Comparison of Noncycloplegic Autorefraction, Manifest Refraction and Cycloplegic Autorefraction in School-aged Children (학령기 근시아동에서 비조절마비 자동굴절검사, 검영법, 조절마비 자동굴절검사에 의한 굴절검사값의 비교)

  • Purpose: The study was conducted to compare the values of auto-refraction, manifest refraction and cycloplegic refraction in school-aged children. Methods: One hundred five myopic school children ranged from 6 to 14 years old (210 eyes, $10.28{\pm}1.59$ years old) were recruited and noncycloplegic auto-refraction (AR) and manifest refraction (MR) were conducted and then underwent cycloplegia and refractive status (CR) again with the auto-refractometer. Results: Refractive powers measured by AR, MR, and CR were highly correlated. However, spherical and cylindrical powers of the subjects measured by AR were measured higher negative power than in CR (p<0.001). From 210 eyes, the discrepancy rate in the spherical and cylindrical powers were 40 eyes (19%) and 19 eyes (9%) of the total subjects, respectively and the differences between noncycloplegic and cycloplegic refractions were higher with the spherical and cylindrical powers increasing. Conclusions: The use of the autorefractometer in children with negative spherical power without cycloplegia may overestimate the actual myopia that subjective refraction is the most important in prescription for the eyeglasses and regression equations would be used to prognose the cycloplegic refraction from the auto-refraction as the basic data for the subjective refraction.

Study on the Analysis of Cycloplegic Autorefraction Value in Infancy (소아기의 조절마비 굴절검사값 분석에 관한 연구)

  • For this study, we analysed the difference of values between noncycloplegic and cycloplegic autorefraction in primary school children. We classified the elementary school one, two, three grade (8~10age) 79children(158eyes) who did not have strabismus or amblyopia. They were examined by using the Autorefractor in domestic market before and after cycloplegic. Discrepancies beyond 0.50D in spherical and cylindrical power and 20 degree in cylindrical axis were regarded as significant. The percentages of discrepancy were greater in spherical power between noncydoplegic and cycloplegic autorefraction(p<0.05). The percentages of discrepancy were not greater in cylindrical power between noncycloplegic and cycloplegic autorefraction. The percentages of discrepancy were greater in cylindrical axis between noncydoplegic and cycloplegic autorefraction(p<0.05). In conclusion, for the primary school children who did not wear glasses, the prediction table of this study will be helpful to reduce the test error in visual acuity and refractive error tests.

A Study on Usefulness of Static Retinoscopy in Eyes Opened for Hyperopic School-aged Children (양안개방상태에서 학령기 원시안 아동의 정적 검영법의 유용성 고찰)

  • Purpose: We compared static retinoscopy in eyes opened with cycloplegic refraction depending on the hyperopia for school-aged children. Methods: There were 59 eyes (30 patients) who were divided into 3 groups - the mild hyperopia (+0.25 D ~ +1.00 D), moderate hyperopia (+1.25 D ~ +2.00 D) and high hyperopia (+2.25 D or more). They all had 0.8 visual acuity or more. Autorefraction and retinoscopy were performed prior to cycloplegic refraction, and then copmared with manifest refraction and cycloplegic refraction. Results: Hyperopia measured with static retinoscopy tends to be measured higher than manifest refraction for school-aged children. Changes of spherical power was statistically significant (p < 0.001). Changes of astigmatism was not statistically significant (p > 0.05). Conclusions: The difference between cycloplegic refraction and static retinoscopy was not significant for hyperopic school-aged children. The use of retinoscopy was limited for opticians because of legal constraints. The usage of static retinoscopy in eyes opened for optician should be generalized under the conditions not using the cycloplegic.

Difference between Manifest Refraction (MR) and Cycloplegic Refraction (CR) with Age and Myopic Value (연령과 근시도에 따른 현성굴절검사와 조절마비굴절검사의 비교)

  • Purpose: The purpose of this study is to investigate difference between manifest refraction (MR) and cycloplegic refraction (CR) with age and myopic value. Methods: Manifest and cycloplegic refractions were carried out on the patients of 229 myopic patients ($25.3{\pm}11.9$ years old). Results: The average results from a pre- and a postcycloplegic refraction showed a reduction of 0.19 D in myopia, 0.02 D in astigmatism and $1.85^{\circ}$ in astigmatism direction. Change of myopic value was higher in 30~40 age. Total 73 eyes of 229 patients who showed 0.50 diopter (D) in spherical, and a high degree of myopia group showed a higher discrepancy rate. Conclusions: The results found in the comparison of the value of the manifest refraction and cycloplegic refractions showed changes of myopic value was not significant with age of patient, but the group of high degree of myopia showed higher discrepancy rate.

Correlation Between Selective Motor Control Test and Functional Performance Evaluation in Children With Spastic Cerebral Palsy (선택적 운동 조절 척도와 기능적 수행도 평가 간의 상관: 경직형 뇌성마비 아동을 대상으로)

  • The purpose of this study was to investigate the relationship among functional evaluation systems, the Selective Motor Control Scale (SMC scale), the Gross Motor Function Classification System (GMFCS), the Gross Motor Function Measure (GMFM) and Activities of daily living in children with spastic cerebral palsy and to provide the foundation data about SMC scle for evaluation system of abilities of selective motor control in children with spastic cerebral palsy. For this, sixty eight children with spastic cerebral palsy were participated in this study. The children were evaluated by using the SMC scale for their selective motor control ability and by using the GMFCS and GMFM for their gross motor function. The activities of daily living were assessed by using the Functional Independence Measure of Children (WeeFIM). There were a significant correlation between the SMC scale and the GMFCS (r = -.485, p < .05). The good correlation between the SMC scale and GMFM was found (r = .482, p < .05). The activities of daily living were not a significant correlation with SMC scale (r = .019, p > .05). The SMC scale in practice will provide usefulness for assessment of abilities of selective motor control in children with spastic cerebral palsy.

The Effect of cycloplegia in Emmetropia with Use of Cycloplegia (조절마비제로 정시안이 된 눈의 조절마비 효과)

  • The purpose of this study is observed effect of cycloplegia in emmetropia with use of cycloplegia. We examined the naked visual acuity, mainfest refraction, 105eyes with emmetropia(under SE ${\pm}0.50D$, Cyl ${\pm}1.00D$) after the use of cycloplegia. We used Nidek ARK-700 autorefractometer. Sexual difference of mainfest refraction error showed in male -0.67D, female -0.92D and difference of the CR and the MR male 0.5D, female 0.81D the total mean 0.69D. The naked visual acuity were in male 0.82, female 0.74, total mean 0.77. Age comparison, difference CR and MR were total mean 0.69D and 13 years group 1.1D were the highest. The naked visual acuity were in the highest 10 years group 0.86. Effect of cycloplegia was $0.69D{\pm}0.79$ in emmetropia and visual acuity was $0.77{\pm}0.21$. The optician, it will not be able to use cycloplegia, must pay attention spectacle prescription unnecessary and over correction in condition below -0.75D MR, above 0.77 visual acuity.

Effects of Exercise Interventions on Postural Control for Children With Cerebral Palsy in Korea: A Systematic Review (국내 뇌성마비 아동의 자세조절 향상을 위한 운동 중재 효과: 체계적 고찰)

  • Objective : The purpose of this study is to prove the effects of exercise intervention on postural control of children with cerebral palsy in Korea by systematic review. Methods : Total 18 studies were selected through Korean research database engine such as DBpia, KISS and RISS for the systematic review in this study. Key words used were 'cerebral palsy', 'postural control' and 'balance' and publication time period was limited only after October 2006 in the research selection. All the selected studies were reviewed in terms of quality level of evidence and methodological quality level, and then the results were analyzed with Patient, Intervention, Comparison, Outcome (PICO). Results : For the level of evidence, it is found that 8 studies show Grade I, 5 studies show Grade III, and 2 studies show Grade V. In terms of Methodological quality, majority of the studies showed 'Fare' level. Majority of target population of the studies was children with spastic diplegia. Most research design of the studies were randomized controlled trials. The results in several suited showed that the postural control exercise used in Korea has positive impact on improvement of motor and balance ability. Conclusion : Impact of the exercise intervention used in Korea for postural control of children with cerebral palsy has been evidenced in this study. It is proposed that further research need to be done considering several types of cerebral palsy.

The Effect of Intensive Weight Shift Training on Seated Postural Balance in Children With Spastic Cerebral Palsy: A Case Study (집중체중 이동훈련이 뇌성마비 아동의 체간조절 능력에 미치는 영향: 사례연구)

  • Objective : This study investigated the effect of intensive weight shift training (IWST) on the ability to balance in a sitting posture of children with spastic cerebral palsy (SCP). Methods : One child with spastic diplegia participated in this study for a total of 8 weeks using the ABA experimental design. For the pre-intervention period (A1), general physical therapy (GPT) for children with SCP was performed. The intervention period (B1 and B2), GPT and IWST were conducted for children with SCP, and GPT was conducted again for the post-intervention period (A'1 and A'2). Trunk control capacity was measured using the Korean Trunk Control Measurement Scale (TCMS-K) and Biorescue (RM Ingenierie, France). Results : All TCMS-K variables increased from A1 to B2, and decreased from B2 to A'2. The total area with limit of stability (LOS) increased from A1 to B2, and decreased from B2 to A'2. The ratio of the left/right (Lt/Rt) and anterior/posterior (Ant/Post) LOS area was closer to 1, meaning symmetry, in B2 than in A1. The ratio of the Lt/Rt LOS area decreased further from 1 in A'2 than in B2. The ratio of the Ant/Post LOS area was closer to 1 in A'2 than in B2. Conclusion : IWST had a positive effect on the improvement of balance in the sitting posture of a child with SCP. The results suggest that IWST might help to improve the balance abilities of children with SCP for independent sitting, postural control, and activities of daily living.

Effect on Myopia Progression Wearing Eye-glasses for School Children (학령기 아동의 안경착용이 근시진행에 미치는 영향)

  • Purpose: We analyzed the influence of myopic's eye-glasses wearing on myopia progress after cycloplegic refraction. Methods: The 33 people (66 eyes) were school children from 8 years to 12 years having no experience eye-glasses wearing, they were taken cycloplegic refraction at the 100th, the 200th and 300th days in order to evaluate myopia progression. We investigated the eye-glasses wearing group (experimental group, 32 eyes) and the non-eye-glasses wearing group (control group, 34 eyes). The eye-glasses power of the experimental group were -0.50 D, -0.75 D, -1.00 D, -1.25 D and -1.50 D. We compared experimental group with control group for myopia progress according to period, age, and refraction error and investigated the myopia progress according to the eye-glasses power of experimental group. Results: At the 300th day from the first cycloplegic refaction, spherical equivalent for the experimental group increased as -1.03${\pm}$0.43 D (t=13.36, p<0.001) and for the control group increase as -0.61${\pm}$0.35 D (t=10.05, p<0.001) and two groups were statistical difference. Myopia power for experimental group increased 60.75%, for control group increased 56.66% at the 300 days. According to eye-glasses power increased 41.19${\pm}$15.25% at -1.50 D, 36.74${\pm}$19.29% at -1.25 D, 56.57${\pm}$20.21% at -1.00 D, 87.26${\pm}$49.38% at -0.75 D and 106.69${\pm}$59.60% at -0.50 D. Conclusions: The myopia power for the eye-glasses wearing group was 0.46 D faster than the non-eye-glasses wearing group at the 300th day from the first cycloplegic refraction. We will consider the effect of non-eye-glasses wearing to protect the progressing myopia and prescribe the under correction for school children having no experience eye-glasses wearing.

The Effects of Sensory Integrative Intervention on the Self-regulation Capacities of Children With Cerebral Palsy: Single-Subject Research Design (감각통합치료가 뇌성마비 아동의 자기조절 행동에 미치는 효과)

  • Objective : The purpose of the study was to investigate effects of sensory integration therapy on self-regulation capacities of children with cerebral palsy. Methods : Subjects of this study were four years old girl and six years old boy, living in P city. Both were diagnosed as diplegia. Research design used in this study is ABA' design for a single-subject research. The experimental period was 12 weeks with 22 sessions of treatment. It was divided into three phase: 6 sessions for baseline phase (A), 14 sessions for treatment phase (B), 2 sessions for baseline phase (A'). There was no treatment on the baseline phases, and sensory integration therapy focused vestibular and proprioceptive stimuli was conducted on the treatment phase. At each session, subjects' behaviors has been recorded with a Digital Camera for 10 minutes and then the occurrence frequency of problematic behavior was identified based on the operational definition of it. Results : In comparison with the baseline phase (A), the occurrence frequency was reduced during the treatment phase for three types of problematic behavior associated with the self-regulatory capacities. The tendency of reducement was maintained during baseline phase (A'). Conclusion : Based on the result, it was implied that sensory integration therapy has a positive effect on the self-regulation capacities in children with cerebral palsy.
