  • The recurrence of an anterior mediastinal mass was discovered incidentally on the chest film for an asymptomatic 72-year-old female. She underwent the mass resection at the same site in 1989 and was diagnosed as a paraganglioma of the anterior mediastinum. She had poorly controlled hypertension which was converted into normal blood pressure after the first operation. During the follow up, the catecholamine levels were within normal limits, and there were no evidence of recurrence of the tumor. However, 6.3 years later, the mass recurred at the same anterior mediastinum. The patient had no hypertension and catecholamine levels were still within normal limits. The recurring mass was successfully removed and had the same histological findings - recurring paraganglioma.

  • Squamous cell carcinoma is the most common malignant tumor of the gingiva. Oral cancer accounts for 5 percent of all malignant tumors in the body, and 6 percent of these occur in the gingiva. We present one case of squamous cell carcinoma of the gingiva in 23-year-old female. She complained of gingival swelling and pain during mastication between left maxillary first molar and second molar. Since she had received scaling 2 years ago, food impaction occurred frequently in this site and an ulcerative lesion recurred several times. When she was referred from local clinic, she had a large, irregularly ulcerated lesion of palatal gingiva between left maxillary first molar and second molar, accompanying induration in center and slight elevation around ulcerative margin. The etiology could not be defined but we could diagnose by careful history taking and excisional biopsy of the recurrent ulcerative lesion. In addition, computed tomography and nuclear medicine imaging were undertaken. As a result, we finally diagnosed as moderately differentiated squamous cell carcinoma without metastases. It is concluded that if ulcerations do not respond to therapy in the usual manner, it should be biopsed and histologically evaluated for the definitive diagnosis and treatment.

  • Squamous cell carcinoma(SCC) associated antigen is a subfraction of TA-4, a tumor-associated antigen first described by Kato and Torigoe in 1977. TA-4, obtained from squamous cell carcinoma cancer tissue of the uterine cervix, has been characterized as a glycoprotein with a molecular weight of approximately 45,000 daltons. SCC antigen has been studied in other squamous cell malignancies including lung, esophagus, head and neck, anal canal, and skin. SCC antigen is shed naturally through sweat, saliva and other body fluids. Contamination of specimens, tray, bead dispenser or other accessories with sweat, saliva or aerosols can cause falsely elevated values. To reduce the possibility of contamination, gloves should be worn in all phases of assay preparation, and when handling specimens, accessories or reagents that will be used in SCC and Thyroid function test(TFT).

  • Several investigators have shown that human papillomavirus(HPV) appear to play an etiologic role in oral and paranasal sinus carcinoma. It was known that 15-25% of head and neck squamous cell carcinoma(SCC) showed HPV-positive infection. Among them, HPV 16 was the most common type but HPV 18 was observed only 2-4% of HPV-positive head and neck cancers. In recent, we treated uncommon 2 oral SCC cases that associated with HPV infection. One is a case of tongue SCC after bone marrow transplantation(BMT), and the other is a case of SCC occurring with aspergillosis in the maxillary sinus. After surgery, HPV 16 and 18 were detected in the surgical specimens by the histological and polymerase chain reaction(PCR) examination. In this report, we present these cases with a review of literature.

Immunohistochemical study of p53 and mdm-2 in Squamous Cell Carcinoma and Leukoplakia of Head and Neck. (두경부 편평상피세포암과 백반증에서 p53과 mdm-2의 면역조직화학적 연구)

  • The mutation of p53 is the most common genetic alteration found in human cancers and has oncogenic properties. mdm-2 is a recently discoverd that controls the p53 activity by binding of its protein, so negative feedback loop has been suggested in which p53 induces mdm-2 expression. The purpose of this study was to analyze the expression of p53 in leukoplakias, mdm-2 in squamous cell carcinomas, and relationship between p53 and mdm-2 expression in leukoplakias and squamous cell carcinomas. The results were as follows : 1) The p53 was expressed 33.4% in leukoplakias 2) The mdm-2 was expressed 8.3% in leukoplakias and 22.7% in squamous cell carcinomas. 3) The expression rate of p53 was higher in specimens negative for mdm-2 than in specimens positive for mdm-2, but there was not significant relationship between p53 and mdm-2 expression. In conclusion p53 was thought to participate in early phase of oncogenesis, and mdm-2 was thought to have a role as a oncogene in squamous cell carcinoma of head and neck. Though there was not significant relationship between p53 and mdm-2 expression, mdm-2 was thought to inhibit p53 activity.

  • Lichen planus is a relatively common chronic inflammatory disease involving the skin and mucous membranes showing small flat polygonal papules. The accurate etiology is unknown but it's suggested that cell-mediated immune response to an induced antigenic changes in skin or mucosa. Oral lichen planus was regarded as an benign lesion but oral lichen planus was classified as premalignant lesion by WHO criteria. It was not known that progress of malignat transmmission in the the patient with oral lichen planus, and chronic inflammatory disease including oral lichen planus showed malignacy in oral cancer unrelated common risk factors(Ex: tabacco, alcohol). Although malignant development in the patient with oral liche planus was various greatly in the literature, from 0.5% upward to 5%. It has been reported that a specific clinical type of oral lichen planus, hyperkeratotic or erosive had a higher chance of transformation into an squamous carcinoma. Clinician has to follow-up check of at least one or two visit per year to detect of malignancy of oral lichen planus and improved prognosis with squamous cell carcinoma. At this case with the middle aged women with squamous cell carcinoma developed from oral lichen planus of more than a decade of persisting, we try to discuss the malignacy of oral lichen planus and cosideration with follow-up.

  • Background : Cell adhesion molecules knave been Implicated In the various stages of tumor progression and metastasis. ICAM-1 plays a important roles in cell-cell interactions in inflammatory and immune response of several diseases. Recently, elevated levels of sICAM-1 in circulation was reported as association with liver metastasis in gastric, Colonic, gall bladder and pancreatic cancer, with reduced survival in malignant melanoma. This study was performed to measure the sICAM-1 in patients with lung cancer and to evaluate the relations between staging of lung cancer and level of sICAM-1. Methods : Serum sICAM-1 was measured in 36 patients with lung cancer according to the pathologic types and clinical staging before therapy and in 8 controls with ICAM-1 ELISA kit. Results : Serum sICAM-1 levels were elevated in patients with lung cancer except small cell type. Also progression and metastasis of lung cancer associated with elevation of sICAM-1 levels. Conclusion : These results suggest that higher levels of serum ICAM-1 reflect the progression and metastasis of lung cancer and it may be used as a marker with diagnostic and prognostic significance.

  • The carcinoma of the maxillary sinus occupies about 0.25% of all human cancers. The radiations therapy for maxillary sinus hs not been practiced widely in this country. The authors treated a case of relatively advanced maxillary sinus cancer with combined radium and external irradiation with a succesful result. The clinical course and treatment are introduced and evaluated in detail. The authors believe that the carcinoma of the maxillary sinus is a curable disease even though it is relatively advanced one.

Association between RASSF1A Methylation and Clinicopathological Factors in Patients with Squamous Cell Carcinoma of Lung (편평상피폐암에서 암억제유전자 RASSF1A의 메틸화와 임상 및 병리소견과의 연관성)

  • Background : RASSF1A, which is one of tumor suppressor genes, is frequently inactivated by hypermethylation of the promoter region in a variety of human cancers, including lung cancer. This study was performed to investigate the association between RASSF1A methylation and the clinicopathological factors in patients with squamous cell carcinoma of the lung. Methods : Eighty-one samples from the patients with squamous cell carcinoma of lung were examined. The promoter methyation of RASSF1A was analyzed by methylation specific PCR and sequencing. Statistical analysis was made to examine the association between RASSF1A methylation and the clinicopathological parameters. Results : RASSF1A methylation was observed in 37.0 % (30 of 81) of the patients with squamous cell carcinoma of the lung. RASSF1A methylation was found to be associated with cellular differentiation(p=0.0097) and the overall survival(p=0.0635). However, there was no association between RASSF1A methylation and the other clinicopathological parameters, such as the pathological TNM stage, the recurrence rate, lymph node invasion and the amount of cigarettes smoked. Conclusion : RASSF1A methylation might be associated with a poor prognosis in patients with squamous carcinoma of the lung. A larger scale study is needed.

Surgical Treatment of Stage IIIA Non Small Cell Lung Cancer(NSCLC) (제 IIIA기 비소세포 폐암의 수술 성적)

  • Background: Surgery has been considered the most effective and standard treatment modality in non-small cell lung cancer(NSCLC). However in stage IIIA lung cancer, the role of surgery is still controversial. To evaluate the role of surgery for stage IIIA NSCLC, we investigated the survival after surgery and the prognostic factors. Material and Method: We evaluated 158 consecutive cases of stage IIIA NSCLC patients operated on between 1990 and 1996. There were 130 male patients and 28 female patients, and the mean age was 58.5 years. All patients except one underwent lung resection beyond lobectomy and extended mediastinal dissection. Postoperative adjuvant therapy were undertaken in 145(94.8%) patients. All patients(153) were followed and the mean follow-up period was 21.4months. Result: Twenty nine cases of the postoperative complications developed in 25 patients (15.8%). There were 5 operative mortality cases(3.2%) and the main cause of death was acute respiratory distress syndrome (ARDS). Local or distant recurrences developed in 84 patients(54.9%). The 5-year survival of 153 patients was 29.6% and the median survival time was 18.0 months. The 5-year survival of non N2 disease group(36.8%) was better than that of N2 disease group(26.6%)(p=0.35) and the 5-year survival of squamous cell carcinoma (38.1%) was better than that of adenocarcinoma(25.7%)(p=0.39) however there were no significant differences. Regarding the postoperative adjuvant therapy, in combined therapy group(84 patients), radiotherapy group(37 patients) and chemotherapy group(24 patients), the 5-year survival were 31.3%, 32.4%, and 14.6% respectively. There was no difference of survival between radiotherapy and combined therapy group(p=0.31), however the survival of the combined therapy group was better than the chemotherapy group(p=0.005). The survival of the complete resection group(31.9%) was better than the incomplete resection group(16.6%) however there was no significant difference(p=0.19). Conclusion: These observations indicate that the good 5-year survival(29.6%) in patients with stage IIIA NSCLC result from the agressive surgical treatment including extensive mediastinal nodes dissection.
