A Study on the Perceived Language and Non-verbal Communication and Lapport and Customer Satisfaction of Hospital Users : Focused on the Control Effect of Service Authenticity (병원이용객이 지각한 언어·비언어커뮤니케이션과 라포, 고객만족에 관한 연구 : 서비스진정성 조절효과를 중심으로)

  • This study is a descriptive survey using a self-reported survey method to understand the effect of language and non-verbal communication of medical service providers on customer satisfaction through Lapport and to determine the degree of impact on Lapport by adjusting perceived service authenticity. The data collection period was from April 5 to April 30, 2021. Using the convenience sampling method, data were collected from users of medical institutions located in Busan and Gyeongsangnam-do, and a total of 306 valid questionnaires were used as statistical analysis data. The collected data were analyzed using the IBM SPSS statistics version 25.0 and AMOS 20.0 programs through a coding process. In the results of this study, it was found that the language and non-verbal communication of medical service providers influenced customer satisfaction through Lapport, and the perceived service authenticity in the relationship between each variable acts as a moderating effect. Considering that the language and non-verbal factors of the medical service provider are important in the interaction between the medical service provider and the hospital user, the medical service provider should take full account of the language and non-verbal factors and help the medical institution communicate with the hospital user.

Health Behavior Associated with Outpatient Utilization (외래서비스 이용과 건강행태)

  • Objectives: It attempted to analyze influencing factors on the utilization of outpatient services which were adopted to predisposing, enabling, and need factors in Anderson model. Methods: The current study analyzed "2007 Korean National Health Nutrition Survey" data, which selected 3,335 people nationwide by proportional systematic sampling. This study analyzed data of persons who used outpatient services in two weeks. It adopted Anderson Model to control contextual factors including socioeconomic factors. The study compared means and fitted logistic regression models and multilevel model. Results: The logistic regression model showed that persons purchased private medical insurance were less likely to use outpatient services than the persons did not purchase private medical insurance. Persons with hypertension and diabetes mellitus, overweight, and problem drinkers were more likely to use outpatient services. Persons with high school graduates or higher in education level and experience of accidents or intoxications were more likely to use outpatient services according to the multilevel analysis of mixed model which treated region as random effect. Conclusion: Higher level of perceived stress increased the probability to use outpatient service than lower level of perceived stress. As number of days a person had exercised increased, the probability to use outpatient service decreased. Overweight and problem alcohol drinking increased the probability of outpatient service use. Further research should be conducted to find more factors influencing outpatient service use.

Sex Differentials in the Utilization of Medical Services by Marital Status (배우자유무에 따른 남녀간 의료서비스 이용의 차이)

  • It has been well known that women live longer but suffer more from degenerative diseases than men do, which, in turn, results in frequent utilization of medical care services among women. Previous studies, mostly based on Western society, have discussed that women, particularly married women, are more likely to utilize medical care services than men even after considerations of different health status and demographic and socioeconomic characteristics. Social interests in women's health and socialization process of caring for health among women are known as causes. This study examines sex differentials in the medical care service utilization in Korean society, particularly focusing on marital status. Feministic perspectives in Korea have argued that sex division of labor within family puts women in a position that has to continuously provide physical and emotional services to other family members, which keeps them from taking care of their own health. This study empirically tests if this feministic perspective holds true in Korea. Results show that Korean women and those with spouse are more likely to utilize medical care services than their men and the spouse-less counterparts, net of other risk factors. However the effect of existence of spouse on the medical care utilization is much smaller among women than men. These results suggest that the feministic perspective is in part applicable to explain sex differentials on medical care service utilization in Korea.

Development of a convergence inpatient medical service patient experience management model using data mining (데이터마이닝을 이용한 융복합 입원 의료서비스 환자경험 관리모형 개발)

  • The purpose of this study is to develop a convergence inpatient medical service patient experience management model(IMSPEMM) that can help in the management strategy of a medical institution to create a patient-centered medical culture. Using the original data from the 2018 Medical Service Experience Survey, 593 people with medical services inpatient(MSI) over the age of 15 were analyzed. By using the decision tree model, we developed a prediction model for overall satisfaction(OS) with the inpatient medical service experience(IMSE) and the intention to recommend patient experience(RI), and were classified into 4 and 7 types. The accuracy of the model was 68.9% and 78.3%. The OS level of IMSE was the nurse area and the hospital room noise management area, and the RI decision factor was the nurse area. It is significant that the IMSPEMM for MSI was presented and confirmed that the nurse area and the noise management area of the hospital room are important factors for the inpatient experience. It is considered that further research is needed to generalize the IMSPEMM.

우리나라 보건의료 정보체계와 정보화사업의 현황 및 문제

  • 국가 보건의료체계(health care system)가 올바르게 기능하기 위해서는 의료체계를 구성하는 각 요소들간의 유기적인 상호 작용이 대단히 중요하다. 정보체계는 이러한 유기적인 상호 작용을 위해 보건의료체계의 각 영역에서 꼭 필요한 시스템이다. 보건의료 서비스를 효율적으로 생산하기 위해서 투입하는 자원의 개발과 배치 체계는 물론 생산된 보건의료 서비스를 소비자가 효과적으로 이용할 수 있도록 하는 서비스 전달체계(delivery system)의 전과정에서 보건의료정보체계는 중요한 인프라가 되고 있다. 정보체계는 궁극적으로 보건의료체계가 지향하는 목적을 달성할 수 있도록 짜여져야 한다. 다시 말해, 의료체계의 각 하부영역에서 의료이용의 접근성(accessibility), 형평성(equity), 서비스의 의과학적 질(scientific technical quality), 포괄성(comprehensiveness) 등 보건의료체계가 추구하는 가치를 정보체계에서도 그대로 담아 낼 수 있어야 한다. 또한 정보체계를 구축하기 위한 요건으로 각 사업 영역별 표준화, 사업 영역간의 상호관계, 공공과 민간의 역할 분담, 정보 관련 시선과 인력, 정보기술 등의 인프라, 각 사업 영역별 정보화에서 담아야 할 정보 내용(contents, DB) 등에 대한 사전 기획이 수립되어야 한다. 그 동안 보건의료분야의 정보화사업은 1996년부터 보건소기관의 행정, 진료, 보건 업무지원을 위한 보건소정보화사업과, 1995년부터 초고속공공응용서비스 사업의 일환으로 추진해 온 전염병, 혈액유통, 장기이식, 외래진료예약, 원격진료, 응급의료, 원격치매진료 시스템 구축사업 등이 있으며, 응급의료, 혈액관리, 장기이식, 전염병정보, 외래진료예약시스템을 통합한 보건의료정보통합서비스시스템(1998년) 구축사업 등이 있었다. 이러한 사업들에 대해 현재 평가되고 있는 문제점들로는 정보화 사업에 대한 전략과 비젼이 부재한 상태에서 지나치게 정보기술 중심으로 추진되다 보니 보건의료 정보의 공유나 활용이 전혀 이루어지지 않거나 매우 제한적이었고, 시범사업의 경우는 초기 사업을 추진하기 위한 주체가 명확하지 않았고, 관련 인력의 훈련과 양성이 안 되었으며, 사업 확대를 위한 법적 기반의 취약, 정보화 사업이 부실한 데 대한 평가 기준과 대안 제시 등이 매우 취약한 것이 문제점으로 나타났다. 특히, 정보 내용과 DB 구축을 위한 표준화 등에 대한 사전 설계 부재는 개발된 정보시스템의 운영을 결정적으로 어렵게 만든 요인이 되고 있다. 전반적으로 기존의 정보화사업은 사업 추진을 위해서 꼭 필요한 비젼 수립, 비젼에 따른 목표 설정, 전략 개발, 관리체계 구축, 운영방안 수립 등 단계적인 정보화 계획이 부재한 것으로 평가되고 있어 보건의료정보체계 구축을 위한 종합 비젼 기획(ISP)이 시급한 실정이다.

The Effect of Perceived Medical Services Quality by Outpatient on the Hospital Image, Satisfaction and Re-use Intention (외래환자가 지각한 의료 서비스의 질이 병원 이미지, 만족도 및 재이용 의도에 미치는 영향)

  • The purpose of this study is to investigate the effect of perceived medical service quality by outpatient on hospital image, satisfaction, and re-use intention in a general hospital. A total of 176 questionnaires were administered to outpatients who presented to a S general hospital in Daejeon. The results of this study are as follows. First, medical service quality had significant effects on re-use intention, satisfaction, and hospital image. Second, the mediating effect of satisfaction on the relationship between medical service quality and re-use intention was not significant but the mediating effect of hospital image on the relationship was significant. Third, satisfaction and hospital image had significant effects on re-use intention. Consequently, this study verified the influence of perceived medical service quality by outpatient on hospital image, satisfaction, and re-use intention in a general hospital. Further, considering that hospital image, rather than satisfaction, mediates the relationship between perceived medical service quality and re-use intention, it suggests that much more general satisfaction is required in the marketing activities of hospitals.

Development of Authentication Service Model Based Context-Awareness for Accessing Patient's Medical Information (환자 의료정보 접근을 위한 상황인식 기반의 인증서비스 모델 개발)

  • With the recent establishment of a ubiquitous-based medical and healthcare environment, the medical information system for obtaining situation information from various sensors is increasing. In the medical information system environment based on context-awareness, the patient situation can be determined as normal or emergency using situational information. In addition, medical staff can easily access patient information after simple user authentication using ID and Password through applications on smart devices. However, these services of authentication and patient information access are staff-oriented systems and do not fully consider the ubiquitous-based healthcare information system environment. In this paper, we present a authentication service model based context-awareness system for providing situational information-driven authentication services to users who access medical information, and implemented proposed system. The authentication service model based context-awareness system is a service that recognizes patient situations through sensors and the authentication and authorization of medical staff proceed differently according to patient situations. It was implemented using wearables, biometric data measurement modules, camera sensors, etc. to configure various situational information measurement environments. If the patient situation was emergency situation, the medical information server sent an emergency message to the smart device of the medical staff, and the medical staff that received the emergency message tried to authenticate using the application of the smart device to access the patient information. Once all authentication was completed, medical staff will be given access to high-level medical information and can even checked patient medical information that could not be seen under normal situation. The authentication service model based context-awareness system not only fully considered the ubiquitous medical information system environment, but also enhanced patient-centered systematic security and access transparency.

Factors Affecting the Satisfaction for Medical Service and Reuse Intention of Patients at Dental Clinic in Gyeongnam Province (경남 일부지역 치과의원 내원 환자들의 치과 의료서비스 질 만족도와 재이용 의사에 미치는 영향 요인)

  • This study was conducted to provide with baseline data with the purpose of increasing the values of medical services. Self-administered questionnaire survey was conducted on 236 patients at a dental clinic follow-up visit in dentist clinic Gyeongnam area from June 2013 to August 2013. All statistical analyses were performed using SPSS. The motivation visiting the dental clinic is that the first one is introduction from the family and friends, the second is accessibility, the third is conspicuity and the last one is awareness of the dentist. The main variables in the process of treatment are service system, kindness, satisfaction of service, efficient of re-call system. The relief of discomfort at revisit show the highest score in the process of implant treatment and intention of revisit hereafter do in the prostheses process. In the correlation between main variables, service system and relief of discomfort at revisit (r=0.440, p<0.001), kindness and satisfaction of medical service (r=0.675, p<0.001), revisit hereafter and satisfaction of service (r=0.387, p<0.001) and efficiency of re-call system and revisit showed the highest correlation. The influence on satisfaction of dental service show meaningful level in kindness (p<0.001) and efficiency of re-call system (p<0.05). The intention of revisit is affected meaningfully by relief of uncomfort (p<0.05), service system (p<0.05), kindness (p<0.01) and efficiency of re-call system (p<0.01). In summary, the personal network of patients is most important variable at intention for revisit of dental clinic. As satisfaction of kindness and efficiency of re-call system is higher, satisfaction of medical service and intention for revisit are shown higher. Therefore further research for improvement of satisfaction for medical service and of intention of revisit at the dental clinic should be carried out.

Factors influencing quality of health care: Based on the Korea health panel data (한국의료패널 자료를 활용한 의료서비스 질 영향 요인)

  • The purpose of this study was to identify factors affecting quality of health care. Methods: The data were derived from the 2011-2013 Korea health panel survey (beta version 1.0). The data were analyzed using SPSS 21.0 with descriptive statistics, ${\chi}^2$-test, and multiple logistic regression analysis. In general characteristics, common factors influencing the quality of health care were age, marital status, education level, and subjective health status. In variables related to health care utilization, unmet healthcare needs, and limitation of dental care utilization were the significant factors affecting quality of health care. The results of this study show that various factors influence quality of health care. These findings can be used to develop strategies to improve health care.

The Effect of Doctor's Payment Method on Patient's Medical Care Use: Revisit of the Patient's Asymmetric Information Problem (환자의 의료이용에 대한 의사의 지불방식의 효과: 재방문 환자의 비대칭적 정보의 문제)

  • Although the patient's problem with access to health information has been improved due to rapidly developing information technologies, such as the internet, some patients still do not have enough ability to understand, interpret, and analyze the health information. Given this view on the patient's asymmetric information problem, if a doctor provides sufficient effort to help patients understand and interpret medical information, the efficiency of patient's medical care use could be improved. This paper shows firstly that the patient's inefficient use of medical care originates from his information problems, such as the misperception of the effectiveness of medical care and secondly suggests that if the doctor makes sufficient effort to correct patient's information problems, the inefficiency can be ameliorated. This paper also suggests the manipulation of a doctor's payment method can lead a doctor to provide optimal level of efforts which can in turn lead patients to use the optimal level of medical care. With an optimal level of effort, a doctor can more easily achieve a patient's compliance with the newly recommended amount of medical care.
