Nursing Protocols of Cold Application in Different Medical Institutions in Korea (전국 의료기관의 냉 요법 실무지침 및 임상간호사의 냉 요법 실무 현황)

  • Purpose: The purpose of this study was to evaluate nursing protocols for superficial cryotherapy in different medical institutions. Methods: The study was conducted with a cross-sectional descriptive design. The medical institutions including general hospitals, hospitals, and geriatric hospitals were randomly selected. A total of 435 nurses from 126 institutions completed a questionnaire. Data were collected from December 2014 to June 2015. Results: Forty-two institutions (39.5%) had nursing protocol for cryotherapy. The nurses reported that durations of cold application were 2 minutes to 5 hours. Frequently used cold therapy devices in order of frequency were frozen gel packs, ice packs, and frozen IV fluid bags. There were variances in the duration of cold application according to the types of institution ($x^2=7.78$, p=.020) and nursing units ($x^2=26.42$, p<.001). In addition, intervals of cold application were different according to the nursing units (x=12.23, p=.032). There were differences in cold application instruments by regional groups (x=70.38, p<.001). Most of the nurses (95.6%) responded that national nursing protocol for superficial cryotherapy were needed. Conclusion: There were difficulties in providing consistent nursing interventions because of the practical differences and absence of evidence-based guidelines for cryotherapy. The researchers recommend that basic studies with various instruments be conducted and proper nursing protocols be developed for cryotherapy.

Topics for Evidence-Based Clinical Nursing Practice Guidelines in Korea (국내 근거기반 임상간호실무지침의 주제 선정)

  • Purpose: This study was to identify topics for evidence-based clinical nursing practice guidelines in Korea. Methods: Data were collected from 330 staff nurses from 10 general hospitals and 53 nurses in charge of nursing education in 110 hospitals with over 500 beds. Using open questions, the nurses identified activities which could not be verified, which lacked consistency among nurses, clinical units and/or hospitals, which were not based on the up-to-date knowledge and which needed reform. The data were analysed by content analysis using a qualitative methodology. Results: Collected data consisted of 1882 clinical topics, which were classified into 50 topics, 207 mid-categories, and 456 sub-categories. The most frequent topics in order of frequency were medications, central line management, intravenous injections, urinary catheterization, perioperative nursing care, skin tests, pressure ulcer care, blood transfusions, laboratory examination-culture, respiratory care which were performed routinely in clinical setting by staff nurses. Conclusion: The research findings indicate the urgent need to develop evidence-based clinical nursing practice guidelines related to these research findings. Further research is needed to identify topics related to health promotion, and symptom/management of health problem.

Influences of Nursing Professional, Self-Leadership and Caring-character on Nursing Practice Readiness among Nursing Students (간호대학생의 전문직관, 셀프리더십, 돌봄인성이 실무준비도에 미치는 영향)

  • This study was to increase the practical preparation of nursing students by identifying how their professionalism, self-leadership, and caring character affect their practical preparation. This study targeted 182 fourth-year students with at least one year of clinical practice experience attending nursing departments at four universities in two regions from April 10 to May 12, 2023. Collected data were analyzed by descriptive statistics, independent t-test, one-way ANOVA, Scheffe's test, Pearson's correlation coefficients, and multiple regression analysis using SPSS/WIN 23.0 program. As a result of this study, the factor affecting the practical preparation of nursing students was found to be caring personality (β=0.72, p<0.001), the explanatory power of the variables was 51.3%(F=189.41, p<0.001). The results showed that caring personality had an effect on practical readiness. Therefore, we believe that it is necessary to develop various programs related to caring character in order to increase the practical preparation of nursing students.

The role type analysis of nursing leader for effective environment at the practice field that nursing students recognize - Q methodology - (간호 대학생이 인식하는 실무현장의 효과적 환경을 위한 간호리더자의 역할유형분석)

  • Nursing students do a variety of learning at the practice field during clinical practice. They can recognize the importance and effectiveness of nursing leader. The purpose of this study was to categorize the types by analyzing what are important aspects of nursing leader's role to make effective practice environment nursing students experienced from training course. This research was used Q methodology to understand how nursing students recognize deeply. Participants were involved 30 nursing students who have experienced nursing practices. Q samples included 36 statements gathered from interviews with students who experienced clinical practices and literatures regarding types of nursing leader's role. Furthermore, major factors were analyzed and categorized by using QUANL PC program. As a result of research, there are 4 types of nursing leader's role. The first type is "Maintaining justice and level-headed", the second type is "Creating environment for motivation", the third is "Respecting character and growing capacity up" and the fourth is "Relationship center through confidence". Through the research process, this study is about to make evidences by understanding role of nursing leader for effective environment of practice field.

Validity and Reliability of the Korean Version of Student Evidence-Based Practice Questionnairs(S-EBPQ) (한국어판 간호대학생의 근거기반실무 측정도구(S-EBPQ)의 타당도 및 신뢰도 검증)

  • This study aimed to evaluate the reliability and validity of the Korean version of Student Evidence-Based Practice Questionnaire (S-EBPQ) which developed by Upton, Scurlock-Evans, and Upton. Data were collected from 209 nursing students with previous experience of education in evidence-based practice and clinical placement. Using SPSS 25.0, construct validity, concurrent validity and reliability were evaluated. The Korean version of S-EBPQ consisted of 21 items in four domains. The measurement explained 67.0% and the reliability using Cronbach's alpha was .927. The Korean version of S-EBPQ can be used to evaluate the ability of evidence-based practice among nursing students. The tool could be also used to examine the effectiveness of educational programs for evidence-based practice which would contribute to improving the quality of nursing education in Korea.

Updates of Evidence-Based Nursing Practice Guidelines for Central Venous Infusion Therapy (근거기반 중심정맥 주입요법 간호실무지침 개정)

  • Purpose: This study was conducted to update nursing practice guidelines for intravenous infusion published in 2017. Methods: The guideline update process was carried out using 22 steps developed by NICE and SIGN. It was agreed to update domains related to central venous infusion therapy. Contents related to peripheral infusion would be updated later. Results: Updated guidelines for central venous infusion therapy consisted of 6 domains and 195 recommendations. The number of recommendations by domain was 11 for general instruction, 14 for central vascular access devices (CVAD) and add-on devices, 13 for nursing management before insertion of CVAD, 30 for management during insertion of CVAD, 51 for management after insertion of CVAD, and 76 for complications. A grade was 29 (14.9%), B grade was 87 (44.6%), and C grade was 79 (40.5%) in the strength of recommendations. A total of 37 (19.0%) recommendations were newly developed and 23 (12.3%) previous recommendations have been modified. The newly developed recommendations were mainly related to the infection control methods. Conclusion: The updated guideline is focused on safe maintenance of central venous infusion therapy. Through this guideline, it is hoped to minimize the occurrence of complications and improve the standardization and efficiency of nursing practice.

The Effect of Readiness to Self-Directed Learning on Nursing Practice Competence (간호사의 학습 관련 자기주도성이 간호실무 수행능력에 미치는 영향)

  • Purpose: The purpose of this study is to investigate nurses readiness to self-directed learning and its correlations with nursing practice competence in a tertiary hospital in Seoul, Korea. Methods: A cross sectional survey design was utilized to assess nurses readiness to self-directed learning and job performance. The data used in this study were obtained from 286 nurses who have been working on general ward for over 1 year. Data were analyzed by ANOVA, t-test, Pearson's correlation analysis, and multiple regression using SAS 9.1 program. Results: The mean score of the readiness to self-directed learning is 3.56 and that of nursing practice competence is 2.71. The readiness to self-directed learning was statistically different according to level of education, and clinical work experience. Nursing practice competence was also significantly different according to level of education, clinical work experience, marital status, age, and working division. The readiness to self-directed learning and nursing practice competence seem to have significant positive correlation to each other (r=.555, p<.001). The readiness to self-directed learning explains 32.0% of nursing practice competence (F=20.20, p<.001). Conclusion: These findings indicate that program development for enhancing the readiness to self-directed learning is needed to improve nursing practice competence. We also need continuous efforts to improve nurses initiatives and creativity.

Change in the Nursing Professionalism, Role Conflict and Job Satisfaction of the Nursing Students before and after their first Clinical Experience (첫 임상실습이 간호대학생의 전문직관, 역할갈등, 직무만족에 미치는 영향)

  • This study was attempted to provide nurse students with basic materials to improve their clinical practices, and to investigate changes in their views on Nursing Professionalism, Job satisfaction, and role conflicts. After experiencing clinical practices, their nursing professionalism(t=-2.184, p=.031) and job satisfaction (t=-2.068, p>.001) increased but role conflict (t=-3.729, p>.001) increased as well. They were satisfied with their jobs, building up nursing professionalism through clinical practices. However, they had also acquired time on thinking about their future jobs more deeply than before practices and realizing their roles in real sense. In particular, they felt stress in some of subcategories, personal inner conflicts (t=-3.729, p>.001), role obscurity (t=-5.720, p>.001) so it is necessary to review standards on their job and task in nurse education and clinical practices.

  • The research about nursing students' experience on the clinical environment observation is essential to developed effective strategy for clinical education and to improve quality for clinical practice of nursing education. This study is aimed to above goal by the report about nursing students' experience on emergency room environmental observation. The number of participants is 30 nursing students who are senior in the practicing at emergence room. Data analysis was centered on scrutinizing the contents of the participants' replies in order to deduce any relevant meaning. Also, data were analyzed by the meaning of learning based on self-examination through the practical learning. The positive and negative topics stated. Nursing students' experience on emergency room observation will supply the thinking about their own self-portrait in near future, and will give self reflection about correct nursing interventions to nursing students.

Effects of Nursing Practice Environment and Positive Psychological Capital in Clinical Nurses on Turnover Intention (임상간호사의 간호근무환경과 긍정심리자본이 이직의도에 미치는 효과)

  • The purpose of this study was to examine influences on turnover intention in the clinical nurses. The data were collected from 193 nurses working in the three general hospital located Y city. The data were analyzed by descriptive statistics, T-test, ANOVA, Pearson's correlation coefficient, and stepwise multiple regression. The result of the stepwise multiple regression indicates the nursing practice environment, positive psychological capital and clinical career predict 41.8% (F=7.35, p<.001) of turnover intention. Therefore, it is necessary to develop changing the nursing practice environment and positive psychological capital strategy program for decrease turnover intention in clinical nurses.
