Electronic Cigarette or Vaping-Associated Lung Injury Manifested as Acute Eosinophilic Pneumonia: A Case Report (급성호산구성폐렴으로 발현된 전자담배 관련 폐 손상: 증례 보고)

  • Electronic cigarette or vaping-associated lung injury (EVALI) is a disease defined by lung injuries caused by e-cigarette use. It predominantly manifests in forms of organized pneumonia or diffuse alveolar damage but rarely as acute eosinophilic pneumonia (AEP). This report describes a 34-year-old male with acute respiratory symptoms and a vaping history of only nicotine. Chest CT revealed peripheral distributing multiple patchy consolidations and ground-glass opacities dominant in both lower lobes, bilateral diffuse interlobular septal thickening, and bilateral pleural effusion without cardiomegaly. Bronchoalveolar lavage fluids showed increased eosinophilia levels, while infectious laboratory results were all negative, enabling the diagnosis of both AEP and EVALI. Herein, we report a rare case of only-nicotine vaping EVALI manifested as AEP.

Improvement of Active Contour Model for Detection of Pulmonary Region in Medical Image (의학 영상에서 폐 영역 검출을 위한 Active Contour 모델 개선)

  • In this paper, we extracted the contour of lung parenchyma on EBT images with the improved active contour model. The objects boundary in conventional active contour model can be extracted by controlling internal energy and external energy as energy minimizing form. However, there are a number of problems such as initialization and the poor convergence about concave part. Expecially, contour can not enter the concave region by discouraging characteristic about stretching and bending in internal energy. We controlled internal energy by moving local perpendicular bisector point of each control point in the contour and implemented the object boundary by minimizing energy with external energy The convergence of concave part could be efficiently implemented toward lung parenchyma region by this internal energy and both lung images for initial contour could also be detected by multi-detection method. We were sure this method could be applied detection of lung parenchyma region in medical image.

  • Pleural metastasis is the most common cause of malignant diseases involving the pleura, and characterized by pleural effusion, nodules, and thickening. Pleuroparenchymal fibroelastosis (PPFE) is a disease characterized by apical pleural thickening and subjacent parenchymal fibrosis. We report a case of a 60-year-old male with lung cancer in the left lower lobe and underlying PPFE combined with left apical pleural metastasis. Initially, asymmetric left apical pleural thickening due to pleural metastasis was mistaken for PPFE. Additionally, we describe the imaging and histopathological findings of PPFE, including MRI findings.

Quantitative CT Evaluation for Lung Volume and Density in Dogs (개에서 정량적 컴퓨터단층촬영을 이용한 폐용적과 폐밀도의 평가)

  • In this study, we analyzed the computed tomography (CT) measurements of lung volume and density in dogs with relation to body weight, age, sex, and breed. The multi-detector CT examination of the thorax was performed on dogs without respiratory or cardiovascular diseases. Three-dimensional reconstruction of CT images facilitated measurement of lung volume and density. There was a statistical significant correlation between body weight and lung volume (p < 0.0001). Lung density significantly decreased with an increase in body weight (p = 0.0078). However, no correlation was seen between these lung parameters and either sex or age of the dogs. In conclusion, this study shows that body weight is an important factor to consider when interpreting total lung volume and density values measured by quantitative CT. We highlight the need for further study using quantitative CT in identifying the potential effects of sex, age, and disease status on these parameters.

  • 본 논문은 흉부 전산화단층촬영 영상에서 폐 영역을 자동으로 분할하는 알고리즘과 폐결절을 자동으로 검출하는 알고리즘에 관한 연구 내용을 담고 있다. 폐 분할 알고리즘은 gray-level thresholding과 morphologic 영상 처리기법을 이용하였고, 폐결절 자동 검출 알고리즘은 추출된 결절 후보의 size, compactness, mean of gray level 값을 분석하여 혈관과 결절을 구분하였다. 개발한 폐결절 자동 검출 시스템은 실험한 영상에 포함된 폐결절 117개 중 55%인 64개를 검출하였고, 3.4 False Positive/section이었다.

  • With the ongoing coronavirus disease 2019 (COVID-19) pandemic, there is an increasing interest in the sequelae and care in recovered patients. Although the long-term sequelae of COVID-19 are still unknown, recently published reports suggest that some of the patients have persistent symptoms and show radiologic abnormalities after discharge. Herein, we present cases of four patients with previous COVID-19 infection manifesting pulmonary sequelae, including pulmonary fibrosis or organizing pneumonia pattern with persistent dyspnea after recovery.

  • A meandering pulmonary vein (MPV) is a rare pulmonary vascular anomaly characterized by an abnormal course of the pulmonary vein draining into the left atrium. We report the case of a 55-year-old female who was diagnosed with a right MPV on pulmonary angiography. Enhanced chest CT revealed a vascular structure with an abnormal course that drained into the right superior pulmonary vein in the right upper lobe, which resembled a pulmonary arteriovenous malformation (AVM). Pulmonary angiography performed to discriminate between an MPV and pulmonary AVM showed no feeding artery, normal parenchymal staining, and drainage to the left atrium via the culprit vessel. Thus, the patient was diagnosed with MPV. When an MPV is difficult to differentiate from other vascular anomalies on enhanced chest CT, pulmonary angiography can be helpful. By recognizing the angiographic findings of an MPV, unnecessary treatment can be prevented.

Imaging of Lung Metastasis Tumor Mouse Model using $[^{18}F]FDG$ Small Animal PET and CT ($[^{18}F]FDG$ 소동물 PET과 CT를 이용한 폐 전이 종양 마우스 모델의 영상화)

  • Purpose: The purpose of this study is to image metastaic lung melanoma model with optimal pre-conditions for animal handling by using $[^{18}F]FDG$ small animal PET and clinical CT. Materials and Methods: The pre-conditions for lung region tumor imaging were 16-22 h fasting and warming temperature at $30^{\circ}C$. Small animal PET image was obtained at 60 min postinjection of 7.4 MBq $[^{18}F]FDG$ and compared pattern of $[^{18}F]FDG$ uptake and glucose standard uptake value (SUVG) of lung region between Ketamine/Xylazine (Ke/Xy) and Isoflurane (Iso) anesthetized group in normal mice. Metastasis tumor mouse model to lung was established by intravenous injection of B16-F10 cells in C57BL/6 mice. In lung metastasis tumor model, $[^{18}F]FDG$ image was obtained and fused with anatomical clinical CT image. Results: Average blood glucose concentration in normal mice were $128.0{\pm}23.87$ and $86.0{\pm}21.65\;mg/dL$ in Ke/Xy group and Iso group, respectively. Ke/Xy group showed 1.5 fold higher blood glucose concentration than Iso group. Lung to Background ratio (L/B) in SUVG image was $8.6{\pm}0.48$ and $12.1{\pm}0.63$ in Ke/Xy group and Iso group, respectively. In tumor detection in lung region, $[^{18}F]FDG$ image of Iso group was better than that of Ke/Xy group, because of high L/B ratio. Metastatic tumor location in $[^{18}F]FDG$ small animal PET image was confirmed by fusion image using clinical CT. Conclusion: Tumor imaging in small animal lung region with $[^{18}F]FDG$ small animal PET should be considered pre-conditions which fasting, warming and an anesthesia during $[^{18}F]FDG$ uptake. Fused imaging with small animal PET and CT image could be useful for the detection of metastatic tumor in lung region.

  • 흉부 X선 영상을 판독함에 있어서 중요한 정보중의 하나로 폐실질의 조직 특성이 이용된다. X선 영상에서 뼈 혹은 심장, 굵은 혈관등은 X선의 투과율이 적어 시각적으로 밝고 균일한 재질로 나타나며, 공기가 채워져 있는 폐실질은 어둡고 산소/이산화탄소 교환에 관계되는 미세한 조직들에 따라 균일하지 않은 재질로 나타나는 특성을 보이고 있다. 본 연구에서는 공간적인 주위조직의 경보를 이영하여 현지의 부분을 예측하여 인식하도록 수정된 경쟁 순환 신경망을 이용하여 흉부 X선 영상에서의 순수한 폐실질 부위를 영역 분할한다.

Hardware-based Level Set Method for Fast Lung Segmental ion on CT Abdomen Image (복부 CT 영상에서 빠른 폐 분할을 위한 그래픽 하드웨어 기반 레벨 셋 기법)

  • 본 논문에서는 복부 CT 영상에서 폐 부위를 빠르게 분할하기 위하여 그래픽 하드웨어를 사용한 레벨 셋 기법을 제안한다. 제안방법은 다음과 같이 세 단계로 구성된다. 첫째, 레벨 셋 기법을 그래픽 하드웨어로 효율적으로 구현하기 위하여 초기 레벨 셋 값 설정과 설정된 레벨 셋 값을 텍스처메모리에 저장한다. 둘째, 레벨 셋 기법의 가장 중요한 부분인 속도함수를 그래픽 하드웨어의 빠른 연산을 이용하여 계산하고, 레벨 셋 값을 갱신한다. 셋째, 갱신된 레벨 셋 값을 통하여 제로-레벨 셋을 찾는다. 본 논문에서는 제안 방법을 평가하기 위하여 일련의 복부 CT 영상을 사용하며, 육안평가 및 수행시간 면에서 기존 소프트웨어 기반 레벨 셋 기법과 비교분석한다. 실험결과 본 제안방법은 소프트웨어 기반 레벨 셋 기법과 분할결과를 동일하게 유지하면서 평균 9배 빠르게 폐 부위를 분할하였다.
