Purpose: This study compared the marginal bone loss around dental implant that were placed in the canine areas of the mandibles and finded the survival rate of implants, marginal bone loss around implants and prosthetic complications in 10 patients treated with overdentures retained with Locator attachments. Materials and methods: Ten patients who had received implant retained overdentures in the mandibules using two implants and Locator attachments at Daegu Catholic University Medical Center from 2004 to 2010 were included in this study. Evaluations of the survival rate of implants, marginal bone loss and prosthetic complications were performed. Results: Implants placed in this study showed a 100% survival rate and the average annual bone loss was 1.03 mm ${\pm}$ 0.20 mm in the first year. The patients have verbally indicated that they are comfortable and that their overdentures function well. But, implant retained overdentures had various prosthetic complications such as male change, relining, rebasing and denture fracture. Conclusion: Implant retained overdentures using two implant and Locator attachments in the fully edentulous mandibles is considered as a stable way for long terms because of minimal invasive operation, simple prosthetic process and easy repair.

Implants placed in this study showed a 95% survival rate and the average marginal bone loss was 1.21 mm($SD{\pm}0.60mm$) in 94.3 weeks of functional loading. Implant-retained overdentures had various prosthetic complications such as male change, metal cap loosening, Locator attachment loosening, denture teeth change, relining and denture fracture. Implant-retained overdentures using two implants and Locator attachments in the edentulous mandibles is considered as a stable way for long terms because of minimal invasive operation, simple prosthetic process and easy repair.

Treatment options for edentulous patients are complete denture and implant prosthesis. A two implant-retained overdenture can be considered the first treatment in the edentulous mandible, but there is no clear consensus of treatment for edentulous maxilla. Implant-retention/support overdenture shows better retention and stability than complete denture and is less expensive and more esthetic than implant-supported fixed prosthesis. CM $LOC^{(R)}$ $Pekkton^{(R)}$ attachment is a solitary type attachment and evaluated to have excellent abrasion resistance and retention with a female part made of poly-ether-ketone-ketone. Meanwhile, SR Ivocap system is injection molding method and discussed to show few changes in the vertical dimension of denture and have excellent fracture resistance. In this case, we restored maxillary arch with a four implant-retained overdenture using CM $LOC^{(R)}$ $Pekkton^{(R)}$ and SR Ivocap system, and mandibular arch with a removable partial denture. Through this procedure, satisfactory outcomes were achieved both in functional and esthetic aspects.

Several types of attachments have been used for implant supported and/or retained overdentures. Locator is one of the stud type attachments and it has been generally used. However, the colored matrix is resilient and vulnerable to wear, so frequent post-insertion maintenance is needed. To solve this problem, it is necessary to introduce innovative attachment system. Overdentures using Air Gap attachment (AGA) has improved masticatory function and facial esthetics. AGA is made of metal, so it could be more resistant to wear or friction than the other resilient attachments. Nevertheless, AGA plays a role in stress breakers because it allows movement during denture movement with predetermined gap. In addition, both pre-existing implants and natural teeth were successfully used for connection of AGA. AGA could replace other unsplinted type of attachments. Overdentures using AGA could provide satisfactory result in terms of function, esthetics and retention. However, long term follow up is needed.

There are various methods for restoring the dentition of completely edentulous patients. Removable complete dentures have the advantage of being relatively economical, but they can be uncomfortable to wear. With the introduction of implant prosthodontics, various options such as implant-supported overdentures and hybrid prostheses have become available. If there is inadequate remaining ridge or limited financial resources, an overdenture supported by a few implants with additional attachments may be more suitable. In this case, due to severe peri-implantitis and other complications, the implants were removed. Subsequently, four implants (two on each side) were placed in the maxilla and a milled-bar with attachment was fabricated for each side of the maxilla.

Long-term alveolar bone resorption in edentulous patient causes difficulty in denture use. Applying an implant overdenture with 2 to 4 implants to edentulous patient is easily approachable. Moreover, it improves denture stability, support, and retention. Milled bar, the attachment used in implant overdenture, can be used to induce better stability and retention to the supporting structure than conventional bar. It has become convenient to use due to the development of CAD/CAM system which had allowed the simplification of dental techniques. In this case, application of conventional maxillary full denture and mandibular overdenture made of CAD/CAM milled bar with 4 implants showed satisfactory results in the patient who had used upper and lower full dentures for a long time.

Compared with traditional full dentures, mandibular implant overdentures have the advantage in that good support and retention can be obtained even with two implants. When manufacturing a mandibular implant overdenture using two implants, it is important to place the implant in the correct position. The long-term prognosis of overdenture is good when two implants are placed in the direction vertical to the occlusal plane and parallel to each other at the canine position. However, it is difficult to place two implants in the correct position in edentulous patients, and if you use surgical guides in these cases, you can get help in placing the implants in a prosthetically advantageous position. This case, a 57-year-old male patient, came to our hospital with all upper and lower teeth removed due to periodontal disease. Therefore, the maxilla was restored with a traditional full denture and the mandible was restored with an implant overdenture using two implants, which resulted in satisfactory functional and esthetic results.

Implant overdenture exhibits higher retention, masticatory function, and patient's satisfaction compared to conventional dentures. Particularly, in treatment of severe alveolar bone resorption, implant overdenture can be considered as the first treatment option. The types of attachments used for implant overdenture can be classified into solitary type, which implants are not connected to each other, and bar type in which implants are connected. In the case of solitary type commonly used in clinical practice, parallel relationship is important. When it is not established, there is a higher risk of attachment wear, retention loss, and frequent visits for maintenance. In this case, satisfactory results have been obtained with implant overdentures using milled bar on two unparallel implants placed on the mandible.

Loss of tooth results in remodeling and resorption of surrounding alveolar bone which causes atrophic edentulous ridge and gradually decreasing gingival attachment. As a result, edentulous patients face difficulty in using dentures due to pain, decrease of support, decline of masticatory efficiency of complete denture. To improve this, overdenture with implant in the mandible and attachment are considered as a treatment of choice as a favorable treatment. In this case, a patient with edentulous ridge for long period is rehabilitated by complete denture in maxilla and implant overdenture using Locator$^{(R)}$ attachment in mandible.

Periodontal and prosthodontic maintenance of implant overdenture is a very important factor for the long term success of the prosthesis and implants. Failure of maintenance can lead to prosthetic failure due to peri-implant bone loss and fracture and wear of the prosthesis or components. In this case, the existing gold milled bars were reconstructed with cobalt chrome milled bar in a manner that does not interfere with the external shape for the retreatment of fractured implant overdenture by maintenance failure. Two implants of mandible were selected strategically and the CM LOC attachments were connected to the two implants, and implant overdentures were fabricated. As a result, prosthesis with a functional and esthetic design that facilitates good hygiene management of the patient was delivered, which is advantageous for long term maintenance, and regular check-ups were scheduled for proper maintenance.
