Intervention of Swallowing Disorder in Dementia Patient: A Systematic Review (치매환자의 삼킴장애 중재에 관한 체계적 고찰)

  • Purpose : The purpose of this study was to establish scientific evidence though a systematic review, of the existing interventional methods and their respective effects, for swallowing disorders among dementia patients. The interventional methods were based on patient characteristics. Methods : An electronic literature search was performed using five electronic databases from 2010 to 2020. The keywords were 'Dementia', 'Alzheimer disease', 'Deglutition disorder', 'Swallowing disorder', 'Dysphagia'. Five articles met the inclusion criteria. Results : One study with moderate-to-severe and four studies with severe dementia patients were included. The interventions consisted of one remedial intervention, three compensatory interventions, and one comprehensive intervention. Compensatory intervention, including behavioral and psychological management, were used in moderate-to-severe patients. For the severe patients, remedial, compensatory, and comprehensive intervention were used to address the feeding tube problem. The interventions used in each study had a positive effect on the outcome variables associated with swallowing ability. Conclusion : The purpose of the intervention and the intervention factors changed depending on the patient's dementia stage. The intervention was effective when considering the progressive stage of the patient. These results may serve as a guide in designing effective interventions for swallowing disorders in dementia patients.

Determinants of Length of Stay in Geriatric Hospitals - Focused on Alzheimer Dementia's Inpatients (요양병원 입원 알츠하이머 치매노인의 재원일수 결정요인 분석)

  • This study purposed to analyze the determinants of length of stay for patients having alzheimer dementia in geriatric hospitals. Sample data was collected from the National Patient Sample (NPS) produced by the Health Insurance Review & Assessment Service (HIRA) in 2009. Total 538 alzheimer inpatients over 65 years were used in the analysis. Patients were classified into 5 risk groups to represent the case-mix differences of patients. Organizational characteristics of geriatric hospitals such as number of beds, doctors, and nurses were included in the analysis model. In multivariate regression analysis, hospital variables were not statistically significant in explaining the length of stay. Two risk group were statistically significant in analysis, However, their signs of relationship with the dependent variable were opposite to the expectations. These results suggest that the characteristics of patients and hospitals did not have impacts on the length of stay. There will be needs to test the effects of other factor such as social needs which represents the socio-economic status of the family for the patients.

  • Since the late 1990s, public nursing hospitals have been established, and as of 2018, 79 hospitals account for 5.1% of all nursing hospitals. As part of the study to conduct the operation evaluation of public nursing hospitals to be implemented from 2020, the research established a function to foster public nursing hospitals as specialized institutions for dementia patients and competent institutions for treating rehabilitation patients during the period of recovery, and developed an evaluation system to evaluate their behavior as medical services, public nature and public institutions.

  • 본 연구는 주기적인 알츠하이머 병의 중증도 모니터링을 위해 스마트 인솔을 통한 보행 특징 추출과 머신러닝 기반 중증도 분류의 성능에 대해 살펴보았다. 최근 고령화가 가속화되는 추세에 있어 치매 환자가 급증하고 있으며, 중증도가 심해질수록 필요한 치료 비용 및 노력이 급증하기 때문에 조기 진단이 최선의 치료 전략으로 보여진다. 환자 친화적이고 저비용의 관성 측정 장치가 내장된 스마트 인솔만을 사용하여 다양한 보행 실험 패러다임에서 환자의 보행 특징을 추출하고, 이를 알츠하이머 병의 중증도 진단을 위한 머신러닝 기반 분류기를 훈련시켜 성능을 평가한 결과, 숫자세기와 같이 뇌에 부하를 주는 하위 작업이 포함된 복합 보행을 측정한 데이터셋을 사용하여 훈련된 분류 모델이 일반 걷기 데이터셋을 사용한 모델보다 성능이 높게 나타나는 것이 관찰되었다. 본 연구는 안전하고 환경적 제약이 적은 방법을 사용하여 시기 적절한 진단뿐만 아니라 주기적인 중증도 모니터링 시스템의 일환으로 활용될 수 있을 것이다.

  • VR based dementia service is emerged as elderly population is increasing with potential welfare cost. Current Dementia Center cannot solve the complicated problems, including limited size, case management, medical support, medical cost and care cost, and the lack preventive environment. VR based dementia service is suggested from three reasons: First, VR shows easy access and convenience; Second, development of cognitive health is easy and fun; Third, Predictive way of cognitive decline can be possible with Big Data.

Factors Affecting the Quality of Life of Patients With Dementia: A Systematic Review of Literature Focusing on the Subjects Admitted to an Overseas Long-term Care Facility (치매 환자의 삶의 질에 미치는 요인 분석: 국외 요양시설에 입소한 대상자를 중심으로 체계적 문헌고찰)

  • Objective: The purpose of this study was to perform a systematic literature review focusing on international studies to identify various factors affecting the quality of life(QOL) of dementia patients admitted to long-term care facilities. Methods: From January 2000 to July 2018, the articles published in foreign journals were searched through CINAHL and MEDLINE databases. The main search terms were'dementia or Alzheimer's'. 'quality of life', 'long-term care', 'care home', 'nursing home', 'care institution', 'residential care', 'small- scale setting'. The first 1706 articles were searched, but 10 studies were selected using the selection and exclusion criteria. Results: Analysis of ten cross-sectional studies showed that factors such as physical functioning status, ability to perform activities of daily living, and cognition showed a positive correlation with QOL. Depression and anxiety, severity of dementia, and neuropsychiatric symptoms were found to be negatively correlated with QOL. In addition, two longitudinal studies have confirmed that factors affecting QOL are affected by individual functional status and social environmental factors rather than the size and form of care facilities. Conclusion: This study summarized 10 papers and analyzed them through a systematic review of literature. We found that factors such as individual characteristics and social environment determine QOL of patients with dementia. In this study, various intervention methods to improve QOL of patients with dementia should be developed and used in long-term care facilities by identifying the factors affecting QOL of dementia patients and using them.

Evaluation of Muscle Load and Fatigue According to the Shape of Severe Dementia Patients' Clothing (중증 치매환자복 형태에 따른 근육 부하 및 피로도 평가)

  • The purpose of this study is to obtain information necessary for the development of patient clothes that can reduce physical fatigue of caregivers by quantitatively measuring the muscle load and fatigue. The patient clothes used in this study can be broken down into three types: A type (back center zipper open suit), B type (top-to bottom separated patient clothes), and C type (front zipper open suit). The EMG measurement sites are as follows: hand muscle (brachioradialis), upper arm (biceps, triceps), shoulder (anterior deltoid, medial deltoid, posterior deltoid, upper trapezius), and waist (erector spinae); additionally, the EMG signals were measured. Through this experiment, muscle load, muscle energy consumption, and muscle fatigue generation tendency were analyzed. The results of the study revealed that the C type patient clothes required the most strength in the muscles of the shoulders, upper arms, hands, and back when being put on and taken off compared to other patient clothes. The A type clothes required a relatively large force in opening the zipper. In terms of muscle energy consumption, B type generally called for more strength when it came to the zip-up and putarmsup motions. With regard to the cover the body and put legs/hips up motions, C type used the highest amount of muscle energy, whereas A type used relatively little energy. In terms of the occurrence of muscle fatigue during the putting on and taking off of the patient's clothing, there was a difference in the area and degree of muscle fatigue in the A, B, and C types, and there was also a tendency for muscle fatigue to occur when performing repetitive movements.

Analysis of domestic dementia research trend for integrated study (융복합 연구를 위한 국내 치매 관련 연구동향 분석)

  • The purpose of this study is to contribute to research on dementia and policy making by reviewing research trends of dementia for 15 years and it was collected from 12,588 dementia - related research data for 15 years at Google 's scalar site. The results of this study confirm that the research on dementia, one of the most common geriatric diseases, is continuously increasing. Second, research on dementia is increasing due to social influences such as government support. Third, we suggested that dementia is changing from a viewpoint of diseases to be managed at the facility and treatment of severe dementia patients to a paradigm from the viewpoint of management of mild patients and community participation. Fourth, dementia seems to be shifting from a serious and negative awareness to a social atmosphere that is accepted as a slow but manageable disease. This is changing to a dementia-friendly social environment, which is making a very positive environmental change. This study contributes to the field research by presenting the trend of domestic research on dementia.

Association between Cognitive function, Behavioral and Psychological Symptoms of Dementia and Temporal Lobe Atrophy in Patients with Alzheimer's Disease and Mild Cognitive Impairment (알츠하이머형 치매 및 경도인지장애 환자에서 인지기능 및 행동심리증상과 내측두엽 위축의 연관성)

  • Objectives : The aim of this study was to compare severity, neurocognitive functions, and behavioral and psychological symptoms of dementia (BPSD) according to the degree of temporal lobe atrophy (MTA) in Korean patients with dementia due to Alzheimer's disease and mild cognitive impairment due to Alzheimer's disease. Methods : Participants were 114 elderly subjects diagnosed with Alzheimer's disease or mild cognitive impairment in this cross-sectional study. MTA in brain MRI was rated with standardized visual rating scales (Scheltens scale) and the subjects were divided into two groups according to Scheltens scale. Severity was evaluated with Clinical Dementia Rating (CDR) and Global Deterioration Scale (GDS). Neurocognitive functions was evaluated with the Korean version of Short Blessed Test (SBT-K) and the Korean version of the Consortium to Establish a Registry for Alzheimer's Disease assessment packet (CERAD-K). BPSD was evaluated with the Korean version of the Neuropsychiatric Inventory (K-NPI). Independent t-test was performed to compare severity, neurocognitive functions, and BPSD between two groups. Results : The group with high severity of MTA showed significantly lower scores in CDR, SBT-K, MMSE-KC, modified Boston naming test, word list recognition, and word list memory (p<0.05). There were no differences in K-NPI scores between two groups. Conclusions : Severity and neurocognitive functions of dementia had significant positive association with MTA, but BPSD had no association with MTA. Evaluating MTA seems to have potential benefit in diagnosing and treating neurocognitive impairments in the elderly. Further evaluation is needed to confirm the association between certain brain structures and BPSD.

Development and Evaluation of Customized Clothing for Patients with Severe Dementia (중증 치매환자복 개발 및 사용성 평가)

  • The purpose of this study is to develop clothing customized for dealing with patients with severe dementia. Based on the results of previous studies, The research patient clothing was designed to reduce the physical fatigue experienced by caregivers when dressing and undressing patients by changing the position and shape of the split in the patient's clothing. This study used qualitative and quantitative methods to measure the extent to which these modifications improved the ease of dressing and undressing the research patient. The research patient clothing was developed by moving the rear-center zipper to the side and changing the zipper from being half-open to fully open. Muscle energy consumption and fatigue generation were analyzed using EMG signals at the following sites: brachioradialis, biceps, triceps, anterior deltoid, medial deltoid, posterior deltoid, upper trapezius and erector spinae. Results indicated that the modified research patient clothing required less muscle energy and the occurrence of muscle fatigue decreased overall compared to traditional patient clothing. This was supported by the qualitative subjective evaluation, which revealed that dressing and undressing was easier with the modified clothing. In conclusion, repositioning of the back zipper to the side and the fully open slit shape significantly reduced caregiver fatigue when dressing and undressing patients.
