Detection of Hidden Proximal Caries using Q-ray view in Primary Molars (Q-ray view를 이용한 유구치의 숨은 인접면 우식증 탐지)

  • The purpose of this study was to evaluate the ability of Q-ray view (All-in-one Bio, Seoul, Korea) in detection of proximal caries in primary molars with sound marginal ridges. Thirty two children aged 3-9 years (average $5.6{\pm}1.3$ years old) were chosen, and two examiners evaluated 100 proximal surfaces of primary molars with sound marginal ridges. The teeth were examined with; (a) visual examination, (b) Q-ray view, (c) DIAGNOdent (KaVo, Biberach, Germany) and (d) digital periapical radiography. Kappa statistic was used to assess the agreement between each examination method and the degree of caries progression. The kappa values for enamel caries were 0.15 (visual examination), 0.10 (Q-ray view), 0.25 (DIAGNOdent) and 0.68 (digital periapical radiography). The kappa values for dentinal caries were 0.34 (visual examination), 0.56 (Q-ray view), 0.44 (DIAGNOdent) and 0.70 (digital periapical radiography). Although Q-ray view showed low diagnostic ability in detection of enamel caries, it was effective in detection of hidden proximal caries extended into dentin. Q-ray view would be a useful and simple device which could aid pediatric dentists in detection of hidden proximal caries in primary molars especially when examining uncooperative children or disabled persons.

SHEAR BOND STRENGTH OF "ONE-BOTTLE ADHESIVE" SYSTEM IN PRIMARY DENTIN. (One-Bottle 상아질 결합제의 유치 상아질에 대한 전단 결합 강도에 관한 연구)

  • To test the shear bond strength of a new "one-bottle adhesive" system to primary dentin two commercially available one-bottle adhesives (Prime & Bond NT, Single bond) and conventional three step system(Scotchbond Multi-Purpose Plus) were included for comparison. And We observe the interfacial morphology by scanning electron microscope. 90 primary molar teeth were embedded in acrylic and buccal and lingual surface were polished to 320 grit to create standardized dentin surface for testing. After bonding of composite resin to sample surfaces according to the manufacturer s direction and 1000 times thermocycling in dwell time 30 second, Shear bond strengths of adhesives to dentin were determined using universal testing machine and analyzed by ANOVA test. Another groups of specimens were treated by hydrochloric acid to secure the resin only and those tags were evaluated under SEM for their length and forms and the morphology of the bonding sites were also observed. The result are as follows. 1. Group I(Prime & Bond NT) showed higher shear bond strength than group iI(Single Bond) and III(Scotchbond Multi Purpose Plus) but no statistically significant difference was founded between groups(p>.05). 2. Relating long resin tags of $70-120{\mu}m$ were observed in samples of all groups under SEM. We could observed hybrid layer, resin tag and many lateral branches in every group. But, we observed in group III rare lateral branched than other two group and discontinuous hybrid layer.

MICRO-TENSILE BONDING STRENGTH OF REGIONAL PRIMARY MOLAR DENTIN (유구치 상아질의 각 부위에 적용된 수종의 복합레진 접착제의 미세인장접착강도에 관한 연구)

  • The purpose of this study was to evaluate the micro-tensile bond strength (${\mu}TBS$) of four luting resin to regional dentin of human primary teeth. Dentin from non-carious primary molars were prepared from different regions (s, superficial dentin; d, deep dentin; c, cervical dentin), and divided into groups based on anatomical locations and types of luting resins (Scotchbond Multi-purpose : SB ; One-Step : OS ; AdheSE Bond : ASE ; G-Bond : GB) : SB-s, SB-d, SB-c; OS-s, OS-d, OS-c; ASE-s, ASE-d, ASE-c ; GB-s, GB-d, GB-c. Luting resins were used according to the manufacturers' instructions, to bond $Light-Core^{TM}$ Core Build-Up Composite) to the exposed dentin specimens in the light-curing mode. After storage for 1 day, ${\mu}TBS$ was tested at a cross-head speed of 1 mm/min. Data were analyzed with T-test and two-way ANOVA. The bonding interface and fractography analyses were performed with SEM. The results were as follows : 1. ${\mu}TBS$ to superficial dentin was significantly higher than to deep dentin for SB(p<0.05). But there were no significant differences in regional ${\mu}TBS$ among OS, ASE, GB(p>0.05). 2. There were no significant differences in ${\mu}TBS$ to superficial dentin among each groups. But, in deep dentin, ${\mu}TBS$ of SB-d was significantly lower than those of OS-d, ASE-d, and GB-d(p<0.05). ${\mu}TBS$ of OS-d was significantly higher than those of GB-d(p<0.05), but there were no significant differences in ${\mu}TBS$ of ASEd. There were no significant differences among ${\mu}TBS$ of ASE-d, OS-d, and GB-d.

Detecting of Proximal Caries in Primary Molars using Pen-type QLF Device (펜-타입 QLF 장비의 임상적 유구치 인접면 우식 탐지 성능)

  • The purpose of this in vivo study was to assess the clinical screening performance of a quantitative light-induced fluorescence (QLF) device in detecting proximal caries in primary molars. Fluorescence loss, red autofluorescence and a simplified QLF score for proximal caries (QS-proximal) were evaluated for their validity in detecting proximal caries in primary molars compared to bitewing radiography. Three hundred and forty-four primary molar surfaces were included in the study. Carious lesions were scored according to lesion severity assessed by visual-tactile and radiographic examinations. The QLF images were analyzed for two quantitative parameters, fluorescence loss and red autofluorescence, as well as for QS-proximal. For both quantitative parameters and QS-proximal, the sensitivity, specificity and area under receiver operating curve (AUROC) were calculated as a function of the radiographic scoring index at enamel and dentin caries levels. Both quantitative parameters showed fair AUROC values for detecting dentine level caries (△F = 0.794, △R = 0.750). QS-proximal showed higher AUROC values (0.757 - 0.769) than that of visual-tactile scores (0.653) in detecting dentine level caries. The QLF device showed fair screening performance in detecting proximal caries in primary molars compared to bitewing radiography.

A STUDY ON THE BONDING OF COMPOMER TO DECIDUOUS DENTIN (컴포머와 유치 상아질의 결합에 관한 연구)

  • The purpose of this study was to evaluate the bonding of compomer to deciduous dentin which is known to have been developed to improve the weak properties of glass ionomer cement and composite resin. 120 sound primary molars were used for the shear bond strength test and another 24 for the scanning electron microscopic evaluation. Each material was ailed into polyethylene mold attached to exposed dentinal surface($3{\times}4mm$ in diameter) of sample blocks. Shearbond strength was measured using Universal testing machine and data were analyzed statistically with Oneway-ANOVA and Scheffe test. Scanning electron microscopic observation was performed in order to evaluate the pattern of distribution and penetration of resin tags and hybrid layer. Compomer groups(II-V) showed significantly higher bond strength values than glass ionomer group(I)(p<.05). Etching-compomer groups(III, V) showed the significantly higher bond strength than non-etching compomer groups(II, IV)(p<.05), but slightly lower values than composite resin group(VI) with no statistically significant difference(p>.05). No significantly different bond strength was found between compomer groups of different bonding system(p>.05). Scanning electron micrographs showed more irregular distribution of short and thin resin tags in non-etching compomer groups(II, IV) whereas the more regular and intimate distribution of long and thick tags in etching compomer groups(III, V) and composite resin group(VI). The evaluation of hybrid layer also showed more regular formation of thicker layer in etching compomer groups(III, V). Based on the results of present study, the use of compomer as an esthetic restorative material for primary molars might be justified.

EFFECT OF SELF-ETCHING PRIMER APPLICATION ON THE CHARACTERISTICS & STRENGTH OF DENTIN BONDING IN PRIMARY TOOTH (자가부식 프라이머의 처리가 유치의 상아질과 레진 간의 결합 형태와 강도에 미치는 효과)

  • The purpose of this in vitro study was to evaluate the efficacy of self-etching primer which was developed to simplify the bonding procedures by measuring the shear bond strength and observing the interfacial morphology. 90 flat dentinal surfaces were prepared by grinding the buccal and lingual areas of caries-free human deciduous molars. After bonding of composite resin to sample surfaces according to the manufacturer's direction and thermocycling, shear bond strengths were measured using Universal testing machine(Instron). Another groups of specimens were treated by hydrochloric acid to secure the resin only and those tags were evaluated under SEM for their length and forms and the morphology of the bonding sites were also observed. The result as follows. 1. Group III showed higher shear bond strength than group I and II but no statistically significant difference was founded between group I and II(p>.05). 2. Adhesive failure was predominant in group II whereas dentin detachment was the main failure pattern in group I and III. 3. Relating long resin tags of $100-200{\mu}m$ were observed in samples of all groups under SEM. In group I, homogeneously long resin tags were arranged rather tight whereas rather loosely arranged resin tags of various length were found in group II. Lateral branching of resin tags was the characteristic finding observed in group III.

A STUDY ON THE EFFECT OF DENTIN ADHESIVE TO FLUORIDE RELEASE OF COMPOMER (상아질 결합제가 컴포머의 불소유리에 미치는 영향에 관한 연구)

  • It is well known that glass-ionomer cement and compomer can release fluoride which can inhibit the progression of dental caries. The purposes of this study were to evaluate whether the fluoride from fluoride-containing filling materials can penetrate the dentin bonding adhesive and the dentin bonding adhesive can increase the bond strength of resin-modified glass ionomer. The amount of fluoride release from resin-modified glass ionomer and compomer was measured during 28 days of period and statistically analyzed by t-test. The bond strength of each material with/without dentin bonding adhesive was measured and also statistically analyzed by t-test. The distribution of fluoride from each material into the tooth was explored by electron probe microanalysis system(EPMA). The experimental teeth used were second primary molars about to exfoliate. The amount of fluoride release from each material was diminished by dentin bonding adhesive during 28 days(p<0.05) and the bond strength was not increased by dentin adhesive in resin-modified glass ionomer. The bond strength of group VI was not detectable. The distribution of fluoride from each material into teeth was according to dentino-enamel junction and dispersed into pulp from pulpal floor. The widest distribution was shown in resin-modified glass ionomer cement filled without the application of dentin bonding adhesive.

  • Ankylosis is defined as a fusion of cementum or dentine with alveolar bone. Due to the loss of the periodontal ligament on the ankylotic area, the tooth is incapable of continued eruption and hence is unable to follow the normal vertical development of the neighboring teeth and alveolar process. A 6-year-old female was referred to the Dept. of Pediatric Dentistry for ankylosis of primary molars and congenital missing of permanent premolars on both jaws. She had neither specific past medical history nor trauma and infection history in oral and maxillofacial region. Radiographic finding is that the maxillary primary molars were the early onset of ankylosis and had fast root resorption rate. However the mandibular primary molars were ankylosed later and being resorbed slower than maxillary primary molars. The object of treating this case is to maintain the proper alveolar bone growth and retention of deciduous molars. The point of managing this case is as follows: Proper treatment (observation, restoration, or extraction) should be established after thorough consideration of the time of onset, the root resorption rate, progression of infraocclusion and the development of alveolar bone support. We should consider the timing of extraction of the ankylosed teeth without problem of neighbouring alveolar bone growth and tilting of adjacent teeth in the view of growth spurt. Early diagnosis is important to avoid many of the complications with infraoccluded primary molars.

  • The aim of this study was to evaluate the effect of chemo-mechanical caries removal system($Carisolv^{TM}$, Medi Team, Sweden) for resin adhesion to carious primary and permanent dentin compared with conventional drilling method. The buccal surface of 92 primary molars and 92 permanent molars were used. Exposed dentins were occurred artificial caries. 32 tooth of primary molars and 32 tooth of permanent molars were prepared to observe treated dentin surface with $Carisolv^{TM}$ and conventional drilling method by SEM. Other tooth were prepared to measure resin-dentin shear bonding strength according to caries removal methods and dentin adhesive system. Two adhesive systems and a composite resin were used; single bonding agent(Scotchbond Multi-Purpose Plus, 3M) and self-etching bonding system(Prompt L-pop, 3M ESPE), and a composite resin (Z-250, 3M). The results were as follows : 1. The removal effect of carious dentin on $Carisolv^{TM}$ was stronger on the primary dentin than that to permanent dentin, and dentin surface became rougher with treated $Carisolv^{TM}$ than drilling method. 2. Acid-etched dentin surfaces were showed smoothening without smear layer. 3. In specimen applied single bonding system hybrid layer and adhesive layer were $2-4{\mu}m$ and $10-15{\mu}m$ in thickness, whereas self-etching bonding system were showed only thin hybrid layer($1-2{\mu}m$). 4. The shear bonding strength of group applied single bonding agent was higher than that applied self-etching priming system(P<0.05). 5. The shear bonding strength of group applied $Carisolv^{TM}$ and self-etching priming system were slightly higher than that applied conventional drilling method and self-etching priming system(P>0.05).

Shear Bonding Strength of Three Cements Luted on Pediatric Zirconia Crowns and Dentin of Primary Teeth (3종 시멘트로 접착한 소아용 기성 지르코니아 전장관과 유치 상아질의 전단결합강도)

  • The aim of this study was to evaluate the shear bond strength of three luting cements and to identify the effect of thermocycling. Zirconia discs were made similar to the inner surface of a preformed pediatric zirconia crown ($NuSmile^{(R)}$ ZR crown: ZRCr). The similarity between the zirconia discs and the inner surface of a ZRCr was confirmed by scanning electron microscope. Three luting cements were $Ketac^{TM}$ Cem Permanent Glass Ionomer Luting Cement (KGI), $RelyX^{TM}$ Luting Plus Cement (RLP), $RelyX^{TM}$ Unicem Self-Adhesive Universal Resin Cement (RUR). Three luting cements were bonded according to the manufacturer's instructions for 60 zirconia discs and 60 dentin of primary teeth. Total of 120 specimens were divided into two subgroups: One was not aged, and the other was tested with 5500 thermocycling. Shear bond strength was measured using a universal testing machine, and the fracture patterns were observed with SEM. On the zirconia discs and the dentin of primary teeth, shear bond strength of RUR was higher than that of KGI and RLP, and there were statistically significant differences by cement type. The shear bond strength differences for RUR were not statistically significant depending on thermocycling.
