Study on the Infection Control and Dental waste Disposal in Dental Clinic Located in Seoul City (서울시 치과의원의 진료실 감염관리 및 폐기물 처리 실태 조사연구)

  • This study was aimed to investigate the practice on the infection control and dental waste disposal in dental clinic. The data used in this study had been collected in 100 dental clinics located in Seoul city from August to October, 2001. It was shown that examination instruments, oral surgery instruments, and periodontal instruments were almost sterilization in dental clinic. It was high in the recognition of individual protection but it was low in the degree of practice. Therefore, the practice in infection control is very important to dental team for safety of patients and operators.

Intradermal Alcian-Blue Injection Method to Trace Acupuncture Meridians (경락추적을 위한 피내 알시안 블루 주입방법)

  • Objective In this article, we report on the intradermal Alcian blue staining method for tracing the meridians of acupuncture. Methods 1% Alcian blue solution was injected into acupoints by using a 0.5mL insulin syringe with a 31-gauge needle, then the skin was incised and was observed under a stereoscopic microscope. The specimens were examined by using immunohistochemical methods and were observed under a confocal laser scanning microscope. Results A threadlike structure, which was visualized with Alcian blue, existed in dermis layer and proceeded to hypodermis. In this structure, characteristic alignments of rod- shaped nuclei and $1-2{\mu}m$ sized DNA granules were observed. Furthermore, abundant blood capillary plexuses, peripheral nerve endings, and a corpuscle-like structure(about $300{\mu}m$ in diameter) were visualized in the skin tissues of acupoints. Conclusion It was concluded that the specific threadlike and corpuscle-like structures corresponded to superficial Bonghan duct and corpuscle, respectively.

Intracameral Epinephrine Injection after Phacoemulsification on Pupil Dilation during Phacovitrectomy for Proliferative Diabetic Retinopathy (유리체절제술과 수정체유화술 동시 수술 시 백내장수술 후 앞방내 에피네프린 주입과 동공 확대)

  • Purpose: We explored the mydriatic effects of injected intracameral epinephrine after phacoemulsification (PE) combined with phacovitrectomy to treat proliferative diabetic retinopathy (PDR). Methods: We enrolled 96 patients (96 eyes) who underwent phacovitrectomy to treat PDR; we used ImageJ software to measure pupil sizes and the ratios of pupil to cornea area (PCA) before and after PE and after pars plana vitrectomy (PPV). We compared pupil sizes between those who received intracameral epinephrine (0.001% w/v) after PE but before PPV (study group, 46 eyes) and patients not so treated (control group, 50 eyes). Results: The PCA ratios of the study group were $0.52{\pm}0.11$ before PE, $0.43{\pm}0.12$ after PE, and $0.51{\pm}0.11$ after PPV, respectively. Changes in pupil size were significant (p < 0.001, p < 0.001 respectively). The PCA ratios of the control group were $0.52{\pm}0.10$ before PE, $0.39{\pm}0.15$ after PE, and $0.43{\pm}0.15$ after PPV, respectively. Changes in pupil size after PE were significant (p = 0.011) but the change after PPV was not (p = 0.056). Conclusions: Intracameral epinephrine given after PE but before PPV effectively dilates the pupils during phacovitrectomy to treat PDR.

Scanning Electron Microscopic Study on the Wax Structures in Lipstick According to Storage Temperature After Mold (립스틱 성형 후 보관온도에 따른 왁스구조의 주사전자현미경적 연구)

  • This study shows that ceresin wax, candellila wax and microcrystalline wax mixed together with liquid paraffin oil to produce lipsticks (LS-1, LS-2) and capric/caprylic triglyceride oil added to produce lipsticks (LS-3, LS-4). After each type of lipsticks were molded, LS-1 and LS-3 was put into a cooling chamber ($5^{\circ}C$). LS-2 and LS-4 was put into a cooling chamber ($5^{\circ}C$) for 18 hours and kept in an incubator ($45^{\circ}C$) for 5 hours and put again into a cool chamber ($5^{\circ}C$). After that, the wax's three dimensional network structure was observed under scanning electron microscopy. Regardless of the kind of oil, the LS-1 and LS-3 wax structure had more distinct shape than the lipstick wax structure of LS-2 and LS-4. Also, regardless of the kind of wax, the three dimensional network structure was modified as the storage temperature increased. As a result, the lipstick's molding temperature increased, the wax's structure size also increased and the shape irregularly modified. This modification causes sweating phenomenon which affected lipstick's surface rheological property.

Effects of Foot Massage and Supportive Communication by Hospice Volunteers on Depression, Anxiety, and Mood of Cancer Patients Who Undergo Intravenous Chemotherapy at Out-patient Department (호스피스 자원봉사자의 발마사지와 지지적 의사소통이 외래 항암화학요법 환자의 우울, 불안 및 기분에 미치는 효과)

  • Purpose: This study was done to investigate effects of foot massage and supportive communication provided by hospice volunteers on depression, anxiety, and mood of cancer patients who are going under intravenous chemotherapy at out patient department (OPD). Methods: A non-equivalent control group non-synchronized design was used. Sixty-four participants were recruited from out-patients setting in a university hospital by a convenient sampling. Two trained- hospice volunteers first provided regular hospice volunteer service to 34 patients in the control group. Subsequently, foot massage and supportive communication were provided to 30 participants in the experimental group. After the intervention, all participants answered the structured questionnaire with visual analog scales on depression, anxiety, and mood including general characteristics. Data were analyzed using descriptive statistics, two sample t-test, and paired t-test by PASW 17. Results: After the intervention, changes in negative moods, i.e. strain and powerlessness in the experimental group were significantly different from those in control group; strain and powerlessness in the experimental group were significantly decreased by foot massage and supportive communication provided by hospice volunteers. Conclusion: The current findings provide an empirical evidence of a brief and efficacious intervention for cancer patients. Further studies are warranted with larger sample size to validate the effectiveness of foot massage and supportive communication as a solid intervention for cancer patients who are undergoing intravenous chemotherapy at OPD.

  • Side scan sonar System occupies an important position as one of marine survey equipments. The purpose of this research is to express sonar' scan images in underwater and compare with the measured size, shape and the quality of the material. Also we confirm the effectiveness of obtained images using the Side scan sonar.

Assessment of decontamination of gutta-percha cone and the change of surface texture after rapid chemical disinfection (화학소독제 처리 후 가타파차 콘의 멸균 효과 및 표면 성상의 변화 평가)

  • The purposes of this study were firstly to identify the microbial species on gutta-percha (GP) cones exposed at outpatient clinics using polymerase chain reaction, and secondly to evaluate the rapid sterilization effect of two chemical disinfectants at chair side. It also evaluated the alteration of surface texture of GP cones after 5-min soaking into two chemical disinfectants. A total of 100 GP cones from two endodontic departments were randomly selected for microbial detection using PCR assay with universal primer. After inoculation on the sterilized GP cones with the same microorganism identified by PCR assay, they were soaked in two chemical disinfectants: 5% NaOCl and 2% chlorhexidine for 1, 3, 5, and 10 minutes. The sterilization effect was evaluated by turbidity and subculture. The change of surface textures using a scanning electron microscope was also examined after 5 min-soaking in two chemical disinfectants. Results showed that four bacterial species were detected in 17 GP cones, and all the species belonged to the genus Staphylococcus. Two chemical disinfectants were effective in sterilization with just 1 minute soaking. On the SEM picture of NaOCl-soaked GP cone, a cluster of cuboidal crystals was seen on the cone surface. Present data demonstrate that two chemical disinfectants are useful for rapid sterilization of GP cone just before obturation at chair side, while CHX-soaked GP cone has cleaner surface without crystal precipitation than that of NaOCl-treated cone.

A study on the strategies to lower technologist occupational exposure according to the performance form in PET scan procedure (PET 검사실 종사자의 업무 행위 별 방사선피폭 조사에 따른 피폭선량 저감화를 위한 연구)

  • Purpose For nuclear medicine technologists, it is difficult to stay away from or to separate from radiation sources comparing with workers who are using radiation generating devices. Nuclear medicine technologists work is recognized as an optimized way when they are familiar with work practices. The aims of this study are to measure radiation exposure of technologists working in PET and to evaluate the occupational radiation dose after implementation of strategies to lower exposure. Materials and Methods We divided into four working types by QC for PET, injection, scan and etc. in PET scan procedure. In QC of PET, we compared the radiation exposure controlling next to $^{68}Ge$ cylinder phantom directly to controlling the table in console room remotely. In injection, we compared the radiation exposure guiding patient in waiting room before injection to after injection. In scan procedure of PET, we compared the radiation exposure moving the table using the control button located next to the patient to moving the table using the control button located in the far distance. PERSONAL ELECTRONIC DOSEMETER (PED), Tracerco$^{TM}$ was used for measuring exposed radiation doses. Results The average doses of exposed radiation were $0.27{\pm}0.04{\mu}Sv$ when controlling the table directly and $0.13{\pm}0.14{\mu}Sv$ when controlling the table remotely while performing QC. The average doses of exposed radiation were $0.97{\pm}0.36{\mu}Sv$ when guiding patient after injection and $0.62{\pm}0.17{\mu}Sv$ when guiding patient before injection. The average doses of exposed radiation were $1.33{\pm}0.54{\mu}Sv$ when using the control button located next to the patient and $0.94{\pm}0.50{\mu}Sv$ when using the control button located in far distance while acquiring image. As a result, there were statistically significant differences(P<0.05). Conclusion: From this study, we found that how much radiation doses technologists are exposed on average at each step of PET procedure while working in PET center and how we can reduce the occupational radiation dose after implementation of strategies to lower exposure. And if we make effort to seek any other methods to reduce technologist occupational radiation, we can minimize and optimize exposed radiation doses in department of nuclear medicine. Conclusion From this study, we found that how much radiation doses technologists are exposed on average at each step of PET procedure while working in PET center and how we can reduce the occupational radiation dose after implementation of strategies to lower exposure. Histopathological studies on the macrophage behavior in lymphomyeloid tissues of tilapia, Oreochromis niloticus (나일틸라피아의 임파조혈조직내 대식세포(大食細胞) 분포변화(分布變化)에 관한 병리조직학적(病理組織學的) 연구(硏究))

  • To elucidate the distributional pattern of macrophages within lymphomyeloid tissues according to the disease process, tilapias, teleostean fish, were intraperitoneally injected with live Edwardsiellatarda and its extracellular product(ECP) respectively. And then histopathological examination for the spleen and gead kidney were carried out for the individuals which had not any clinical signs. In the group injected with live E. tarda, macrophages were densely organized into MMC-like structures with showing some degree of recovery in histological arrangement. At the 2nd week, overall structures of the lymphomyeloid tissues became normal, accompanying the disappearance of most of macrophage groups. Also in case of ECP injection, quite similar findings were observed. Moreover, macrophage collections and hypertrophied ellipsoids were recognized at 1hour after the injection of ECP in head kidney and spleen, respectively. These results suggested that characteristic behaviors of macrophages in lymphomyeloid tissues would be used as important morphological criteria for early diagnosis of edwarsiellosis or possibly of other infectious diseases.

Endophthalmitis after Removal of an Intralenticular Foreign Body in Place without Symptoms for 20 Years (20년 동안 증상없이 잔존한 수정체 내 이물제거 후 발생한 안내염)

  • Purpose: We report a case of postoperative endophthalmitis in the cataract patient, associated with removal of an intralenticular foreign body that had remained in place without symptoms for 20 years. Case summary: A 45-year-old male visited our outpatient clinic complaining of gradual visual loss in his right eye over the past 3 months. In slit-lamp examinations, anterior capsular opacification, nuclear sclerosis, and posterior subcapsular opacity were observed in the right eye. Twenty years before, a tiny metallic projectile had hit his right eye, but slit-lamp examination at the time of injury did not reveal any intraocular foreign body. We decided to undergo cataract surgery. During phacoemulsification, a metallic foreign body was found in the lens and safely removed; then an intraocular lens was implanted. As hypopyon was evident 3 days later, we injected intravitreal antibiotics and applied fortified antibiotic eye drops to the right eye. The anterior chamber inflammation improved and the best-corrected visual acuity recovered to 1.0. Conclusions: Surgeon should be aware of that endophthalmitis could develop after cataract surgery with removal of an intralenticular foreign body that had been in place for 20 years. But did not trigger inflammation or cause any symptoms as the cataract progressed.
