Academic interest has been drastically increased for the health disparities due to socioeconomic factors. For those who have a chronic illness such as diabetes, various psychosocial barriers related to illness management might affect and aggravate this disparity. This study focused on illness management barriers experienced by people with diabetes, and examined the differences by their Medicaid beneficiary status. The between group differences in illness management barriers, family support and health status were examined as well as the association between illness management barriers and health status. The sample of this study consists of 144 community dwelling adults who have diabetes, recruited from an outpatient diabetes clinic and a community welfare center. Medicaid beneficiaries reported poorer health status, and experienced more illness management barriers compared to their counterparts. Statistically significant differences were observed in barriers due to mental health, financial status, and lack of support. The between group difference in health status remained significant after controlling for the effect of demographic characteristics and illness related factors. Social work practitioners working with this population should address these illness management barriers to reduce socioeconomic health disparity.
목장 경영의 성공과 실패에는 여러 가지 요인이 작용하지만 연중 얼마나 적은 소가 도태되는가 하는 것 또한 아주 중요하게 생각해 보아야 할 사항이다. 낙농에 있어 거의 대부분의 질병이 산욕기에 발병하는데 산욕기(産褥期)란 분만으로 인한 상처가 완전히 낫고, 자궁이 평상시 상태가 되며 신체의 각기관이 임신 전의 상태로 회복되기까지의 기간으로 대개 분만 후 6~8주간을 말한다. 산욕기에 주로 발생할 수 있는 다양한 질병(산욕기 부전마비, 난산, 급성유방염, 제4위 전위증 등)들이 조기에 발견되어 적절히 치료되지 못하고 만성화되면서 전해질불균형, 에너지와 단백질의 섭취부족에 의한 영양장애로 허약과 지방간 및 케토시스 등으로 진행되어 결국 도태됨으로서 목장 경영에 있어 막대한 영향을 끼치는 것이다. 이러한 산욕기 질병의 주 원인은 건유기 사양관리에 있다. 건유기간 동안 체점수(BCS)의 변화를 최대한 줄이면서 분만전의 스트레스를 얼마만큼 줄일 수 있도록 관리하는가에 달려있다. 다시말해 분만 전 마지막 한달간에 건유에서 분만으로의 이행과정을 얼마나 매끄럽게 연결 지어주는가에 따라 산욕기 질병의 발생률이 결정되는 것이다. 본 고에서는 건유기 사양관리 소홀로 발생되는 산욕기 부전마비에 관해서 자세히 설명해 보고자 한다.
The study aimed to identify risk factors for falls as well as hospitalization status according to disease and demographic characteristics of demented inpatients by investigating the in-depth Injury Patient Surveillance System data collected by Korea Centers for Disease Control and Prevention(KCDC). Older adults over 60 years old who were diagnosed with dementia were included(n=1,732). Their data were analyzed after being assigned to either a fall group or a non-fall group. STATA was used for statistical analyses, such as frequency analysis, chi-square (χ2) test, and logistics regression. It was found that 8.0% of the demented inpatients experienced falls. According to the analysis on category of fall and non-fall group were statistically significant difference in age and Charlson Comorbidity Index(CCI) and bone density deficiency. Based on the logistic regression analysis of factors affecting falls, older adults over 80 are 2.386 times more likely to fall and based on a target with a CCI of 0, the risk of falls is 0.421 times lower, finally based on those without bone density disorder, the fall risk for those with bone density disorder was 3.581 times higher. Therefore, we expect that the important about the factors relating to falls identified in this can not only be found valuable for educating inpatients with dementia and care-givers, but also be used as reference that supports clinical professionals to make decisions on falls management for patients with dementia.

동물용의약품은 가축질병의 예방 및 치료의 목적으로 이용되어 그동안 우리나라 가축의 질병방제와 사료효율의 증진으로 양축농가의 소득증대와 아울러 축산이 대규모로 발전할 수 있게 하는데 기여한 바가 크다. 그러나 특히 항생제나 합성 항균제의 오남용에 따른 가축의 장내 세균과 병원성 세균의 항생항균제에 대한 내성획득으로 질병치료 효과의 저하, 축산식품을 통하여 이들 제제의 잔류물질이 인체내로 들어가 부작용을 일으키거나 세균의 내성인자가 사람 병원체에 전달되어 사람의 질병치료에 장애요인이 될 수 있는 문제점이 제기되고 있다. 그리고 공중위생전인 측면에서 항생항균제, 호르몬제, 생물학적제제(백신 등), 마취제, 마약류, 등의 요주의동물용의약품을 체계적으로 관리할 필요성이 제기되고 있다. 본 주제에서는 동물용의약품 중 현재 가장 많은 량이 이용되고 있는 항생제를 중심으로 사용실태와 오남용 사례를 살펴보고자 한다.
The purpose of this study is to examine the factors associated with the probability and levels of the health service utilization among the disabled elderly in Korea. The sample includes 2,111 people older than 65 who are extracted from the 2008 National Survey on People with Disabilities. More than half (54.3%) of the sample experienced at least 1 outpatient physician visit within two weeks and 26.7% were hospitalized within a year. The key factors associated with the outpatient visits were health insurance status, the existence of chronic disease, self-rated health, the Activities of Daily Living (ADLs), as well as renal impairment. Similarly, the utilization of inpatient care was related to health insurance status along with the existence of the internal organ disabilities such as cardiac or respiratory disorders. The study implies the need for the health care policies regarding the prevention of chronic diseases, dependency for daily activities of the elderly, and a management system that specifically targets those with internal organ disabilities. Moreover, the study suggests that financial supports for the low-income group would be helpful to increase their access to health service utilization.

We extract the symptom of panic disorder from the context awareness environment. It extracts body context information through natural movement that exists in everyday life and uses a component of panic disorder. The ontology theory can be used to provide information on the degree of symptoms of panic disorder through inference process. For the components of panic disorder to the information processing based on ontology are defined as Classes. Panic disorder index is expressed through ontology modeling so that the condition of panic disorder can be known. The derivation of panic disorder component and panic disorder index will enable context awareness based information service for panic disorder. The context information is periodically synchronized with the context awareness on based device. Panic disorder can be used to improve the lifestyle of panic disorder.
The purpose of this study was to identify factors influencing sleep disturbance among benign prostate hyperplasia (BPH) patients in South Korea. A trained reporter filled out the questionnaire by explaining and reading the sentences to 164 BPH patients in J city. The questionnaire consisted of international prostate symptom score (IPSS), anxiety, depression, sleep disturbance. There was a statistically significant difference in sleep disturbance of participants according to income, regular exercise, and number of disease. There were statistically significant correlations between sleep disturbance and IPSS (r=.45, p<.001), anxiety (r=.59, p<.001), depression (r=.42, p<.001). The influential factors for sleep disturbance were anxiety (${\beta}=.41$, p<.001), income (${\beta}=-.36$, p<.001), IPSS (${\beta}=.28$, p=.021). These factors accounted for 41.2% of the variance in sleep disturbance. The result of this study can be used as a basic data for development of a management strategy considering convergent relationship between IPSS and anxiety in order to decrease sleep disturbance of BPH patients.

Objective : The purpose of this study was to investigate the risk factors of dehydration from the subjects who underwent anthropometric and blood parameters testing during a comprehensive health screening. Methods : For the study analysis, 5,391 samples with valid data of the levels of Sodium($Na^+$), BUN (Blood Urea Nitrogen) and FBS(Fasting Blood Sugar) were selected to calculate a dehydration indicator of plasma osmolality. The study data was collected from the health screening examinees who visited Sahmyook Medical Center Seoul Adventist Hospital Comprehensive Health Check-up Center from 2014.01.01 to 2015.12.31. The relationship between dehydration and age group, BMI, disease exposures(hypertension, diabetes mellitus, dyslipidemia, kidney disorder) were analyzed by gender. Results : The odds ratio of dehydration showed statistical significance from age ${\geq}50$ in both male and female, respectively. The female obese group was vulnerable to dehydration while the male study group showed no statistical significance in the BMI difference. The disease exposed groups(hypertension, diabetes mellitus, dyslipidemia, kidney disorder) were vulnerable to dehydration. Also, the more types of disease carried by the exposed patients, the higher odds ratio and susceptibility to dehydration. Conclusions : Aging, increasing BMI, and exposed to diseases were found to be the risk factors for vulnerability to dehydration. To prevent dehydration, special caution to be taken for those in the ${\geq}50s$ group, along with controlling BMI and chronic diseases. Further studies are suggested to investigate the risk factors of dehydration that may affect increasing plasma osmolality as a potential stimulus mechanism in disease outbreaks.

This study was conducted to examine the therapeutic compliance and its related factors in rural women with osteoporosis. A questionnaire survey was performed from April to May in 2000 for 140 osteoporotic patients who were diagnosed from April to June in 1999 through community health program. The study employed the health belief model for predicting and explaining sick role behavior. The analysis techniques employed included contingency table analysis and path analysis using LISREL. The major results of this study were as follows: Of the subjects, 12.1% were continuously complaint, 53.6% were intermittently compliant, and 34.3% were non- compliant to calcium supplement therapy. As the result of path analysis, the therapeutic compliance was significantly higher(${\mid}T{\mid}$ >2.0) as patients had higher perceived severity of disease, lower perceived barriers of treatment, and when patients thought their disease status as severe. As the patients had higher educational level, more experience of mass media contact or health education about osteoporosis, and when family had more concern for patient treatment, they had higher perceived susceptibility of complication(bone fracture)${\mid}T{\mid}$ >2.0). The patients had higher perceived severity(${\mid}T{\mid}$ >2.0) as they had more educational level, more advice for treatment from their doctors, and when family had more concern for their treatment. In order to improve the therapeutic compliance in rural osteoporotic women, it would be necessary that the patient should recognize their disease severity properly. And the perceived barriers should be removed through supportive environments for osteoporosis treatment such as doctor 's more advice and family 's more concern for treatment. In addition, effective and continuous management system for osteoporotic patients should be established.
