임플란트 보유자의 진료기관 내 구강보건교육경험에 따른 구강건강관리 수준 (Oral hygiene maintenance in patients with dental implants following oral health education)

  • Objectives: The aim of this study was to investigate the impact of oral hygiene education provided by dental hygienists in patients with dental implants. Methods: Data were obtained from a survey that involved 300 patients who had one or more dental implants. Based on the calculated sample size (n = 274), 300 questionnaires were used for further analyses. Results: The patients who received oral hygiene education had higher levels of satisfaction after dental implant surgery (p<0.05). Patients who had repeated educational sessions, in the form of interactive or hands-on sessions or lectures, with continuous feedback had higher scores for oral hygiene maintenance (p<0.05). Conclusions: This study indicated that patients who received oral hygiene education, in the form of interactive or hands-on sessions or lectures, with continuous post-educational feedback were better at maintaining oral hygiene.

  • Background: The number of patients with disabilities who require general anesthesia for dental treatment is increasing. Methods: We reviewed the cases of 538 patients with disabilities who underwent outpatient general anesthesia for dental treatment at the clinic for the disabled in Seoul national university dental hospital according to the types of disabilities and comprehensive treatments. Results: The patients mean age was 17.4 years and fifty-eight percent of the patients had epilepsy, autism, mental retardation and cerebral palsy. About twenty-five percent of the patients were medically compromised such as brain disease, heart disease and genetic problem. In 142 cases, more than two dentists from the clinical departments systemically co-worked to provide the patients with comprehensive treatment as accurately and quickly as possible under general anesthesia. Conclusions: The number of medically compromised patients was high and the system of comprehensive dental treatment was well run.

치과진료실에서 흔한 긴급상황의 처치 (How to cope with medical emergencies in the Dental Clinic)

  • Conceptually, the emergency is classified as an urgency and an emergency. The Urgency is not immediately life threatening, but could become so if not resolved promptly. So, it requires prompt patient care same as in the emergency situation. However, the emergency is immediately life threatening and requires immediate action, such as calling 119 and basic life support. Most medical emergencies in the dental clinic cases are urgencies. The incidence of true emergencies is approximately 1/1,000,000. Adequate managing medical urgencies are important because the chance of encountering medical emergencies in the dental clinic is high and higher these days especially because of rapidly aging Korean society. Many dentists indicate that many dentists feel difficulties to recognize and treat medical problems. This paper reviews the concept of medical emergencies and how to cope with commonly occurring urgencies in the dental clinic such as loss of consciousness, hypoglycemia, hyperventilation syndrome. The best treatment for medical urgencies and emergencies in the dental clinic is prevention. Also, it is required to make preparation for emergency situations such as CPR education for dentists and being well-acquainted with equipment and drugs for the emergency care as well as to systemic medical evaluation, patient monitoring, and sedations for controlling patient's anxiety and pain. In this paper, simple algorithms based on guidelines for common urgencies in the dental clinic are suggested. In conclusion, every dentist has competencies to do the urgency care adequately and basic life support. Also, advanced cardiac life support is strongly recommended when sedation is performed in the clinic.

당뇨병 환자의 청각장애에 대한 관찰 (Clinical Observation on Hearing Impairment in Diabetes Mellitus Patients)

  • 1976년 1월부터 동년 12월까지 본병원 당뇨병진료실을 방문하여 당뇨병으로 진단된 환자 중 청력검사를 시행한 43례(남자 25례, 여자 18례)와 그중 청각장애자 8례를 대상으로 임상적 소견 및 그 상호관계를 관찰하여 다음과 같은 결론을 얻었다. 1) 당뇨병환자 43례 중 청각장애자는 8례이며, 청력형은 고음급추형 및 고음점경향이 대부분이다. 2) 중연속 및 고연령층에 청각장애자의 출현빈도가 높다. 3) 이식기간과는 무관하나, 혈당치 및 혈청총 cholesterol치와는 어느 정도 상관관계가 있다. 4) 강막증, 고혈압증 및 신경증 등 합병증을 동반한 례에 청각장애자가 많다. 5) 당뇨병환자의 청각장애의 성립요인은 단일한 것이 아니고 여러가지 요소가 관여하는 것으로 생각된다.

일부지역 치과위생사의 치과진료실 실내공기질에 대한 만족도 및 자각증상 비교 (The satisfaction and subjective symptom level by indoor air quality in dental parlor in Capital and Jeon-nam area)

  • Objectives : A precedent research has documented that indoor air pollution is closely associated with increased the risk of symptom and decreased in job satisfaction. This study was conducted in an effort to assess the relationship of indoor air quality to self-perceived symptom and satisfied with working environment. Methods : This research is based on self-filling survey which 393 dental hygienists who work in seoul and Jeon-nam area participated on October 2010 through January 2011. This survey was compared and analyzed about the level of satisfaction and subjective symptom by the indoor air quality that dental office's working environment. Results : The result of satisfaction degree of hospital working environment was pretty low about office air. Most people mentioned that they were sore and dull all over the back, shoulder, and neck. Also, they were drowsy and exhausted in work place. Conclusions : In conclusion, it is need to improve their quality of life and mental health by developing proper air conditioning system and using one in their work place.

e-Healthcare 환경 내 개인정보 보호 모델 (Privacy Information Protection Model in e-Healthcare Environment)

  • 인터넷 등의 정보기술의 발전은 기존의 의료기술에 빠른 변화를 가져오면서 e-Healthcare가 사회적 이슈로 등장하고 있다. 의료정보화 패러다임의 새로운 전환점이라 할 수 있는 e-Healthcare는 국내에서 의료정책방안이나 기술개발을 하고 있지만, 아직 의료정보화의 기반이 되는 인프라는 부족한 수준이며 개방된 인터넷 환경 내 역공학적 측면으로 민감한 의료정보 유출 및 프라이버시 침해에 대한 문제가 대두되는 실정이다. 본 논문에서는 앞서 제시한 문제점의 해결방안으로 e-Healthcare환경 내 개인의 의료정보 보호를 위한 역할기반의 접근제어 시스템(HPIP - Health Privacy Information Protection)을 네 가지 주요 메커니즘(사용자 신분확인, 병원 권한확인, 진료기록 접근제어, 환자진단)으로 제안하였으며, 실 환경에서 효과적으로 활용될 수 있도록 프로토타이핑을 통해 그 가능성을 타진해 보았다.

공공의료기관 간호사의 조직몰입, 환자안전문화인식이 환자안전간호활동에 미치는 영향 (The Effect of Organizational Commitment and Perceived Patient Safety Culture on Patient Safety Nursing Activities in Public Hospital Nurses)

  • Purpose: This study was conducted to identify and analyze organizational commitment, patient safety culture, and patient safety nursing activities on patient safety in public hospitals. Methods: The data were collected from 190 nurses in 6 public hospitals within the Gyeonggi region and were analyzed using SPSS 23.0 for descriptive statistics and multiple regression. Results: Patient safety nursing activity was positively correlated with the number of participants in patient safety training programs, organizational commitment, and patient safety culture awareness. Patient safety culture awareness was positively correlated with the number of participants in patient safety training programs and organizational commitment. Organizational commitment was positively correlated with age and total work experience. Factors affecting patient safety activities, frequency of patient safety training, and patient safety culture awareness were significant influencing factors with an explanatory power of 30.1%. Conclusion: In order to increase patient safety nursing activities in public hospitals, systematic patient safety training policies are required. In addition, diverse interventions are required to increase organizational commitment.

국내 반복입원의 현황과 환자 특성: 외래진료 민감질환을 중심으로 (Current Status of Repeated Hospitalization in South Korea: Focused on Ambulatory Care Sensitive Conditions)

  • Purpose: Repeated hospitalization could be a proxy of unnecessary or preventive admission in South Korea where barriers to hospitalization are relatively low. This study aimed to estimate the current status of repeated hospitalization due to ambulatory care sensitive conditions (ACSC) in South Korea. Methods: Using the National Health Information Database, repeated hospitalization databases were constructed in units of episodes for patients who had been admitted more than twice between January 2017 and December 2018. The number of hospitalizations, total in-hospital days, and total medical expenditure were calculated and compared by patient characteristics in both of the entire patient group and the ACSC patient group. Results: Of total hospitalization episodes, 26.6% reported repeated admission, and 6.7% of repeated hospitalization was due to ACSC. A total of 183,110 patients with ACSC had been admitted an average of 2.9 times and spent an average of KRW5,630,118. In other words, KRW1,309 billion had been spent for repeated hospitalization due to ACSC. The scale of medical expenditure was relatively large in the highest and lowest socioeconomic status. Conclusion: Repeated hospitalization for ACSC can be considered a simple and intuitive indicator when assessing unnecessary hospitalizations or evaluating healthcare policy.

  • 최근 저출산 문제로 인한 노동인구의 감소로 노동력이 감소하며 인건비가 증대되고, 의료 기술의 발달과 고령화로 인해 병실의 수용 인원이 증가했다. 특히 중환자, 거동이 불편한 환자, 노인 환자와 같은 간병인의 도움과 의료 종사자의 지속적인 진료의 필요성이 증대되고 있다. 환자의 상태를 일정 시간마다 확인해야 하는 불편함, 간병인 고용의 필요로 기본적인 병원비와 인건비가 증가되고, 의료업계 종사자들의 부담도 늘어나는 추세이다. 따라서 이러한 문제를 해결하기 위해 중앙 관제실에서 환자의 상태를 실시간으로 확인하고 자동으로 병실을 관리하는 시스템을 도입하고자 한다. 정보를 실시간으로 전달함으로써 의료진의 부담을 줄이고 위기 상황에 즉각적으로 대응하며, 자동 병실 환경관리와 환자 편의를 위한 의료 환경을 지원하는 시스템을 제안한다.

구강안면통증클리닉에 처음 내원한 턱관절장애 환자들의 내원 목표 (Expectations of the First Visit to Orofacial Pain Clinic for the Patients with Temporomandibular Disorders)

  • 환자가 진료실에 들어설 때 어떤 기대와 목표를 가지고 있는 지 파악하여 치료에 적절히 반영한다면 환자의 치료만족도를 향상시키고 보다 나은 치료효과를 얻을 수 있을 것이다. 본 연구는 TMD 환자의 첫 내원 시 면담과 치료에 대한 기대감을 조사하고, 이러한 내원 목표가 과거치료의 유무나 통증 정도 및 통증으로 인한 일상생활의 제한 정도 같은 요소들에 따라 달라지는지를 조사하고자 하였다. 지난 3개월(2010년 8월부터 11월) 동안 단국대학교 치과대학 부속병원 구강내과에 내원하여 TMD로 진단된 18세 이상의 성인 환자를 대상으로 담당의사와의 첫 면담 전 대기실에서 환자기대조사 설문지와 간이통증목록(Brief Pain Inventory, BPI)을 작성하게 하여 분석하였다. 322명의 TMD 환자들의 남녀 비는 1:1.5 이었고 평균연령은 약 36세이었다. 본 연구 결과 TMD 환자들이 병원을 찾을 때 가장 중요한 Top 3의 기대목표는, '통증이 완전히 나았으면 좋겠다(cure of pain)', '왜 아픈지 이유를 알고 싶다(understanding)', '자신의 증상에 대해서 의사와 충분히 상의하고 싶다(doctor-patient communication)'로 조사되었다. 성별이나 병력기간, 과거치료 경험에 따른 환자 내원 목표의 차이는 관찰되지 않았다. TMD를 세분하였을 때, 근육장애, 특히 복합(관절-근육)장애 환자군에서 통증조절을 가장 중요하게 생각하는 비율이 상대적으로 높았고, 자신이 아픈 이유를 알고자 기대가 큰 반면, 관절장애군은 의사소통과 검사에 대한 기대가 상대적으로 더 컸다(p<0.005). 또한 통증과 통증으로 인한 일상생활 제한이 심할수록 통증조절을 가장 중요하게 생각하는 환자의 비율이 증가하였으며, 통증 강도가 낮은 환자들의 경우 의사-환자간 communication에 대한 기대가 상대적으로 컸다(p<0.05). 이러한 결과로 볼 때 TMD 환자의 치료에서 효과적인 통증조절은 가장 우선되어야 할 치료목표이지만, 질병의 원인과 기전에 대한 충분한 설명과 진단과 치료에 대한 의사-환자 간의 충분한 의사소통을 통하여 환자의 진료 만족도를 더욱 향상 시킬 수 있을 것으로 판단된다. 질병에 대한 환자이해를 돕고 의사소통을 충분히 하기 위해 환자중심의 상담과 자료의 적극적인 활용이 필요하겠다.
