The Effects of Breast Self-examination Program Enriched Environmentally among Healthy Women (환경보강 유방자가검진 프로그램의 효과)

  • Purpose: The purpose of this study was to determine the effects of an environmental enrichment program on barrier, benefit, confidence, and compliance of breast self-examination in women at 3 months after instruction. Methods: Nonequivalent control group pretest-posttest design was conducted among 62 healthy women. Data were collected through the self-reported questionnaires from December 2008 to March 2009. Women were assigned to one of two treatment groups: (a) no-support of environmental enrichment with the instruction control group; (b) the support of environmental enrichment with the instruction experimental group. All women received the same instruction on breast self-examination once a week for two weeks. Data were analyzed with paired t-test, McNemar test and ANCOVAs of controlling for age run using SPSS/WIN 17.0. Results: The experimental group was significantly lower than control group on perceived barrier (F=5.91, p=.02) and higher than control group on compliance of breast self-examination (F=4.57, p=.04) after environmental enrichment program. However the environmental enrichment program did not make significant differences on benefit (F=0.01, p=.91) and confidence (F=0.77, p=.38). Conclusion: Findings suggest that the environmental enrichment should be needed to support women's breast self-examination and expanded for compliance of breast self-examination to promote the secondary prevention of women breast cancer.

  • 당뇨병은 고혈당을 조절 안할 경우에는 돌이킬 수 없는 신체적 합병증은 물론 정신적 손상까지 받지만, 고혈당은 노력만 하면 조절이 가능하며, 정상에 가까울 정도로 조절되면 정상인과 같은 삶을 영위할 수 있다. 그러나 보건의료 전문가의 인슐린 주사 요법, 식사 및 운동요법 등의 일방적인 처방만으로 성공적인 당조절을 기대하기에는 어려움이 있다. 그래서 당뇨병 환자에게 당뇨병이라는 것이 무엇이며, 어떻게 이 병을 스스로 조절할 수 있게 만드느냐가 더욱 중요한 것이다. 따라서 우리나라에서도 당뇨병에 대한 관심이 높아짐에 따라 각 지역과 의료기관마다 다양한 당뇨병 교육 프로그램이 실시되고 있다. 특히 소아 당뇨병 환자는 이러한 전통적인 당뇨병에 관한 교육을 통해서 당뇨병에 관한 지식은 많이 가지고 있지만, 당뇨병 자가 조절을 위한 환자 역할 행위(sick-role behavior)를 변화시키는데는 그다지 성공절이라 할 수 없다. Bandura(1977)는 자기 효능은 ‘할 수 있다는 자신감’으로 수행 성취, 대리 경험, 언어적 설득, 정서적 각성에 관한 정보에 의해 결정된다고 하였다. 따라서 이러한 정보들은 자기 효능을 증진시키기 위한 중재의 방향을 제시해 준다. 오늘날 이러한 정보를 활용한 중재 프로그램 중에서 가정과 학교를 떠나 자연 환경 속에서의 집단 활동을 통하여 사회 학습 경험을 하는 캠프 프로그램에 대한 관심이 높아지고 있다. 캠프에 참여한 당뇨병 아이들은 캠프 활동속에서 다른 동료 아이들이 자신이 갖고 있는 문제들을 성공적으로 해결해 나가는 것을 관찰하여 대리 경험하게 될 때 희망을 가지게 되며, 당뇨병을 가진 다른 동료들과 공통의 경험을 공유할 수 있는 기회를 갖게 되어 그들은 자신이 더 이상 혼자가 아니며 남과 ‘다르지’도 않다는 것을 깨닫게 되어 점차 자신감을 가지고 살아갈 수 있게 된다. 본 연구는 캠프 프로그램이 소아 당뇨병의 자기 효능을 증진시키고, 당뇨병 환자 역할 행위 이행 정도를 높여주는지를 규명해 봄으로써, 소아 당뇨병 환자를 위한 효과적인 간호 중재 방안을 제시하고자 비동등성 대조군 전후 실험 설계의 유사 실험 연구를 시도하였다. 1996년 8월 10일 부터 12월 12일까지 종합병원에서 추후 관리하고 있는 소아 당뇨병 환자중 선정 기준에 맞는 환자 41명을 연구대상으로 하였으며, 그 중 19명은 실험군으로 Bandura의 자기 효능 증진을 위한 정보원을 활용한 캠프 프로그램을 5박 6일간에 걸쳐 실시하였고, 다음 22명은 대조군으로 캠프 프로그램을 실시하지 않았다. 자료수집은 자기 효능 척도와 환자 행위 역할 이행 척도를 캠프 프로그램을 실시하기 전에 사전 조사를 하고 중재 후 4주째 사후 조사를 하였다. 수집된 자료는 SPSS/PC+로 Chi-square test, t-test, ANCOVA, Pearson correlation을 이용하여 분석하였으며, 그 결과는 다음과 같다. 캠프 프로그램은 소아 당뇨병 환자의 자기 효능을 증진시키고 환자 역할 행위 이행을 높여주는데 효과적 이었다. 소아 당뇨병 환자의 자기 효능은 환자 역할 행위 이행과 순 상관 관계가 있어, 자기 효능이 증진될수록 환자 역할 행위 이행 정도가 높아졌다.

The Effects of an Empowerment Education Program for Kidney Transplantation Patients (신장이식환자를 위한 능력증강교육프로그램 효과)

  • Purpose: This study was conducted to develop an Empowerment Education Program (EEP) for kidney transplant patients and to test the program's effects on uncertainty, self-care ability, and compliance. Methods: The research was conducted using a nonequivalent control group with a pretest-posttest design. The participants were 53 outpatients (experimental group: 25, control group: 28) who were receiving hospital treatment after kidney transplants. After the pre-test, patients in the experimental group underwent a weekly EEP for six weeks. The post-test was conducted immediately after, and four weeks after the program's completion in the same manner as the pre-test. For the control group, we conducted a post-test six and ten weeks after the pre-test, without and program intervention. A repeated measure ANOVA was performed to compare the change scores on main outcomes. Results: Uncertainty was significantly lower in the experimental group than in the control group, both immediately after (t=-3.84, p=<.001) and 4 weeks after (t=-4.51 p=<.001) the program, whereas self-care ability (t=5.81, p=<.001), (t=5.84, p=<.001) and compliance (t=5.07, p=<.001), (t=5.45, p=<.001) were significantly higher. Conclusion: Kidney transplant patients who underwent an EEP showed a decrease in uncertainty and an improvement in self-care ability and compliance. Thus, our findings confirmed that an EEP can be an independent intervention method for improving and maintaining the health of kidney transplant patients.

A Study of the Effects of Health Contracting on Compliance with Health Behaviors in Clients with Hypertension (자가간호증진을 위한 건강계약이 고혈압자의 건강행위 이행에 미치는 영향)

  • It is generally accepted that the delivery of health care is undergoing many changes specially those related to acute, contagious disease care and to the increase of chronic illnesses which can not be cured but are controlable. The health care practitioner can not be soley responsible for the control of their clients' care. Because the clients will play a vital role in controlling their illnesses, long term participation by both the health care provider and the client is necessary. Since most individuals with hypertension do not experience signs or symptoms, the disease is difficult to detect and even when diagnosed, clients do not comply well with their hypertension regimens. The noncompliant client is at increased risk for compliants involving the heart, brain, kidney and other organs. In an effort to explore methods of increasing patient participation in and adherence to treatment programs for hypertension, the researcher used health contracting to promote self care. The research questions are; 1) Will the health contracting increase compliance in health behavior and reduce the blood pressure\ulcorner 2) If clients comply with their regimens will this reduce their blood pressure\ulcorner The research design utilized in this study was a quasi-experimental design. A purposive sample, was abtained from two churches in the 1. area, consisting of 64 clients with hypertension. The data was collected from the middle of January to the 1st of September 1985. Randomization was only of the two church groups into experimental and control groups. Compliance with health behavior related to the hypertensive regimen, blood pressure and body weight were measured, compared and analyzed. In the experimental group measurements were made 6 times; one month before the education program after education program when health contracting was done and 4 more times once a month for 4 months. In the control group measurements were made 3 times; one month before the education program after the education program, and once 4 months later. There was no health contracting. The data were analyzed by t-test, Pearson correlation and ANOVA according to purpose of the study. The result of this study may be summarized as follows: The result related to the hypothesis on the effect of health contracting are as follows: H$_1$; “The hypothesis that the experimental group, with a health contractual agreement will demonstrate increased compliance levels for health behavior than the control group” was supported(t=-5.29, df=62, p=.000). H$_2$; “The hypothesis that the experimental group, with a health contractual agreement, will demonstrate a greater reduction in blood pressure than the control group” was supported (for systolic blood pressure t=2.72, df=62, p=.009, for diastolic blood pressure t=1.95, df=62, p=.050). H$_3$; The hypothesis that the greater the compliance of the client with health behavior the lower the client's blood pressure will be was partially supported (for systolic pressure r=-.2981, p=.008, for diastolic pressure r=-.1720, p=.087). From the examination of the results of this study it can be concluded that the interaction between the nurse and the client, contracting to define goals and reinforcing compliant behavior, leads to improved compliance with health care behaviors and thus to an increase in the effectiveness of nursing care. Further consideration need to be given to the inclusion of the concept of health contracting in primary nursing and to further research in this area.

The Effects of the Breast Self-Examination Participant Education program on Breast Self-Examination Compliance and Health Promoting Behaviors in Adult Women (유방 자가 검진 참여 교육 프로그램이 유방 자가 검진 이행과 건강 증진 행위에 미치는 영향)

  • This study is carrying out a before and after experiment design for the non-equal comparative group to identify the effects of the breast self-examination education on breast self-examination participant education compliance and health promotion in women. The subject of this study was 58 women residing in Chungju. Their age ranged from 20 to 40. These women were not pregnant or did not breast feed, as well as they did not have any breast disease, at the time of survey. They were available for the response to the questionnaires, and understood the purpose of this study. They also agreed to participate in the study, and responded to the 3rd time questionnaires to the end. Thus, brochures and lectures were provided side by side to a group of 19 of those women, and only brochures were provided to another group of 39 of those women. With regard to education, a brochure and a program using a breast model were applied. Then, the frequency of the breast self examination compliance, breast self examination capability and the relations between the breast self examination and activities to promote health were measured, before the education, after 4 weeks of education and after 12 weeks of education. Before education and after 4 weeks of education. I collected the questionnaires myself visiting them, and after 12 weeks of education, the questionnaires were collected by mail. In relation to the study tool, the breast self examination activity was measured by two measuring tools: breast self examination activity frequency and breast self examination compliance capability. As for the frequency, the number of self examination for the period of 3 months, before the questionnaire survey, was measured in the form of self report. In relation to the tool to measure the breast self examination capability, the BSEPRI tool, which was developed by Wood in 1994, was used. Here, as the score was higher, the capability was indicated to be higher. The translated and revised version of Health Promoting Lifestyle Profile II (Walker, Sechrist & Pender. 2002) was used as a tool for health promotion. As the measured score was higher, the health promoting activity was indicated to be higher. The results of this study were as follows: 1. As a result of checking the breast self examination compliance frequency between the experiment group that received the breast self examination participant education and the comparative group that did not receive the education, there was a significant difference in interaction between groups by time, as time passed by. 2. As a result of checking the breast self examination compliance capability between the experiment group and the comparative group, there was a significant difference in interaction between groups by time, as time passed by. 3. As a result of carrying out a repetitive measurement analysis, between the experiment group that received the breast self examination education and the comparative group that did not receive the education, which was carried out to validate the hypothesis that the former would have higher health promoting activities than the latter, there was no significant difference after the breast self examination education was conducted.

A Scoping Review on Educational Programs Provided to Kidney Transplant Patients in Korea (국내 신장이식 환자에게 제공된 교육 프로그램에 대한 주제범위 문헌고찰)

  • The purpose of this study is to provide evidences throught scoping review of education intervention studies for post kidney transplantation management in the kidney transplant patients in Korea. Through the search engines RISS, KISS, DB pia and Pubmed, 13 articles were selected by searching for theses and academic journals published prior to the search date of May 21, 2023. According to the study result, relevant studies were 13, and the education programs provided to kidney transplant patients were classified into 1) overall health management after transplantation, 2) self-efficacy promotion exercise program, 3) exercise therapy, 4) drug education, and 5) stress coping. Knowledge, implementation, self-efficacy, anxiety, uncertainty, stress, and quality of life were confirmed as effects of the intervention. Based on the results of this study, it is necessary to develop integrated educational materials and systematic self-management programs that can manage various health problems that may occur after kidney transplantation, and it is suggeste that research on educational interventions for kidney transplant patients in nursing.

Relationship of Fatigue, Family Function and Self-esteem of Middle-aged Women in Menopausal Periods (폐경기 중년여성의 피로, 가족기능과 자아존중감 간의 관계)

  • The purpose of this study was to examine the relations between fatigue, family function and self-esteem among middle-aged women in menopausal periods. Data were collected through self-administered questionnaire survey form 133 middle-aged women. The data collection was done from March 2nd to April 10th, 2012. As a results, there were significant differences in fatigue by education level and menstrual status. And, there were significant differences in family function according to education level and marital status. Also, there were significant differences in self-esteem by education level, marital status and monthly income. Fatigue was negatively related to family function and self-esteem. And family function was positively related to self-esteem. These results suggest that nursing intervention should be developed to strengthen family function and self-esteem as a way to regulate fatigue for middle-aged women in menopausal periods. Especially, it is necessary to pay more attention to health management of middle-aged women belonging to menopausal transition, with low-income, low-education level.

Influencing Factors for Post-traumatic Growth in Patients with Breast Cancer (유방암 환자의 외상 후 성장 영향 요인)

  • This study was undertaken to investigate the factors that influence post-traumatic growth in patients with breast cancer. The study subjects were 100 outpatients with breast cancer who visited the outpatient clinic of a university hospital. The instruments used for this study were Korean Post-traumatic Inventory (K-PTGI), Modified Medical Outcomes Study Social Support Survey (mMOS-SS), Self-Efficacy Scale for Self-Management of Breast Cancer (SESSM-B) and Cancer Coping Questionnaire (CCQ). The data were analyzed by t-test, ANOVA, Pearson-correlation coefficient, and stepwise multiple regression using SPSS/WIN 21.0. The mean score of post-traumatic growth was $3.4{\pm}0.9$ out of 5. Post traumatic growth had statistically significant correlation with social support, breast cancer self-management self-efficacy, and coping behavior. In stepwise multiple regression analysis, post-traumatic growth was significantly influenced by therapeutic compliance related self-efficacy and positive reframing coping behavior and it was account for 29% of the total variance. These results suggest that these influencing factors should be considered in developing the nursing intervention to improve the post-traumatic growth in patient with breast cancer.
