  • RP(Rapid Prototyping) 기술은 점점 그 활용범위를 넓혀가는 추세이며, 의료분야에서도 임플란트의 제작등에 RP 기술의 도입이 시도되고 있다. 뼈나 치아 등의 인체부속물들은 속성상 직접 그 형태를 파악하기가 힘들고, 따라서 그 대체물인 임플란트의 제작을 위해 MRI나 CT등의 장비가 사용되고 있다. 의료분야에까지 이르는 RP의 활발한 활용에도 불구하고, 아직 국내에서는 RP에 대한 인식이 부족하고 그 활용에 대한 연구가 거의 없는 상황이다. 본 논문은 MRI, CT등의 2차원 평면데이터로부터 직접 RP 기계의 표준입력화일인 STL 파일을 생성시키기 위한 방법을 제시한다. 그리고 MRI로 스캐닝된 사람의 머리뼈 형상을 삼각메쉬 형태의 STL 파일로 변화하여 RP 기계를 통한 실물 모형 제작이 가능하도록 한 예를 설명하였다.

  • 본 논문은 시차를 두고 촬영된 두 장의 치과용 디지털 X-Ray 영상으로부터 임플란트 시술환자에서 자주 발생하는 임플란트 주위염을 검출하는 자동화된 시계열 분석 방안을 제안한다. 분석자에 따라서 결과의 차이를 보이는 수동적인 시계열분석방법의 문제점을 극복하고 보다 객관적이고 정략적인 결과를 얻는 방안을 제시한다. 보다 신속하고 정확한 검출을 위해서 본 연구에서는 치아의 구조 분석을 통해서 환부 탐색 영역을 줄이고 작은 변화도 민감하게 검출할 수 있는 환부영역을 분리할 수 있는 방안을 제안한다. 또한 분리된 환부의 특성을 수치적으로 표현할 수 있는 방안도 함께 제시된다.

  • Guided implant surgery is useful in anterior implant case in terms of fixture installation and temporary crown delivery. For the aesthetic prosthetics in anterior implant, the position of the implant fixture is crucial. Guided surgery is the top-down procedure and we designed prosthetics first and then determine the position of the fixture. Guided surgery can reduce the stress of dentist with difficult anterior implant case.

  • The treatment of patients with severe periodontitis should be proceeded step-bystep through an accurate diagnosis of each patients' individual tooth and with a strategic treatment plan. Implant-supported fixed prosthetic restoration has the advantage of high patient satisfaction and stable vertical dimension compared to the removable partial denture. However, multiple teeth defect areas lacking hard tissue may be disadvantageous in aesthetic failure and longer treatment time. In addition, it takes a certain period of time to manufacture and install a conventional fixed prosthesis, and during this process, the provisional prosthesis must satisfy the mechanical, biological, and aesthetic requirements of teeth. The purpose of this article is to describe the fabrication of implant-supported fixed prosthesis through a step-by-step approach in a partially edentulous patient.

Types and Distribution of Implant Dental Clinic Patients in Busan Area (부산지역 치과환자의 임플란트 유형 및 분포 경향)

  • The purpose of this study was to examine the types of implant, the type of bone graft, periodontal status and the distribution of implant among patients in a region. The subjects in this study were 299 patients who received implant treatment Busan area. The number of their implants was 871 in total. The findings of the study were as follows: As for the type of implant, home-made implants accounted for 83.6 percent, and imported ones represented 16.4 percent. Regarding bone graft, The presence or absence of disease made a significant difference to that($x^2$=14.66, p<.01). As to periodontal status by gender, age and disease, the periodontal state was better among those who were female(y=-2.73, p<.01), who were younger(F=14.20, p<.001) and who had no disease(t=-4.67, p<.001). The intergroup gaps were statistically significant. Concerning the distribution of implant, The distribution of implant was statistically significantly different($x^2$=33.14, p<.01). Age made a statistically significant difference to that($x^2$=74.09, p<.001). As to links between periodontal status and the number of implant, The intergroup gaps were statistically significant($x^2$=38.28, p<.01).

Comparison of the SNR in the MR images on dental implant material (치아 임플란트 재료에 따른 자기공명영상의 SNR 비교)

  • Tooth implant is located in oral cavity and affects neck, skull base, and facail image. These magnetic inhomogeneities are usually frequency encoding direction which cause artifacts due to change of signal strength and geometric distortion. First, to evaluate signal to noise ratio (SNR) of magnetic resonance image caused by tooth implant this study uses meat phantom which is similar to human body and is consisted with fat, muscle, and water to measure signal to noise ratio. Second, signal to noise ratio by using custom-made fixed phantom is measured, and then signal to noise ratio size of different tooth implant types is compared and analyzed. The measured signal to noise ratio values of Brushite, HSA, Metal, and RBM for meat phantom were 2.76, 2.22, 1.88, and 1.57 on T1 SE, 1.88, 1.78, 1.65, and 1.79 on T2 FLAIR, 2.28, 2.25, 2.88, and 2.05 on T2 FSE, and 2.74, 1.94, 1.67, and 1.48 on T2 GRE. The measured signal to noise ratio values of Brushite, HSA, Metal, and RBM for fixed water phantom were 1.2, 1.06, 1.12, and 1.22 on DWI, 1.93, 1.87, 1.93, and 2.06 T1 SE, 1.83, 1.76, 1.82, and 1.92 on T2 FLAIR, 1.85, 1.79, 7.86, and 1.97 on T2 FSE, and 1.97, 1.93, 1.99, and 2.06 on T2 GRE. By considering through the results, patients and dentists need to consider some impacts from testing many aspects although their main purpose of having tooth implants is a dental restoration. Moreover, depending on the tooth implant characteristics of individual patients this study results can be used as baseline data when choosing test protocol.

Hybrid telescopic double crown denture on maxillary few remaining teeth and 2 mandibular implants in case of failed implant overdenture (상악의 소수 잔존치와 실패한 하악의 임플란트 오버덴처의 재수복 임상증례: 프릭션핀을 이용한 하이브리드 텔레스코픽 이중관법)

  • Double crown prostheses can be used in patients who have a few remaining teeth and poor periodontal condition because of secondary splinting of abutments, vertical loading, decrease of the length of lever arm due to fulcrum line located on margin of inner and outer crown. Successful results of treatments using double crown prostheses for the partially edentulous patients who have a few remaining teeth and implant overdenture using a small number of implants have been reported. In this case, there were a few remaining teeth with a very poor periodontal condition in maxilla, and there were a failed implant with severe alveolar bone resorption and shrinkage in the mandible. The main objective of this report is to introduce our case because a double crown partial denture showed satisfactory results in functional and esthetical aspects during more than one-year follow-up period.

Prosthetic restoration of the maxillary anterior teeth using implantation and forced eruption: Case report (인위적 정출술과 임플란트 치료를 통한 상악 전치부 보철치료)

  • When a tooth adjacent to implant has coronal damages caused by severe dental caries or fracture, the clinical crown lengthening by forced eruption makes it possible to get esthetic restoration due to the prevention of alveolar crestal bone resorption and loss of interdental papilla. A 54-years-old male patient wanted prosthetic treatment because his anterior 3 unit bridges had fallen out. A right maxillary central incisor showed mild dental caries but a right maxillary canine lost most clinical crowns. Forced eruption combined with a gingival fiberotomy of a right maxillary canine was performed for 1 month after the dental implant had been simultaneously placed with bone grafts on a right maxillary lateral incisor. About 5 months after implant placement, 2nd surgical operation was performed. The provisional restorations were adjusted to make esthetic gingival contour for 8 weeks. The porcelain fused gold restorations were fabricated and set. The patient was satisfied with the final restorations in esthetic and functional aspect.
