On rationale for government intervention is the failure of competition in the market. Health care markets are characterized by such unique aspects as information asymmetry, prevalence of insurance, and cost-increasing competition based on the adoption of costly medical technology. Therefore, government policy to guarantee a sufficient number of providers in markets may not lead to socially beneficisal outcomes such as higher quantity and lower price. This paper examines the unique nature of health services and its implications for competition, the evidence that competition may not reduce health care ex[enditures, and policy tools that government can use to encourage competition which contributes to supporting a sustainable health care system.

단순 병원 전산화 수준이었던 의료 정보 서비스는 지능화된 의료 기기를 통하여 언제 어디서든 의료 서비스를 제공받을 수 있는 u-헬스케어 서비스 형태로 진화하고 있다. 특히, 홈-네트워크 기술의 성숙과 고도화된 유무선 의료기기의 발달은 홈-헬스케어 서비스의 확대를 촉진시키고 있다. 그러나 홈-헬스케어 서비스는 거주자의 생명이나 안전과 매우 밀접할 뿐 아니라 주로 매우 개인적인 정보를 다룬다는 점에서 정보보호 문제에 매우 민감하다. 따라서 본 고에서는 신뢰성과 안전성을 보장하는 홈-헬스케어 서비스를 제공하기 위한 정보보호 요구사항 및 이슈들을 살펴보고 아울러 이와 관련된 대안 기술들을 구체적으로 살펴본다.

의료기기는 사람의 생명과 직접적으로 연관되어 있기 때문에 다른 분야의 기기보다 안전성에 대한 검증이 필수적이다. 의료 분야에서는 안전성 검증을 위해 기기의 허가 심사 조건으로서 소수의 피험자를 대상으로 수행되는 임상 시험이 존재한다. 그러나 임상 시험의 경우 의료기기를 직접 사람에게 적용하여 검증을 진행하기 때문에, 인체에 미칠 위해성을 고려하여 전임상 시험을 수행하고 있다. 하지만 전임상 시험은 동물이나 가상의 물체를 대상으로 수행하여 실제 사람에 대한 적용이 아니기 때문에, 임상 시험에 비해 검증에 대한 효력을 갖지 못한다. 따라서 본 연구에서는 피험자의 안전을 보장할 수 있고, 임상 빅데이터에 축적된 실제 환자의 사례를 활용한 신뢰성 있는 검증 방안을 제안하고자 한다. 그러나 현재 식품의약품안전처에서 제공되고 있는 의료기기 품목군은 개발하고자 하는 의료기기의 임상 빅데이터 기반 검증 가능성을 식별하기 어렵다. 그러므로 본 논문에서는 의료기기에 대한 다양한 특성 분석을 통해 임상 빅데이터 기반 검증 가능성을 식별하기 위한 프로세스를 제안한다. 제안하는 프로세스에서는 의료기기의 검증에 요구되는 데이터의 식별을 통해 임상 빅데이터를 이용한 테스트 데이터 수집 및 이를 활용한 신뢰성 높은 검증을 가능케 한다.

2014년 2월 정보통신 진흥 및 융합 활성화 등에 관한 특별법(ICT 특별법)이 시행되었고, 2014년 6월 의료법개정에 따른 원격의료서비스 시범사업이 가능해졌다. 본 연구에서는 기존의 병원을 방문하여 진료 후에 약 처방을 받던 일반적인 내시경 의료서비스와 병원 방문 없이 전화를 이용한 진료 및 처방과 함께 약을 배송해 주는 원격의료서비스를 의료서비스 제공자들을 대상으로 FGI(Focus Group Interview)를 통하여 비교하였다. 본 연구는 향후 보다 나은 내시경환자를 위한 원격의료서비스 설계를 위한 기초자료로 활용될 것으로 기대한다.

The telemedicine system provide an efficient means to monitor the status of health and measure bio-information even though the limitation of distance and time. The telemedicine system can be divided into two categories. The first one is Remote Support Service which can monitor the patient including home environments and give those diagnosis information to medical office. and the other one is Alarm Service that can be used for tile emergency status. For the convenience of the patient we will design and implement portable healthcare device and the telemedicine system under the wireless ZigBee network environments.

With the paradigm shifts towards consumer-centered health service, it is expected that more health care consumers will keep their health information and manage their own health in the future. Thus, this study was conducted to compare "Understanding", "Utilization" and "Management" of Personal Health Record(PHR) between medical users(healthcare students) and health care providers(medical students). We collected data from 208 health and medical students via using self-reported questionnaires form April to June, 2011. The collected data were analyzed using frequency, t-test, Chi-square on SPSS 19.0 version. There was no significant difference in "Understanding" of PHR between two groups. Looking at the order of the importance of PHR contents, two groups equally emphasized medical records, surgical history, and test results. There was significant difference in both time and effectiveness of PHR(p=0.02). Intergrated type of PHR was preferred by both groups. Recently, PHR reflects needs and demands of users more than ever. However there are many limitations to promote the utilization. In the future, it is necessary to implement targeted strategies for the elderly groups and specific types of disease.

Under the COVID-19 pandemic, the importance of Official Development Assistance(ODA) in health and medical service is emphasized more. The number of cases is expected to increase in the future. Therefore, it is necessary to provide differentiated education and training for each type of occupation as a follow-up to realize the operations management and maintenance of medical equipment supported by the ODA project. In this study, for an effective follow-up and operations management of medical equipment supported by the ODA project, the differences in educational training achievement between users and maintainers were analyzed through a t-test. As a result of this study, there were significant differences between users and maintainers of medical equipment in use and manage six types of priority management target medical equipment(Q5) and an instruction manual understanding of medical equipment(Q6). However, there was no difference in the need and purpose of education and training, satisfaction with an instructor, and the need for cross-professional communication.

The present study was carried out to evaluate the impacts of the qualities of dental medical services on service value, consumer satisfaction and consumer loyalty for 221 patients who visited 17 dental hospitals or clinics located in Busan and Ulsan using questionnaires and obtained the following conclusion. By gender, males had higher perceptions of para-language than females did in the qualities of the dental medical services, and by the level of education, college students and university graduates had higher perceptions of body language and physical appearance. Scores of service value, consumer satisfaction and customer loyalty were higher in males, college students and university graduates, and married patients, but those were not statistically significant. It turned out that the qualities of the dental medical services were correlated with service value, consumer satisfaction and consumer loyalty. As the perception of para-linguistic and verbal communication in the qualities of the medical services increased, service value increased; as the perception of verbal communication and physical appearance increased, consumer satisfaction increased; and as the perception of verbal communication, physical appearance and body language increased, consumer loyalty increased. To sum up the above findings, the qualities of dental medical services influence service value, consumer satisfaction and consumer loyalty, so in the future, it will be necessary to study quality dental medical services with which patients are satisfied.

This study is the quasi-experimental research of nonequivalent groups and is designed to measure and compare the awareness and knowledge of 2 groups of undergraduate nursing students - one group who received the education of biomedical ethics and brain-death organ donation and the other group who did not receive the education - regarding biomedical ethics and brain-death organ donation. We conducted this research for the 76 undergraduate nursing students of a certain university located in "C" city. Collected data were analyzed by Fischer's exact test and t-test, ANCOVA using SPSS 23.0 program. As result of the research, it was found that there was significant difference in the knowledge of brain-death organ donation between the above 2 groups (t=35.21, p=.000). In this regard, the education of biomedical ethics should be included in the curriculum of undergraduate nursing students in consideration of factors influencing the awareness of biomedical ethics.

The purpose of this study is to find out the differences in between national and private medical institutions' perception towards aging, and to analyze how their different perceptions effect each of their later life preparation. A total of 783 questionnaires were distributed among national and private medical institution workers. Frequency analysis, t-test, hierarchical regression analysis was conducted from the collected data via SPSS 21.0. The results were the following. Firstly, there was a partial difference in their perception of aging, government policy satisfaction and later life preparation. National medical institutions were better than private medical institutions in all three terms. Also, perception of aging and government policy satisfaction were higher-than-average in both institutions, but in later life preparation, both scored lower-than-average. Secondly, the factors affecting preparation for aging were significantly different between these two institutions. Also, how each factors affected later life preparation was partially different. In conclusion, effects of factor was difference in aging preparation between national and private medical institutions workers. We need to provide as basic data for education and policy making for aging preparation.
