Complete denture rehabilitation in the edentulous patient with severe mandibular bone resorption and vascular malformation using closed mouth impression and monoplane occlusion: A case report (심한 하악골 흡수와 혈관기형을 보이는 무치악 환자에서 폐구인상법과 단일평면교합법을 이용한 총의치 수복 증례)

  • A patient failed to use conventional complete dentures due to severe mandibular bone resorption and soft tissue hyperplasia on lips, tongue and oral vestibule from vascular malformation. The treatment of choice would be an implant-retained overdenture, however, he refused owing to high cost. The closed mouth impression technique using BPS principle, CR record registration with gothic arch tracing and monoplane occlusion using non-anatomic teeth were used to fabricate complete dentures for this patient. Patient satisfaction, the retention and the stability of complete dentures were highly improved as a result.

A retrospective randomized study of success rates according to abutment selection in DENTIS submerged implant with an internal hex connection (DENTIS 내부연결형 서브머지드 임플란트에서 지대주 선택에 따른 성공률의 후향적 연구)

  • PURPOSE. The purpose of this study is to determine the efficacy of the DENTIS submerged-type implant with an internal hex connection and to build corresponding abutment-selection criteria. MATERIAL AND METHODS. A total of 204 patients received submerged implant fixtures with an internal hex connection at the Dong-A University Hospital Dental clinic in Busan from January 2013 and May 2016. Three specific abutments, UCLA abutments, customized abutments, ready-made abutments, were randomly selected. Implant success was defined as the basis of the International Congress of Oral Implantologists(ICOI, 2007) criteria. The relationship between the implant success rate and the abutment factor was analyzed using the Kruskal-Wallis test(P<.05). RESULTS. A total of 508 implants were placed in 204 patients. After a mean observation period of 38.6 months, 493 out of 508 implants were in normal function, yielding an overall success rate of 97.05%. A total of 15 implants failed: 10 in the maxillary molar area, 4 in the mandibular molar area, and 1 in the mandibular incisal area. All of the implant failures occurred in a single-implant prosthesis, especially high in the maxillary molar area. The Kruskal-Wallis analysis showed that abutment selection has no significant correlation with implant failure(P>.05). CONCLUSION. DENTIS submerged implants with an internal hex connection showed predictable results with a success rate of 97.05%. It is no influence on the success rate in the selection of submerged implant abutment with an internal hex connection.

Comparison of patient satisfaction with digital and conventional impression for prosthodontic treatment (보철 치료 시 디지털 및 전통적 인상채득에 대한 환자 만족도 비교 연구)

  • Purpose: The present study aims at researching the subjective satisfaction of patients who have experienced both conventional impression taking and digital impression taking to measure the possibility of wide clinical application of digital impression. Materials and methods: The study surveyed 170 adult patients over the age of 20, between October 2015 and April 2016, who voluntarily consented to participation and who experienced both conventional impression and digital impression at five dental hospitals that use intraoral digital impression. A total of 128 surveys were used for data analysis, involving frequency analysis, multiple response frequency analysis, descriptive statistics, and contingency table analysis, with the significance level set at 0.05. Results: Responses on the reason for taking impressions using the digital method appeared in the order of 'for implant treatment' (43.8%), 'for crown treatment' (30.5%), and 'for inlay treatment' (15.6%). Patients satisfaction was higher for digital impression taking than conventional impression taking (P<.05). As the preferred choice of impression, digital impression (60.2%) was higher than conventional impression (11.7%). Responses on the reason for choosing digital impression taking appeared in the order of 'no vomiting reflex' (35.1%), 'reliability of 3D digital scanning' (33.8%), and 'short time' (33.8%). Conclusion: The patients preferred digital impression taking to conventional impression taking in terms of satisfaction.

Comparison of the accuracy of digital impressions and traditional impressions: Systematic review (디지털 인상법과 전통적 인상법의 정확도 비교: 체계적 고찰)

  • Purpose: This study systematically examines literatures on the suitability of prostheses and accuracy of obtained impressions to see if digital impressions using intraoral scanners can replace traditional impressions. Materials and methods: A MEDLINE/PubMed search and manual search was performed for studies written in English about accuracy of digital impression published in dental journals from August 1, 1997, to July 31, 2017. Depending on criteria, the data for the selected articles were independently organized into standardized spreadsheets by 2 reviewers. Results: Among the total 35 studies met the inclusion criteria, there were 26 studies comparing the suitability of prostheses, and 9 studies comparing the accuracy of impressions through scan data without prostheses. Most studies used prostheses to compare the accuracy of impression techniques. Conclusion: This review suggests that making single crown or mesio-distally short prostheses with digital impressions is clinically reliable in natural teeth. However, there is still a limit to making mesio-distally long prostheses with digital impressions from the lack of related studies. Digital impression cannot fully replace traditional impressions in implant prostheses yet.

Comparison of Shear Bond Strength of Veneer Ceramics to Co-Cr Alloys Produced by Selective Laser Melting and Casting Technique (선택적 레이저 용융 그리고 전통적인 주조 기술에 의해 제조된 Co-Cr 합금에 대한 전장용 세라믹의 전단 결합 강도 비교)

  • Selective laser melting (SLM) manufactures an alloy using laser as a heat source, and has recently been introduced in the dental industry. However, there is a lack of analytical research on metal-ceramic restorations achieved by SLM. This study evaluates and compares the metal-ceramic bond strength of Co-Cr alloys produced by selective laser melting and casting methods. Co-Cr samples required for this study were produced through the sintering process of ceramics, by applying the SLM and CAST methods. The metal-ceramic bond strength was measured by applying the shear bond strength test. In order to determine the area fraction of adherent ceramic, Si content of the specimen was measured using scanning electron microscopy SEM/ EDS. Results of the metal-ceramic bond strength and AFAC were analyzed by t-test (α = 0.05). No significant difference was observed comparing the bond strength of SLM and CAST Co-Cr alloys (P> 0.05). However, the SLM group had much better ceramic adherence than the CAST group (P < 0.001). Moreover, oxidation characteristics were similar for both SLM and CAST Co-Cr alloys, but metal structures were different. These results imply that although the bond of ceramic and Co-Cr alloy is not related to the manufacturing method, SLM alloys impart better ceramic adherence. This indicates that alloys made with SLM can be used to fabricate upper implant prostheses in the future. In particular, it is expected to overcome the shortcomings of the CAST method, and save time and cost.

  • Purpose: This study was to evaluate the accuracy of how to use the Mechanical Torque-Limiting Devices. Materials and methods: Three different implant manufacturers (Dentium, Osstem, Shinhung) were prepared for each two implant torque controllers. Divided into two groups depending on the method used for removal torque was measured. Repeated measures of ANOVA test (${\alpha}=.05$) was used as statistics to evaluate the effect of repeated loading number on the removal torque. Independent t-test was used to evaluate the difference in removal torque of two groups. Results: The removal torque significantly decreased as the number of loading repetition increased (P<.05). There was significant difference between two groups. Conclusion: Loosen the handle part of the implant torque controllers spring resilience to recover one group compared to the group that did not, showed a little more closed to the reference value.

A Study on Optimal Shape of Stent by Finite Element Analysis (유한요소 해석을 이용한 스텐트 최적형상 설계)

  • Stents are widely used as the most common method of treating coronary artery disease with implants in the form of a metal mesh. The blood flow is normalized by inserting a stent into the narrowed or clogged areas of the human body. In this study, the mechanical characteristics of a stent are investigated according to the variations of its design parameters by the Taguchi method and finite element analysis. A stent model of the Palmaz-Schatz type was used for the analysis. In the analysis, an elasto-plastic material model was adopted for the stent and a hyper-elastic model was used for the balloon. The main interest of this study is to investigate the effects of the design parameters which reduce the possibility of restenosis by adjusting the recoil amount. A Taguchi orthogonal array was constructed on the model of the stent. The thickness and length and angle of the slot were selected as the design parameters. The amounts of radial recoil and longitudinal recoil were calculated by finite element analysis. The statistical analysis using the Taguchi method showed that optimizing the shape of the stent could reduce the possibility of restenosis. The optimized shape showed improvements of recoil in the radial and longitudinal directions of ~1% and ~0.1%, respectively, compared to the default model.

  • Complete denture prosthodontics includes replacement of natural teeth and associated maxillary and mandibular structures for full edentulous patients. The final objectives of prosthodontic treatment include esthetic and functional rehabilitation. Because the esthetic demand of senior patients has increased esthetic requirement should not be overlooked in denture treatment. In complete denture treatment, attempts have been made to reproduce tooth wear, arrangement, recession of cervical gingiva, staining, improved gingival shade and so on. This 61 year old female patient came to the Department of Prosthodontics with the chief complaint that dentures made 5 years ago in a local clinic were broken and did not fit well. The patient put more emphasis on fabrication of natural and esthetic dentures than recovery of masticatory function. Remake of maxillary complete denture and mandibular implant retained overdenture was planned. In order to create esthetic and natural contour, Aesthetic Color Set Easy system$^{(R)}$ (Candulor dental GmbH, Wangen, Germany) was used to make complete dentures reproducing various gingival shades. Patient's esthetic satisfaction was high. When complete denture prosthodontics treatment involves appropriate expression of individual characteristics in addition to proper selection of esthetic materials, treatment will give satisfaction to both dentist and patient.

A Study on the Retentive Force of Stud Attachments for Implant Overdenture (임플란트 피개의치를 위한 개별유지형 어태치먼트의 유지력에 관한 연구)

  • The purpose of this study was to compare the changes of retentive force in different stud attachment systems for implant retained overdenture. Two commercially available attachments with different retentive forces were investigated: Kerator (pink, blue, red) and O-ring (orange, red). Two implant fixtures were vertically embedded in base mountings. Five pairs of each attachment were evaluated. A universal testing machine was used to evaluate the retentive force of two attachments during wear simulation. Surface characteristics of each attachment system were evaluated with scanning electron microscopy. Five pairs of each attachment were evaluated. Kerator pink showed the highest initial retention. After 2,500 cycles of wear-simulation, Kerator pink noted the largest decrease in retention. According to results of surface analysis, Worn surfaces were obtained in matrices and patrices. Heavy wears were observed in matrices. After 2 year-wear simulation, most attachments exhibited retention loss. Attachments using different kind of material exhibited dissimilar surface alterations.

Dental institution selection factors for implant surgery among young adults aged 20-64 years (임플란트 수술환자의 치과의료기관 선택요인: 청장년층 20~64세를 대상으로)

  • Background: This study was aimed at investigating factors influencing the selection of dental institutions for implant surgery among young adults (age: 20-64 years) and identifying differences in these factors according to general characteristics. Methods: We conducted an internet survey for approximately 2 months, from April to July 2022, and analyzed data from a total of 128 people. Dental institution selection factors included three items of convenience of transportation, five items of physical environment, three items of image of institution, five items of image of dental staff, five items of dental service, and four items of basic dental elements. The importance score for each item was investigated on a five-point scale. Results: The importance score for each dental institution selection factor was the highest for dental service (4.42 points), followed by basic dental element (4.00 points), physical environment (3.89 points), image of institution (3.81 points), convenience of transportation (3.76 points), and image of dental staff (2.78 points). The importance score for each item was the highest for dentists' technique, followed by cleanliness, reliability, and dentists' attitude. Women had higher average scores for all factors compared to men, with statistical significance in scores for convenience of transportation, physical environment, image of institution, dental service, and basic dental elements (p< 0.05). Conclusion: To attract patients seeking implants to a dental institution, attention should be paid to the patients' institution selection factors. In addition, the environment of a dental institution should be created in such a way that it favors patients' selection factors.
