Body mass index(BMI) and lipid metabolism in patients admitted in long-term care hospitals (요양병원 입소 환자의 신체질량지수와 지질대사)

  • This is a non-experimental and retrospective study aimed at determining the effects of long-term hospitalization on the body mass index (BMI) and lipid metabolism in long-term hospitalized patients. The study subjects included 120 patients aged 40-65 years who were hospitalized for >3 months in 2 long-term care hospitals in Gyeonggi-do, South Korea. In this study, the BMI and levels of total cholesterol, triglycerides (TG), high-density lipoprotein (HDL), and low density lipoprotein (LDL) at admission and 3 months after hospitalization were compared and analyzed, and the related changes over time were followed up. The general characteristics of the subjects were analyzed by using descriptive statistics and frequency analysis. In addition, logistic regression analysis was performed to determine the effects of the general characteristics on the BMI and Dyslipidemia. The changes in the BMI and blood lipid levels between admission and 3 months after hospitalization were analyzed using the paired t-test. The results showed that with regard to the changes in the blood lipid levels, the triglyceride levels significantly increased 3 months after hospitalization (p<.05). These findings imply that long-term hospitalization for care and rehabilitation after acute-phase treatment should be considered a potential high-risk factor for dyslipidemia, which could be prevented or alleviated by providing the patients with health education, including exercise and dietary education.

Analysis of the Correlation Between Ultrasonography Diagnosis Fatty Liver Grade and Hyperlipidemia and Obesity Indicators by Age Groups (연령대별 초음파 진단 지방간 등급과 고지혈증 및 비만 지표 간의 상관관계 분석)

  • The purpose of this study was to analyze the correlation between the grade of fatty liver diagnosed by ultrasonography and hyperlipidemia and obesity indicators by age groups. A total of 1,470 patients of various sex and age groups were examined by abdominal ultrasonography at the H Center in Chungbuk. The patients were classified as normal and fatty liver by age groups (below twenty, the thirties, forty, fifties, and over sixties). The grade of fatty liver was classified in detail as Grade 1 (mild fatty liver), Grade 2 (moderate fatty liver), Grade 3 (severe fatty liver). We selected indicators of hyperlipidemia as total cholesterol, triglyceride, low-density lipoprotein cholesterol, and high-density lipoprotein cholesterol. The obesity indicators were height, weight, body mass index, and waist circumference. The demographical descriptive statistic analysis and frequency analysis by age groups were performed. The difference of average and correlation between hyperlipidemia and obesity indicators were analyzed. As a result, patients over 60 have fatty liver regardless of sex. there was a difference between triglyceride, high-density lipoprotein cholesterol, weight, body mass index, and waistline for all age groups. The degree of fatty liver was highly correlated with waist size and body mass index for all age groups.

The Effects of 12-Week Training for the Physical Fitness and Cardiovascular Factors to Examine Physical Fitness on Firefighters Test-Taker (소방공무원 수험생의 체력검정을 위한 12주간 훈련이 체력요인, 심혈관계요인에 미치는 영향)

  • The purpose of this study was to investigate the effects of 12-week training on changes in physical fitness and cardiovascular factors for firefighters. For this purpose, 40 men in their 20s and 30s who agreed to participate voluntarily were recruited. They were divided into four groups: the firefighters' physical fitness test training group (hereinafter referred to as PT group), firefighters' physical fitness test and aerobic training group (hereinafter referred to as PT+AR group), firefighters' physical fitness test and both aerobic and anaerobic training group (hereinafter referred to as PT+CO group). Physical fitness factors (grip strength, back muscle strength, seated forward bend, standing long jump, sit-ups, 20-meter shuttle run), cardiovascular factors (total cholesterol, triglycerides, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol, glucose, waist circumference, systolic blood pressure, diastolic blood pressure) and the relationship between Framingham Heart Risk Score and physical/cardiovascular factors were compared and analyzed, and the following conclusions were obtained. Aerobic training, anaerobic training, and combined training, including 12 weeks of firefighter physical examinations, all had positive effects on fitness and cardiovascular factors, which would be an appropriate way for firefighter examinees to improve physical strength and reduce the risk of cardiovascular disease.

Positive effect of Saengmaeg-san intake on blood lipid and arteriosclerosis index during high-intensity training (고강도 트레이닝 시 생맥산 섭취가 혈중 지질 및 동맥경화 지수에 미치는 긍정적인 영향)

  • The purpose of this study was to investigate the effect of Saengmaeg-san intake on blood lipids and arteriosclerosis index in male tennis players. Seventeen male tennis players enrolled in university were assigned to a Saengmaeg-san intake group (n=9) and a placebo control group (n=8) according to whether they took Saengmaeg-san during 4-week high-intensity tennis training. Tennis training was conducted 5 times a week for 4 weeks, and the exercise intensity was performed at 70-90% of the spare heart rate. The intake of Saengmaeg-san was 110ml at a time before breakfast, before, during, during, and after exercise, and after dinner, a total of 7 times, 770ml per day. For all data, mean and standard deviation were used. Two-way repeated measures ANOVA was used to examine the effect of time and treatment, and Pearson's Correlation was used to examine the relevance of differences in blood lipid responses after treatment. As a result of this study, the Saengmaeg-san intake group significantly improved blood lipids (triglyceride, total cholesterol, high-density lipoprotein cholesterol, low-density lipoprotein cholesterol) and arteriosclerosis index, and there was a significant correlation appeared between ∆TG, ∆LDL-C, and ∆TC. As a result, the intake of Saengmaeg-san during high-intensity training of male college student tennis players can have a positive effect on blood lipids and arteriosclerosis index, suggesting that it can be an effective sports drink as an exercise supplement.

Association between Hypertriglyceridemic-Waist Phenotype and Metabolic Abnormalities in Hypertensive Adults (고혈압자의 고중성지방혈증-허리 표현형과 대사이상 사이의 연관성)

  • The hypertriglyceridemic-waist (HTGW) phenotype can predict the risk of coronary artery disease. This study evaluated the relationship between the HTGW phenotype and metabolic abnormalities in hypertensive adults. The test was conducted on adult hypertension patients aged 20 or older who underwent health checkups from January 2018 to December 2021 at general hospitals in Gyeonggi Province. The HTGW phenotype was triglyceride ≥150 mg/dL and waist circumference ≥90 cm for men and ≥85 cm for women. The prevalence of the HTGW phenotype in the study subjects was 17.9%. After adjusting for age, gender, and BMI, the odds ratio of the HTGW group was 5.09 (95% confidence interval [CI]: 3.545~7.309) for low HDL-cholesterol, 1.68 (95% CI: 1.176~2.411) for high LDL-cholesterol, was 2.92 (95% CI: 2.009~4.235) for high total cholesterol, 3.39 (95% CI: 2.124~5.412) for diabetes and 1.85 (95% CI: 1.286~2.674) for hyperuricemia compared to the normal triglyceride levels with the normal waist (NTNW) group. The area under curve values of the HTGW phenotype for diagnosing metabolic syndrome were 0.849 for all subjects, 0.858 for men, and 0.890 for women. In conclusion, the HTGW phenotype is closely related to metabolic abnormalities and is a useful indicator for monitoring adult hypertensive patients with metabolic syndrome.

Upregulation of Connexin43 Expression in Mitral Valves in a Rabbit Model of Hypercholesterolemia (고콜레스테롤혈증을 유발한 토끼의 승모판막에서 Connexin43 발현의 증가)

  • Background: Connexin 43-mediated gap junctional communication plays an important role in atherosclerosis. Numerous studies have demonstrated a correlation between mitral valve annular calcification and atherosclerotic disease. However, the relevance of connexin 43 to mitral valve disease remains unclear. We hypothesized that the mechanism contributing to mitral valve disease is associated with alterations in cell-to-cell communication mediated by changes in Connexin 43 expression. Material and Method: Twenty male New Zealand rabbits were divided into two groups: animals in group 1 (n=10) were fed a normal chow diet, whilst those in group 2 (n=10) received a diet containing 1% cholesterol for 12 weeks. After sacrificing the animals, the mitral valves were excised and analyzed with immunohistochemical staining and Real-time Reverse Transcriptase polymerase chain reaction (real time RT-PCR). Result: Myofibroblasts and macrophages were found concentrated within the endothelial layer on the ventricular side of the leaflet in the cholesterol diet group. Immunohistochemial staining showed elevated expression of connexin43 in the cholesterol diet group. Real-time RT-PCR revealed increased connexin43 mRNA levels in mitral valves from hypercholesterolemic animals. Conclusion: Our finding that connexin43 expression is increased in mitral valves of hypercholesterolemic rabbits suggests that alterations in cell-to-cell communication via connexin43 containing gap junctions play a role in the development of mitral valve disease in hypercholesterolemia.

Quality Characteristics and Evaluation of Physiological Activities of Moju Made with Hovenia dulcis Thunb. (헛개나무를 이용한 모주의 품질 특성 및 생리활성(in vivo) 효능 검증)

  • The purpose of this study was to assess the quality characteristics of Moju made with Hovenia dulcis Thunb. and its physiological effects on ICR mice. According to the sensory score, we selected Moju made with 1% Hovenia dulcis Thunb. among Moju made with 0, 0.5, 1, 5, and 10% Hovenia dulcis Thunb. Compared to Moju made without Hovenia dulcis Thunb., Moju made with 1% Hovenia dulcis Thunb. had higher proportions of moisture (86.77 g/100 g) and carbohydrates (11.86 g/100 g). The mean values of the physicochemical analyses were as follows: pH 4.91, acidity 0.28, $^{\circ}Brix$ 12.63, reducing sugar 68.97, alcohol content 0.1, alcohol density 0.998. Moju made with 1% Hovenia dulcis Thunb. did not have effects on DPPH radical scavenging activity; however, superoxide dismutase activity was significantly higher than that of Moju made without Hovenia dulcis Thunb. For assessing physiological activities, 4-week-old male ICR mice were divided into six groups (n=10): normal control group (NC), ethanol-administered group (EC), EC plus low-dose Moju made with 0% Hovenia dulcis Thunb. (MCL), EC plus high-dose Moju made with 0% Hovenia dulcis Thunb. (MCH), EC plus low-dose Moju made with 1% Hovenia dulcis Thunb. (MDL), and EC plus high-dose Moju made with 1% Hovenia dulcis Thunb. (MDH). Serum triglyceride (TG) level was reduced by 11.17% and 19.61% in the MDL and MDH groups, respectively, compared to the EC group. Serum total-cholesterol levels of MDL and MDH groups were significantly lower as compared to the EC group. Serum high-density lipoprotein-cholesterol levels of the MDL and MDH groups were significantly higher than those of the EC group. Liver TG levels were significantly reduced in the MCL and MDL groups. From these results, Moju made with Hovenia dulcis Thunb. demonstrated antioxidant activity and reduction of hyperlipidemia markers. Therefore, Moju made with Hovenia dulcis Thunb. can serve as a non-alcoholic beverage and functional food source.

Screening test for Dendropanax morbifera Leveille extracts: in vitro comparison to ox-LDL-induced lipid accumulation, ethanol-induced fatty liver and HMG-CoA reductase inhibition (황칠나무 추출물의 고지혈증 완화 효과 스크리닝)

  • The objective of this study was to compare the antihyperlipidemic effects of different Dendropanax morbifera leaf extracts in vitro. The extracts differed in terms of specimen age, harvesting season, and extraction method. RAW 264.7 cells were pretreated with these extracts and stimulated by oxidized low-density lipoprotein. Ethanol was used to induce toxicity in HepG2 cells. Cellular lipid accumulation was quantified using oil red O staining in both these cells. The extracts were evaluated for their inhibitory effects on 3-hydroxy-3-methylglutaryl coenzyme A (HMG-CoA) reductase. RAW 264.7 cells treated with the 60% ethanol extract of an 8-year-old specimen harvested in November exhibited the lowest lipid accumulation. The 30% ethanol extract of a 5-year-old specimen harvested in May exhibited the greatest protection from ethanol-induced cytotoxicity in HepG2 cells. The hot water extract of an 8-year-old specimen harvested in May showed the greatest inhibition of HMG-CoA reductase. These results showed that D. morbifera extracts prepared from leaves that are harvested in May possess the highest antihyperlipidemic effects.

Association between Low Density Lipoprotein Cholesterol and Pulmonary Function among Premenopausal women in their 40s: a Retrospective Cohort study (40대 폐경 전 여성에서 저밀도 지단백 콜레스테롤과 폐 기능의 연관성: 후향적 코호트 연구)

  • The aim of this study was to identify the correlation between low density lipoprotein (LDL) cholesterol and the pulmonary function. This study enrolled premenopausal women in their aged 40s, who visited the health promotion center of a general hospital more than two times. A total of 384 subjects were classified into four groups based on their LDL-cholesterol levels; A, LDL(mg/dL)<100 at both initial and follow-up; B, LDL<100 at initial but ${\geq}100$ at follow-up; C, $LDL{\geq}100$ at initial but <100 on follow-up; and D, $LDL{\geq}100$ at both the initial and follow-up test. The result showed no significant differences in the pulmonary function between the four groups. Multiple linear regression analysis, which was adjusted for age, body mass index, smoking, exercise, and follow-up duration, showed a significant negative relationship between the changes in the LDL and the changes in the $FEV_1/FVC$ (${\beta}=-0.109$, S.E.=0.029, P<0.001), but not in the $FEV_1$ and FVC. In conclusion, there was a significant but weak relationship between the LDL and pulmonary function. Further larger studies will be needed.

Association of instant noodle intake with metabolic factors in Korea: Based on 2013~2014 Korean National Health and Nutrition Examination Survey (성인 남녀에서 라면 섭취에 따른 대사적 지표 평가 : 2013~2014 국민건강영양조사 자료를 이용하여)

  • Purpose: The aim of this study was to investigate the relationship between instant noodle intake and metabolic factors in Korean adults. Methods: Study subjects were 5,894 (male 2,293, female 3,601) aged 19~64 years who participated in the 2013~2014 KNHANES. Information on frequency and consumption of instant noodles was obtained by the food frequency questionnaires method in KNHANES (Korean National Health and Nutrition Examination Survey), and subjects were classified according to age, sex, and instant noodle consumption (INC). Results: The frequency and consumption of instant noodles was 1.2 times/week and 1.2 servings in subjects. High INC group (${\geq}1$ serving/week) was significantly younger in age compared with the low INC group (< 1 serving/week). However, the high INC group had significantly higher waist circumference, metabolic factors (triglyceride, cholesterol, and HDL-cholesterol), and dietary intake (energy intake, fat, and sodium density) compared with the low INC group. Hyperglycemia showed association with higher risk of highest quartile of INC after adjustments for multiple confounding factors, including age, gender, household income, education, smoking, and alcohol compared with the lowest quartile (OR: 1.4, 95% CI: 1.1-1.8). In female, abdominal obesity showed association with higher risk of highest quartile of INC after adjustments for multiple confounding factors compared with the lowest quartile (OR: 1.6, 95% CI: 1.2-2.2). Conclusion: Consumption of instant noodles was associated with increased prevalence of abdominal obesity and hyperglycemia in women. These findings suggest an association of instant noodle consumption status with metabolic risk.
