Quality Assurance of Gastric Cancer Surgery (위암 수술의 질 관리)

  • Quality assurance may be defined as the complete set of systemic actions that is required to achieve a better treatment result by standardizing treatment and by using various audit programs. In general, application of a quality assurance program in surgery is considered to be more difficult than it is in chemotherapy or radiotherapy. However, recently, the importance of quality assurance in the surgical field has been emphasized in clinical trials comparing different surgical procedures and evaluating the role of postoperative adjuvant therapy. In the case of gastric cancer surgery, excellent quality assurance programs have rarely been applied in most large prospective clinical trials. Although the quality assurance in Dutch trial was conducted very systemically and strictly, the situation is quite different from ours. On the other hand, several quality assurance programs in Japanese trials comparing D2 and D2 plus para-arotic lymph node dissection seem to be applicable to Korean clinical trials. Several factors, including selection of appropriate surgeons based on personal experience and annual number of operations, standardization of surgical procedures by education and consensus, development of a unified database program, application of standardized perioperative management, and standardization of pathologic examination, are required to guarantee a successful multi-institutional prospective clinical trial. In contrast, one needs to realize that protocols that are too strict and sophisticated can make the enrollment of patients and surgeons more difficult and can promote protocol violation during the clinical trials.   • 의료영상은 환자의 질병을 진단하고 치료방침을 결정하는데 중요한 도구로 자리매김하고 있다. 최근 의료영상을 인공지능 연구가 국내외에서 활발하게 진행되고 있다. 특히 대규모의 의료영상들을 학습시켜 질병과 상태를 정밀 진단할 뿐만 아니라 예측하는 소프트웨어를 개발 하는 상황이다. 그러나 의료영상은 DICOM 표준에 따르고 있지만 태그정보의 사용은 의료기기와 의료기관마다 상이하다. 따라서 의료영상에 대한 메타 데이터의 표준화에 어려움이 있다. 본 논문은 이러한 의료영상 데이터를 표준화 할 수 있는 방법을 제안한다. 그리고 제안한 표준화 데이터로 변환할 수 있는 ETL 소프트웨어의 수행결과를 보이고, 조건에 따라 머신러닝 학습 데이터셋을 생성하는 결과를 제공한다. 향후 제안한 의료영상 표준화와 ETL 소프트웨어는 다양한 수요자 중심의 표준화된 데이터셋을 제공할 수 있는 플랫폼의 주요기능으로 활용 될 것으로 기대한다.

  • 혀의 상태는 인체 내부의 생리적 병리적 특성의 변화를 나타내므로, 한의학에서 중요한 지수가 된다. 한의학에서 설진 방법은 환자의 설질과 설태의 변화를 관찰함으로써 질병을 진찰하는 방법이므로, 편리할 뿐만 아니라 비침습적이고, 널리 쓰이고 있다. 그러나 설진은 광원, 환자의 자세, 한의사의 상태와 같은 검사 환경에 의해 영향을 받는다. 표준화된 진단을 위한 자동 진단 시스템을 개발하기 위하여 질병의 예후를 판단할 수 있는 설태 분류 방법은 필수적이지만, 컬러의 경계가 모호하므로 설태와 설질을 구분하기는 매우 어렵다. 이 논문에서 분할된 설체 내에서 컬러를 계층적으로 분류하여 설태를 분류하는 방법을 제안한다. 또한 설태 영역을 정확하게 분할하도록 하였다. 제안된 방법은 표준화된 진단을 가능하도록 한다.

Improvement of customer satisfaction with "Explanation Nurse Unit" (설명간호사실 운영 활성화를 통한 외래 환자 만족도 증진)

  • 문제: 본원의 외래 상황은 진료의가 짧은 시간 내에 많은 환자를 진료해야 하고, 진료 환자 모두에게 충분히 만족하도록 설명해주는 것은 매우 어려운 실정이었다. 2007년의 의료기관 본 평가에 앞서 병원 자체 평가 결과 외래환자의 설명 만족도는 57점으로 저조하였다. 목적: "이해하기 쉽게 설명하는 서울대학교 병원" 이라는 슬로건 하에 설명간호사 제도를 운영하기 시작하여, 설명간호사실 운영 활성화를 통한 외래 환자 만족도를 향상시키고자 한다. 의료기관: 서울시 종로구에 소재한 대학병원 질 향상 활동: 설명간호사실 방문율 증가 및 one-stop service care을 실현할 수 있는 방안을 모색하였고, 검사 시행 이유, 귀가 후 주의사항 검사 결과, 검사일정 외래 진료 안내 브로셔 및 표준화된 교육 자료를 개발하였다. 또한 설명간호사의 역할 및 업무의 확립을 위한 업무기술서를 개발 및 설명간호사실 환경을 개선하였다. 개선효과: 설명간호사실의 방문율이 내과는 2배, 신경과는 3배 정도 증가하였다. 표준화된 교육 자료 개발로 설명 요구도를 충족시켰고, '환자의 알 권리'와 '설명을 들을 권리'를 보장하였다. 업무기술서 개발로 설명간호사의 업무 및 역할을 확립하였으며 설명간호사실의 환경 개선으로 개인 신상 비밀을 보호받을 권리를 보장하였다. 외래 설명만족도가 2007년 57점에서 2008년 74점으로 향상되었다. 설명간호사실 자체 설문조사에서도 97%가 만족한다고 답하였고, 100% 모두 재방문 의사를 보였다. one-stop service care를 제공함으로써 외래가 복잡하다는 부정적인 이미지를 긍정적인 이미지로 변화시켰다.

Effects and Development of Clinical Competency Evaluation using Standardized Patients among Nursing Students: Based on Abdominal Surgical Patients (표준화 환자를 활용한 임상수행능력 평가방법 개발 및 적용효과: 복부수술 후 환자간호)

  • Purpose: This study was to develop the clinical competency evaluation, and to examine the effects of the developed evaluation by comparing it with existing evaluation on clinical competency, communication skill, and self-efficacy of nursing students. Methods: Design was a randomized control group post-test design. The subjects were 102 senior nursing students(Experimental group: 48, Control group: 54) at K university in seoul, Korea. The experimental group took the clinical competency evaluation using standardized patients and the control group took the existing evaluation using a doll. The clinical competency and communication skills were measured by evaluators, and self-efficacy was self reported by the nursing students. Results: The experimental group had higher scores in clinical competency, communication skills, and self-efficacy than those in the control group. Conclusion: Through these results, practice education of nursing education programs need to activate the clinical competency evaluation using standardized patients.

Psychiatric Nursing Simulation Practice Education Effect Using Standardized Patients (표준화 환자를 활용한 정신간호 시뮬레이션 실습 교육 효과)

  • This study was conducted by collecting data from September to November 2021 for third-year students enrolled in the Department of Nursing at a university located in D city in order to confirm the educational effect of psychiatric nursing simulation practice using standardized patients. For data analysis, using the SPSS/WIN 22.0 program, the results before and after the simulation class on general characteristics, communication ability, problem-solving ability, learning self-efficacy, and learning satisfaction were compared and verified. As a result of the study, communication ability (t=-3.199, p=.004) and learning satisfaction (t=-3.342, p=.003) significantly increased after education. However, problem-solving ability (t=-1.875, p=.072) and learning self-efficacy (t=-1.432, p=.164) were not statistically significant. Therefore, it is necessary to continuously operate education applying standardized patients to improve the communication ability and learning satisfaction of nursing students.

A Study on Small Area Variations of Hospital Services Utilization in Heart Diseases (심질환의 지역간 입원의료이용 변이에 관한 연구)

  • The purpose of this study is to provide preliminary data for political measures to minimize the variations by understanding the regional variations and trends of hospital services for heart diseases, and analyzing the factors that could occur any variations. The various data collected from nation-wide inpatient services conducted separately by small region show that there have been some differences in income level, supply of medical resources, standardized rate of hospitalization by sex and age, health level in the residence, and the length of stay per head. Then it indicated that the number of special medical equipment per 10,000 people and the rate of vigorous physical activity have the highest influence over the regional variations in using hospital services. On the other hand, the number of sick-beds per 10,000 people, the number of special medical equipment, and the present smoking rate have significant degree of influence on the length of stay per head. Thus, it is imperative for the authorities to set aside health promoting policies and to distribute the medical resources equitably throughout the country and to enhance the accessibility of local residents to the health care services.

The Effects of Psychiatric Nursing Simulation on Anxiety and Self-confidence about Clinical Placement of Nursing Students (정신질환자간호 시뮬레이션교육이 간호학생의 임상실습에 대한 불안과 자신감에 미치는 효과)

  • Nursing students experienced anxiety and fear of building relationships with psychiatric patients due to lack of professional knowledge about metal illness and skill to take care of the patients. The simulation education is known as an effective teaching-learning method since being performed in a non-threatening environment similar to clinical setting. The purpose of this nonequivalent quasi-experimental study was to identify the effect of simulation on anxiety and self-confidence about the psychiatric-mental health nursing practice. The third year nursing students at a three year nursing program were participated in this study. For the experimental group (N=28) the simulation education using the standardized patient based on the scenario for patients with schizophrenia was performed, and for the control group (N=18), the debated education applying the case study same as for the experimental group was made in the class. Using SPSS, t-test and ANCOVA were analyzed to compare the anxiety and self-confidence between the experimental and control groups. As a result of the study, the anxiety score on psychiatric-mental health nursing clinical practice of the experimental group significantly decreased, compared to that of the control group, but the self-confidence score for the performance of psychiatric nursing care showed no significant differences between the groups. Accordingly the simulation education can be an effective educational method to reduce anxiety about psychiatric-mental health nursing clinical practice for nursing students.

Effects of using Standardized Patients on Nursing Competence, Communication Skills, and Learning Satisfaction in Health Assessment (표준화환자를 활용한 건강사정 실습교육이 간호학생의 간호수행능력, 의사소통능력 및 학습만족도에 미치는 효과)

  • Purpose: The purpose of this study was to evaluate the effects of using standardized patients on nursing competence, communication skills, and learning satisfaction in health assessment. Method: This study was a nonequivalent control group non-synchronized post-test design. Study participants were recruited from third-year nursing students at a university. The data were analyzed using the SPSS/WIN 18.0 program. Results: The mean scores of nursing competence and communication skills were significantly higher in the experimental group than the control group. However, the two groups were not statistically different in learning satisfaction. Conclusion: The findings suggest that the use of standardized patients compared to the traditional method is an effective educational method for health assessment in nursing competence and communication skills. Therefore, it is necessary to develop various scenarios and apply standardized patients for health assessment. In addition, further research is required to evaluate its effects.

Development of a Multimedia Learning DM Diet Education Program using Standardized Patients and Analysis of Its Effects on Clinical Competency and Learning Satisfaction for Nursing Students (표준화환자를 활용한 당뇨식이교육 동영상학습이 간호학생의 임상수행능력과 학습만족도에 미치는 효과)

  • Purpose: The purpose of this study was to develop a multimedia learning program for patients with diabetes mellitus (DM) diet education using standardized patients and to examine the effects of the program on educational skills, communication skills, DM diet knowledge and learning satisfaction. Methods: The study employed a randomized control posttest non-synchronized design. The participants were 108 third year nursing students (52 experimental group, 56 control group) at K university in Seoul, Korea. The experimental group had regular lectures and the multimedia learning program for DM diet education using standardized patients while the control group had regular lectures only. The DM educational skills were measured by trained research assistants. Results: The students who received the multimedia learning program scored higher for DM diet educational skills, communication skills and DM diet knowledge compared to the control group. Learning satisfaction of the experimental group was higher than the control group, but statistically insignificant. Conclusion: Clinical competency was improved for students receiving the multimedia learning program for DM diet education using standardized patients, but there was no statistically significant effect on learning satisfaction. In the nursing education system there is a need to develop and apply more multimedia materials for education and to use standardized patients effectively.
