The Clinical Study of Clarithromycin for the Treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease (만성 폐쇄성 폐질환 환자의 급성 호흡기 감염에 대한 Clarithromycin의 임상 효과)

  • To evaluate the efficacy and safety of clarithromycin in acute exacerbation of chronic obstructive pulmonary disease, we administered clarithromycin(250mg) twice in a day in 30 patients with acute exacerbation of chronic obstructive pulmonary disease from September to November in 1996. Twenty eight cases of 30 patients were cured(93.4%) and 2 cases(6.7%) show clincal improvement. Three cases were improved within 3 days of treatment and 24 cases were improved between 5 days and 12 days of treatment. There were no significant side effects. These results suggest that clarithromycin will be effective as a first line therapy in patients with acute exacerbation of chronic obstructive pulmonary disease.

  • Kawasaki disease is generally diagnosed base on its clinical features. Sometimes unusual or atypical presentations make the diagnosis of Kawasaki disease difficult. We experienced an unusual case of Kawasaki disease presented with unilateral parotitis in a 23-month old girl. Despite of intravenous antibiotics treatment, fever and unilateral parotid swelling persisted. Skin rashes, conjunctival injections, and coronary abnormalities showed up on the 8th day of fever. After the intravenous immunoglobulin and salicylates treatment, all symptoms disappeared. Although unilateral parotitis is very unusual presentation of Kawasaki disease, in case of no response to antibiotics, Kawasaki disease should be included in the differential diagnosis.

Extended-Spectrum $\beta$-lactamase Genes Acquired Multidrug-Resistant Klebsiella pneumoniae in a Dog and Its Owner (개와 보호자에서 Extended-Spectrum $\beta$-lactamase 유전자를 획득한 다약제내성 Klebsiella pneumoniae)

  • A 2-year-old female Pomeranian dog was referred with multiple pelvic fractures. The surgical correction was performed for the fractures. However, after the surgery, purulent exudation was occurred in the surgical site. Antibiotic susceptibility test revealed that the isolated bacteria are resistant to penicillins, cephalosporins, aminoglycosides, quinolones, and trimethoprim/sulfamethoxazole. Bacterial identification and extended-spectrum $\beta$-lactamase (ESBL) confirming test indicated that the isolated bacteriae is ESBL-producing Klebsiella pneumoniae. Minimum inhibitory concentration (MIC) and maximum bactericidal concentration (MBC) tests revealed that meropenem, one of carbapenems, is the only effective antibiotic. The patient was treated with meropenem for 5 days. After 10 days, the exudation was disappeared and the infection was cured. The molecular typing of the ESBL revealed that TEM-1 ESBL is present in the bacteria isolated from the patient. The bacteria isolated from the owner's palm also revealed that TEM-1 and SHV-1 ESBLs are present.

Tricuspid Valve Endocarditis (삼첨판막 심내막염)

  • Some tricuspid valve endocarditis can be controlled effectively with specific antibiotic treatment. However, surgical intervention Is necessary when there are continuing sepsis, moderate or severe heart failure, multiple pulmonary emboli, and echocardiographycally demonstrated vegitations. We are repoting a 19 year-old male patient who was admitted for the treatment of infective endocarditis. He previously had an operation for ventriculer septal defect (perimembranous type) about 9 years ago . An echocardiogram showed a large vegetation on the anterior cusp area and a left to right shunt through VSD, which was previously closed with dacron patch. A valve replacement in addition to antibiotic therapy was recommended for the patient. The patient underwent on operation : tricuspid valve replacement was done with 51. Jude medical valve prosthesis (33 mm), and in addition to above procedure, removal of vegetation and direct closure of VSD were done Postoperative echocardiogram showed that replaced tricuspid valve functioned well and vegeta ion and shunt flow were not observed. The patient recovered without complication and discharged at Postoperative day 25. Early aggressive surgical intervention is indicated to optimize surgical results, and this case seems to be a typical right sided bacterial endocarditis, which is caused by residual VSD. We are reporting a case of tricuspid valve endocarditis with a review of the literature. (Korean J Thorax Cardiovasc Surg 1996 ; 29: 440-3)

  • A group of patients with Kawasaki disease (KD) initially present with cervical lymphadenitis or deep neck infection. These unusual KD presentations lead to unnecessary antibiotic therapy or surgical intervention, thereby delaying intravenous immunoglobulin treatment and increasing the risk of coronary artery damage. We present four KD patients whose initial presentations mimicked a retropharyngeal abscess. Nonsuppurative cervical lymphadenitis or suspected neck abscess unresponsive to intravenous antibiotics could signal the possibility of KD.

  • An 8-year-old spayed Yorkshire Terrier Dog was presented to the Veterinary Teaching Hospital of Kyungpook National University because of the recurrent superficial pyoderma. At the presentation, pustules and papules were present throughout the body. Numerous rods with a few cocci were observed on impression smears and they were confirmed to be Proteus mirabilis and Staphylococcus pseudointermedius consecutively. The patient was treated with systemic enrofloxacin and amoxicillin-clavulanic acid based on the results of antimicrobial sensitivity tests with once a week basis 4% chlorhexidine shampoo. An excellent clinical response was achieved in 2 weeks of therapy and the lesions were fully resolved in 6 weeks. The possibility of P. mirabilis infection should not be overlooked by clinicians in dogs with recurrent superficial pyoderma although it's been considered to be rare.

  • 70세 남자가 내원 7일전 발생한 우측 흉부 통증, 고열 및 가래를 동반한 기침으로 입원하였다. 백혈구와 CRP(C reactive protein)의 상승, 흉부 방사선 소견상 우하엽에 폐렴을 시사하는 소견이 관찰되어 지역 사회 획득성 폐렴에 준한 경험적 항생제를 투여하였다. 처음 두 번의 연속적인 가래 배양 검사에서 항생제 감수성이 있는 Acinetobacter baumannii균이 동정 되었으며 항생제 치료 후 임상 증상 및 방사선 소견이 점차 호전되었다. Acinetobacter baumannii는 중환자실에서 주로 발생하는 병원 획득성 폐렴의 주요 균으로 알려져 있으며 지역 사회 획득성 폐렴의 흔한 원인균은 아니다.

  • Chronic granulomatous disease in childhood is a rare inheritable disorder of phagocytic cells in which defective production of the reactive intermediates of oxygen predisposes the patient to severe recuring pyogenic infections. The lung is the most common site of infection and pulmonary disease is the primary cause of death for greater than 50% of children with chronic granulomatous disease. Although the role of surgery in management of this disease remains undefined, rapid diagnosis of the underlying pulmonary problem is crucial to determine the most appropriate antimicrobial therapy and surgical techniques such as lobectomy of involved areas lead to more rapid recovery and thus allow the antibiotics to be more efficacious in these cases. We have treated a one month old male baby who had the chronic granulomatous disease with pulmonary infection. Wide surgical resection of the affected lobe and use of antibiotics and antifungals were carried out with good clinical results. He was well after the operation.

Screening and Characterization of Probiotic Strains for Prevention of Bacterial Fish Diseases (어류의 세균성 질병 예방을 위한 Probiotic균주의 선발 및 특성)

  • The purpose of the present study was to screen the effective of lactic acid bacteria (LAB) as probiotics, which are able to protect aquacultural fish pathogenic bacteria, and investigate their characterization. Twenty strains of lactic acid bacteria were isolated from fish intestine, fermented fish foods and kimchis. These bacteria were screened for antagonistic activity against fish pathogenic bacteria. Seven tested LAB strains were able to inhibit the fish pathogenic bacteria, including Vibrio anguillarum, Edwardsiella tarda, and Streptococcus sp.. Of the probiotic candidates, BK19 strain isolated from fermented pollack viscera indicated the largest inhibition activity. Moreover, this strain showed a resistance over low pH and antibiotic agents. Therefore this probiotic candidate BK19 was finally selected and identified as a probiotic strain. This particular probiotic bacteria was identified as Lactobacillus sakei BK19 by biochemical characteristics and 165 rRNA PCR amplification.

Peptide isolated from Hermetia illucens larvae inhibits mice from Klebsiella pneumoniae infection in the kidney (동애등애유충에서 분리된 펩타이드의 신장에서의 폐렴간균 감염 억제 효능)

  • Overuse of antibiotics has significantly contributed to an increase in microbial antibiotic resistance, causing difficulties in the suppression of microbe-borne infectious diseases. In this study, we determined the anti-Klebsiella pneumoniae effect in the kidneys of mice induced by peptides isolated from H. illucens larvae. Mice were intranasally infected with a high dose of K. pneumoniae and 1 day later, peptides were introduced through the intramuscular route. Mice were sacrificed on day 10 upon K. pneumoniae infection to determine the bacterial loads in the kidneys. Mice receiving peptide treatment demonstrated significantly reduced bacterial loads, reduced bodyweight loss, and higher survival in a dose-dependent manner compared to control. These results indicate that peptide isolated from H. illucens larva inhibits K. pneumoniae infection in the kidney. The peptide from H. illucens larva could be a potential candidate for the development of an effective antibacterial drug.
