Anti-platelet Effect of Carvacrol Extracted from Thuja Orientalis L.;A Possible Mechanism Through Arachidonic Acid Pathway (백자인에서 추출된 Carvacrol의 항혈소판 효과)

  • 목적 : 백자인에서 추출된 Carvacrol이 혈소판 활성화와 혈액 응고에 미치는 영향에 대해 알아보고자 하였다. 방법 : Carvacrol의 항혈소판 효과의 기제를 밝히기 위해 토끼 혈소판으로 Arachidonic Acid 유리,TXB2, PGD2, 12-HETE의생성을 방사선 크로마토그래피 분석을 사용하여 측정하였다. 결과 : 1. U46619를 제외하고 Collagen과 AA에 의해 유발된 응고는 Carvacrol 농도에 따라 억제되었다. 2. Collagen으로 인하여 자극된 AA 유리에 대한 Carvacrol의 유의한 억제 효과는 나타나지 않았다. 3. AA로 유발된 TXB2, PGD2와 12-HETE의 생성억제에 대한 실험에서 Carvacrol은 유의한 억제가 있는 것으로 나타났으며,농도의존적으로 억제되었다. 결론 : Carvacrol은 항혈소판 작용이 있는 것으로 볼 수 있다. 이는 한의학에서 활혈거어 작용으로 해석될수 있으며,타박상,윌경곤란증,탈모증 등 어혈질환의 예방 및 치료와 관련된 약침개발에 기초가 될수 있을 것으로 사료된다.

  • 항혈전성 약물로서 그의 항 혈소판 작용력은 dipyridamole보다 강하나 심혈관계 등에 대한 부작용이 적어 clinical efficiency가 유의하게 높은 약물개발에 대한 연구는 임상적 응용성 뿐 아니라, 혈소판-응고기전의 규명에 기여할 것으로 사료되는 바 본 연구에서는 cyclic nucleotide phosphodiesterase(PDE-I)들의 항혈소판 작용을 검토하여 그들의 혈관 내피세포와 혈관평활 근세포의 중식에 대한 영향을 항혈소판성 작용을 보이며 혈관세포들의 증식에 없어서는 안되는 spermine의 그것과 비교 검색하였다. Johnson 등(1985)의 방법에 따라서 제조한 aequorin부하-가토혈소판의 thrombin(0.25 units: TB)에 대한 응집반응에서, pyridazinone 유도체인 KR30075, sodium nitroprusside(SNP), imazodan, isobutylmethylxanthine(IBMX), rolipram, 및 spermine의 응집억제성 $IC_{50}$/ (M)은 각각 2.21 $\times$ $10^{-7}$, 1.26 $\times$ $10^{-6}$, 6.96 $\times$ $10^{-6}$, 7.78 $\times$ $10^{-6}$, 8.11 $\times$ $10^{-4}$, 및 4.28 $\times$ $10^{-3}$ M으로써 이들은 TB-응고반응에 동반되는 혈소판 [Ca$^{++}$]$_{i}$-증가에 대한 각각의 $IC_{50}$/과 차이를 보이지 않았으며, 유의한 상관성을 보였다.

The Effects of Anti-Thrombotic Activities and Cardiovascular Improvement of Fermented Garlic Extracts (발효마늘 추출물의 항혈전 및 심혈관개선 효과)

  • The purpose of this study was to investigate the effects of Anti-Thrombotic Activities and Cardiovascular Improvement of Fermented Garlic Extracts. The incidence of cardiovascular diseases (CVDs) is increasing rapidly in developed countries, with CVDs now representing the leading cause of morbidity and mortality. Natural products and ethnomedicines have been shown to reduce the risk of CVDs. Garlic is a medicinal plant used throughout the world for its anti-inflammatory, antioxidant, and antiplatelet activities. We hypothesized that fermented preparations of these products may possess stronger antiplatelet effects than the non-fermented forms owing to the increased bioavailability of the bioactive compounds produced during fermentation. Therefore, we compared these compounds via in vitro and ex vivo platelet aggregation assays by using standard light transmission aggregometry and ex vivo granule secretions from rat platelets. We found that fermented preparations exerted more potent and significant inhibition of platelet aggregation both in vitro and ex vivo. Likewise, ATP release from dense granules of platelets was also significantly inhibited in fermented preparation-treated rat platelets compared to that in non-fermented preparation-treated ones. We concluded that fermented preparations exerted more potent effects on platelet function both in vitro and ex vivo, possibly as a result of the increased bioavailability of active compounds produced during fermentation. We therefore suggest that fermented products may be potent therapeutics against platelet-related CVDs and can be used as antiplatelet and antithrombotic agents.

Phosphodiesterase-억제제와 항우울성 약물들의 Thrombin성 혈소판-응집반응에 미치는 억제작용에 관한 연구

  • 항우울약인 rolipran(RP)등 phosphodiesterase-억제 약(PDE-1)들이 thrombin(TB: 0.25 U/ml)에 의한 혈소판 응집에 미치는 작용을 가토-혈소판에서 일차 검토하였다. 신 PDE-1인 KR-30075(KR)의 $IC_{50}$/은 sodim nitroprusside의 것보다 낮았고 PDE-1들은 혈소판내 cAMP와 cGMP를 증가시켰으며 특히 KR은 타 PDE-1와 달리 I $P_3$를 감소시켰다. 아울러 rolipram은 cGMP와 I $P_3$를 증가시켰으나, amitriptyline(AT), sertraline(57), chlorpromazine(CP) 및 spermine은 I $P_3$를 증가시켰다. 그러나 이들과 PDE-1들은 강도의 차이는 있으나 모두 TB에 의한 혈소판응집을 모두 억제하였다. 따라서, PDE-1 중 IBMX(2$\times$$10^{-5}$M), KR(5$\times$10$_-7/M), 및 rolipram(10$_{-3}$M) 그 외에 항우울약인 AT(1.5 $\times$10$_-4/M) 와 ST(10$_-4/M) 및 항정신병약인 CP(10$_-4/M)둥이 혈소판내 I $P_3$, [C $a^{++}$], Tx $B_2$, 및 PG $I_2$ 함량과 단백-인산화의 TB에 의한 변동에 미치는 영향을 검토하였다. 그 결과 TB에 의한 혈소판내 I $P_3$, [C $a^{++}$], Tx $B_2$, 및 PG $I_2$ 함량의 증가가 PDE-1들과 항우울약들에 의하여 억제되었다 단, 항우울성약들과 CP는 정상 혈소판 I $P_3$를 증가시켰다. 아울러 혈소판-단백인산학에서 TB는 41-43 kD와 20 kD의 인산화를 현저히 증가시키며 19 kD의 인산화는 감소시켰고, PKC의 기질인 41-43 kD와 20 kD의 단백인산화가 PDE-1들과 항우울약들 뿐 아니라 CP에 의하여 현저히 억제되었다. 단, 20 kD 인산화에 대한 AT의 억제작용은 미약하였고, cAMP와 PDE-1들은 22kD 인산화를 증가시켰다. AT, ST, 및 CP는 A23187에 의한 41-43 kD 인산화는 현저히 억제하나 20 kD 인산화에는 영향을 미치지 않았고, PMA(3.2$\times$$10^{-7}$ M)에 의한 단백인산화에 대하여는 더 미약한 억제-효과를 나타내었다. 이상의 결과는 PDE-1과 항우울약들의 항혈소판작용은 PKC-기질인 41-43 kD와 20 kD의 인산화를 억제함에 기인되는 것으로 사료된다.다. 것으로 사료된다.다.

Cudrania Tricuspidata root extract (CTE) has an anti-platelet effect via cGMP-dependent VASP phosphorylation in human platelets (꾸지뽕나무 뿌리 추출물의 cGMP에 의한 VASP 인산화 기전을 통한 항혈소판 효과)

  • Cudrania tricuspidata has been reported to have many biological activities, including anti-inflammatory, anti-cancer, and antioxidant properties. However, the effects of C. tricuspidata root extract (CTE) on human platelet aggregation induced by collagen as well as the signaling pathways involved remain unknown. In the present study, we investigated the effect of CTE on human platelets. CTE inhibited platelet aggregation via down-regulation of thromboxane A2 (TXA2) by blocking cyclooxygenase-1 (COX-1) activity and intracellular Ca2+ mobilization in collagen-induced platelets. CTE also reduced the phosphorylation of phospholipase C (PLC) γ2 and syk. CTE regulated platelet aggregation via cyclic guanosine monophosphate (cGMP)-dependent phosphorylation of vasodilator-stimulated phosphoprotein (VASP) Ser239. In addition, administration of CTE (50 and 100 mg/kg) significantly reduced hyper-aggregated platelet aggregation by collagen (5 ㎍/mL) without hepatotoxicity in HFD (high fat diet)-fed rats. Taken together, these results suggest that CTE has anti-platelet effects both in vitro and in vivo. Therefore, CTE may be an effective therapeutic and preventive agent for cardiovascular disease, and is a safe and natural product.

The Effect of Second-Generation Antipsychotics Administration for 1-Year on the Platelet Activities in Patients With Schizophrenia: Retrospective Study (조현병 환자에서 1년간의 2세대 항정신병약물 지속적 투여가 혈소판 활성도에 미치는 영향: 후향적 연구)

  • Objectives : This study was conducted to investigate the effect of 1-year administration of second-generation antipsychotics (SGAs) on the platelet activity in patients with schizophrenia through a retrospective review of the medical records. Methods : The mean platelet component (MPC) value was used as an index of the platelet activity. The included subjects (N=24) were the patients who were confirmed to have taken SGAs continuously for one year after the first MPC measurement had been performed. The change of MPC was verified through a paired sample t-test. Results : The result revealed that the mean MPC value was significantly decreased from 26.5±1.4 g/dL to 25.6±1.8 g/dL after 1-year administration of SGAs from the time of the first MPC measurement. Conclusions : This study suggests that 1-year administration of SGAs may be related with increased platelet activity, and that close monitoring for risks such as cerebrovascular/cardiovascular or thromboembolic diseases may be necessary during SGAs treatment in clinical practice.

Antioxidation, Antimicrobial and Antithrombosis activities of natural products (천연물 추출물의 항산화, 항균 및 항혈전 활성에 관한 연구: 기능성 소재로서의 가능성)

  • In this experiment, we evaluated the anti-microbial, anti-oxidant and anti-platelet activities using 70% ethanol extract of natual products, Natural products (Lycium chinense, Aralia cordata, Achyranthes japonica, Mentha canadensis, Eriobotrya japonica, Belamcanda chinensis, Phlomis umbrosa, Lespedeza cuneata, Kalopanax septemlobus, Nelumbo nucifera) were extracted with 70% ethanol, freeze-dried, and used in the experiment. All natural products exhibited antibacterial activities, and Aralia cordata, Achyranthes japonica, Eriobotrya japonica, Belamcanda chinensis, and Nelumbo nucifera showed strong antimicrobial effect. Next, we investigated the antioxidant activity, polyphenol and flavonoid content and antiplatelet activity of the natural products. As a result, Aralia cordata, Achyranthes japonica, Eriobotrya japonica, Belamcanda chinensis, and Nelumbo nucifera showed antiplatelet effect and Ca2+ inhibitory activity. Therefore, it was confirmed that the five natural product extracts have a potential to prevent antibacterial, antioxidant, and antiplatelet-related diseases, and it is suggested that they can be developed as materials for functional foods and pharmaceuticals in the future.

Effects of Aralia continentalis Kitagawa on Antiplatelet and Antioxidative Activities. (독활의 항혈소판 및 항산화 효과)

  • The dried roots of Aralia continentalis Kitagawa is known to have the potential for anti-inflammation and anti-rheumatic, but their effects on thrombosis are not clear. In this study, we evaluated the anti-platelet and antioxidative activities of A. continentalis Kitagawa. Methanol extract and its various fractions of A. continentalis Kitagawa inhibited ADP-induced platelet aggregation, and EtOAc fraction showed strongest inhibition in a concentration-dependent manner with a $IC_{50}$ value of 217.7 ${\mu}g/ml$. Moreover, the EtOAc fraction contained 77.7% of 1,1-diphenyl-2-picrylhydrazyl(DPPH) and 43.1% of 2,2'-azinobis (3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) radical-scavenging activity at 0.1 mg/ml. In addition, the methanol and EtOAc fraction dose-dependently inhibited thrombin-stimulated platelet adhesion to collagen or fibrinogen. Collectively, these results suggest that the polyphenol-rich EtOAc fraction from A. continentalis Kitagawa can reduce platelet hyperactivation by scavenging free radicals. Thus, the EtOAc fraction of A. continentalis Kitagawa is a potential source for inhibition of platelet-dependent thrombosis.

Does a Preoperative Temporary Discontinuation of Antiplatelet Medication before Surgery Increase the Allogenic Transfusion Rate and Blood Loss after Total Knee Arthroplasty? (항 혈소판 제제의 술 전, 일시적 중단은 슬관절 전치환술 이후의 실혈량 및 동종수혈의 필요성을 증가시키지 않는가?)

  • Purpose: The aim of this study was to determine if preoperative temporary discontinuation of antiplatelet medication (aspirin, clopidogrel, or cilostazol) is a safe procedure that does not increase early postoperative bleeding and allogenic blood transfusion after a total knee arthroplasty. Materials and Methods: A retrospective analysis was conducted among consecutive patients who underwent navigation assisted primary total knee arthroplasty performed by a single surgeon, from January 2013 to December 2016. A total of 369 patients enrolled in this study were divided into two groups, 271 patients with no history of antiplatelet therapy and 98 patients who underwent 7 days of temporary withdrawal of antiplatelet therapy. Comparative analysis between the two groups, on the variation of hemoglobin and hematocrit during the first and second postoperative days, was conducted to determine the amount of early postoperative bleeding and the frequency of allogenic blood transfusion during hospitalization. Results: The variation of hemoglobin, hematocrit during the first and second postoperative days and the frequency of allogenic blood transfusion between no history of antiplatelet medication and discontinuation antiplatelet medication before 7 days from surgery were similar in both groups. Of the 369 patients, 149 patients received a blood transfusion during their hospitalization. Compared to patients who did not receive a blood transfusion, those who did received blood transfusion were significantly older in age, smaller in height, lighter in weight, and showed significantly lower preoperative hemoglobin and hematocrit values. No statistically significant differences in sex, preoperative American Society of Anesthesiologists scores, and the history of antiplatelet medication until 7 days prior to surgery were observed between the two groups according to blood transfusion. Conclusion: Compared to patients with no history of antiplatelet medication, the temporary discontinuation of antiplatelet medication 7 days prior to surgery in patients undergoing antiplatelet medication did not increase the amount of postoperative bleeding or the need for allogenic blood transfusion.

Inhibitory Effects of Scopoletin in Collagen-induced Human Platelet Aggregation (콜라겐으로 유도한 사람 혈소판 응집에 미치는 Scopoletin의 억제 효과)

  • Platelet aggregation is essential for the formation of a hemostatic plug in the case of blood vessel damage. On the other hand, excessive platelet aggregation may cause cardiovascular disorders, such as thrombosis, atherosclerosis, and myocardial infarction. Scopoletin, which found in the root of plants in the genus Scopolia or Artemisia, has anti-coagulation and anti-malaria effects. This study examined the effects of scopoletin on human platelet aggregation induced by collagen. Scopoletin had anti-platelet effects via the down-regulation of thromboxane $A_2$ ($TXA_2$) production and intracellular $Ca^{2+}$ mobilization ($[Ca^{2+}]_i$), which are aggregation-inducing molecules produced in activated platelets. On the other hand, scopoletin increased both the cyclic adenosine monophosphate (cAMP) and cyclic guanosine monophosphate (cGMP) levels, which are known as intracellular $Ca^{2+}$-antagonists and aggregation-inhibiting molecules. In particular, scopoletin increased the potently cAMP level more than cGMP, which led to suppressed fibrinogen binding to ${\alpha}IIb/{\beta}_3$ in collagen-induced human platelet aggregation. In addition, scopoletin inhibited collagen-elevated adenosine triphosphate (ATP) release in a dose-dependent manner. The results suggest that aggregation amplification through granule secretion is inhibited by scopoletin. Therefore, scopoletin has potent anti-platelet effects and may have potential for the prevention of platelet-derived vascular diseases.
