Triage Score as a Predictor of need for Tertiary care Center Transport from Scene by Helicopter (소방헬기를 이용하여 직접 내원한 외상환자의 분석: 3차 의료기관으로의 이송의 적절성 평가)

  • Purpose: The number of patients transported by the Helicopter Emergency Medical Service (HEMS) has increased recently. In our review of the Korean HEMS, there was no established helicopter utilization criteria or triage tool on the scene, so many patients with minor injuries were transported to tertiary care centers. The aim of this study is to evaluate the percentage of patients with minor injuries and to propose a more appropriate triage tool for predicting the need for transport to a tertiary care center. Methods: The subjects of this study were 59 trauma patients transported to Asan Medical Center (AMC) from the scene by Seoul HEMS from January 2004 to December 2005. The Triage score (TS), Injury Severity Score (ISS), and modified Canadian Triage and Acuity Scale (mCTAS) were calculated as severity scales. Patients with minor injuries were defined as those with TS=9, ISS${\leq}15$, and mCTAS${\geq}3$. We evaluated the association of TS, ISS, and mCTAS with the appropriateness of transport. Results: Many of the patients transported to tertiary medical centers were classified as having a minor injury: TS=9 group 35 cases (72.9%), ISS${\leq}15$ group 30 cases (62.5%) and mCTAS${\geq}3$ group 27 cases (56.2%). However, 56.2% (27/59) of the patients were appropriately transported according to need for admission or an operation. The more severely injured patients classified by TS, ISS, and mCTAS were more appropriately transported to a tertiary center (p<0.05). Conclusion: Many patients with minor injuries were transported to a tertiary center from the scene directly. The TS can be easily calculated by an emergency medical technician at the scene. Thus, we propose the TS as a useful triage tool for determining the necessity of transport to a tertiary center by helicopter.

  • The aircraft manufacturing industry is a profitable business and has the potential to be an important export business. Our trade deficit needs to be improved in aircraft manufacturing part. Our national aircraft industry, which has been recognized for its quality in many domestic and international Air Shows, has developed the T/A-50 aircraft from the origins of the KT-1. With the ongoing development of the next generation fighter planes and helicopters, business for the next generation domestic air cargo has bright prospects. Judging by the history of the overseas air force dispatch during the Vietnam, Gulf, and Iraqi wars whose main mission was air transportation, there will be many opportunities to show the excellence of Korean air cargo in the future. The purpose of this study is to discover the optimal scale of air cargo development using the AHP method. The authors evaluated the scale of air cargo in size and capacity based on the following 5 standards used Korean developing airliner: usability, economics, technology, safety, and environmental friendliness. Then, the authors modified specific standards suitable for air cargo based on literature review Decision-making groups were chosen for suitability of expert staff in charge because they could represent for leading opinions in this specialized area especially as in practical aspects. The participants of the evaluation were pilots, transportation officers and management staff in the KAI who have experienced over 5years.

A Study on the Utilization of Drone for the Management of Island Areas in Marine National Park - Focusing on Drone Type and Arrivals in Island - (해상국립공원 도서지역 관리를 위한 드론의 활용에 관한 연구 - 드론 유형과 입도객 파악을 중심으로 -)

  • The purpose of this study was to obtain information about the type of drones suitable for the management of entrants and entrants of islands in the marine national park. The research sites were 25 islands in the Hallyeohaesang National Park. The target islands were divided into three zones, and were investigated with different types of drones. The survey period was from October to November, 2019. As a result of the operation of drone airframe, drone with fixed wings was found to be favorable for the management of marine parks in medium and long distances compared to other types, but stopping flights for broadcasting was found to be unsuitable. Drone with rotational wings was found to be suitable for image acquisition and broadcasting through close flight. However, it was deemed suitable for short and medium distance flights because of the fast battery consumption. In the case of helicopter rotorcraft drone, image acquisition and broadcasting were possible, but noise and vibration caused by propellers were disadvantageous. The number of entrants to the islands totaled 410 and the main act was fishing. The proportion of entrants to the islands in Area A was higher than that of other areas, and thus it was deemed more necessary to manage the area. Broadcasting was found to have had a positive effect on the management of fishers.

  • This Study has developed the information system of the rivers based on 3D image GIS by converging the latest information technology of GIS(Geographic Information System), RS(Remote Sensing), GNSS(Global Navigation Satellite System), aerial laser survey(LiDAR) with real time network technology in order to understand the current situation of all the four major rivers and support the administrative management system. The said information system acquires the high resolution aerial photographs of 25cm, aerial laser survey and water depth surveying data to express precise space information on the whole Youngsan River which is the leading project site out of the four river sites. Monitoring the site is made available on the transporting means such as a helicopter, boat or a bus in connection with locational coordinate tracking skill for the moving objects in real time using GNSS. It makes monitoring all the information on the four river job sites available at a glance, which can obtain the reliability of the people to such vast areas along with enhancing the recognition of the people by publicity of four Rivers Revitalizing Project and reports thereof.

Strength Evaluation of High-Strength Concrete Specimens within Reinforcing Bars (철근이 포함된 고강도 콘크리트 공시체의 강도평가)

  • Recently, the safety issue of high-rise concrete buildings damaged by fire, helicopter collisions, earthquakes, and faulty construction has attracted a great deal of interest. It is essential to know the strength of the concrete in order to accurately evaluate its safety for the reinforcement of these buildings. The core drilling method is considered to be the most effective method of assessing the compressive strength of concrete. However, it is very difficult to retrieve the core without the reinforcing bars, because buildings made with high-strength concrete are overcrowded with reinforcing bars. These reinforcing bars are often present in the core specimens, but there are few research studies and no regulations concerning the assessment of the strength of the concrete for high-strength core specimens within reinforcing bars. The purpose of this study is to investigate the effects of the reinforcement arrangement on the strength of the concrete and to present the quantitative values. To complete this research, the compressive strengths of different types of concrete with two different strengths (40 MPa and 60 MPa), two reinforcing bar diameters (10 mm and 12 mm), and 15 types of specimens with or without reinforcement arrangements were prepared and tested. As a result, the strength of the cylinders whose volume is less than or equal to the reinforcement volume of $53.1cm^3$ (about 4 - 13 mm) was predicted to have a low value of up to 60% of the strength of the cylinders without reinforcement.

A Study on Restricted Category Type Certification Procedure of Surion Derivatives Rotorcraft (수리온파생형 회전익항공기 민간 제한형식증명 획득 절차에 관한 연구)

  • For operating military aircraft, military certificate of airworthiness (MCA) must be obtained from military authority. Among procedures of general airworthiness certification, there is a military type certification process that aircraft design complies with military airworthiness certification criteria. The Surion is the first military rotorcraft which has obtained military type certificate, production validation and airworthiness certificate in Korea, and the Surion derivatives for special mission are being operated for government services. Based on Aviation Safety Act, in order to operate the Surion derivatives (military aircraft) for special purpose (such as emergency patient transportation and firefighting), the issuance of special airworthiness certificates was needed from civil airworthiness authority, and the restricted category type certification (RTC) is one of design approvals for special airworthiness certification to be streamlined. This study discussed the procedures for acquiring RTC for special purpose operation of the Surion derivatives classified as military derived aircraft, and suggested procedural ideas to improve Korean RTC system.

  • Helicopter rotor systems are dynamically loaded structures with many composite components such as the main and the tail rotor blades. The fatigue properties of composite materials are extremely important to design durable and reliable helicopter rotor blades. The safe-life methodology has generally been used in the helicopter industry to substantiate dynamically loaded composite components. However, it cannot be used to evaluate the strength reducing effects of flaws and defects that may occur during manufacturing and operational usage. The damage tolerance methodology provides a proper means to overcome this shortcoming; however, it is difficult to economically apply it to every composite component. The flaw tolerant methodology is an equivalent option to the damage tolerance methodology for civil and military rotorcraft. In this study, the flaw tolerant safe-life evaluation is described and illustrated by means of successful application to substantiate the retirement time of composite rotor blades.

Historical Review for the Government Contractor Defense (Government Contractor Defense(정부계약자항변)에 대한 연혁적 고찰)

  • A significant rise in product-liability cost is expected due to the newly passed product liability amendment Bill approved during the assembly plenary session on March 30, 2017. Korean government legal service(KGLS) filed a damage suit against Korea aerospace industries, Ltd.(KAI) and Hanwha Techwin Co., Ltd., the manufactures of the KUH-1 Surion helicopter crashed. KGLS alleged claims under the product liability Act, the warrant liability Act and the non-performance of contract act. The accountability limits of military aircraft manufacturers was a highly divisive issue among related scholars and legal practitioners. The bottom line was that military aircraft manufacturers had no product-liability insurance available. The United States courts have, therefore, developed the government contractor defense(GCD) and it was recognized by the U.S. Supreme Court in Boyle v. United Technologies corporation(1988). product liability insurances for military aircraft manufacturers are excessively expensive and it cannot be added onto the military procurement cost accounting. However, having an aircraft accident without one can be ruinously expensive. Therefore, the manufacturers should promptly set up appropriate risk management measures. This thesis will first review the advance GCD theory, and then find a way to either reform government contract related regulations.

Physician-staffed Helicopter Transport for Mountain-rescued Emergency Patients: a Pilot Trial (의사 탑승 헬기를 이용한 산악 응급 환자 이송: 시범 연구)

  • Purpose: We aimed to compare the transport time, the proportion of direct hospital visit and the emergency procedures between the current mountain rescue helicopter emergency medical service (HEMS) and physician-staffed mountain-rescue HEMS. Methods: During weekends from October 2, to November 21, 2010, 9 emergency physicians participated as HEMS staff in the mountain-rescue HEMS program of the Seoul fire department. Patient demographic data, transport time, proportion of direct hospital visits, and emergency procedures were recorded. We also collected data on HEMS mountain-rescued patients from June 1, to September 1, 2010, and we compared them to those for the study patients. After an eight-week trial of the HEMS, we performed a delphi survey to determine the attitude of the physician staff, as well as the feasibility of using a physician staff. Results: Twenty-four(24) patients were rescued from mountains by physician-staffed HEMS during the study period, and 35 patients were rescued during the pre-study period. Patient demographic findings were not statistically different between the two groups, but the transport time and the emergency procedures were. During the study period, the time from call to take-off was $6.1{\pm}4.1min$ (vs. $12.1{\pm}8.9min$ during the pre-study period, p-value=0.001), and the time from call to arrival at the scene was $15.0{\pm}4.8min$ (vs. $22.3{\pm}8.1min$ during the pre-study period, p-value=0.0001). The proportions of direct hospital visit were not different between the two groups, but more aggressive emergency procedures were implemented in the study group. The delphi survey showed positive agreement on indications for HEMS, rapidity of transport and overall satisfaction. Conclusion: A pilot trial of physician-staffed HEMS for mountain rescue showed rapid response and more aggressive performance of emergency procedures with high satisfaction among the attending physicians.
