Improvement on resolution of mono-filament wire (초음파 팬텀 내 모노필라멘트의 해상력 개선에 대한 연구)

  • The purposes of this study are to improve the ultrasound resolution of various nylon and metallic mono-filament wires, therefore, it was tested that it analyze on nylon mono-filament wire of 0.1 mm in A Co.'s ultrasonic phantom and synthesis of C15 g tissue mimicking materials(TMM), analyze resolution of nylon and metallic mono-filament wires in water and TMM. The results obtained were summarized as follows: 1. Metallic mono-filament wire of 0.1 mm and nylon mono-filament wire of 0.12 mm, 180 denier showed that it cleared dot echo pattern. 2. Metallic and nylon mono-filament wire of 0.2 mm showed that it cleared comet tail echo by reverberation artifact. 3. Nylon and metallic mono-filament wire of 0.1 mm showed that it can used for dead zone and axial resolution test. 4. Nylon mono-filament wire compared with metallic mono-filament wire showed that it satisfy elasticity and construction. 5. Degree of hardness of na not changed mono-filament's echo textures.

A Study on the Effectiveness of Screening for Selective Breast Cancer Using Digital Mammography Centered on General Hospital (디지털 유방촬영술을 이용한 선별적 유방암 검진의 효용성에 대한 연구(2차 병원을 중심으로))

  • This study is a retrospective analysis of the results of tests at three general hospitals in Busan that perform mammography using digital mammography devices. There were 5,320 people in the study, and the results of their breast cancer screening were analyzed to verify the efficacy of breast cancer screening for digital mammography. The average age of patients who performed breast cancer screening was 57.7 years (range 30 to 87 years), and the cancer detection rate was 26, with 4.6 cases per 1,000 people. According to the cancer detection rate by risk factor in patients who conducted breast cancer screening, breast cancer was found in patients without underlying diseases more than in patients with underlying diseases. Additional ultrasound examinations show that the gastronomic rate identified is 3.6%, which is relatively very low compared to that of the Film-Screen system.

  • Diffuse-type tenosynovial giant cell tumor (D-TSGCT), previously known as pigmented villonodular synovitis, is a locally aggressive neoplasm that may arise from the synovium, bursa, or tendon sheath. D-TSGCT is usually monoarticular and can be classified into intra- and extra-articular forms, the latter of which is rarer. Here, we report a case of D-TSGCT in a 64-year-old female that involved the entire flexor and extensor tendon sheaths of both wrists. We describe the ultrasonography and MRI findings, as well as review the relevant literature.

Safety and Efficacy of Ultrasound-Guided Percutaneous Core Needle Biopsy of Pancreatic and Peripancreatic Lesions Adjacent to Critical Vessels (주요 혈관 근처의 췌장 또는 췌장 주위 병변에 대한 초음파 유도하 경피적 중심 바늘 생검의 안전성과 효율성)

  • Purpose To evaluate the safety and efficacy of ultrasound-guided percutaneous core needle biopsy (USPCB) of pancreatic and peripancreatic lesions adjacent to critical vessels. Materials and Methods Data were collected retrospectively from 162 patients who underwent USPCB of the pancreas (n = 98), the peripancreatic area adjacent to the portal vein, the paraaortic area adjacent to pancreatic uncinate (n = 34), and lesions on the third duodenal portion (n = 30) during a 10-year period. An automated biopsy gun with an 18-gauge needle was used for biopsies under US guidance. The USPCB results were compared with those of the final follow-up imaging performed postoperatively. The diagnostic accuracy and major complication rate of the USPCB were calculated. Multiple factors were evaluated for the prediction of successful biopsies using univariate and multivariate analyses. Results The histopathologic diagnosis from USPCB was correct in 149 (92%) patients. The major complication rate was 3%. Four cases of mesenteric hematomas and one intramural hematoma of the duodenum occurred during the study period. The following factors were significantly associated with successful biopsies: a transmesenteric biopsy route rather than a transgastric or transenteric route; good visualization of targets; and evaluation of the entire US pathway. In addition, the number of biopsies required was less when the biopsy was successful. Conclusion USPCB demonstrated high diagnostic accuracy and a low complication rate for the histopathologic diagnosis of pancreatic and peripancreatic lesions adjacent to critical vessels.

Radiographic Status of the Visited Patients at University Hospital Emergency Room (한 대학병원 응급실 내원환자의 방사선촬영 실태)

  • This study was carried out to improve service efficiency and to cope with a emergency situation in emergency radiography, through analysis of the radiographic distriution and literature cited about emergency care. Data collection of radiographic distribution was surveyed for 1270 emergency outpatients who visit during JAN, 2009at ER of the general hospital in Gwang city. The results is as follows : Emergency radiography rate of simple radiography was 56.6%, special radiography 2. 5%, CT 34.2%, and ultrasonography 6.7%, In simple radiography rate. a high rate was distributed on male(63.6%), thoracicsurgery part(90.0%), admission patient(77.9%), and long stayed patient at ER. In special raiography rate, a high rate was obsurved in urologic part(28.6%), and in CT rate, observed neurosurgerty part(49.2%) and neurologic part(36.7%). Ultrasonography rate was high for female(8.8%) and internal medicine part(15.9%). There are distributed regional radiography rate in radio-graphic type that chest(55.3%) is high in the simple radiography, urinary system(1.2%) in the special study, and brain(40.0%) in the CT. Regional radiography rate according to diagnostic department also was showed highly for head(64.6%) in neuro surgery, chest(90.0%) in thoracic surgery, abdomen(58.0%) in general surgery, spine(40.0%) in neuro surgery, and pelvis(15.9%), upper extrimity(20.5%), and lower extrimity(31.8%) in orthopedic surgery each. Mean radiographic case number per patient of simple radiography was sinificant on sex, age, transfer relation in both total and radiopraphic patients(p<0.05). Mean radiographic case number was highly distributed on male(2.2 case number) in sex, on thirties(2.7) in age, transferred patient(2.7) in patient type, and on nurosurgery(3.4) in diagnostic charged part. Total radiographic case number in regional party was highly distributed on chest(998 case number.) Considering the above results, emergency radiographer should take care of the elder patient in emergency radiography and get hold of injury mechanism to decrease possible secondary injury during radiography. Because of high radiography rate of urinary system in special study, related instrument. All radiographer who take charge emergency patient should cope with a emergency situation during radiography. Because head trauma patients is very important in patient care, especilly in CT at night, charged doctor should be always sitted with CT room and monitoring patient. Radiography was reqested by many diagnostic department in ER. Considering that rate of simple radiography is high, special room for emergency radiopraphy should be established in ER area, and the radioprapher of this room should be stationed radiologic technician who is career and can implement emergency patient care and The disposition of men which is appropriate with emergency patient increase is necessary.

Ultrasound-Guided Axillary Brachial Plexus Block, Performed by Orthopedic Surgeons (정형외과 의사가 시행한 초음파 유도 액와 상완 신경총 차단술)

  • Purpose: The purpose of this study was to assess the effectiveness and complications of an ultrasound-guided axillary brachial plexus block performed by orthopedic surgeons. Materials and Methods: From March to May 2017, an ultrasound-guided axillary brachial plexus block was performed on a total of 103 cases of surgery. A VF13-5 transducer from Siemens Acuson X300 was used. The surgical site was included in the range of the anatomic sensory distribution of the blocked nerve, except for the case where an operation time of more than 2 hours was expected due to multiple injuries and the operation of the upper arm. The procedure was performed by 2 orthopedic surgeons in the same method using 50 ml of solution (20 ml of lidocaine HCl in 2%, 20 ml of ropivacaine in 0.75%, 10 ml of normal saline in 0.9%). The success rate of anesthesia induction during surgery, anesthetic induction time, anatomical range of operation, duration of postoperative analgesia and complications were investigated. Results: The results from the 2 practices were similar. The anesthesia was successful in 100 out of 103 patients (97.1%). In these patients, the average needling time was 5.5 minutes (2.5-13.2 minutes), the average induction time to complete anesthesia was 18.4 minutes (5-40 minutes), and the average duration of postoperative analgesia was 402.8 minutes (141-540 minutes). The post-anesthesia immediate complications were dizziness in 1 case, nausea and vomiting in 4 cases, and peri-oral numbness in 2 cases, but surgery was performed without problems. All these 7 cases with complications recovered on the same day. A total of 3 cases failed with anesthesia, and they were treated by an injection with local anesthesia in the operation room in 2 cases and switched to general anesthesia in 1 case. Conclusion: An ultrasound-guided axillary brachial plexus block, which was performed by orthopedic surgeons allows anesthesia in a brief period and the high success rates of anesthesia for certain surgeries of the elbow and surgeries on forearm, wrist and hand. Therefore, it can reduce the waiting time to the operating room. This technique is a relatively safe procedure and dose selective anesthesia is possible.

Enhancement of Immune Activity of Spirulina maxima by Low Temperature Ultrasonification Extraction (저온 초음파 추출에 의한 Spirulina maxima 면역활성 증진)

  • The marine microalga Spirulina maxima was extracted using water or ethanol at 100 or $80^{\circ}C$ and by ultrasonification in water at $60^{\circ}C$. The ultrasonification technique generated the highest yield (19.8%). To be therapeutically useful, the extraction should yield a product with low cytotoxicity and high immunity against skin infections. The cytotoxicity of all extracts (1.0 mg/mL) was below 25%. Moreover, the cytotoxicity of the extract generated by ultrasonification was 5%. Extracts prepared in the described manners could inhibit hyaluronidase activity by up to 40% compared to the control. Increased growth of human B, T and NK cells and an increase in cytokine secretion were observed, confirming the interrelationship between both human immune and skin immune activity. The extract prepared by ultrasonification increased the growth of human B, T and NK cells up to $10.3{\times}10^4$ cells/mL, $11.3{\times}10^4$ cells/mL and $19.1{\times}10^4$ cells/mL, respectively. The extract prepared by ultrasonification also greatly increased the secretion of both IL-6 and $TNF-{\alpha}$. Moreover, it was estimated that protein, Na and leucine occupy a high ratio. Accordingly, this study has confirmed that extracts prepared as described have the potential to effectively increase skin immunity.

Enhancement of Immune-Potentiation of Cichorium endivia L. by Ultrasonification Extraction Process (초음파 추출물을 이용한 치콘의 면역활성 증진)

  • Immune-potentiation of Chicorium endivia L. were investigated on follows extracts associated with ultrasonification process at 60 kHz and showed the highest promotion of human B and T cell growth, about $10{\sim}20%$ compared to the control. The secretion of TNF-${\alpha}$ and IL-6 was also enhanced by the addition $(0.5mg/m{\ell})$ of the extracts. NK cell activation was Improved up to 1.37 times higher than the control, through adding extracts. It was also found that extracts from C. endivia L. could yield higher nitric oxide production from macrophage than Lipopolysaccaharides (LPS). It can be concluded that, in general, the extracts treated with ultrasonification has higher immune activity than others, possibly by higher yielding immune-modulatory activity than conventional extraction process. The optimum condition for the extraction of C. endivia L. is ethanol extraction at $60{\sim}100^{\circ}C$ associated with ultrasonification.

  • The application of the ultrasonic nozzle has been extended because it is possible atomization of liquid material. In this study, the driving characteristics of the ultrasonic nozzle on the driving circuit were investigated. And the characteristics of the ceramic oscillator were investigated for the temperature stability. The ceramic oscillator were made the Pb[(Sb$\sub$1/2/,Nb$\sub$1/2/)$\sub$0.035/-(Mn$\sub$1/3/Nb$\sub$2/3/)$\sub$0.065/- (Zr$\sub$x/Ti$\sub$l-x/)$\sub$0.9/]O$_3$with mole ratio of Zr/Ti. The ceramis oscillator were need the curie temperature of the over 300[$^{\circ}C$] for the temperature stability. When the Zr/Ti ratio was 49/51, it's curie temperature is 322[$^{\circ}C$] and the electromechanical coupling factor(k$\sub$p/) and mechanical quality factor(Q$\sub$m/) showed the values of 0.555, 1,214, respectively The resonance frequency of ceramic oscillator were from 40KHz to 45KHz. So that, the driving circuit were made a possibility that the frequency are variable. The driving current of ultrasonic nozzle showed the value of maximum 80[mA]. Also, The surface temperature of ceramic oscillator showed 80[$^{\circ}C$] at driving time 10[min]. We knew that the ultrasonic nozzle had stabile driving above 10[min.].

3-D Underwater Object Restoration Using Ultrasonic Transducer Fabricated with 1-3 Type Piezoceramic/Polymer Composite and Neural Networks (1-3형 복합압전체로 제작한 초음파 트랜스듀서와 신경회로망을 이용한 3차원 수중 물체복원)

  • In this study, the characteristics of Ultrasonic Transducer fabricated with PZT-Polymer 1-3 type piezoelectric ceramic/polymer composite are investigated. 3-D underwater object restoration using the self-made ultrasonic transducer and modified SCL(Simple Competitive Learning) neural network was presented. The ultrasonic transducer was satisfied with the required condition of commerical ultrasonic transducer in underwater. The modified SCL neural network using the acquired object data $16\times16$ low resolution image was used for object restoration of $32\times32$ high resolution image. The experimental results have shown that the ultrasonic transducer fabricated with PZT-Polymer 1-3 type piezoelectric ceramic/polymer composite could be applied for SONAR system.
