The purpose of this study is to analyze the program according to the local characteristics of the elderly welfare center and to present the development direction of the elderly welfare center in the future. The questionnaire was administered to 344 elderly welfare centers nationwide and 304 questionnaires were used for analysis. Based on the regional characteristics of the elderly welfare center, three types were derived and analyzed. The results of this study are as follows: First, the projects which showed difference according to the regional characteristics of the elderly welfare center were the functional recovery program, counseling, emotional support, and community welfare program. Second, it was found that the elderly welfare centers had different programs according to their areas. The results showed that the elderly welfare centers had different programs, group programs, self-help groups, beauty services, employment support education, economic education, retirement preparation education, home helper service, generation integration program, mobile welfare business and regional cooperation project. Based on these results, it is suggested that the development direction of the elderly welfare center is necessary to develop the program of the elderly welfare center according to the characteristics of the region and it is necessary to guarantee autonomy so that the elderly welfare center can operate the program.

One of the most important things in the researches on diseases is to attach more importance to prevention of a disease and preservation of health than to treatment of a disease, also to foods rather than to medicines. In this context, the most significant concern in examining a patient is to find the presence of disease, and, if any, to diaguose the type of disease, after which a pharmacotherapy is followed. In this paper, various diagnosis methods of Oriental medicines are discussed. And ocular inspection, the most important method among the 4 disease diagnoses of Oriental medicines, is studied. Observing a person's shape and color has been the major method for ocular inspection, which usually has been dependent upon doctor's intuition as of these days. We are developing an automatic system which provides objective basic data for ocular inspection. As the first stage, we applied the signal processing techniques to automatic feature extraction of faces for ocular inspection. Firstly, facial regions are extracted from the point of frontal view, which was followed by extraction of their features. The experiment applied to 20 persons showed that frontal face regions are perfectly extracted, as well as their features, such as eyes, eyebrows, noses and mouths. Future work will seek to address the issues of morphological operation for a few unfinished extraction results, such as combined hair and eyebrows.
Functional recovery of cerebrovascular accident (CVA) patients were studied by examining functional independence measure (FIM) to evaluate the functional state of the patients at admission to and at discharge from the hospital and its relationship with the family support. Study subjects consisted of 129 CVA patients, who were admitted and received rehabilitation treatment at K Medical Center of Oriental Medicine from August 3 to December 18, 1997. The results were as follows: 1) Total FIM score was $72.37{\pm}25.16$ at admission and $101.67{\pm}22.13$ at discharge. The difference of average score was 29.30, which was statistically significant by paired t-test. 2) The largest difference between FIM scores at admission and at clischarge was observed in items of walking and wheel-chair riding, and the smallest clifference in items of social interaction. 3) The recovery was faster with motor function than with cognitive function, because the difference of FIM scores at admission and at discharge was much larger with motor function. 4) Recovery was better in groups under age 49 than in groups above age 70. Functional recorvery was prominent especially in groups with normal sensory state and speech functions, and groups without urinary incontinence. Recovery was less significantly in patients with paraplegic patients hospitalized longer than 2 months, patients with family all the time, and patients with CVA over 11 days. 5) We could not find any relationship between functional recovery and family support. FIM scores were lower in groups of old age(r=-0.325), long stayed in hospital (r=-0.426), and long period of time after the onset of disease(r= -0.339) with a reciprocal correlation between FIM scores and these parameters. 6) Stepwise multiple regression analysis was done to evaluate factors to affect the recovery from CVA. FIM score at admission could explain 51.2 % of the functional recovery. Important factors were periods of hospitalization, state of sensory function, age, and education (listed in decreasing order of importance). In total, they could explain 64.89% of the functional recovery. These results indicate that functional recovery of CVA patients, who were admitted to oriental medicine hospital for rehabilitation treatment, could be estimated by measuring FIM scores. Recovery was significantly better at discharge from the hospital than at admission and motor function recovery rate was much faster than that of cognitive function. 2. Recommendation Based on these results, we recommend following further studies. 1) Comparative study of recovery of motor function and of sensory function would be necessary by measuring FIM scores once a week to evaluate the recovery of CVA patients. 2) It would be interesting to see whether there is any difference of functional recovery between patients treated with either western medicine or oriental medicine. 3) Psychological factors affecting the recovery of CVA patients need to be studied.
In the traditional medicine, the basidiocarps of Poria cocos (Fr.) Wolf naturally collected have been widely used as a traditional remedy for dropsy, diarrhea, gonorrhea and nervous disease more than 3500 years ago. It was recently found to be composed of the particular compounds having antitumor effects. Therefore, its demand has been gradually increased, whereas it is still dependent upon the natural harvest. The artificial cultivation of P. cocos was carried out to observe the morphological characteristics and to investigate the cultural characteristics of P. cocos isolates collected. The morphological characteristics of its basidiocarps was observed. The physiological aspects of its isolates were also investigated.
This was a qualitative study on medical aid patients to understand the cause and process of statistical difference of health service utilization between medical aid and health insurance patients. The main results were the following; 1) There was few overuse of health service in medical aid patients. The reason of heavy utilization was mainly due to the complicated disease. Some of them were considered to overuse physical therapy and oriental acupuncture. 2) In case of medical aid patients, medical cost was paid by their welfare benefit of government or by the support of family or neighbors. They usually could not adequately use the services of uninsured benefit or large hospitals due to the cost. Some patients just endured the pain. There was still discrimination for medical aid patients in some medical institutions. 3) The health officials and institutions did not provide sufficient information to medical aid patients about the policy of medical cost support. 4) Health policies, such as selective clinic system, medial aid case management, approval of extended care, were considered to contribute in preventing unnecessary use of health service. However, this might limit adequate use of medical aid service. In conclusion, there is little evidence of overuse of health service for medical aid patients, which is different from the previous studies. A new plan is necessary, because medical aid patients thought that the necessary health service was not accessible to them.
The purpose of this research is to examine the effect of Simjeongkyeok Sa-am acupuncture treatment for major symptoms of Hwa-byung. Method : In this randomized, single blind, placebo-controlled study, we compared Simjeongkyeok acupuncture with Sham acupuncture in the treatment for major symptoms of Hwa-byung. Likert scale for major symptom of Hwa-byung was measured as the 1st evaluative instrument, and STAXI-K, STAI-K, BDI-K and HRV were also measured as the 2nd evaluative instrument at the before treatment and after treatment. Results : In comparison of Likert scale for major symptoms, total score of after treatment decreased significantly at each point on both groups, but there was no significant difference between both groups. In comparison of STAXI-K, STAI-K, BDI-K, there was no significant difference in variation of score between both groups. But Simjeongkyeok group showed higher ratio variation of STAXI expression than that of Sham group, Also on subjects whose main complaint symptom were burning sensation and whose pattern identification were Qizhi, Simjeongkyeok group showed higher variation of Likert scale score and BDI-K than that of Sham group. The significance was border line around. Conclusion : We considered that Shimjeongkeok treatment will he likely to he recommended for treating Hwa-byung, especially on subjects in each group whose main complaint symptom were burning sensation or whose pattern identification were Qizhi. Also it may also be effective on the management of anger expression.
Objectives : Trend of R&D of herbal medicine for dementia treatment was examined based on the quantitative information analysis for establishing the national strategy of research on dementia treatment with oriental medicine. Methods : Definition was made to clarify the technology for development of herbal medicine for dementia treatment. Based on the initial keyword provided by experts in the field, queries were compounded to conduct search in the search engines of WoS and DWPI. The raw data (papers or patents) extracted from the initial search were examined by expert-review before objects of analysis were determined. Then, the accumulated data was analyzed in terms of year, country and organization, which led to examination of the trend of R&D. And the research performance evaluation for dementia treatment technologies was also made in terms of country, organization and researcher based on the forward citation analysis. The international cooperation intensity was examined on the basis of analysis of network by researcher before analysis results were put together to select lead researchers. Results : According to the quantitative information analysis of 1,330 articles that were selected as analysis objects, the number of papers on natural products research for dementia treatment has increased by around 4.6 times in recent five years. This indicates that the intensive studies have been underway recently. It was found to be the US that had the highest level in research filed of herbal medicine for dementia treatment and the highest capacity of international cooperation for that purpose. On the contrary, Korea had the share of papers at 5.1%, the number of countries in cooperation research at 8, and the article quality index at 0.40, showing that the qualitative level was insufficient, compared to the quantitative outcome. In particular, Korea was found to have no intensity of international cooperation among researchers. In case of patent, the results of information analysis of 305 patents selected as analysis objects demonstrated that China had the highest share while Korea had the very low frequency of patent application quantitatively. Conclusions : In this study, the research to develop herbal medicine for dementia treatment has recently drawn much attention that has spread around the globe. Therefore, these results suggest establishing the strategy to develop technology for dementia treatment with oriental medicine in the future based on quantitative information analysis.
Objectives: This study was conducted to evaluate the clinical effects of group therapy for Panic disorder patients based on Mindfulness & Li-Gyeung-Byun-Qi therapy. Methods: The FFMQ, BDI, STAI, STAXI, Panic attack, Anticipatory anxiety and subjective improvement of three Panic disorder patients were compared pre- and post-treatment when given Mindfulness & Li-Gyeung-Byun-Qi therapy. Results: 1) After the patient in case 1 underwent 5 weeks of group therapy for Panic disorder, the Mindfulness meditation score was slightly improved, anxiety and depression were significantly decreased, and expression of anger was also improved. In addition, the Panic attack and Anticipatory anxiety became more stable in the objective evaluation, while 'Extreme improvement' was shown in the subjective evaluation. 2) After the patients in case 2 and 3 underwent 5 weeks of group therapy for Panic disorder, Mindfulness meditation scores were slightly improved, anxiety and depression were significantly decreased, and expression of anger was also improved. In addition, the Panic attack and Anticipatory anxiety became more stable in the objective evaluation, while 'Moderate improvements' were shown in the subjective evaluation. Conclusions: As per the results in these cases, it was shown that group therapy for Panic disorder utilizing Mindfulness & Li-Gyeung-Byun-Qi therapy was effective to maintain meditation and control the emotions of anxiety, depression, anger and so on. Therefore, it was considered that expansion of clinical utilization through the standardization of a group therapy program for Panic disorder is needed. Furthermore, it was also considered that a comparative study of the effects of previous cognitive programs for Panic disorder according to the objectified and standardized manual is needed in the future.

Schisandra chinensis containing a variety of pharmacologically active lignans has been traditionally used in oriental medicine. In this study, anti-inflammatory compounds were screened from the hexane extracts of S. chinensis by activity-guided fractionation. First, we investigated the regulatory effects on the expression of E-selectin, intercellular adhesion molecule-1 (ICAM-1) and vascular cell adhesion molecule-1 (VCAM-1) with 38 fractions from the hexane extracts of S. chinensis in human umbilical vein endothelial cells (HUVECs). As a result, SCKH1 among the 38 fractions from the hexane extract of S. chinensis was selected for further analysis based on its unique regulatory effect on cell adhesion molecules, especially on VCAM-1, in LPS-stimulated HUVECs. The subsequent activity-guided fractionation of SCKH1 resulted in the purification of SCKH1PAIBPB, which was found to suppress the expression of VCAM-1, MCP-1, IL-6 and IL-8 in HUVECs stimulated with LPS, and to inhibit the adhesive capacity between HUVECs and monocytes. Taken together, our data indicate that SCKH1PAIBPB can be proposed as an effective anti-inflammatory compound that may have a potential therapeutic use for the prevention and treatment of various inflammatory diseases as well as ischemic vascular diseases.
The leaves of Selaginella tamariscina were used for the treatment of many diseases in traditional medicine. In the study, antioxidant, anti-inflammatory, and wound healing activities of the hot-water extract(STW) and 80%ethanol extract(STE) obtained from S. tamariscina were evaluated. As a result, the polyphenol content of STW and STE were 38.108±0.766 mg/g and 17.927±1.064 mg/g, respectively. The DPPH and ABTS radical scavenging activities with the IC50 values of the STW were over 2 times lower than that of the STE. In the MTT assay, RAW264.7 cell viability of two extracts was decreased by about 6% at 1 mg/mL, whereas for HaCaT cell viability increased by 18% at 50 ㎍/mL. In addition, STW and STE suppressed the production of nitric oxide(NO), Tumor-necrosis(TNF)-. COX-2 and PGE2 in lipopolysaccharide(LPS) induced RAW264.7 cells. Furthermore, the STE showed wound healing effect through the promotion of skin cell migration in TNF- stimulated human keratinocytes. These results indicated that the STW and STE have the potential to be used as a new cosmetic active ingredients in skin care.
