Background: Patch angioplasty is an alternative surgical procedure to coronary artery bypass grafting (CABG) for left main coronary ostial stenosis. The purpose of this study is to evaluate the outcome of patch angioplasty by analyzing the short-term and long-term results. Material and Method: Twenty nine patients who had undergone patch angioplasty due to left main coronary ostial stenosis between July 1991 and May 2003 were enrolled in the study. The mean age of the patients was 53.1 $\pm$12.5 years. There were 8 males and 21 females, and there were 12 female patients who had no risk factor for atherosclerosis. Twenty six (89.7%) patients showed isolated coronary ostial stenosis without any distal coronary lesion. Result: Anterior approach was used in 28 patients and superior approach was used in one patient. Transsection of the main pulmonary artery was used in one patient. Concomitant CABG was performed in 4 patients because of left anterior descending artery lesions in 3 patients and unstable postoperative hemodynamic status in one patient. Hospital mortality had occurred in one patient (3.4%) and late mortality also in one patient, therefore the overall 5 year survival rate was 91.2$\pm$6.1%. Seventeen coronary angiographies were done in 13 patients (44.8%) postoperatively. Two distal patch stenoses, 1 proximal patch stenosis, and 1 new right coronary ostial lesion were identified and 3 percutaneous interventions and 1 CABG were performed during the follow-up period. The overall 5 year freedom from reintervention rate was 82.4 $\pm$ 8.5%. Aortic regurgitation less than grade 1 had developed postoperatively in 4 patients and one patient showed progression of preexisting aortic regurgitation from grade II to III. Conclusion: Patch angioplasty in left main coronary ostial lesion showed acceptable short-term and long-term results in this study. However, restenosis at the patch anastomosis site and aortic regurgitation should be carefully investigated during the follow-up period.

As the development of information communication technology and thus the growth of security incidents, there has been increasing demand on developing methodologies and tools for measuring the information security level of organizations for the efficient security management. However, most works from foreign countries are not realistic in constructing the checklists, moreover their tools provide neither the ease of use nor the inexpensiveness, and most domestic works are not properly considering the characteristics of the organizations when measuring the information security level. In this study, an efficient information security levelling tool is suggested, which applies the multiple variable weights for security levelling according to the characteristics of organizations and the fuzzy technique to reduce the user's subjectivity and the genetic algorithm to establish the security countermeasure.
The aim of this study was to examine whether there is an association between vitamin intakes and established periodontitis in Korean adult population. The 6,245 subjects aged over 19 years old, who participated in health survey, oral examination and nutrition survey were selected for this study from the database of the Fourth Korean National Health and Nutrition Examination Survey. Established periodontitis was defined as ${\geq}$ code 3 in community periodontal index. Vitamin intake was assessed with the food-frequency questionnaire. In analysis, participants were classified by quintile of vitamin intakes. We also considered covariates as socio-demographic characteristics, health-related behaviors including physical activities, systemic diseases and oral health-related behaviors. Multiple logistic regression was performed to assess the crude and adjusted associations. All analyses considered a complex sampling design using SAS 9.2. In crude analysis, less intake of vitamin A, retinol and vitamin B2 significantly increased the risk of periodontitis (vitamin A, odds ratio [OR] Q1=1.00, Q2=0.73, Q3=0.80, Q4=0.77, Q5=0.78; retinol, OR: Q1=1.00, Q2=0.86, Q3=0.73, Q4=0.62, Q5=0.55; vitamin B2, OR: Q1=1.00, Q2=0.70, Q3=0.63, Q4=0.67, Q5=0.68). However, after adjusting for socio-demographics, general and oral health status and behaviors, only vitamin B2 was significantly associated with established periodontitis (OR: Q1=1.00, Q2=0.72, Q3=0.73, Q4=0.76, Q5=0.84). An adequate vitamin B2 intake was significantly associated with a decreased risk of periodontitis. This finding shows that nutrient intake is slightly correlated with periodontitis in Korean adult population. Further studies are needed to understand this association between nutrients intake and periodontitis in more details.

Objective: The objectives were to estimate the rate of the injury in the elderly over the past year and to identify factors related to injury in the elderly in South Korea. Method: Using data from the 2008 Community Health Survey, 43,049 elderly persons, aged 65 years and older, were selected as study subjects. Their experience of injury during the past year and other variables, including socio-demographic factors, health-related factors, and diagnosed chronic diseases, were used. The chi-squared test and multiple logistic regression analysis with weighted analysis were conducted and statistical significance was set at p<0.05. Result: The rate of injury in the elderly during the past year was 5.1%. The most common types of the injury were falling/slipping down and traffic accidents. Factors related to injury in the elderly were living alone, current drinking, depression, poor self-rated health, stroke, and osteoporosis. Living alone (odds ratio 1.23, 95% CI: 1.05-1.45), current drinkers (OR 1.19, 95% CI: 1.05-1.35), poor self-rated health (OR 1.72, 95% CI: 1.43-2.08), depression (OR 1.23, 95% CI: 1.17-1.68), and history of stroke (OR 1.40, 95% CI: 1.17-1.68), and history of osteoporosis (OR 1.45, 95% CI: 1.26-1.66) were related to an increased risk of injury. Conclusions: Intervention programs that consider the risk factors related to injury should be developed and implemented to decrease and prevent injuries in the elderly.

As the type of Hazardous and Noxious Substances(HNS) becomes various and the transport volume of HNS increases, HNS spill incidents occur frequently on land and the sea. In view of various damages to human lives and properties by HNS spills, it is necessary to educate and train professional personnel in preparation for and response to potential HNS spills. This study shows the current state of response systems and education courses against HNS spill incidents on land and the sea to compare those with each other between land and sea in Korea. Incident command system on land are basically similar to that at sea, but leading authority which is responsible for combating HNS spills at sea is changeable depending on the location of HNS spill, as it were, Korea Coast Guard(KCG) is responsible for urgent response to HNS spill at sea, while municipalities are responsible for the response to HNS drifted ashore. Education courses for HNS responders on land are established at National Fire Service Academy(NFSA), National Institute of Chemical Safety(NICS), etc., and are diverse. Education and training courses for HNS responder at sea are established at Korea Coast Guard Academy(KCGA) and Marine Environment Research & Training Institute(MERTI), and are comparatively simple. Education courses for dangerous cargo handlers who work in port where land is linked to the sea are established at Korea Maritime Dangerous Goods Inspection & Research Institute(KOMDI), Korea Port Training Institute(KPTI) and Korea Institute of Maritime and Fisheries Technology(KIMFT). Through the comparison of education courses for HNS responders between land and sea, some recommendations such as extension of education targets, division of an existing integrated HNS course into two courses composed of operational level and manager level with respective refresh course, on-line cyber course and joint inter-educational institute course in cooperation with other relevant institutes are proposed for the improvement in education courses of KCG and KOEM(Korea Marine Environment Management Corporation) to educate and train professionals for combating HNS spills at sea in Korea.

Purpose: Purpose was to explore associations between sleep duration and metabolic syndrome (MS) risks, and to determine factors associated with self-rated health (SRH) of adults with MS compared to other adults. Methods: This is a secondary data analysis based on the Fifth Korea National Health and Nutrition Examination Survey KNHANES V (N=12662). Study instruments included sleep duration, MS risk factors, SRH and health-related quality of life (HRQoL). Results: Mean age of participants was $43.68{\pm}12.26years$. Fifty-eight percent were women, and 18.3% were identified as having MS. Systolic blood pressure (SBP), diastolic blood pressure (DBP), and SRH were significantly different according to sleep duration (p <.05) among all participants. In the non MS group, male gender, younger age (19~30 and 41~50 age brackets) upper income level, sufficient sleep duration, and high density lipoprotein (HDL) were positively associated with SRH, whereas, lower education levels (${\leq}$ middle school), glucose level, and waist circumference were negatively associated with SRH (p <.05). In the MS group, lower income, lower education levels (${\leq}$ middle school), glucose level, and waist circumference were negatively associated with SRH, whereas, having an occupation was positively associated with SRH (p <.05). Conclusion: Results suggest that tailored approaches are required for prevention and control of MS and sleep duration of each individual should be considered rather than applying standardized guidelines. However, as sleep quality was not included in the analysis, further investigations regarding influence of sleep quality on MS and SRH and controlling for other lifestyle and health behavior factors are required.
Objectives The purpose of this study is to evaluate the relationship between the severity of white matter changes (WMC), risk factors and cognitive domains, including executive function profiles. Method Forty nine subjects over 55 years with subjective memory complaints were assessed with MRI and neuropsychological tests. The WMC were assessed by MRI T2-FLAIR images and divided into 3 groups of mild vs. moderate vs. severe and 2 groups of mild-moderate vs. severe by using Mantyla's criteria and Fazeka's criteria. The risk factors were examined in hypertension, heart disease history and chemistry Lab. Medical conditions which affect to cognitive dysfunction and definite dementia were also excluded. Results Comparing 3 groups, hypertension was identified as a risk factor of the WMC. Comparing 2 groups, total cholesterol and LDL were identified for as the risk factor of WMC. The severity of WMC was significantly associated with cognitive disturbances and their main effect on cognition was working memory and inhibition. Conclusion The risk factors of the WMC in the elderly were hypertension, hyperlipidemia. The severity of WMC appears to be associated with executive dysfunction in the elderly.

The depression of the elderly decreases satisfaction in life and is also an important risky factor of their suicide. The subjects of the study were 615 males and 904 females aged above 65 who had the 2008 Korea National Health and Nutrition Examination Survey on the relations between presence of spouses and depression given by the Korea Center for Disease Control and Prevention. Through a chi-square test and a multiple logistic regression, the relations between general characteristics, contraction, health characteristics and depression were analysed. As a result of the multiple logistic regression for which age, family income, subjective health status, presence of activity limitation and stress were revised, the odds ratio of the males without spouses was 2.37(95% CI 1.03-5.45), which indicates that they had more depression than female subjects. However, for the female subjects, there were no relations between presence of spouses and depression.

The purpose of this study is to examine the effect of subjective overwork on health and the moderating effect of risk perception in the relationship between subjective overwork and health. The data used in this study are the fourth Korean Working Condition Survey (KWCS) conducted by Korea Occupational Safety and Health Agency (KOSHA) in 2014. The items related to actual working hours, subjective overwork, health, and risk perception were used. A total of 18,508 workers who are 1) waged workers, 2) work more than three days a week, 3) work more than 21 hours a week, 4) work longer than preferred work hours or work as longer as they desired were selected. Hierarchical regression analysis was performed to identify the effect of actual work hours, subjective overwork, prevalence of risk, lethalness of accident damage on health and to determine the moderating effects of prevalence of risk and lethalness of accident damage. As a result, actual work hours, subjective overwork, prevalence of risk, lethalness of accident damage had an effect on worker's health problems. Second, lethalness of accident damage had a moderating effect in the relationship between subjective overwork and health. These results suggest that organizations should manage worker's environmental factors and other threats that could increase the lethalness of accident damage.
Recent research has identified that self-reported voice problems are a risk indicator for voice disorders. However, previous studies concerning the general population did not take into account the influence of gender on self-reported voice problems. The purpose of the present cross-sectional study was to determine the gender differences in risk factors of self-reported voice problems in the Korean adult population using national survey data. This study utilized data from the Korea National Health and Nutritional Examination Survey 2008. Subjects inclued 3,622 people (1,508 male and 2,114 female) aged 19 years and older living in the community. Data were analyzed using t-test, one-way ANOVA, and multiple logistic regression. The prevalence of self-reported voice problems was 5.9% in males, and 8.1% in females Females had higher incidents of self-reported voice problems than males. Adjusting for covariates, in males, age (OR=2.47, 95% CI: 1.07-5.70), pain and discomfort during the last two weeks (OR=3.64, 95% CI: 2.20-6.01) were independently associated with self-reported voice problems (p<0.05). In women, age (OR=1.96, 95% CI: 1.18-3.26), education (OR=2.09, 95% CI: 1.06-4.12), smoking (OR=2.70, 95% CI: 1.48-4.93), thyroid disorders (OR=2.58, 95% CI: 1.47-4.53), pain and discomfort during the last two weeks (OR=1.75, 95% CI: 1.21-2.54) were independently associated with self-reported voice problem (p<0.05). Self-reported voice problems related risk factors differed according to gender. These findings suggest that there needs to be different program strategies that reflect gender differences in self-reported voice problems.
