Synergistic Antibacterial Activity of Fig (Ficus carica) Leaves Extract Against Clinical Isolates of Methicillin-resistant Staphylococcus aureus (병원내에서 분리된 메티실린내성 황색포도상구균에 대한 무화과잎 추출물의 항미생물효과)

  • Fig (Ficus carica L.) belongs to the mulberry tree (Moraceae) which is one of the oldest fruits in the world. It has been used as a digestion promoter and a cure for ulcerative inflammation and eruption in Korea. The present study investigated the antimicrobial activity of methanol (MeOH) extract of fig leaves against methicillin-resistant Staphylococcus aureus (MRSA) isolated in clinic. The MeOH extract (MICs, 2.5 to 20 mg/mL; MBCs, 5 to 20 mg/mL) was demonstrated as antibacterial activity in isolates MRSA 1-20. The administration of the MeOH extract in combination with oxacillin or ampicillin induced a reduction of ${\geq}$4-8-fold in all tested bacteria, which was considered to be synergistic based on a FICI of ${\leq}$0.375-0.5. Furthermore, time-kill study was found that a combination of MeOH extract with oxacillin or ampicillin produced a more rapid decrease in the concentration of bacteria CFU/mL than MeOH extract alone. The results suggest that fig leaves could be employed as a natural antibacterial agent in MRSA infection care products.

  • Toxic shock syndrome is an acute disease involving multiple organs. I described here a case of TSS associated with intranasal packing. Four major criteria, which are involvement of multiple organ systems, fever, hypotension or shock, and rash with subsequent desquamation should be fulfilled for the diagnosis. The exact pathogenesis is not well understood, however it is thought to be due to the effects of an enterotoxin produced by certain strains of staphylococcus aureus.

  • Tooth avulsion usually causes inflammatory root resorption and ankylosis, and ankylosis cause severe functional and esthetic problems, especially in childhood. A 7-year-old female visited the Dept. of Pediatric Dentistry, Yonsei University with the chief complaint of avulsive trauma to the upper right incisor which was left dry for 40 minutes. Tooth was irrigated with saline and replanted immediately and splinted. Anti bacterial agent and anti inflammatory agent were prescribed. After 4 months of replantation slight external root resorption and apical radiolucency was seen at radiographic examination, therfore pulp extirpation and calcium hydroxide($Vitapex^{TM}$) canal filling were carried out. After 16 months, root canal was filled with gutta-percha, and bleaching treatment was done. Treatment results were satisfactory both esthetically and functionally for 8 years and 5 months.

  • Escherichiae-like organisms were isolated from rectal specimens of 56 children who were either in preschool age or in elementary school. The isolated strains were subjected to tests to screen enteropathogens producing heat-labile enterotoxin and susceptibility test to various antibiotics by disc diffusion method on agar plates. Production of heat-labile enterotoxin by the strains was assyed in the sensitive and reproducible cultured adrenal tumor cell system. The assay was sterodogenesis of the cell in the presence of heat-labile enterotoxin. Among 56 strains, gave positive reaction in the test of toxin production. This meant that about 10% of the children population objected to the study harbored the toxigenic strain of enteropathogenes. Some of these toxigenic strains were resistant to the antibiotics employed in the test. This study suggested that considerable population in Korea may harbor entertoxigenic E. coli as a part of intestinal normal flora. The toxigenic strains which are resistant to antibiotics may bring issue of public health in the future.

  • A patient with post-pneumonectomy empyema was treated sucessfully by modification of Clagett's operation after closure of bronchopleural fistula using a transsternal, transpericardial approach. His primary disease was pulmonary tuberculosis, and he had a past history of left upper lobe lobectomy 34 year ago. Recently recurred pulmonary tuberculosis with aspergilloma in the remaining left lung, empyema with bronchopleural fistula had developed on the post-operative 4th day after completion pneumonectomy. Closed thoracostomy was done at the lowest point of the left pleural cavity immediately. The pleural cavity was irrigated with small amount of normal saline through pigtail catheter. The 2nd operation was done by closure of bronchopleural fistula using a stapler through transsternal, transpericardial approach, and then the pleural space was irrigated with normal saline with Tobramycin which shows sensitivity to isolated organism from pleural cavity. After negative conversion of pleural fluid culture, we performed modified Clagett's operation under local anesthesia. The patient had no evidence of recurrence of empyema and discharged from hospital after 10 days of the 3rd procedure.

  • Primary sternal osteomyelitis is a rare disease. Primary sternal osteomyelitis occurring during childhood is extremely rare; therefore, only eleven cases have been reported in the English language literatures. The predisposing factors of primary sternal osteomyelitis are malnutrition, immune deficiency, intravenous injection, blunt chest trauma, and sickle cell anemia. Drainage of pus with antibiotic therapy is the treatment of choice. We report a case of primary sternal osteomyelitis occurred in a 16-year old boy, who had no predisposing factors, with review of literatures.

  • 개흉술에 의한 수술적 폐쇄방법 및 경도관 동맥관 폐쇄술은 동맥관 개존중의 치료방법으로 발전되어 왔으나, 이러한 술식은 개흉술에 의한 합병증, 지속적인 단락 및 용혈의 가능성, 기구의 이동이나 색전증의 발생, 지속적인 단락시 세균성 심내막염 방지를 위한 지속적인 항생제 사용등의 단점이 있어 저자들은 흉강경을 이용한 수술을 시행하였다. 총 6례의 환자에서 흉강경을 이용한 동맥관 개존증 수술을 시행하였으며. 이중 대동맥 외막(adventitia) 박리중 지형이 잘 되지 않았던 1례에서는 소개흉술(minithoracotomy)로 전환하였다. 술 후 이시행한 이학적 검사상에서 모든 환아에 심잡음이 소실되었음을 확인할 수 있었으며, 흉부 방사선 검사상 점진적인 폐혈관음영의 감소를 관찰할 수 있었으며 수술중 동맥관 파열이나 불완전한 동맥관 폐쇄, 기흉, 애성(hoarseness) 등의 합병증은 발생되지 않았다. 환아들은 술후 평균 3.4일째 퇴원하였으며, 퇴원후 외래추적 관찰검사시 시행한 심초음파 검사상 동맥관의 재개통이나, 잔류단락은 관찰되지 않았다. 본원에서는 개흉술 및 경도관 동맥관 폐쇄술의 단점을 방지할수 있으며, 성공적인 동맥관 폐쇄, 작은 피부절개 반흔에 의한 미용적 효과, 짧은 재원기간등의 장점을 가진 흉강경을 이용항 동맥관 결찰술을 시해앟여 문헌 고찰과 함께 보고하는 바이다.

Comparison Between Antimicrobial Susceptibility Test and mecA PCR Method for Reading of Methicillin-Resistant Staphylococcus aureus (메티실린 내성 황색포도알균 판독에 있어 항균제 감수성 검사와 mecA PCR법의 비교)

  • Methicillin-resistant Staphylococcus aureus (MRSA) is one of major pathogen causing hospital infection and several diseases such as purulent infection, bacteremia. The isolation ratio of MRSA is gradually increased up to 80% in the hospital, which makes a limitation for treatment of antibiotics because the isolated MRSA show resistance to methicillin as well as other antibiotics. This study proposes that mecA detecting methods which are not commonly used because of cost in the hospital is a more accurate method than Susceptibility Testing to detect a MRSA. We compared Staphylococcus aureus ATCC 29213 as a negative control and 20 MRSA strains isolated from patients by these two methods. We amplified mecA gene by polymerase chain reaction (PCR) and confirmed the PCR products by sequencing. All of the MRSA showed oxacillin and cefoxitin resistance whereas 85% (16/19) of the strains had mecA wildtype. These results suggest that some of the MRSA are mecA mutants therefore mecA genotyping reinforces the MRSA detection by antibiotic susceptibility test.

Mixture of Edwardsiella tarda specific Bacteriophage and Bacillus subtilis KM-1enhanced bactericidal activity against Edwardsiella tarda (Edwardsiella tarda의 특이 Bacteriophage와 Bacillus subtilis KM-1혼합액이 Edwardsiella tarda 에 미치는 항균효과)

  • The present study was performed to investigate an antibacterial activity of specific bacteriophage (phage) and Bacillus subtilis KM-1 (B. subtilis) mixture against Edwardsiella tarda (E. tarda). An appropriate number of phage showing the most effective antibacterial activity was $2{\times}10^5$ PFU/ml with $1{\times}10^7$ CFU/ml of B. subtilis 36 h post incubation. On the other hand, B. subtilis showed a dose dependant manner in inducing antibacterial activity in the presence of phage ($2{\times}10^5$ PFU/ml). The phage and B. subtilis mixture showed higher antibacterial activity against E. tarda than phage or B. subtilis only. These results suggest that the phage and B. subtilis mixture could be utilized as an alternative to antibiotics in the control of fish diseases caused by E. tarda.

Antimicrobial Effects of Essential Oils for Multidrug-Resistant Acinetobacter baumanii (다제내성 아시네토박터 바우마니의 에센셜 오일에 대한 항균효과)

  • Acinetobacter baumannii is categorized as a red alert pathogen that is increasingly associated with a high mortality rate in infected patients because of its resistance to extensive antibiotics. This study evaluated the antibacterial activities of some essential oils (tee tree, rosemary, and lavender oils) against 18 clinical isolates of multidrug-resistant A. baumannii (MRAB). The carbapenemase screening Hodge test showed that all 20 strains of A. baumannii were resistant to imipenem. The identification of multidrug-resistant microbes was carried out using the VITEK system. The antimicrobial activity of essential oils was tested by a disk diffusion method against MRAB. In the disk diffusion method, tea tree showed the largest increase in inhibition size compared to lavender oil, and rosemary had no antibacterial effect. These results proved the antimicrobial effect of multidrug resistance A. baumannii. Tee tree oil would be a useful alternative natural product for the treatment and prevention of most common human pathogens and MRAB infections. This is expected to be used as an antimicrobial agent, such as hand disinfectant using natural essential oil in the future.
