The Effects of Organizational Commitment and Perceived Patient Safety Culture on Patient Safety Nursing Activities among Nurses in Comprehensive Nursing Care Units (간호·간병통합서비스 병동 간호사의 조직몰입과 환자안전문화인식이 환자안전간호활동에 미치는 영향)

  • Purpose: The purpose of this study is to identify the effects of organizational commitment (OC) and perceived patient safety culture (PPSC) on patient safety nursing activities (PSNA) among nurses in comprehensive nursing care units. Methods: Participants were 173 nurses working at five general hospitals in Chungcheong area. Data were analyzed using descriptive statistics, 𝑥2 test, t-test, ANOVA, Pearson's correlation coefficient, and multiple regression analysis with SPSS/WIN 23.0 programs. Results: The mean scores of the OC and PPSC were 3.28±0.50 and 3.85±0.35, respectively. The mean score of PSNA was 4.55±0.41, and PSNA was significantly different by the experience of participating in hospital's safety culture campaigns (t=2.70, p=.008). The results of the multiple regression analysis showed that 'patient safety knowledge and attitudes' (β=.27, p=.006) and 'unpunished environment to error' (β=.22, p=.004) as the sub-categories of PPSC were affecting factors on PSNA with an explanatory power of 26.0% (F=6.40, p<.001). Conclusion: The results of this study suggest that in order to promote PSNA among nurses in comprehensive nursing care units, it is necessary to develop a program to enhance patient safety-related knowledge and attitudes. In addition, the hospital's organizational efforts such as operating safety campaigns and creating an unpunished environment to error should be needed.

Influencing Effects of Job Stress, Professional Autonomy, Reciprocity on the Job Embeddedness of Comprehensive Nursing Care Unit Nurses (간호·간병통합서비스 병동 간호사의 직무 스트레스, 전문직 자율성, 호혜성이 직무착근도에 미치는 영향요인)

  • Purpose: The purpose of this study was to identify the influencing effects of job stress, professional autonomy, and reciprocity on the job embeddedness among comprehensive nursing care unit nurses. Methods: The participants in this study were 147 nurses who have worked for over 6 months in Comprehensive Nursing Care Unit. Data were collected from January 3 to January 31, 2022 from six general hospitals with more than 300 beds in three cities in G, G and P. Results: As the results of hierarchical regression analysis, job embeddedness was lower when nurses had clinical experiences for 1 to under 5 years (β=-.49, p<.001), 5 to under 10 years (β=-.27, p=.035), 10 to under 20 years (β=-.54, p<.001) compared to those who had clinical experiences for more than 20 years. Also, job embeddedness was higher when there was greater balance within team caregiving of reciprocality (β=.27, p<.001) and intrinsic reward (β=.22, p=.003), and lower role conflict (β =-.27, p<.001). Conclusion: The results of the study showed that job embededness would increase if the role conflict of comprehensive nursing care unit nurses reduces, if there was a culture that the members can make cooperative relationship with other health care professionals and, if there was an appropriate intrinsic reward depending on their work experience.

Factors Influencing Nursing Care Needs of Patients in Comprehensive Nursing Care Service Wards (간호·간병통합서비스 병동 환자의 간호필요도 수준과 영향요인)

  • Purpose: This is a retrospective secondary data analysis study based on real-world data to analyze the level of nursing care needs of patients in a comprehensive nursing care service ward, and identify factors influencing nursing needs. Methods: Study participants included patients admitted to two comprehensive nursing care service wards at a tertiary general hospital located in Gyeonggi-do, Korea. After obtaining permission from the health and medical information team of the target hospital, data were collected from their electronic medical records. Nursing care needs were measured using seven items on nursing activity and four items on daily living activities developed by the National Health Insurance Service (NHIS). The collected data were analyzed using the SPSS version 29.0 with frequency and percentage, mean and standard deviation, minimum and maximum values, t-test, ANOVA, Pearson's correlation coefficients, and multiple linear regression. Results: The level of nursing care needs of patients in comprehensive nursing care service wards was found to be higher for patients with pressure sores (β=.33), older patients (β=.26), patients who underwent procedures (β=.15), patients with present guardians (β=.15), and patients with more comorbidities (β=.10). The total explanatory power was 51.0%. Conclusion: It is necessary to accurately identify patients' nursing care needs and provide nursing care according to priority by considering the characteristics of patients in comprehensive nursing care service wards.

A Study on the Subjectivity of Comprehensive Nursing Service in Early Postpartum Mothers: Q Methodology (산욕초기 산모의 간호간병통합서비스 인식에 관한 주관성 연구 : Q 방법론)

  • This study attempted to examine the subjectivity of early postpartum mothers' perceptions of comprehensive nursing care services targeting at early postpartum mothers hospitalized at general wards, and applied Q methodology to find types of subjectivity and characteristics of the types. P-sample 21 subjects were asked to read Q sample 25 statements about comprehensive nursing care services and indicate their level of agreement to those statements on a 9-point scale. The data were collected during the period from March 16 to March 31, 2020, and the collected data were analyzed using PC-QUANL program. The results of the study showed differences in the respondents' perceptions, which led to the classification into three types. "Type 1" is "those calling for continuous emotional management for early postpartum mothers." "Type 2" is "those seeing the role of a family in the recovery period as important." The respondents of this type considered a bond of sympathy should be formed in the family in relation to the new tasks of the mother's recovery and the child's birth. "Type 3" is "those calling for differentiated operation guidelines according to the motivation for hospitalization." The respondents of this type recognized that the operation guidelines limiting the residing of guardians need to be improved for early postpartum mothers.

Role Ambiguity of Comprehensive Nursing Care Unit Nurses: A Concept Analysis (간호·간병 통합서비스 병동 간호사의 역할 모호성 개념분석)

  • Background: The purpose of this study was to identify role ambiguity of comprehensive nursing care unit nurses. Methods: A concept analysis method by Walker and Avant was used to understand role ambiguity of comprehensive nursing care unit nurses. Results: The antecedents of role ambiguity of nurses at comprehensive nursing units were shortage of nurses, unclear admission criteria, and demands for customized nursing care according to severity. Attributes include ambiguity in role delegation, patient placement ambiguity, and professional ambiguity among nursing staff. The consequences were diminished job satisfaction due to excessive workload, difficulty in resolving role ambiguity due to the lack of work analysis studies, and poor outcome of nursing indicators. Conclusion: Improvement of nationwide awareness for comprehensive nursing care unit is required. Clear division at scope of practice for nursing staff in accordance of each medical institution's characteristics is essential. Nurses at comprehensive nursing care unit should understand nature of role ambiguity that occurs as they work in large groups. Nurses should promote communications between nursing staff and they must have volition to improve status quo. An additional research of comprehensive nursing care on the causes of role ambiguity in the practice of nursing care for ward nurses is needed, and management measures should be sought at the organizational level.

Factors affecting Nursing Satisfaction of Inpatients in Comprehensive Nursing Care Service Ward (간호·간병통합서비스 병동 입원환자의 간호만족도에 영향을 미치는 요인)

  • Objectives: The purpose of this study is to evaluate the factors affecting patient satisfaction for the nursing service of comprehensive nursing care service unit in Tertiary hospital Methods: This study employed a descriptive survey to investigate the nursing satisfaction and the study data was collected from 74 patients among 83 patient who were admitted to the nursing care integration service ward of an tertiary hospital for 10 days from May 9, 2018 to May 19, 2018. Data was analyzed by descriptive statistics, t-test, ANOVA, and multiple linear regression using SPSS 21 program. Results: The average with nursing satisfaction was significantly different according to marriage type and KPCS level. It was found that physical factor and educational factor have statistically significant effect on nursing satisfaction. As physical care increase by one unit, patient satisfaction increase by 0.226(95% CI; 0.061-0.390) and also satisfaction increase by 0.443(95% CI; 0.070-0.816) as education care increase by one unit. Conclusions: As a result of study, it was found that it is necessary to provide patients with more careful nursing service in physical and educational perspective, in order to improve the service quality of comprehensive nursing care service unit.

Association of Health Outcomes with Frailty in Community-Dwelling Korean Older Adults: An Integrative Review (국내 지역사회 거주 노인의 허약과 건강결과 간의 관계에 대한 통합적 고찰)

  • Purpose: Frailty is associated with an increased risk of adverse health outcomes. We aimed to review the relationships between frailty and health outcomes in community-dwelling Korean elderly individuals. Methods: Whittemore and Knafls' framework for conducting integrative reviews was used. PubMed, Cumulative Index to Nursing and Allied Health Literature, and six Korean databases were searched. For analysis, the study included articles written in English and Korean published between January 1960 and June 2018. Of the total 1,488 studies found in the databases, we analyzed 15 studies that met the quality of the evaluation criteria. Results: The prevalence of frailty in Korean elderly individuals ranged from 6.5% to 11.7% when divided into three levels of frailty. The health outcomes assessed in relation to frailty were divided into five domains: quality of life, physical health, psychosocial health, health behavior, and health care quality. Frailty was negatively associated with all five domains. Conclusions: Our study suggested that nurses should be aware of the limitations in the physical and cognitive functions of frail elderly individuals and provide tailored interventions for Korean elderly individuals. Furthermore, a large-scale study is needed to develop the Korean model of the frailty assessment tool and to verify the conceptual model of this study.

Development and Validation of a Education Nurse Specialist-Led Education Satisfaction Scale for Nurses Working at Comprehensive Nursing Care Service Wards (교육전담간호사가 주도하는 교육 프로그램에 대한 간호·간병통합서비스 병동 간호사의 교육 만족도 측정 도구 개발 및 타당도 검정)

  • Purpose: The purpose of this study was to develop and validate a education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service wards. Methods: A methodological study was conducted. A total of 237 nurses working at comprehensive nursing care service wards in a general hospital participated in this study. The scale was developed through literature reviews, in-depth interview, development of preliminary items, verification of content validity, development of secondary items, evaluation of construct validity, and extraction of final items. Analysis included exploratory factor analysis, pearson's analysis, and reliability analysis using cronbach's α. Results: The education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service ward consisted of 21 items. Two factors (critical thinking disposition and clinical competency) were identified which explained 63.5% of the total variance. Cronbach's α of each factors were >.95. Conclusion: The results suggested that the education nurse specialist-led educational satisfaction scale for nurses working at comprehensive nursing care service wards demonstrated acceptable validity and reliability. Items of the instrument can assess the level of satisfaction with regards to education led by education nurse specialist among nurses working at comprehensive nursing care service wards.

Relationships among Non-Nursing Tasks, Nursing Care Left Undone, Nurse Outcomes and Medical Errors in Integrated Nursing Care Wards in Small and Medium-Sized General Hospitals (중소종합병원 간호·간병통합서비스 병동 간호사의 비간호 업무, 미완료 간호와 간호사 결과, 의료오류 간의 관계)

  • Purpose: This study aimed to identify the degree of non-nursing tasks and nursing care left undone in integrated nursing care wards, and examine their relationships with nurses' burnout, job satisfaction, turnover intentions, and medical errors. Methods: A cross-sectional questionnaire survey was conducted. Data were collected using self-report questionnaires from 346 nurses working in 20 wards of seven small and medium-sized general hospitals, and analyzed using multiple regression and multiple logistic regression analysis with the SPSS WIN 25.0 program. Results: The mean score for non-nursing tasks was 7.32±1.71, and that for nursing care left undone was 4.42 ± 3.67. An increase in non-nursing tasks (β = .12, p = .021) and nursing care left undone (β = .18, p < .001) led to an increase in nurses' burnout (F = 6.26, p < .001). As nursing care left undone (β = .13, p = .018) increased, their turnover intentions also (F = 3.96, p < .001) increased, and more medical errors occurred (odds ratio 1.08, 95% confidence interval 1.02~1.15). Conclusion: Non-nursing tasks and nursing care left undone are positively associated with nurses' burnout, turnover intentions, and the occurrence of medical errors. Therefore, it is important to reduce non-nursing tasks and nursing care left undone in order to deliver high quality nursing care and in turn increase patient safety.

A Study on the Image of a Nurse in Korean Modern Novels (한국 현대소설에 나타난 간호사 이미지 연구)

  • This study analyzed how the literary works embodying nurses accommodate various discourses about nurses and reproduce them in literature. The subject of analysis is a Korean modern novel featuring a nurse, and 29 works were selected from 1927 to 2016. The analysis method is the content analysis of the novel among the qualitative research methods. The unit of analysis adopts a narrative or dialogue that deals with the image of a nurse in a novel as a unit of analysis, The image determining factors of previous studies were integrated and categorized into 4 types and considered. As a result of analyzing the image of a nurse in the novel, First, traditional image types include Lee Kwang-soo 『Love』, Kim Eui-jung 『Doctor Han』, Jo Jung-rae 『Han River』, Gong Ji-young's 「Field of Stars」, Baek Min-seok 「Poor Little Hans」 Second, social image types include Kang Kyung-ae 「Dark」, Kim Kyung-wook 「Heaven's Gate」, Choi Jeong-hee 「Cheonmaek」 Third, professional image types include Lee Cheong-jun 「Mr. Jo Man-deuk」, 「Discharge」, Choi In-hoon 『The Square』, Kim Yeon-soo 「The Night in the Tunnel Where I Listened to Jusaeng Tudipini」, Jeong Se-lang 『Public health teacher Ahn Eun-young』and Fourth, personal image types include Choi In-ho 「Apprentice Patient」, Kim Jeong-han 「The Third Ward」, Eun Hee-kyung 『Minor League』, Hoon Kim 「Hwajang」, Ha Seong-ran 「The Joy of Eating」, Kim Ji-yeon 「Hippocrates Love Song」, Park Kyung-ri 「Era of Distrust」, Jeong Mi-kyung 「The Lady of Arsenal」 typed as. Through the image of the nurse in the novel, the implications of the novel for human care were discussed.
