Effect of Environmental Change in Radiography Room and Psychological Impact on Young Patients (심리적 안정을 고려한 촬영실 내부 환경변화가 검사에 미치는 영향; 유아 방사선검사의 불안감 해소 중심으로)

  • The attempt to enhance the environment of hospitals has increased recently to minimize young patients' anxiety about tests and inspections. This gives more satisfaction with the medical service to kids and young patients. The department of radiology endeavors to improve the conditions of existing radiography rooms to help young patients psychologically feel relaxed. This facilitates the process of inspections. This paper examines the relationship between the environment of radiography rooms and its effect on young patients' state of mind. 94 patients at the age of five were observed before and after the improvement of environment of the hospital. Positive results about the psychological state of the young patients were shown after the change of the environment. The result of this paper gives an idea that the modification of hospital environment has a critical importance to the young patients' psychological state. By this conclusion it may help to improve the quality of the medical service.

The Design and Implementation of Telemedicine System for Emergency Patients in a Ship (해상응급환자를 위한 원격진료 시스템의 설계 및 구현)

  • Though there were many research and development about telemedicine on land recently, not yet applied to that on the shore. In this paper, when emergency clinic situation were generated in a ship on shore, the telemedicine system was designed and implemented for transfer and clinic service to doctors on the land by measuring basic body signals of patients. Presently, wireless communication and inmarsat telephone are usually used by simple questions and clinic consults to remote doctors when emergency situation were happened in the ship. In this paper, the telemedicine system on the shore were developed for improving this problems by measuring patient's fundamental conditions such as the blood pressure, pulse, the respiratory condition, electrocardiogram, body temperature, patient image and sending these information to remote doctors on land for more accurate prescription. The developed system can supply the high level clinic service to emergency patients on the shore and cope with the emergency situation in ship.

A study on the effect of expected benefits and perceived risks on intention to use untact medical diagnosis and consultation services

  • The purpose of this study is to explain intention to use untact medical diagnosis and consultation services. We carried out the analysis of the survey data using Smart PLS 3.0 to test the hypotheses how the expected benefit variables and perceived risk variables of untact medical diagnosis and consultation services affect intention to use. According to the empirical analysis results, this study confirmed that quality of telemedicine service had a significant effects on perceived usefulness, Perceived Easy of Use. And accessibility had a significant effects on perceived easy of use, cost saving and expected benefits had a significant effects on use Intention of untact medical diagnosis and consultation services. Performance risk and service risk had a significant effect on medical risk. And medical risk had a significant negative(-) effects on use Intention of untact medical diagnosis and consultation services. This study has its meaning because it found out that it deals structurally and expansively with use intention of untact medical diagnosis and consultation services through positive and negative factors.

Study on Dental Care need of the Rural Residents (일부 농촌지역주민의 치과의료요구도에 관한 연구)

  • This study was conducted to analyze the dental care need and related factors of the rural residents. An interview and questionnaire survey was carried out for 524 people who lived in Seongju-gun county of Gyeongsangbukdo, from July 28 to August 8, 2002. The summarized results are as follows: 1. The rate of persons who experienced the oral disease was 52.5 per 100 persons during 1 year and it was highest in the age group of 40-49. Therefore the rate of persons who had experienced the oral disease was significantly higher the younger peoples, worse oral health status and being of the regular treatment source than the other groups. 2. Dental care user was 52.5 per 100 persons during 1 year 3. Yes or no of The dental care need was significant to age, the subjective oral health status, necessity of regular oral health examination 4. Perceived dental need of the Rural Residents was supplement 64%, dental caries 30.0%, peridontal disease 21.0%, extraction 15.7%, sensitive tooth 7.9%, and so on 1.9%.

Changes in the Behavior of Healthcare Organizations Following the Introduction of Drug Utilization Review Evaluation Indicators in the Healthcare Quality Evaluation Grant Initiative (의료질평가지원금 제도의 의약품안전사용서비스 평가지표 도입에 따른 의료기관의 행태 변화)

  • Background: This study aimed to determine the effectiveness of drug utilization review (DUR) evaluation indicators on safe drug use by comparing the changes in DUR inspection rates and drug duplication prescription prevention rates between the pre- and post-implementation of the DUR evaluation indicators of the Healthcare Quality Evaluation Grant Initiative. Methods: This study used DUR data from the Health Insurance Review and Assessment Service in 2018 (pre-implementation) and the evaluation results of the Healthcare Quality Evaluation Grant Initiative in 2023 (post-implementation). The dependent variables were the DUR evaluation indicators, including DUR inspection rate and drug duplicate prescription prevention rate. The independent variable was the implementation of the DUR evaluation indicators, and the control variables included medical institution characteristics such as type, establishment classification, location, DUR billing software company, and number of beds. Results: The results of the analysis of the difference in the prevention rate of drug duplicate prescriptions between the pre- and post-implementation of the DUR evaluation indicators of the Healthcare Quality Evaluation Grant Initiative showed that the prevention rate of drug duplicate prescriptions increased statistically significantly after the implementation of the DUR evaluation indicators. Conclusion: The policy implications of this study are as follows: First, ongoing evaluation of DUR systems is needed. Second, it is necessary to establish a collaborative partnership between healthcare organizations that utilize DUR system information and the organizations that manage it.

A Strategy for the Improvement of Health Care Inequality in the Southwestern region of Korea - Around the island area (서남권 의료불평등 개선을 위한 방안 -도서 지역을 중심으로-)

  • The purpose of this study was to identify the degree of medical inequality in medical vulnerable areas, especially in the southwestern islands, and to prepare improvements. As a research method, 14 pieces were analyzed by systematic literature review with keywords such as 'medical vulnerability', 'medical inequality', and 'island area', and a focus group or in-depth interview (FGI) was conducted on 9 medical personnel in the public medical delivery system to identify the current status and demand. As a result of the study, medical inequality in the southwest region, especially in island areas, was confirmed, and the lack of professional manpower and administrative support system were confirmed through FGI. As a result of the study, it was confirmed that realistic measures should be prepared to increase the efficiency of public health care as well as active administrative support to improve the vulnerability of island areas.

  • The recent medical treatment guidelines and the development of information technology make hospitals reduce the expense in surrounding environment and it requires improving the quality of medical treatment of the hospital. That is, with the new guidelines and technology, hospital business escapes simple fee calculation and insurance claim center. Moreover, MIS(Medical Information System), PACS(Picture Archiving and Communications System), OCS(Order Communicating System), EMR(Electronic Medical Record), DSS(Decision Support System) are also developing. Medical Information System is evolved toward integration of medical IT and situation si changing with increasing high speed in the ICT convergence. These changes and development of ubiquitous environment require fundamental change of medical information system. Mobile medical information system refers to construct wireless system of hospital which has constructed in existing environment. Through RFID development in existing system, anyone can log on easily to Internet whenever and wherever. RFID is one of the technologies for Automatic Identification and Data Capture(AIDC). It is the core technology to implement Automatic processing system. This paper provides a comprehensive basic review of RFID model in Korea and suggests the evolution direction for further advanced RFID application services. In addition, designed and implemented DB server's agent program and Client program of Mobile application that recognized RFID tag and patient data in the ubiquitous environments. This system implemented medical information system that performed patient data based EMR, HIS, PACS DB environments, and so reduced delay time of requisition, medical treatment, lab.

The Effect of Dental Hygienist Prosthetic Medical Service on Dental Satisfaction and Dental Loyalty (치과위생사의 보철 의료서비스가 치과 만족 및 애호도에 미치는 영향)

  • This study was conducted to clarify the role and importance of dental hygienists in improving patient satisfaction in dental medical institutions, and to improve the social position, working conditions, and environment of dental hygienists. This study evaluated the impact of dental hygienists on dental satisfaction, revisits, and doctor recommendations for patients who had received prosthodontic treatment at dentistry in the last 10 years. This study analyzed the data from 166 survey questionnaires from June 1, to July 2, 2020. For data analysis, the SPSS Ver 23.0 program was used to calculate frequency and percentage, mean and standard deviation of the variables, Pearson correlation analysis, and multiple regression analysis. As a result, the average quality of medical services provided by dental hygienists among prosthetics was 4.05, the average dental satisfaction was 4.27, the average revisit was 3.97, and the average recommendation was 4.01. As a result of Pearson's correlation analysis, there was a significant positive correlation between each variable (P<0.01). As a result of multiple regression analysis, the level of knowledge appeared as a factor affecting dental satisfaction (P<0.05). The factors influencing both the revisits and the intention to recommend were the level of knowledge and the level of education (P<0.05).

Determinants of Quality in Outpatient Medical Service (외래 의료서비스 질적 수준의 결정요소)

  • This study was conducted to evaluate the subjective ideas about the determinants of quality in ambulatory care unit among outpatients and medical staff of a university hospital, and to compare the differences of the ideas, between patients themselves and hospital staff. A self-administered questionnaire survey was conducted covering 799 outpatients and 190 hospital staff in March, 1998. The questionnaire included general characteristics and 26 determinants of ambulatory care quality. The following are summaries of the findings: 1. Both of outpatients and hospital staff perceived, "Physician's knowledge" as the most important determinant of medical care quality. 2. In respect of 7 determinants related to physician's knowledge and skill, both outpatients and hospital staff perceived "physician's knowledge and skill" as important determinants. The scores of determinants such as, "Not doing unnecessary examinations", and "Assignment of adequate number of patients and duty schedule for the physician" were significantly different between outpatients and hospital staff. 3. In respect of 4 determinants related to doctor-patient relationship, both outpatients and hospital staff perceived "attention to patient's complaints" as the most important determinant. The scores related to the determinants such as "kindness of physician" and "explanation of treatment outcome" were significantly different between outpatients and hospital staff. 4. Among the amenities related determinants, "Modern facilities and equipments" were perceived as the most important determinant in both group. 5. In respect of 8 determinants related to non-financial accommodation, outpatients perceived, "Waiting hours for treatment" as the most important determinant, and hospital staff perceived, "Kindness of hospital staff". 6. In respect of 4 determinants related to financial accommodation, outpatients perceived, "Fare account of medical cost" as the most important determinant, and hospital staff perceived, "Increasing reimbursements". Further comprehensive research should be made on the evaluation of perceptions of medical care quality, both of outpatient and inpatient care, among patients and hospital staff. So good quality in medical care will be achieved based on clients' needs.

Analysis of Hospital Services for Elderly Inpatients (노인 입원환자 병원서비스 분석)

  • Background : This study is to identify the inappropriate hospital services for elderly inpatients over 65 years in general hospital with acute care functioning. Consequently elderly inpatient care and the management of long-term care facilities are key issues for current government health policy. Method : The survey was conducted for two months for all inpatients over 65 in 7 general hospitals, 6 work sampling days randomly selected. In each survey day, the subjective judgement by medical staff on the degree of acute care needs and by nursing unit manager on hospital services of each inpatients was also conducted. Result : The total number of cases collected are 2,541 elderly inpatients, according to subjective judgements by medical staff on inpatient condition. However 46.8% of cases are turned out to be non-acute care group. The frequency of medical services provided to non-acute group are 2~3 vital sign checks per day 78.2%, IV injection 40.1%, antibiotics medication 20.2%. Conclusion : Lots of elderly patients' who are staving in acute hospitals, at present need to be transferred to long-term care facilities. However, there was been shortage of long-term care facilities. It is expected to identify the need of elderly inpatients and therefore, to provide cost-effective, appropriate and good quality health services to elderly inpatients depending on their needs.
