이 Resendahl의 문헌을 읽고 나는 두 가지 면에서 모든 간호학도에게 이 글을 읽기를 강권하고 싶은 생각이 났다. 첫째는 ; 오늘날 우리가 지향하는 이상적인 간호사업을 이룩하기 위하여, 둘째는 ; 현시대와 사외가 우리에게 요구하는 건강사업을 독자적이고 지도적 입장에서 이룩하기 위한 전략에 기본적인 원리로서 교수나 학생, 또한 임상간호학자(임상간호원)가 자기가 하는 일을 원숙하게 이행해 나가기 위해서는 깊이 동감할 수 있고 귀히 참고할 만한 문헌이라고 생각하여 여기 옮겨본다. 1976년 11월 하순 어느 눈 내리는 조용한 주일 아침 탈고하다.
Purpose: The purposes of this study were to examine self-efficacy, severity, social support, and self-care compliance of patients with liver cirrhosis and to identify the factors that affect their self-care compliance. Methods: A total of 160 outpatients with liver cirrhosis visiting the gastroenterology clinic at an university hospital located in Busan were enrolled in this study. Data were collected via self-report questionnaires including self-efficacy, severity of disease, social support, and self-care compliance measures. The collected data were analyzed using t-test, ANOVA, $Scheff\acute{e}-test$, Pearson correlation coefficients, and multiple linear regression analysis. Results: The factors affecting self-care compliance were self-efficacy, severity of disease, age, and sex. These factors explained 28.4% of the variance in self-care compliance of patients with liver cirrhosis. Conclusion: Therefore, nursing interventions that enhance patients' self-efficacy are needed to promote self-care compliance in patients with liver cirrhosis. In addition, patients must be taught of the importance of self-care compliance so that even young patients and those with relatively mild disease. That would also be motivated to comply with self-care instructions.
Purpose: The objective of this study was to identify factors related to self-care adherence in symptomatic patients with heart failure (HF). Methods: Using a cross-sectional design, a convenience sample 209 outpatient clinic patients were recruited at two medical centers. Between October 2011 and August 2012, data were collected using the structured questionnaire. Factors related to self-care adherence were examined using hierarchical multiple regression. Results: Mean age of participants was 67.71 years and a half of them (53.6%) were female. They showed relatively low self-care adherence with mean scores of $61.88{\pm}12.92$. Lower self-care adherence was reported in asking for low sodium items, weighing oneself, checking for ankle edema, and exercising for 30 minutes. The overall model significantly explained 23.9% of variance in self-care adherence. Among the predictors, education, New York Heart Association functional classification, and social support were statistically significant in influencing self-care adherence. The variable of negative emotional status such as anxiety and depression were not found to be significant. Conclusion: These findings demonstrate that social support could help self-care adherence among symptomatic patients with HF. Thus, programs targeting self-care adherence in this population should consider the strategies improving social support.
Purpose: The purpose of this study was to examine the effects of a cognitive behavioral stress management program on diabetic self-care and glycemic control with type 2 diabetic patients. Methods: Thirty three diabetic patients who were older than 40 were recruited from a public health center and conveniently assigned into both experimental(n=16) and control groups(n=17). Participants in the experimental group had attended the weekly cognitive behavioral program for 8 weeks. Data were collected from June 2005 to August 2006 and analyzed by independent t-test using the SPSS WIN program. Results: After an 8 week intervention, participants in the experimental group reported on increasement of diabetic self-care behaviors and an increasement of blood glucose levels, which were significantly different from those in the control group. Conclusion: On the basis of those findings, we concluded that the cognitive behavioral stress management program has positive effects on diabetic self-care and glycemic control for the patients with DM. Further research is needed to identify the long-term effects of the cognitive behavioral program.
Purpose: The purpose of this study was to identify the retention effects of an individualized dietary education program for hemodialysis patients on diet knowledge, diet self-care compliance, and physiological indices, thus to find the most effective time period for re-education. Method: This study utilized one-group repeated pretest-posttest design. The participants were 52 hemodialysis patients in C hospital, Gyeonggi-Do. Data were collected at 4 and 12 weeks after the education from January through April 2016. Results: There was significant increases in diet knowledge even 12 weeks after the education (p= .007). Diet self-care compliance showed a significant increase at 4 weeks (p= .001), but a decrease at 12 weeks after the education. The level of blood natrium was significantly decreased between 4 and 12 weeks after the education (p= .006). The weight was significantly decreased at 12 weeks after the education. Conclusion: It has been identified that re-education for hemodialysis patients should be implemented between 4 and 12 weeks after education in order to maintain patients' diet self-care compliance, an ultimate aim of diet education. By helping them with their self-care compliance, the patients would maintain their physical and psychological function optimally, thus contributing to a better quality of life among hemodialysis patients.
Purpose: This study was conducted to identify the effects of education regarding action plans according to a self-monitoring program on self-management adherence, knowledge, symptom control, and health-related quality of life (HRQoL) among adults with asthma. Methods: Thirty-four patients were randomly assigned to the intervention group and thirty-two to the control group in this study. A tailored 50-minute intervention based on the contents of self-monitoring and action plans developed by the National Heart Lung and Blood Institute was provided to the intervention group. Structured and well developed questionnaires were used to measure the dependent variables. Results: There were no differences in all general and clinical characteristics, and the dependent variables between two groups in the pre-test. In the post-test, there were differences in the level of self-management adherence (t=4.41, p<.001), knowledge (t=2.26, p=.027), symptom control (t=-2.56, p=.013), and total HRQoL (t=2.14, p=.036) between the two groups, although there was a difference only in the sub-domain of emotion (t=2.03, p=.047) in HRQoL. Conclusion: This study found that action plans according to self-monitoring that enhance a participatory interaction in the treatment and care could help patients with moderate to severe asthma to engagead equately in self-care, to control their symptoms, and to improve their HRQoL. Further studies are still needed to identify longitudinal effects of this program.
Purpose: This study aimed to develop an instrument that measured adherence to self-management among patients with hypertension, and to verify the validity and reliability of the developed instrument. Methods: The 34 indicators that underwent the content validity test were provided to 202 subjects for investigation The collected data were analyzed using SPSS Ver. 22.0 and Amos Ver. 25.0. Results: Five factors, 'interaction,' 'lifestyle change,' 'continuing motivation,' 'medical care', and 'weight control' were extracted, and the total cumulative variance was shown to be 61.75%. The test statistic of the regression coefficient was statistically significant according to the confirmatory factor analysis. Conclusion: It is suggested that more research is needed to generalize the instrument to suit more candidates, while continuing to refine the reliability and validity of the instrument.
Purpose: The purpose of this study was to explore the effect of individualized diet education program on dietary knowledge and self-care compliance among hemodialysis patients. Methods: The design of the study was quasi-experimental nonequivalent control group pre-post test design. Fifty patients were assigned to an experimental group and fifty patients were assigned to a control group. Individualized diet education program was applied to the experimental group once every two weeks for twelve weeks. Results: Dietary knowledge increased significantly in the experimental group when compared to the control group. Dietary self-care compliance improved significantly in the post-test of the experimental group, however, the difference was not significant. Conclusion: The individualized dietary education program was effective in improving dietary knowledge. The individualized diet education program needs to be incorporated as an important nursing intervention. Nurses should provide individualized diet education program for hemodialysis patients.
Purpose: The purpose of this study was to investigate the adherence to diabetes control recommendations (blood glucose testing, medication, diet, exercise) in patients with type 2 diabetes at home and to analyse the correlation between adherence and blood glucose level. Method: Participants, numbering 214, were recruited from the endocrinology outpatient department of a tertiary care hospital in an urban city. The data were collected by a self report adherence questionnaire. Glycosylated haemoglobin (HbA1c) was determined by the high-performance liquid chromatography technique and fasting blood glucose was analyzed by the glucose oxidase method. Result: Medication adherence was higher than diet or exercise adherence. The frequency of blood glucose testing was lower for middle school graduates than college graduates. Diet adherence was significantly lower for participants who were obese, who did not have a spouse, and who had hyperglycemia. Medication and diet adherence were negatively correlated with HbA1c. Conclusion: A diet education program should be developed for patients with type 2 diabetes who are obese, who do not have a spouse, and who have hyperglycemia.
Purpose: The purpose of this study was to examine self care compliance, family support, and depression in patients with congestive heart failure. Method: The participants were 105 outpatients with congestive heart failure. Data were collected using a questionnaire survey on self care compliance, family support, and depression. The collected data were analyzed using the SPSS Win 10.0 program. Result: 1. The total score for Self care compliance was 78.34 out of a possible 120 and was significantly high for people in ages between 70-79 years and people who were not employed. Exercise compliance was significantly high in men and classifications I & II of the NYHA class. Compliance with smoking and alcohol cessation were significantly high in women. Medication compliance was significantly high in the group which had experience with hospitalization. Family support was 31.75 out of a possible 40 and showed significant differences depending on spouse status and religion. Depression was 30.18 out of a possible 64 and was significantly high in the group whose educational level was above college graduation or who were under 65 years of age. Conclusion: The results suggest that nurses have to emphasize smoking and alcohol cessation for men, exercise for women and total self care compliance for patients under 65 years of age.
