Burnishing effect on marginal misfit of implant-supported screw-and-cement retained prostheses: A case report (임플란트 지지 나사-시멘트 유지형 보철물에서 보철물-지대주 제거 후 변연부 연마의 효과: 증례보고)

  • When the marginal fit of fixed dental prosthesis decreases, biological and technical complications, such as plaque accumulation, periodontal disease, hypersensitivity, components fracture, cement loss, can happen. The complications affect the long-term success and survival of prostheses. This case report describes a clinical procedure to minimize the marginal gap of implant-supported screw-and-cement retained prosthesis by removing prosthesis-abutment complex and burnishing the interface area. The marginal gap was measured before and after the burnishing using a stereomicroscope and compared. This technique improves the marginal fit, thereby contributing the longevity of the prosthesis.

  • Peri-implantitis is an inflammatory lesion of the periodontium surrounding an endosseous implant, with progressive loss of the supporting peri-implant bone. The main purposes of treatment for peri-implantitis due to biological factors include addressing the inflammation and restoring a healthy but reduced periodontium around the implant, similar to the treatment of periodontitis in natural teeth. The proposed treatment protocol includes surgical treatment, mainly resective surgery, after non-surgical treatment such as oral hygiene instructions, mechanical cleansing of the fixture, and general or topical antiseptic or antibiotic application according to the extent of inflammation. In this article, we present a 6-year follow-up case showing unusual marginal bone regeneration after resective surgery and decontamination of an implant surface for the treatment of peri-implantitis and discuss the possible reasons.

Finite Element Stress Analysis of Bone Tissue According to the Implant Connection Type (2종의 임플란트 내부결합구조체에 따른 치조골상 유한요소응력 분석)

  • The purpose of this study was to make the stress distribution produced by simulated different load under two types of internal connection implant system (stepped and tapered type) by means of 3D finite element analysis, The finite element model was designed with the parallel placement of the one fixtures ($4.0mm{\times}11.5mm$) with reverse buttress thread on the mandibular 1st molar. Two models were loaded with 200 N magnitude in the vertical direction on the central position of the crown, the 1.5 mm and 3 mm buccal offset point from the central position of the fixture. The oblique load was applied at the angle of $30^{\circ}$ on the crown surface. Von Mises stress value was recorded and compared in the fixture-bone interface in the bucco-lingual dimension. The results were as follows; 1. The loading conditions of two internal connection implant systems (stepped and tapered type) were the main factor affecting the equivalent bone strain, followed by the type of internal connections. 2. The stepped model had more mechanical stability with the reduced max. stress compared to $11^{\circ}$ tapered models under the distributed oblique loading. 3. The more the contact of implant-abutment interface to the inner wall of implant fixture, the less stress concentration was reduced.

Sinus floor elevation and implant-supported fixed dental prosthesis in the posterior area, with full-digital system: a case report (완전 디지털 시스템을 이용한 상악동 거상술 및 구치부 임플란트 고정성 보철 수복 증례)

  • A variety of digital technologies are being used throughout the entire implant treatment process of diagnosis, surgery, impression, design, and fabrication of prostheses. In this case, using a digital surgical guide, sinus floor elevation was performed without complications, and the implants were placed in the planned position. After the healing period for osseointegration, CAD-CAM (Computer-aided design-Computer-aided manufacturing) customized abutments and provisional prostheses were delivered. While using the provisional prosthesis, occlusal change was observed. To transfer the intermaxillary relationship and abutment position that reflect occlusal change and axial displacement, double scanning and abutment-level digital impressions were taken. Abutment superimposition was used to capture the subgingival margin without gingival retraction. Then, the definitive prosthesis was designed and fabricated with digital system. We report a case applying digital system, to achieve the predictable result as well as the efficient treatment process from implant surgery to fabricating prosthesis in the posterior area.

Evaluation of the stability of sandblasted, large-grit, acid-etched implants with tapered straight body design (테이퍼드 직선형 SLA 임플란트의 안정성 평가)

  • Purpose: Implant surface modification and implant design are the principle targets for achieving successful primary stability. The aim of this study was to measure implant stability quotient (ISQ) values of sandblasted, large-grit, acid-etched (SLA) implants with tapered straight body design during the healing period, and to determine the various factors affecting implant stability. Materials and Methods: To measure implant stability, resonance frequency analysis (RFA) was performed in 26 patients (13 women and 13 men) with 44 SLA implants with tapered straight body design. Implant stability (ISQ values) was evaluated at baseline and healing abutment connection (12 weeks), and the correlations between RFA and insertion torque (IT), bone quality, and jawbone were determined. Results: The mean ISQ value of the implants was $69.4{\pm}10.2$ at the time of implant placement (baseline) and $81.4{\pm}6.9$ at the time of healing abutment connection (P < 0.05). Significant differences were found between RFA and bone quality and between RFA and jawbone (P < 0.05). No significant differences were found between RFA and IT, insertion area, fixture diameter, and implant length (P > 0.05). Conclusion: ISQ values of SLA implants with tapered straight body design were high at baseline and healing abutment connection. It was concluded that SLA implants with tapered straight body design show improved primary and secondary stability, and that immediate or early loading may be applicable.

Evaluating usability of and satisfaction with two types of dental CAD software (두 종류 치과 임플란트 캐드 소프트웨어의 사용자 편의성 및 만족도 비교)

  • Purpose: This study evaluated the usability of and satisfaction with two types of computer-aided design (CAD) software among users who had experience with dental implant CAD software and those who did not. Materials and Methods: Dental technicians (n = 20) who had previous experience with dental implant CAD Software and students from the College of Dentistry (n = 12) who had never designed implant custom abutments were asked to evaluate two types of CAD Software, Exocad and Deltanine. In addition, the participants were asked to fill out a structured questionnaire (Section 1: Entering basic information and retrieving files; Section 2: Setting conditions before abutment design; Section 3: Setting abutment design; and Section 4: Overall satisfaction). For the statistical analysis of the collected data, Mann-Whitney U test was used (${\alpha}=.05$). Results: The ease of design and satisfaction with the implant CAD Software, evaluated with respect to 21 statements divided into four Stages, were significantly higher for Exocad in both groups for Secion 1. For Sections 2 and 3, participants with experience evaluated Deltanine to be significantly better. For Section 4, both groups evaluated Exocad Software to be better. Conclusion: Overall, the Exocad Software was evaluated as having better usability and offering greater satisfaction. However, in terms of performance in the core of the design process, i.e. Sections 2 and 3, Deltanine was rated higher by the experienced users. Thus, if the user interface design parts are supplemented, Deltanine CAD Software could be put to a wider use in clinics.

Influence of the Dental Implant Abutment Screw Coating Materials on Joint Stability (임플란트 지대주나사 코팅이 결합안정성에 미치는 영향)

  • The aim of this study was to evaluate effect of implant abutment screw coating treatment on joint stability, investigating mechanical properties of these. For this study used $ExFeel^{(R)}$ external hexed implant system and $15mm{\times}1mm$ discs. Experimental group was $1{\mu}m$ TiN, TiCN, TiC coated abutment screws and discs. To know mechanical property, i evaluated adhesion strength, surface hardness, using disc, corrosion test using screw. The results were as follows : rotation angle of coated screws increased than that of non-coated screw because of lower friction coefficient, especially TiC coated screw group had the largest value, but removal torque decreased in all coated screws (p<0.05). Torque loss before and after fatigue test was the smallest in TiC-coated screws, and the largest in non-coated screws (p<0.05), and there was no statistically significant difference between dry condition and wet condition of screws because of higher surface hardness and lower friction coefficient. From the above results, TiN, TiCN, TiC coating group had high abrasion resistance, especially TiC coated group which had low torque-consuming, high rotation angle as low friction coefficient will be considered to influence on implant abutment screw joint stability positively.

Implant-assisted removable partial denture using freely removable abutment in a fully edentulous patient: A case report (완전 무치악 환자에서 자유 착탈가능 임플란트 지대주를 이용한 임플란트 융합 국소의치 수복증례)

  • Implant-Assisted Removable Partial Dentures (IARPDs) treatment is being performed in a fully edentulous patient using implant surveyed prosthesis as an abutment. Implant-supported prosthesis as an abutment of IARPDs is classified into screw-retained and cement-retained type according to the retention type, and each has advantages and disadvantages. The EZ crown system (Samwon DMP, Yangsan, Korea) has a cylinder combined with abutment, and the nickel-titanium spring in this cylinder provides a constant force on the zirconia ball to obtain retention in EZ crown system. In this patient, the natural abutment teeth of the mandibular overdenture was hopeless. We planned implant assisted removable partial denture using anterior implant surveyed prosthesis considering functional and esthetical rehabilitation, cost and patient's needs. When fabricating IARPDs using implant as abutment, we could compensate for the shortcomings of existing implant-supported prosthesis retention type and made the design of removable partial denture easy due to using EZ crown system.
