The paper presents a new scheme and a support system for the analysis sof hyman errors in nuclear power plants based on a cognitive model. We discusse the problems identified in current managerial analysis, and propose a new approach that frames the description of human activities according to a human decision making modle, so that it could provide a better reconstruction of a sequence of event suspected of involving human errors. This sophistcated approach becomes practical for the field application with the support of a computerized aiding system. The model-based event re-construction method is expected to enable the analysts to produce more informative reports, which in turn heop to derive appropriate counter- measures to reduce the possibility of the analyzed human errors.
Gori-I nuclear power plant has been examining the effectiveness and efficiency of the current emergency operation procedures from human factors viewpoint. Previous study showed that some mistakes that the procedures did not comply with the writers' guidelines. Reviewing the current writers' guidelines for emergency operating procedures revealed that they lack of some important human factors rules such as enumeration of switching conditions and detailed action requirements, definite expression for setup points, description for anticipated results, and recommendation for use of present tense, affirmative sentence and active voice. This study suggested a new classification system for the writers' guideline contents supplementing the deficiencies of the current emergency operation procedure text.
Railway system which has latent loss of lives and property by big accident with that human error such as locomotive driver, manager, signaller, and the others involved. So human error management is needed to control this complex system and to confirm safety of it. Rail human error research for managing human resource has grown rapidly in both quantity and quality of output over the past few years. The continual influences of safety concerns, new technical system opportunities, reorganization of the business, needs to increase effective, reliable and safe use of capacity, and increased society, media and government interest have now accelerated rail human factors research programmes in several countries. The objective of this research is to improve safety and to reduce accidents in korean railway system, through the application of research results to the investigation of requirement for human error.
Almost all companies have paid much attention to the safety management ranging from maintenance to operation even at the stage of designing in order to prevent accidents, but fatal accidents continue to increase throughout the world. In particular, it is essential to systematically prevent such fatal accidents as fire, explosion or leakage of toxic gas at factories in order to not only protect the workers and neighbors but also prevent economic losses and environmental pollution. In addition, HRA may be used to detect the human errors which may cause accidents or trace back to any mistake on the part of workers. Usually, HRA technique is used in association with other risk assessment techniques. Moreover, it can serve to enumerate the human errors which may occur during operation or down-time or correct the existing system to reduce the mistakes. This work focuses on the coincidence of human error and mechanical failure for management of human error, and on some important performance shaping factors to propose a method for improving safety effectively of the process industries.
Although the up-to-date(high) technique is developing and automating in many fields, the center of the all fields is closely connected with the human resource. Also, the railway driver is required to have both high technique and high level of mental work. Therefore, this research will grasp the types of the railway driver groups through railway driver's task stress analyze(JCQ and Job Content Questionnaire), and then analyze the human errors which the types can occur. Finally, this research will propose the way to reduce the human errors.
It is recommended that reasonable and well-organized standards of the locomotive's requalification system, as it stands, should be established through comparing with foreign of the country for verifying it's propriety. The purpose of this study is to develop the process of locomotive's requalification system for reducing human errors which has been founded as important cause in railway accidents and safety. Therefore, some effective alternatives suggested in this study may be used as basic data for supporting the Korean railway safety law.
Human reliability attempts to make precise quantitative analyses and predictions of the performance of human-machine(or product) systems. In order to yield more precise human error analysis, precise human error probabilities(HEPs) must be used in the analysis. However, because human behavior is influenced by factors that are called performance shaping factors(PSFs), the effects of PSFs must be considered to obtain precise HEPs, These are called basic HEPs or situation-specific HEPs. This paper presents a theoretical method for obtaining basic HEPs (i.e. , considering PSFs) in quantitative human error analysis. In this method, the weight which characterizes the degree of importance of several PSFs is obtained by the analytic hierarchy process. The quality scores of PSFs in the task situation are obtained by percentile concept. These scores are used in conjunction with the relative Importance weights of PSFs to compute the composite quality percentile score of PSFs in the task situation. Then, a new mapping method of the composite quality percentile score of PSFs into a situation-specific basic HEP is proposed with a numerical example.

The purpose of this study was to develop safety competences, a set of behavioral indicators of each competence and measuring methods of behavioral indicators for preventing human error of nuclear power plants(NPPs). The safety competences and behavioral indicators were derived from the five steps consisted of derivation of preliminary competence items through literature review, content analysis, interview(FGI, BEI), examination of content validity and decision making of final indicators. The results showed that 13 core safety competences and 35 behavior indicators were derived finally. In addition, the methods of measuring safety competences or behavioral indicators such as Behaviorally Anchored Rating Scale (BARS), Behavior Observation Scale (BOS) were developed and suggested.

Most of studies on aircraft accidents have been conducted mainly for fixed-wing aircraft, while the studies on helicopter accidents have been done less even though the helicopter accidents occurred quite more than those of the fixed-wing. There are lots of differences between helicopter and fixed-wing accidents, in aspect of causes and occurrence of accidents as well as aerodynamics and operation. In Korea, helicopter accidents have occurred 2 or 3 times annually since 2000, while the number of fixed-wing aircraft accidents has been reduced considerably. The goal of this study is to solve the present safety problems in helicopter accidents by reviewing the characteristics of past accidents and comparing differences between two types of aircraft.

반응형 실시간 시스템의 특성을 가진 원자력 발전소 제어시스템은 안정성이 크게 중요시되며, 소프트웨어의 오류가 많은 인적 물질적 피해를 줄 수 있는 시스템이므로 높은 신뢰도를 가지고 있는 소프트웨어가 요구된다. 높은 신뢰도를 뒷받침하기 위해 내장되는 소프트웨어는 적합한 정형명세기법과 정형검증기법의 적용이 필요하다. 소프트웨어 개발시 적용되는 정형기법은 명세의 모호성을 제거하고 검증을 수행하기에 용이하게 하므로, 높은 신뢰도를 요구하는 반응형 실시간 시스템인 원자력 분야의 소프트웨어 개발에 유용하다. 그러므로, 본 논문에서는 반응형 실시간 시스템의 특성을 가진 원자력분야의 소프트웨어 개발시 유용하게 적용할 수 있는 정형기법을 기반으로 한 소프트웨어 개발 방법론을 제시하고자 한다.
