EFFECT OF SMEAR LAYER ON THE APICAL LEAKAGE OF WARM VERTICAL CANAL FILLING (도말층이 가온 수직가업 근관 충전후 치근단 누출에 미치는 영향)

  • The purposes of this study were to observe the difference in the root canal wall after hand instrumentation or engine-driven Ni-Ti instrumentation under the scanning electron microscope, and to evaluate the apical leakage provided by continuous wave of canal filling technique with or without root canal sealer and smear layer. Twenty recently extracted human maxillary anterior teeth were instrumented with K-type files or engine-driven Ni-Ti files, Quantec series 2000$^{TM}$ and irrigated with 5.25% NaOCl alone or 15% EDT A and final flush of 5.25% NaOCl. After the instrumentation and flushing, teeth were split in half with a knife and a mallet. They were then examined with a scanning electron microscope Forty-four recently extracted human maxillary anterior teeth were divided into four groups with and without smear layer and then warm vertical canal filling using System-B with or without sealer. The extent of leakage was scored after immersion in India ink for 1 week. The results were as follows: 1. No significant difference of smear layer was observed between K-type file-instrumented group and engine-driven Ni-Ti file-instrumented group. 2. A group without smear layer showed significantly less apical leakage than a group with smear layer when sealer was used for the canal filling (p<0.01). 3. There was no significant difference between a group without sealer and smear layer and a group without sealer and with smear layer (p<0.01). 4. In groups without smear layer, a group with sealer showed significantly less apical leakage than a group without sealer (p<0.01). 5. When root canals were irrigated with NaOCl alone, a group with sealer showed significantly less apical leakage than a group without sealer (p<0.01).

Nocturnal Bruxism and Botulinum Toxin Effect on the Subjects with Masseteric Hypertrophy (야간 이갈이와 교근비대 환자의 보툴리눔 독소 주사 효과)

  • This study aimed to evaluate a relation of bruxism with clinical effects of botulinum toxin type A(BTX-A) injection. 5 bruxers and 5 nonbruxers with bilateral masseter hypertrophy were participated in this study. After injecting 25 unit of BTX-A(Allergen Inc, $Botox^{(R)}$) into each masseter muscle, the thickness of masseter(Mm) and anterior temporalis(Ta) muscles was measured by ultrasonography and the maximum bite force was evaluated during a 9-month period. Self-estimation on the recovery of occlusal force during mastication was done as well. Regardless of presence of bruxsim, all subjects showed significantly reduced Ms thickness(p<0.001) and maximum bite force at $1^{st}$ molars(p=0.027) with their peak at 3 months after injection, which then started to return. No significant difference was observed in Ta thickness and the bite force at the central incisors. While self-estimated occlusal force was the least at 2 weeks after injection and then rapidly returned to the baseline level with full recovery at the time of 6 to 9 months after injection, the maximum bite force measured by bite force recorder did not recover the original value, particularly in the nonbruxer group. It is assumed that nocturnal bruxism can influence recovery of atrophic masseter and decreased occlusal force due to BTX-A injection. These findings suggest a need of occlusal appliance to control bruxism or clenching habit for longer clinical effect of BTX-A injection.

A comparative study on the accuracy of impression body according to the types of impression tray (임플란트 인상 채득 시 트레이 종류에 따른 인상체의 정확도에 관한 비교 연구)

  • Purpose: The objective of this study was to evaluate and compare the accuracy of impression body taking by the closed and the open tray impression technique with 3 types of impression tray. Individual tray, metal stock tray and polycarbonate tray were used. Materials and methods: Nine closed tray impressions were taken by individual tray, metal stock tray and polycarbonate stock tray, respectively with polyether impression material. 9 open tray impressions were also acquired by same manner. Precision analysis on the master models was performed by attaching the reference frameworks with alternate single screws and measuring the vertical fit discrepancy of respective analogues in working cast with a stereo microscope. Data were analyzed by 1 way ANOVA and independent t-test. Results: The average fit accuracy of impression bodies was calculated. With the closed tray impression technique, there were significant statistical differences in vertical fit discrepancy according to the types of tray. The individual tray group showed the lowest value and the polycarbonate stock tray group represented the highest. With the open tray impression technique, there was no significant difference in vertical fit discrepancy. Significant statistical difference in vertical fit discrepancy was found between the open and the closed impression technique with the polycarbonate stock tray. Conclusion: From the results above, more precise impressions could be acquired by the rigid individual tray compared with the polycarbonate stock tray. It was hard to get consistent accuracy impressions by the closed tray impression technique with polycarbonate stock trays.

Esthetic removable partial denture with implants and resin clasp: Case report (임플란트와 레진 클래스프를 이용한 심미 가철성 국소의치 증례)

  • For the rehabilitation of fully edentulous patients, implant-supported removable partial dentures can be considered as one of the treatment options with complete dentures or implant-supported overdentures. If removable partial dentures are used in combination with a small number of implants placed in strategically important positions, it can offer additional stability, retention and support through implants and reduce a burden of surgical procedures compared with fixed implant-supported prostheses with extensive implant placement. Moreover, the economical benefit can be expected as well. The purpose of this case report is to present a treatment in which an implant-supported removable partial denture was fabricated considering residual alveolar bone status and demands after teeth loss in a patient who had been using a distal extension removable partial denture for a long period of time. In anterior area, fixed prostheses were fabricated with implant placement and in posterior area, short implants provide only support for the removable partial denture. In addition, denture base and clasp were made of thermoplastic acrylic resin. Finally, functionally and aesthetically satisfying treatment results can be achieved.

THE STUDY ON THE ORTHODONTIC PATIENTS WHO VISITED DEPARTMENT OF PEDIATRIC DENTISTRY, CHONNAM NATIONAL UNIVERSITY HOSPITAL (전남대학교병원 소아치과에 내원한 부정교합 환자에 관한 연구)

  • This study was aimed to provide an epidemiologic study so that we could accommadate their orthodontic needs adequately and to obtain the reliable quantitative information regarding the characteristics of orthodontic patients who visited the Department of pediatric dentistry, Chonnam National University Hospital from 1986 to 1999, October. The results were as follows. 1 The total number of orthodontic patients were 1,381(male 658, female 723) and the number of annual patients showed irregular trend and the number of male were slightly higher than that of female. 2. According to the investigation made by age group, the group of under 5 years, 6 to 7, 8 to 9, 10 to 11, 12 to 13 and above 14 years showed 8.4%, 29.6%, 34.3%, 21.2%, 5.7% and 0.8%, respectively. 3. The patients corresponding to primary dentition were 15.7% of total patients and mesial step, distal step and flush terminal plane were 83%, 4.6% and 12.4% respectively. By the way 82% of mesial step had anterior crossbite at the same time. 4. The patients corresponding to Angle classification were 84.3% of total patients and Class I malocclusion, Class II div. 1, Class II div. 2 and Class III was 34.7%, 34.6%, 2.1% and 28.6%, respectively.

Distal-extension removable partial denture with anterior implant supported fixed prostheses in a maxillary edentulous patient: Case report (상악 완전 무치악 환자에서 임플란트 고정성 보철물을 지대치로 한 후방 연장 국소의치 수복 증례)

  • Clinicians are faced with many difficulties when planning prosthodontic restorations with implants in a complete edentulous patient. When planning fixed implant prosthetics, it is often necessary to have additional surgery due to highly reduced alveolar bone, as well as high treatment costs and long-term treatment durations can be required. In addition, lack of interocclusal space can be a problem when planning implant supported overdentures. In this study, we planned to place a small number of implants on the anterior maxilla and used them as the abutments for distal-extension removable partial dentures on the posterior side in a maxillary fully edentulous patient. This would reduce the possibility of additional invasive operations such as alveolar bone graft, shorten the treatment time, and be relatively easy for elderly patients to burden. In this case, the patient was provided with a distal-extension removable partial denture and anterior implant fixed prostheses, which was similar to the previous one, and showed good adaptation, and chewing efficiency and esthetics was recovered.

SHEAR BOND STRENGTH OF GIOMER AND SELF-ETCHING PRIMER ON THE DENTIN (Giomer와 자가 산부식 접착제의 상아질에 대한 전단 결합강도)

  • Giomer is fluoride-releasing, resin-based dental materials that comprise PRG(pre-reacted glass ionomer) filler. The purpose of this study was to evaluate the shear bond strength of Giomer using self-etching primer systems to bovine dentin. Bovine incisors were mounted in self-curing orthodontic resin and the facial surfaces were wet ground on SIC paper to expose the dentin. Total 100 samples were made and divided randomly into 4 groups, Giomer group(I), Composite resin group(II) and Compomer group(III), Giomer and single bottle adhesive group(IV). The shear bond strengths of 25 samples per each group were measured using universal testing machine. And data were analyzed statistically with One-way ANOVA and Scheffe test. Giomer group(I) showed the significantly higher bond strength than Compomer group(III)(p<0.05). There was no significant difference between Giomer group(I) and Composite resin group(II)(p>0.05). And there is no significant difference between gourp(I) and group(IV). Based on the results of present study, the use of Giomer as an esthetic restorative material for primary teeth might be justified. It is considered that more study about the fluoride releasing ability is needed to evaluate the anticariogenic effect of giomer.

Utilization of Resin Infiltration for Prolonging of Tooth Whitening Effects (치아 미백 효과의 장기화를 위한 Resin Infiltration의 활용)

  • The present study aimed to evaluate the validity of resin infiltration in improving color stability after tooth whitening. Enamel samples were extracted from 40 healthy bovine upper incisors, and primary staining and whitening were performed. After that, specimens were randomly divided into 3 groups : resin infiltration group (n = 15, RI group), resin adhesive group (n = 15, RA group), and control group (n = 10). Secondary staining was performed on all samples. Coloration was assessed 5 times as follows: initial color, immediately after staining, after whitening, after resin application, and after secondary staining. Color was measured using a spectrophotometer and recorded by using the CIE $L^*a^*b^*$ color space. The color changes after primary staining for the RI, RA, and control groups were $12.16{\pm}3.50$, $12.16{\pm}3.38$, and $15.81{\pm}6.39$, whereas those after secondary staining were $15.21{\pm}7.19$, $15.93{\pm}4.31$, and $26.62{\pm}17.89$. Color changes after secondary staining showed a significant difference between the RI and control groups. In the within-group comparison between primary and secondary staining, there was no significant difference found in the RI group only (p = 0.26). The results suggest that Color stability after tooth whitening can be improved using resin infiltration.

Design of the PAM with High Linearity and Efficiency for Wibro (고선형성, 고효율의 Wibro용 PAM 설계)

  • This thesis is regarding of fabricating wibro PAM. First of all, we need to set specification based on link budget for wibro communication circumstance in order to develop PAM, then we decided specification concerning of wibro PAM by considering TTAS_Ko_06_0082R1 which is standarded in Korea, and IEEE Std. 802. 16d/e which is international standard. We selected the Doherty structure to increase efficiency, and pre-distorter structure to increase linearity. The fabricated PAM has not only a result of $26.5dB{\pm}1.0dB$ gain characteristics and maximum of -14 dB return loss characteristics in full frequency bands and full output ranges, but also a result of 37 dBc at 4 tone IMD characteristics which is improved result of 843 and a result of 31 dBc spurious characteristics which is improved result of 5 dB at 4.77 MHz offset point in status of having 27 % efficiency in the 26 dBm high power amplifier output signal. We confirmed the suggested structure is better than others by comparing with normal structure, balanced structure and Doherty structure without predistorter.

SEMI-LONGITUDINAL STUDY OF GROWTH AND DEVELOPMENT OF CRANIO-FACIAL SOFT TISSUE OF CHILDREN AGED FROM 3 TO 11 (3-11세 아동의 두개 및 연조직의 성장변화에 관한 준종단적 연구)

  • Today's orthodontic treatment goals lie in functional esthetics, and the importance of the latter is increasing gradually in trend. Considering such, study on growth and development of soft as well as hard tissues becomes inevitable. Early studies emphasize mainly on skeletal analysis using cephalometric data. However, more recent studies report that maxillofacial soft tissue, which plays a critical role in determining facial esthetics, is influenced by underlying hard tissue, and yet close relationship between them was not noticed. Cephalometric x-rays were taken of 137 Korean boys and 106 girls with no systemic disease, fair developmental status and normal occlusion for two consecutive years; afterwards, soft tissue analysis, which was divided into four parts, facial form lip position & posture, nose, and thickness, was done to correlate them with general growth.
