The convergent influence of Perceived Stress and the Empowerment on Rehabilitation motive of Stroke patients (뇌졸중 환자가 지각한 스트레스와 임파워먼트가 재활동기에 미치는 융복합적 영향)

  • The purpose of this study has been done to identify the influencing factors on rehabilitation motive of stroke patients. A sample of 138 stroke patients in Long-term Care and rehabilitation hospitals completed questionnaires through face - to - face interviews. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and Stepwise multiple regression. Rehabilitation motive of stroke pacients was correlated with empowerment (r=.467, p<.001), but not with perceived stress(r=.-.186, p=.029). In a multiple regression, empowerment (${\beta}=.432$, p<.001) and primary care-giver (${\beta}=.175$, p=.023) were associated with rehabilitation motive. These factors attributed to 24% of the total variance in rehabilitation motive. Therefore, the study finding suggests that these significant factors should be considered when developing and implementing convergent care strategies for stroke patients in order to promote their rehabilitation motive.

A Systematic Review of the Rehabilitation Motivation Evaluation Tool for Stroke Patients (뇌졸중 환자의 재활동기 평가도구에 관한 체계적 문헌고찰)

  • Objective : In this study, the rehabilitation motivation evaluation tool for stroke patients was identified, and the characteristics of the evaluation tool were analyzed to analyze the conceptual framework and characteristics used in the composition of the evaluation contents. Methods : From 2000 to May 2022, documents registered in major domestic and foreign databases (PubMed, NDSL, Medline, and EMbase) were searched and selected according to the selection and exclusion criteria. The evaluation tools used in the selected documents were analyzed, and the characteristics of this evaluation tool were analyzed. Results : A total of 30 documents were selected, and they were classified into a total of five rehabilitation motivation evaluation tools. The most widely used in Korea was the measurement tool for rehabilitation motivation for the disabled, and the most widely used overseas was the intrinsic motivation inventory. Three evaluation tools constructed the evaluation contents based on the self-determination theory, and two constructed the evaluation contents through expert agreement. The number of evaluation items and 8 to 45 items were varied, and the evaluation scale was also used in various ways from 4 to 7 point scale. Conclusion : It was possible to identify the evaluation tools mainly used for the evaluation of rehabilitation motivation, and to identify the main conceptual framework based on the development of the rehabilitation motivation evaluation tool. The results of this study are expected to be used as basic data for research on the development of rehabilitation motivation evaluation tools for stroke patients in the future.

Factors Convergent Influencing Rehabilitation Motivation among Stroke Patients (뇌졸중 환자의 재활동기에 미치는 융복합적 영향 요인)

  • The Purpose of this study was to examine, the effects of depression, family Support, and medical suport on rehabilitation motivation among stroke patients. Participants in this cross- sectional, descriptive study were 206 stroke patients who completed a self-report structured questionnaire. The data were analyzed using descriptive statistics, t-test, one-way ANOVA, correlation and stepwise multiple regression analyses with IBM SPSS Win 22 Program. Family support, medical support, education and age were strong predictors of rehabilitation motivation in stroke patients. These variables explained 58.3% of the variance in rehabilitation motivation. Depression was not a predictor of rehabilitation motivation. A higher level of rehabilitation motivation was associated with lower levels of depression and higher levels of family support and medical support. These results show that effective strategies for improving family support and medical support. are needed to improving rehabilitation motivation among stroke patients.

Effects of Virtual Reality-Based Activities of Daily Living Training on Activities of Daily Living and Rehabilitative Motivation in Patients With Traumatic Brain Injury: A Pilot Study (가상현실 기반의 일상생활활동 훈련이 외상성 뇌손상 환자의 일상생활활동 및 재활동기에 미치는 효과 : 예비연구)

  • Objective : The purpose of this study was to investigate the effects of virtual reality-based activities of daily living (ADL) training on ADL and rehabilitative motivation in patients with traumatic brain injury. Methods : This study was performed using a pre-post design with seven traumatically brain injured patients. Subjects were subjected to virtual reality-based ADL training for 30 minutes a day, 2 to 3 times a week for 4 weeks. Evaluation was conducted before and after the intervention using the Korean Modified Barthel Index (K-MBI), Cognitive Functional Independence Measure (C-FIM), and Volitional Questionnaire (VQ). Changes before and after intervention were analyzed by Wilcoxon signed-rank test, and correlations were analyzed using Spearman's coefficient. Results : After intervention, patients with traumatic brain injury showed significant improvements in K-MBI (p<.05). There was no significant change in total C-FIM score and VQ score (p>.05). Total C-FIM score correlated significantly with VQ score (p<.05, r=.755). The social cognition domain of C-FIM had a significant correlation with VQ score (p<.05, r=826). Conclusions : Virtual reality-based ADL training can improve ADL performance, but further research is needed to determine whether improvements in social cognition and rehabilitative motivation are possible.

Effects of Song Discussion on Depression and Rehabilitation Motivation in Stroke Patients (노래 가사 토의가 뇌졸중 환자의 우울 및 재활동기에 미치는 효과)

  • This study investigated the effects of song discussion on depression and rehabilitation motivation in stroke patients. Older adults with chronic stroke participated in this study: nine for the experimental group and eight for the control group. The experimental group was divided into three subgroups and participated in 12 sessions over 6 weeks. Target lyrics were selected by the investigator among popular songs from the participants' young adulthood. The song-based discussion was facilitated to address issues targeted at supportive, insight-focused, or reconstructive stage. The control group was provided with delayed intervention. At pre and posttest, the short form of Geriatric Depression Scale and the Rehabilitation Motivation Scale were measured. The experimental group showed significantly decreased depression and significantly increased rehabilitation motivation (p < .01), while the control group showed no significant changes. Positive changes were also observed in all subcategories of rehabilitation motivation in the experimental group, particularly in significantly increased task-oriented motivation and decreased amotivation. This study suggests that song discussion will be effectively applied in rehabilitative settings to address psychological issues of older adults with stroke.

Effects of Depression on the Rehabilitation Motivation of Middle-Aged Stroke Patients - Focused on the Mediating Effects of Resilience (뇌졸중 중년 환자의 우울이 재활동기에 미치는 영향 - 극복력의 매개효과 중심으로)

  • This study was a descriptive correlational study investigating the mediating effects of resilience in the relationship between depression and rehabilitation motivation in middle-aged stroke patients. There was a total of 185 middle-aged patients aged 40 to 64 years, who were diagnosed with stroke at a university hospital and rehabilitated at three local hospitals located in S city and four hospitals in U city. The data were collected using a self-reported questionnaire between the 1st and 31st of December in 2016. The data were analyzed using t-test, ANOVA, Pearson's correlation coefficient, and step-wise causal method using SPSS/WIN 22.0 statistical program. To test the statistical significance of the mediation effect, PROCESS and bootstrapping were used. The mean age of the subjects were $56.26{\pm}6.37$ years. There were 70.3% male subjects, an average depression level was $21.21{\pm}7.09$, an average resilience was $25.52{\pm}9.63$, and rehabilitation motivation was $47.44{\pm}5.87$. Depression was negatively related to resilience and rehabilitation motivation. However, resilience and rehabilitation motivation have a static correlation. These results confirmed that resilience appears to be a complete mediating effect in the relationship between depression and rehabilitation motivation. Therefore, it is important to develop a resilience enhancement program to improve the motivation of rehabilitation for stroke patients.

Influences of Self-esteem, Rehabilitation Motivation on Quality of Life in Stroke Pateints with Rehabilitation Therapy (재활치료 중인 뇌졸중 환자의 자아존중감, 재활동기가 삶의 질에 미치는 영향)

  • This study was conducted to investigate the levels of self-esteem, rehabilitation motivation and quality of life and to identify the influencing factors of quality of life in stroke patients who were receiving rehabilitation therapy. Data were collected from February 22 to March 20, 2016 using self-administered questionares and the data from 216 participants were analyzed using SPSS WIN 21.0 program(SPSS Inc., Chicago, IL, USA). The results of this study were as follows: the mean self-esteem was 3.09/5, rehabilitation motivation was 3.60/5, and quality of life was 2.95/5. Sex, age, education, caregiver, occupation, duration after attack, the degree of disability(dependence), and cognitive function were significantly associated with quality of life. There was positive correlation between quality of life and self-esteem(r=.45, p<.001) and there was positive correlation between quality of life and rehabilitation motivation(r=.27, p<.001). Variables such as self-esteem, duration after attack, the degree of disability(dependence), and cognitive function explained 43.6%(F=14.87 p=<.001) of the variance in quality of life in model 3. Consequently, it is needed to develop and apply nursing intervention and programs for improving self-esteem during rehabilitation in order to improve quality of life of stroke patients.

The Effect of Leisure Activity, Based on the Model of Human Occupation, on Leisure Satisfaction, Activities of Daily Living and Rehabilitation Motivation in Elderly Patients: Implications for occupational therapy (여가활동을 통한 작업치료가 노인 환자의 여가 만족도, 일상생활활동 수행 능력, 재활 동기에 미치는 영향)

  • The purpose of this study was to investigate the effects of leisure-based occupational therapy on leisure satisfaction, activities of daily living, and rehabilitation motivation in elderly patients with chronic diseases. In the study, Subjects were assigned to either the control or experimental group. The experimental group consisted of 10 and the control group of 14 patients. The experimental group receive a Leisure-based occupational therapy intervention, and the control group receive a conventional occupational therapy intervention. Leisure satisfaction, activities of daily living, and rehabilitation motivation were measured to Canadian occupational performance measure(COPM), Korean Activities of Daily Living(K-ADL·K-IADL), and Patient Questionnaire of Rehabilitation Motivation(PAREMO). Evaluation scores were compared before and after intervention to determine whether or not there were differences over time. And we examined the correlation between the variables. The results of this study were as follows: 1) The leisure-based occupational therapy has a significant effect on leisure satisfaction and rehabilitation motivation(p<.01). 2) The conventional occupational therapy has a significant effect on activities of daily living(p<.01). 3) There was a significant correlation between ADL and IADL(p<.01). 4) There was a significant correlation between leisure satisfaction and Rehabilitation Motivation(p<.01). Implications for occupational therapy include that we must offer appropriate approach with considering the interests and value of patients to them. And proposing studies to demonstrate the efficacy of occupational therapy approaches.

Effects on Recovery Oriented Integrated Rehabilitation Program for Chronic Schizophrenic Patients (만성정신분열병 환자를 위한 회복지향의 통합재활프로그램의 효과)

  • This study was to assess the effects of a recovery oriented integrated rehabilitation program on cognitive social function, rehabilitation motivation, and mental health recovery. This program is involved motivation enhancement program with 4 subprograms composed of cognitive rehabilitation, emotion management, vocational rehabilitation, family education program. The subjects were 34 patirents with schizophrenia who had been hospitalized at National Mental Hospital on local area. The study was a non-equivalent control group non-synchronized design with two groups, an experimental group(18 patients) and a control group(16 patients). The collected data were analyzed using SPSS 12. 0. The results of the data show that there was not a significant increase in the symptom, cognitive and social function. But there was a significant increase in readiness of rehabilitation(maintenance, p=.027) and mental health recovery (p=.039). Short term recovery oriented integrated rehabilitation programs in inpatient settings are more available for motivation of rehabilitation and mental health recovery than symptom reduction or functional improvement. Therefore there should be more efforts to create more recovery oriented service provision in clinical settings.

Mediating Effect of Uncertainty on the Relationship between Social Support and Rehabilitation Motivation in Patients with Stroke (뇌졸중 환자에서 사회적 지지가 재활동기에 미치는 영향: 불확실성의 매개효과를 중심으로)

  • Purpose: The purpose of this study was to examine whether the uncertainty mediates relationship between rehabilitation motivation and social support. Methods: Participants were 123 adults patients diagnosed as stroke in a hospital located in a Metropolitan City. Data were collected by self-administered questionnaires measuring uncertainty, family support and medical team support as social support, rehabilitation motivation. Data were analyzed using t-test, one-way ANOVA, Pearson's correlation, and multiple regression analyses. The significance of mediating effect was analyzed by Sobel test. Results: Family support and uncertainty, respectively, were influential on rehabilitation motivation (${\beta}=.39$, p<.001, ${\beta}=-.18$, p=.011). Uncertainty showed partial mediating effect on relationship between family support and rehabilitation motivation, however, the effect was not significant (Z=1.63, p=.103). Conclusion: Interventions improving family support and decreasing uncertainty may be helpful in enhancing rehabilitation motivation.
