaluate the effect of Jeoreongchajeonja-tang administration, and present indications of Jeoreongchajeonja-tang in dizziness. Methods: A retrospective study was conducted on 63 patients who were admitted to the Oriental Hospital of Se-Myung University from August 2012 to June 2019, complaining of dizziness, so who took Jeoreongchajeonja-tang. Results: 1) As a result of the analysis of the general characteristics of patients who took Jeoreongchajeonja-tang with dizziness, women were 2.7 times more than men. The age distribution was: patients in their 70s, 36.5%; in their 50s, 20.6%; in their 60s, 15.8%; and in their 80s, 12.6%. Patients had a history of hypertension (46.0%), diabetes (22.2%), stroke (17.4%), lung disease (12.6%), and cancer (11.1%). According to the diagnosis, 48 patients (76.1%) were assigned to the peripheral dizziness group, 7 patients (11.1%) to the central dizziness group, and 8 patients (12.6%) to other dizziness groups. 2) Compared with the peripheral dizziness group, the central dizziness group was older, their history of hypertension 8.4 times greater, their history of stroke 7.8 times greater, and the duration of herbal medicine administration was longer. 3) The vertigo grade was significantly decreased after taking Jeoreongchajeonja-tang (p=0.000). 4) As age increased by one year, odds ratio of improvement in the vertigo grade decreased by 0.924 times after taking Jeoreongchajeonja-tang (p=0.010). 5) Sex, disease duration, past history, diagnosis, the duration of herbal medicine administration, whether or not Yanggyuksanhwa-tang was combined with the Jeoreongchajeonja-tang and whether or not Western medicine was used did not affect improvement in the vertigo grade after taking Jeoreongchajeonja-tang. Conclusions: In patients with dizziness, Jeoreongchajeonja-tang can be used as a treatment without side effects.

Synthesis and Characterization of Biodegradable MethoxyPoly(ethylene glycol)-Poly$(\varepsilon-caprolactone-co-L-lactide)$ Block Copolymers (메톡시폴리(에틸렌 글리콜)-폴리(카프로락톤-co-L-락타이드) 공중합체의 합성 및 특성 분석)

  • A series of methoxypoly(ethylene glycol) $(MPEG)-poly(\varepsilon-co-L-lactide)$ (PCLA) diblock copolymers were synthesized by ring-opening polymerization of a mixture of $\varepsilon-caprolactone$ and L-lactide with different ratios in the presence of $Sn(Oct)_2$. The characterization of MPEG-PCLA diblock copolymers were examined by $^1H-NMR$, GPC, DSC, and XRD. Kinetic study on ring-opening polymerization of monomer mixtures was carried out in various conditions such as a variation with polymerization time, amount of catalyst, and temperature. The highest conversion obtained in 1.2 ratic of initiator venn catalyst at $110\;^{\circ}C$. The biodegradable characterization of MPEG-PCLA diblock copolymers in aqueous solution was carried out by using GPC for $1\~14$ weeks. The biodegradability of MPEG-PCLA diblock copolymers increased as the L-lactide content of diblock copolymers increased. In conclusion, we confirmed the dependence of polymerization rate according to various conditions. In addition, we can control the biodegradability of MPEC-PCLA diblock copolymers by changing the ratio of PCL and PLA block segment.

  • Purpose : The purpose of this study was to evaluate whether the therapeutic effects of topiramate differ according to the types of migraine. Methods : We recruited 38 children and adolescents with migraine who had been treated with topiramate. The effect of topiramate was evaluated on the basis of the change in the frequency of migraine attacks after treatment. Results : Among patients having migraine with aura, 85.7% showed complete recovery, 1 (7.1%) showed partial recovery, and 1 did not show any recovery. Among patients having migraine without aura, 47.1% showed complete recovery, 29.4 % showed partial recovery, and 23.5% showed no recovery. Among patients suspected with migraine, 1 (20%) showed complete recovery, 1 (20%) showed partial recovery, and 3 (60%) showed no recovery. Conclusion : Our results indicated that topiramate exhibited excellent therapeutic effects for migraine accompanied with aura, and it was effective in migraine without aura. However, the effect of topiramate in patients suspected with migraine was uncertain.

A Study on Anxiety in College Students (불안척도에 의한 대학생의 정신건강 평가)

  • The authors studied anxiety, using Zung's Self-Rating Anxiety Scale (SAS), in the subjects of 3,893 male and 1,976 female college students of Yeungnam University. The authors collected the reports of SAS during the periods from October to November, 1984, and applied ANOVA and t-test on the anxiety scores in order to compare them between various psychosocial factors, and sexes. The results could be summarized as follows: There was significant difference in the mean averages of total anxiety score's between male and female students ; male students scored $36.92{\pm}7.07$, female students scored $39.63{\pm}7.51$, p<0.001. The anxiety scores relating to the items of sweating, apprehension, restlessness, insommia, dyspnea were relatively higher in both groups. The anxiety scores in the items of paresthesias, mental disintegration, tremors, faintness, dizziness were lower in both groups. Two hundred and one male students (5.2%) showed seriously high anxiety scores of 50 or higher, while 201 female students (10.2%) showed the same scores. So the authors inferred that the features of anxiety symptoms were much the same in our country, but females showed more various symptoms and higher level of anxiety than males. In a comparison between male students in different school grades, freshmen showed the highest level of anxiety scores and as the grades of the students became higher, the levels of anxiety scores were lower. The female students who resided in dormitory or other residences, and whose educational fees were paid by brothers or sisters, showed higher level of anxiety scores. There was a strong tendency toward higher anxiety scores in the students who were dissatisfied with their colleges or departments, and who had pessimistic views of self image in the past, present, or future, in both groups.

Synthesis and Characterization of Linear and Branched Copolymers of Poly(ethylene glycol) and $Poly({\varepsilon}-caprolactone)$ (선형 및 분지 구조의 폴리(에틸렌 글리콜)/폴리카프로락톤 공중합체의 합성 및 특성 검토)

  • Linear and branched copolymers consisting of poly(ethylene glycol) (PEG) and $Poly({\varepsilon}-caprolactone)$ (PCL) were prepared to compare the characterization of star-shaped copolymers with various molecular architecture. Linear and branched PEG-PCL (1-arm, 2-arm, 4-arm, and 8-arm) copolymers were synthesized by the ring-opening polymerization of ${\varepsilon}-caprolactone$ in the presence of HCl $Et_2O$ as a monomer activator at room temperature. The synthesized copolymers were characterized with $^1H-NMR$, GPC, DSC, and XRD. As a result of the DSC and XRD, each copolymers showed different thermal properties and crystallinity according to the number of ms. The micellar characterization of linear and branched copolymers in an aqueous phase was carried out by using NMR, dynamic light scattering, AM, and fluorescence techniques. The critical micelle concentration (CMC) and diameters of micelles depended on the number of arms. Most micelles exhibited a spherical shape in AFM. In this study, we characterized star-shaped PEG-PCL copolymers and investigated their molecular architecture effect on the various properties. Furthermore, we confirmed that the micelles termed with linear and branched PEG-PCL copolymers have possibility as a potential hydrophobic drug delivery vehicle.

  • Diblock copolymers consisting of methoxy Poly (ethylene glycol) (MPEG) and poly (${\epsilon}-ca$ prolactone) (PCL), poly(${\delta}-valerolactone$) (PVL), poly(L-lactide) (PLLA), or poly(L-lactide-co-glycolide) (PLGA) were prepared to compare the characterization of diblock copolymers as a drug carrier. MPEG-PCL, MPEG-PVL, MPEG-PLLA, and MPEG-PLGA diblock copolymers were synthesized by the ring-opening polymerization of ${\epsilon}$-caprolactone or ${\delta}$-valerolactone in the presence of $HCl{\cdot}Et_2O$ as a monomer activator at room temperature and by the ring-opening polymerization of L-lactide or a mixture of L-lactide and glycolide in the presence of stannous octoate at $130^{\circ}C$, respectively. The synthesized diblock copolymers were characterized with $^1H-NMR$, GPC, DSC, and XRD. The micellar characterization of MPEG-polyester diblock copolymers in an aqueous phase was carried out by using NMR, dynamic light scattering, AFM, and fluorescence techniques. Most micelles exhibited a spherical shape in AFM. Thus, ore confirmed that the micelles formed with MPEG-polyester diblock copolymers have possibility as a potential hydrophobic drug delivery vehicle because a hydrophobic drug could be preferentially distributed in the micelle core.

Clinical Observation on Effects and Adverse Effects of Choweseuncheng-tang on Obesity Patients (비만 환자에 대한 조위승청탕의 효과 및 부작용에 관한 임상적 고찰)

  • Objective : This study was designed to investigate the effects and adverse effects of Choweseuncheng-tang on obesity patients Methods : Choweseuncheng-tang was given to 20 obesity patients who have no abnormality of GOT, GPT and past medical history. Additionally, electroacupuncture was performed on Fuchao(腑巢) with 60Hz. We compared body weight, body fat and laboratoy test(GOT, GPT, BUN, Creatinine) between before and after treatment. Blood pressure, heart rate and clinical symptoms were checked to investigate the side effects. Decrease of body weight was checked in accordance with Sasang constitutional type, appetite type, and BMI. Results : 6 patients(BMI 23-24.9), 10 patiets(BMI 25-29.9), and 4 patients(BMI over 30) were showed decrease of body weight. We found body fat was decerased $3.16{\pm}1.25%$. BUN, Creatinine, GOT and GPT were increased within normal range. There were adverse event of autonomic hyperactivity(11 patients), gastrointestinal symptoms(7 patients) and others(3 patients). Conclusions : According to above results, Choweseuncheng-tang could be used effectively, but should be taken cautiously on obese patients.

A Case of Intracranial Tuberculoma and Optic Disc Tuberculoma Suspected by Miliary Tuberculosis (속립성 결핵에서 발생된 것으로 사료된 두개내 및 시신경 유두부의 결핵성 육이종 1예)

  • Intracranial tuberculoma results from hematogenous spread of pulmonary, intestinal or urogenital tuberculosis. However, it might be caused by pulmonary tuberculosis, mainly. Clinically, symptoms of intracranial tuberculoma are headache and seizure, its symptoms are simillar to intracranial tumor. A 25-year-old-unmarried shopgirl was visited to this hospital because of headache, dizziness and visual disturbance for couple weeks in Sep. 1995. She had been treated with anti-tuberculosis agents of miliary tuberculosis during past nine months period. Brain MRI revealed intracranial tuberculoma and brain edema but not involved optic nerve. Ophthalmic examination revealed severe papilledema and splinter hemorrhage with bitemporal hemianopsis and central scotoma. This finding was strongly suggested of optic disc tuberculoma. Her symptoms became much better following repeated retrobulbar steroid injection with continuous anti-tuberculosis agents. We report a interesting case with intracranial tuberculoma and optic disc tuberculoma associated by miliary tuberculosis during anti-tuberculous treatment.

The Release of Albumin from PLGA and PCL Wafers Containing Natural and Synthetic Additives for Protein Delivery (단백질 전달체로서 천연 및 합성재료의 첨가에 따른 PLGA와 PCL웨이퍼로부터 알부민의 방출거동)

  • PLGA and PCL copolymers initiated by carbitol as drug carriers were synthesized by ring-opening polymerization of L-lactide (LA), glycolide (GA), and $\varepsilon-caprolactone(\varepsilon-CL)$. Implantable wafers were simply fabricated by direct compression method after physical mixing of copolymers and bovine serum albumin-fluorescein isothiocyanate (BSA-FITC) as a model protein drug. The release amounts of BSA-FITC from wafers were determined by fluorescence intensity using the fluorescence spectrophotometer. Also, the release behavior of BSA-FITC on wafers was controlled by adding the additives such as collagen, small intestinal submucosa (SIS), poly(vinyl pyrrolidone) (PVP), and poly(thylene glycol) (PEG). The wafer prepared by PLGA and PCL exhibited slow release within $10\%$ for 30 days. But, those prepared by a variety of additives exhibited the controlled BSA release patterns with a dependence on the additive contents. furthermore, the wafers containing natural materials such as collagen and SIS showed more zero-order release profile than that with synthetic materials such as PVP and PEG. It was confirmed that the release of BSA from implantable wafers could be easily controlled by adding natural additives.
