  • Airway stenosis in children may be classified into congenital and acquired forms. The incidence of acquired forms is increasing mainly due to increase in prolonged intubation and trauma. In congenital stenosis with significantly compromised airway, the patient may be tracheotomized and allowed some waiting period while expecting spontaneous resolution as the child grows. However, with this treatment policy, there is a considerably higher mortality and morbidity for the acquired disease as to often warrant surgical reconstruction. The authors reviewed the medical records of 22 pediatric patients under the age of 15 who have had airway reconstruction at the authors' department from the beginning of January, 1988 through the end of December, 1992. The prognosis was analyzed in light of the etiology, site and severity of the stenosis, and the type of reconstructive measures.

A LONGITUDINAL STUDY ON THE INTERRELATION BETWEEN PUBERTAL GROWTH AND SEXUAL MATURITY IN MALOCCLUSION (부정교합자(不正咬合者)의 사춘기성장(思春期成長)과 성적(性的) 성숙간(成熟間)의 상호관계(相互關係)에 관(關)한 누년적(累年的) 연구(硏究))

  • To study the sexual maturity at puberty and interrelation between pubertal growth peak and sexual maturity, 68 malocclusions (female 44, male 24) were longitudinally studied for 4 years and cross-sectional samples of 210 malocclusions were studied together. The pubertal growth peak was determined by stature increment and sexual maturity was studied using developmental stages of pubic hair and breast, menarche in female and those of pubic hair in male. The following results were obtained. 1. Mean age at menarche was 12.8 years and it was 11.8 months later than PHV. 2. At pubertal growth peak, developmental stage 2 (54.2%) and 3 (29.2%) of pubic hair were most in male, and stage 1 (72.7%) and 2 (20.5%) were most in female. 3. At pubertal growth peak, the developmental stage 2 (50%) of breast was most and the stage 3 (43.2%) was next. 4. In interrelation between skeletal growth and sexual maturity, PHV had most high correlation with menarche and stage 3 of breast, and menarche had most high correlation with stage 3 of breast. 5. Appearance of remarkable sexual maturity suggested that the PHV was passed already.

Simplified Approach to Repair of Complete Atrioventricular Septal Defect (완전방실중격결손수술의 단순화)

  • Recent advances in understanding the anatomy of complete atrioventricular septal defect have led to alternative methods of repairing these defects. Here we report two cases of complete atrioventricular septal defect repair by direct closure of the ventricular element of the defect. Follow-up for average of 3 months suggests that, when direct closure can be performed, the results can be comparable with those of the standard technique. Our initial success with this approach is encouraging; however, longer follow-up and more experiences are required to establish whether it will be broadly applicable.

Long-Term Clinical Results of the St. Jude Medical Valve in Mitral Position (St. Jude 승모판막의 장기 임상성적)

  • A total of 217 patients underwent single mitral valve replacement [MVR] with the St. Jude Medical valve between September 1984 and the end of 1992. There were 86 males and 131 females with a mean age of 34.6 $\pm$ 14.4 years[range 5 months-61 years]. A previous valve replacement had been performed in 46 patients [21.2 %]. An early mortality rate was 7.4 % [5.2 % in primary MVR; and 15.2 %in re-replacement MVR]. Early survivors of 201 patients were followed up for a total of 934.5 patient-years[mean 4.7 $\pm$ 2.1years]. A late mortality rate was 2.5 % or 0.54%/patient-year. The linearized rates of thromboembolism, valve thrombosis and anticoagulation-related bleeding were 1.301 %, 0.214 % and 0.428 %/patient-year, respectively. The actuarial survival including operative mortality was 89.9 % $\pm$ 2.1% at postoperative 10 years. The freedom from thromboemolism was 91.3 %$\pm$ 2.5% and the actuarial estimate of incidence free from late deaths and all complications were 80.9 % $\pm$ 3.8 % at 10 years. There were no mechanical failures. In summary, the St. Jude Medical prosthesis performed satisfactorily with an acceptable rate of late complications.

Clinical Results of Double Mitral and Aortic Valve Replacement with the St. Jude Medical Prosthesis (쎈트쥬드 중복판막치환의 장기 임상성적)

  • A total of and consecutive 87 patients underwent concomitant double mitral and aortic valve replacement with the St. Jude Medical prosthesis between January 1985 and December 1993. They were 44 males and 43 females with the ages ranging from 18 to 59 years[mean$\pm$SD: 40.9$\pm$9.5 years . Fifteen patients[17.2% had a history of previous cardiac valve replacement. There were 2 early deaths[2.3% , and 85 early survivors were followed up for a total of 352.6 patient-years[mean$\pm$SD: 4.1 $\pm$2.6 years . All were anticoagulated with coumadin keeping the target international normalized ratio within the range of 1.5 and 2.5. There was a single late death[late mortality of 0.284%/patient-year . Thromboembolism was the most frequent complication[1.985%/patient-year , and bleeding related to anticoagulation was experienced in one patient [0.284%/patient-year . The incidences of prosthetic valve endocarditis and of paravalvular leak were also low[0.284%/patient-year, respectively . The survival including operative mortality was 96.1%$\pm$2.2% at 10 years. The actuarial probabilities of freedom from thromboembolism and from all events were 77.9%$\pm$11.1% and 72.4%$\pm$10.7%, respectively, at 10 years. There was no structural failure of the prosthesis. Results from a series of clinical studies suggest strongly that the use of lower intensity of anticoagulation therapy lowers the thromboembolic as well as bleeding rates in patients with the ST. Jude Medical prosthesis.

  • Desmoplastic small round cell tumor (DSRCT) is rare and a recently described, poorly differentiated malignant tumor and it usually presents with widespread intra-abdominal involvement. We report a case of DSRCT arising form the mediastinum which was treated with multimodality anticancer therapy in 15 year-old girl.

Mid- and Long Term Outcome of Fontan Procedure: Extracardiac Conduit Fontan versus Lateral Tunnel Fontan (폰탄 술식의 중장기 성적: 심장외 도관 술식과 가측터널 술식의 비교)

  • Background: This paper reviews our experience retrospectively to examine the clinical results and effectiveness of lateral tunnel (LT) and extracardiac conduit (ECC) Fontan procedures at a single institution. Material and Method: One hundred and sixty five Fontan procedures were performed (67 LT and 98 ECC) between January 1996 and December 2006. Preoperative and postoperative hemodynamic values, arrhythmia, hospital and intensive care unit stay, chest tube drain, morbidity and mortality were reviewed. Result: The overall operative mortality in the LT and ECC groups was 4.5% (3) and 2.0% (2), respectively. There was a significant difference in the immediate postoperative transpulmonary gradient (LT $8.5{\pm}2.5$ vs ECC $6.6{\pm}2.4$, p-value<0.001) and central venous pressure (LT $18.3{\pm}3.8$ vs ECC $15.6{\pm}2.4$, p-value=0.001) between the two groups. The mean follow-up in the LT and ECC groups was $74.1{\pm}31.5$ and $38.1{\pm}29.1$ months, respectively. There was one late death. The actuarial survival at 10 years in the LT and ECC groups was 92% and 89%, respectively. In arrhythmia, the ECC patients showed a slightly low incidence but the difference was not statistically significant. Conclusion: Both the LT and ECC Fontan procedures showed comparable early and mid-term outcomes in terms of the surgical morbidity and mortality, postoperative hemodynamics, and mid-term survival. The ECC Fontan procedure reduces the risk of arrhythmia in the follow up period.

The Early Results of Tricuspid Valvuloplasty with Using the Edwards MC3 Annuloplasty System (Edwards MC3 Annuloplasty System을 이용한 삼첨판 성형술의 조기 성적)

  • Background: Functional tricuspid regurgitation (TR) greater than or equal to a mild grade requires tricuspid valvuloplasty, and tricuspid valvuloplasty with ring annuloplasty has shown good outcomes. We report here on our early experience with the Edwards $MC^3$ annuloplasty system (Edwards LifeSciences, Irvine, CA). Material and Method: From November 2004 to July 2006, 72 patients with tricuspid annular dilatation and TR underwent tricuspid valvuloplasty with using the Edwards $MC^3$ annuloplasty ring. Sixty-eight patients were operated on via median sternotomy and four patients were operated on using robotic assisted minimal invasive thoracotomy. The patient population included 21 males and 51 females and their mean age was $53.9{\pm}12.3$. The mean grade of TR, as assessed by the preoperative echocardiography, was $2.2{\pm}1.0$. The mean NYHA functional class was $3.1{\pm}0.8$. The mean left ventricular ejection fraction was $57.0{\pm}9.9$%. Result: The TR and NYHA functional class, as assessed by postoperative echocardiography, was significantly reduced (mean=$0.4{\pm}0.6$ and $2.0{\pm}0.7$, respectively p<0.001). There was one case of hospital mortality. One patient required permanent pacemaker insertion for third degree atrioventricular block. Conclusion: Our study shows that the Edwards $MC^3$ remodeling ring is easy to implant and it effectively corrects functional TR with excellent clinical and echocardiographic outcomes. Further follow-up and a larger clinical series are required to establish the long-term stability of this repair technique.

Comparison of Outcomes after Curative Resection of Primary Lung Cancer between 50 Year or Younger and 70 Year or Older Patients (50세 이하와 70세 이상 원발성 폐암 환자에서의 근치적 수술 후 성적 비교)

  • Background: Previous series have suggested that younger patients with primary lung cancer exhibit a more aggressive disease course with a worse prognosis, as compared to older patients, although this issue is still debatable. Material and Method: We reviewed the medical records of 79 patients (32 patients 50 years and younger (Group I) and 47 patients 70 years and older (Group II)) who underwent curative resection for primary lung cancer between July 2000 and June 2008. Result: The median age of the patients was 46.5 years in Group I and this was 73 years in Group II. The older patients were more likely to have major comorbidities (44% versus 77%, respectively; p=0.003). Histological examinations identified that the minor histological types (excluding non-small cell lung cancer (NSCLC)) were predominantly found in the Group I patients (16% versus 2%, respectively; p=0.037). For the TNM staging of the NSCLC, with excluding the minor histologic types, a higher proportion of patients had stage III disease in Group I (33% versus 13%, respectively; p=0.038). There was no significant difference in major morbidity (16% versus 30%, respectively; p=0.148) and operative mortality (0% versus 4.3%; p=0.512) between the groups. The mean follow-up interval was 33 months (range: $1{\sim}98$ months) for patients in both groups. For the patients with NSCLC, the five-year overall survival rate was 52.3% for Group I and 53.7% for Group II (p=0.955). The rate of freedom from recurrence at five years was significantly lower for the Group I patients than for the Group II patients (39.4% versus 70.4%, respectively; p=0.027), and only being a member of Group I impacted recurrence, based on the Cox proportional hazard analysis (p=0.034). Of the patients who had recurrence, four patients in Group I underwent aggressive surgical treatment. All of these patients exhibited long-term survival (range: $46{\sim}87$ months). Conclusion: In our study, the early outcome and long-term survival were similar for the younger and older patients after curative resection of primary lung cancer. However, we think that younger patients require meticulous follow-up as they had a tendency to proceed to surgery with advanced stage disease, a higher recurrence rate than did the older patients and the survival rates were improved, even for the recurred cases, with early aggressive treatment.

Result of Radiation Therapy of Cerebellar Medulloblastoma - with Emphasis on the Neuraxis Dose - (전중추신경계 조사선량을 중심으로 한 수아세포종의 방사선치료성적)

  • Treatment of cerebellar medulloblastoma has been much improved with modern surgical technique for gross total tumor removal and adequate radiation therapy for the whole craniospinal axis. Questions have been arosen about the optimal radiation dose for the preventive treatment of whole cranium and whole spinal axis. Recently, many authors have reported their treatment results as comparable to older data, using lower than conventional dose of 3,600 cGy-4,000 cGy. For 50 patients treated between 1981 and 1990 at the Department of Radiation Therapy of SNUH, retrospective analysis was done for the treatment result, especially the neuraxis control, by radiation dose for the presymptomatic area of the disease. Analysis only by total spinal dose did not give any significant difference. But further analysis by following patient group; 3,600 cGy/150 cGy (n=6), 3,000 cGy/150 cGy (n=10), 2,400 cGy/150 cGy (n=17) and 2,400 cGy/100-120 cGy (n=11) showed significant improvement of neuraxis control by decreasing order (p =0.003). There was no significant difference in overall survival between the groups. For the 19 patients who had been confirmed initially as having no neuraxis disease, TDF 30 was the cur-off value that could prevent neuraxis failure (p =0.004). We couldn't define any TDF value that give reasonable control for the patient group with positive CSF study at initial diagnosis.
