Groyne to control the direction and velocity of flow in rivers is generally installed for the purpose of protecting riverbanks or embankments from erosion caused by running water. In particular, as interest in river restoration and natural river improvement increases, groynes are proposed as a key hydraulic structure for local flow control and riparian habitat establishment. Groynes are installed mainly in groups rather than as individual structures. In case of groynes installed as a group, flow around the groynes change according to spacing in between the groynes. Therefore, groyne spacing is regarded as the most important factor in groyne design. This study aimed at examining changes of flows around and within the area of groynes that take place according to the spacing of groynes installed in order to propose the optimal spacing for upward groynes. To examine flow characteristics around groynes, this study looked at flows in main flow area and recirculation flow area separately. In main flow area, it examined the impact of flow velocity increasing as a result of conveyance reduction that is exerted on river bed stability in relation to changes in the maximum flow velocity according to installation spacing. As a factor causing impacts on scouring and sedimentation within the area of groynes, recirculation flow in the groyne area can lead problems concerning flow within the area and stability of embankment. As for recirculation area, an analysis was conducted on the scale of rotational flow and the flow around embankment that exerts impacts on stability of the embankment. In addition, a comparative analysis was carried with reference to changes of the central point of rotational flow that occur within the area of groynes. As a result of compositely examining the results, the appropriate installation spacing is proposed as min. four times-max. six times considering a decrease in flow velocity according to the installation of upward groynes, river bed stability and stability of embankments against counterflow within the area of groynes.
In proceeding with orthodontic treatment, the prediction for the shape, growth rate and growth direction of mandible plays a major role to set up the treatment plan and determine its period and prognosis. Various approaches being made so far have shown that the linear and angular measurement using lateral cephalograms are relatively accurate to estimate them. This study was purposed to find the shape of mandible more clearly by preventing the overlap of the Condyle head area which appears in lateral cephalogram, and to estimate its growth rate by comparing the growth quantity and ratio via lateral area measurement. This experimental was performed against 40 patients total, of which Class I of 14, Class II of 9 and Class III of 17 consist. Wide open lateral cephalograms of 40 patients were taken over average period of 4 Year 3 Months, then the linear and angular measurements were carried out with 11 itemized lists. Autocad Rl4 application program was utilized to draw their appearance, measure and compare their lateral area. As a result of study, conclusions were made as follows; 1. Mandibular body length (gonion-menton) tended to increase in order of CIII, CI and CII, and Mandibular body length of CIII group had a tendency to grow twice faster than that of CII group. 2. In lateral items such as Go-Me, A-Cd, B-Cp, E-F and G-H, CIII showed a significant increase on the year-average quantity and rate of the growth, and especially apparent difference was observed in CIII group rather than CII group. 3. For the 4 Year 3 Months period, the year-average growth quantity of lateral area of the mandible was $1.0cm^2$ for Class I, $0.8cm^2$ for Class II and $1.4cm^2$ for Class III, which corresponds to $11.9\%,\;11.8\%\;and\;20.3\%$ of growth ratio respectively. Thus, growth ratio almost 2 times more than other groups was observed in group CIII while growth ratio between group CI and CII has little difference. 4. Considering the results as above, it can be proposed that the difference in size of the mandible between groups is caused by the difference in the growth rate and growth quantity of the mandible, which generated in the middle of growth, rather than the difference in size of congenital Jaw-bone.
A given facial type can be considered as a syndrome in which various features are aggregated, so a single parameter is not sufficient to accurately identify a given facial type. This study was designed to identify & characterize the skeletal types that blend under the headline-'Cl III,deepbite'. Cephalograms of thirty-four untreated mixed dentition patients, selected mainly on the basis of clinical impression of Cl III with reduced lower face heights were studied. The following conclusion can be drawn. 1. Cl III malocclusion with reduced lower face height could be classified into three types. 2. Subtype 1 was identified by the following features : strong ramus, more anteriorly positioned upper molars without alveolar hypoplasia, acutely reduced Mn. plane angle. 3. Subtype 2 was characterized by a short ramus, sharply reduced postrior alveolar height, and normal Mn. plane angle. In general, this type had hypoplasia tendency in the vertical dimension. 4. In subtype 3, the AUFH occupying more percentage than ALFH was a outstanding feature. Ramal height was in normal range, alveolar hypoplasia and slightly reduced Mn. plane angle was observed. 5. The features of the subtypes were reflected in certain indices, which can be regarded as discriminative index. LAFH: if reduced, regardless of subtypes, indicates reduced lower ant. face height consistently. FHR: when this ratio is increased, it indicates subtype 1. FHI: when this ratio is in normal range, it indicates subtype 2. FPI: if reduced greatly, it indicates subtype 3.

Information on the limits of treatment could allow for more rational treatment Planning and better results after treatment. From this point of view, this article has attempted to discuss the limits of orthodontic tooth movement. A relatively wider range of tooth movement is expected after Class III surgical-orthodontics than after conventional orthodontic treatment in general. The purposes of this Paper were: first, to evaluate the reliability of teeth position measuring gauge; and second, to elucidate the limits of orthodontic tooth movement. Dental casts of fifty-fine subjects were analyzed by using Set-up model checker (InVisitec Co., Korea) before and aster the Class III surgical-orthodontic treatment. The changes of maxillary and mandibular dental arch widths were also measured from the canines to the second molars. To test the inter-examiner reliability, randomly selected casts were measured by another examiner. Descriptive statistics and paired t tests were used to explain the tooth movement during treatment. The results showed a relatively good reliability of measuring instruments and a very diverse range of tooth movement. Collective changes by the orthodontic tooth movement evaluated in Class III surgical-orthodontics allowed for a suggestive interpretation of specific treatment patterns. Arch width changes during the inter-arch coordination were mainly the result of tipping in both buccal segments. Based on the results of this study, the possibility of a change in dentition as a result of orthodontic treatment should be understood in order to launch a well-organized plan of treatment.
The aim of this study was to investigate clinical assessment, panorama & MRI findings and cephalometric characteristics in 42 patients with condylar resorption, who visited in the Department of Oral Medicine Kyungpook National University Hospital at 2006. The results were as follows; 1. Clinical assessment 1) Female was 34 and male was 8, females were predominant. Distribution of age showed as follows; 10s was 14, 20s was 13, 30s was 7, 40s was 3, 50s was 4 and 60s was 1 patient. 10s and 20s were predominant. 2) Most of the patients had parafunctional habit. 2. Findings of panorama & MRI 1) Most of the patients had degree of Grade II condylar resorption by panorama taking. 2) Most of the patients had disc dislocation and belonged to the degree of stage IV by MRI taking. 3. Cephalometric Characteristics 1) SN, SAr and saddle angle in female patients were significantly smaller and SN in male patients showed only significantly smaller than normal group. 2) SNA showed no difference from the normal group in both patients. SNB was smaller and ANB was lager in female patients than normal group. 3) SN-GoMe and FMA increased in patients. 4) Total posterior facial height & ramus height were significantly smaller. 5) Mandibular body length did not show any significant difference.
Purpose: To determine the prognostic values of the hematologic parameters checked preoperatively in gastric cancer patients, we evaluated and compared the relationship between hematologic parameters and clinicopathologic factors of gastric cancer patient. Materials and Methods: The medical records of 357 consecutive patients who had undergone surgery for gastric cancer at the Department of Surgery, Hanyang University Hospital, between Dec. 2,000 and Dec. 2003 were reviewed. To exclude any adverse effect of invasive procedures to hematologic parameters, the samples taken immediately at outpatient department was used. The normal range of serum albumin was defined above 3.5 g/dl, serum hemoglobin above 12 g/dl, and serum platelet count under $400\times10^{3}{\mu}l$. Patients were defined as group 1 when any of these parameters was abnormal, and defined as group 2 when all parameters were normal. The relationships between hematologic parameters and survival rate were investigated. Results: The mean values of platelet count increased, but level of serum albumin and serum hemoglobin decreased significantly according to the advancement of the disease stage (P=0.000). The differences of depth of tumor invasion and lymph node metastasis between the group 1 and the group2 was statistically significant (P=0.001). Three-year survival difference between group 1 and group 2 was significant (P=0.037). Conclusion: The hematologic parameters checked preoperatively in patients of gastric cancer are simple and cheap, meanwhile reflect the general condition of the patients. Any presence of anemia, hypoalbuminemia, or thrombocytosis can predict the progression of the disease and poor survival rates.
Purpose: A primary adenosquamous carcinoma of the stomach is relatively rare, accounting for only about 0.5% of all gastric cancers. However, its histopathologic characteristics are still unclear, and the most appropriate form of therapy has not been established yet. Materials and Methods: We retrospectively reviewed the clinicopathologic features of 8 patients with pathologically confirmed primary adenosquamous carcinomas out of 8,268 patients who underwent gastric cancer surgery at Samsung Medical Center between September 1994 and December 2004. Results: The median age of the 8 patients was 49 ($41{\sim}69$) years, and the male : female ratio was 5 : 3. In 3 patients, the tumor was located at the mid body of the stomach, and in 5 patients, at the lower body or antrum. The tumor sizes were $2.5{\sim}8cm$. Seven patients showed metastases to the regional lymph nodes. The UICC stage distribution were: 5 stage II, 2 stage III, and 1 stage IV. In the stage IV patient, a palliative gastrojejunostomy was performed, and he died 5 months after surgery. Of the 7 patients who underwent a radical gastrectomy and adjuvant chemotheratpy, the median survival was 34 ($12{\sim}66$) months, 2 patients died of cancer recurrence, and 4 patients are being followed up without evidence of recurrence. Conclusion: As for an adenocarcinoma of the stomach, a radical gastrectomy including regional lymph node dissection and postoperative adjuvant therapy should be performed for appropriate treatment of an adenosquamous carcinoma of the stomach.
Background : A clinical study was carried out on 153 new cases with small cell lung cancer registered at Presbyterian Medical Center, Chonju during the 7 years from 1986 to 1992. They were analyzed by sex and age distribution, symptoms and signs, classification of stage and site and its treatments. Especially, an effort was made to compare the overall survival time between limited stage and extensive stage. Methods : Among 806 lung cancer patients diagnosed by biopsy or cytologic evaluation for the 7 years, 153 patients was shown small cell lung cancer. These 153 cases was analyzed retrospectivery through patient's records, letters or telephones. Results : The results of evaluation of small cell lung cancer are as follows. Over 85 percent of the small cell lung cancer patients were over 50 years of age and prominent clinical features were cough(86.3%), sputum(75.8%) and dyspnea(54.9%). One hundred and five patients(68.7%) was staged to have limited stage. Mean survival time of the chemotherapy and chemoradiotherapy in limited stage has significant difference and its survivals are 5.3 months and 15.0 months. Patients whose disease was staged as limited, regardless of whether or not chemotherapy was administered, had a median survival time of 10.9 months, compared with 4.8 months for those with extensive stage. Conclusion : Lung cancer is one of the malignant diseases tend to increase gradually in Korea and proven to be the most common cancer next to the gastric cancer among various cancers in males found at the Presbyterian Medical Center in the past seven years. This report is a retrospective view of the clinical therapeutic results of the small cell lung cancer patients. Especially at the limited stage, the combined therapy revealed higher survival rate than the chemotherapy alone. For a more accurate evaluation. a prospective view, without any bias, of patients selected at random is needed.
Objective : Cavitary lesion in pulmonary tuberculosis sometimes makes problems in the course of treatment. Especially, retreatment cases tend to respond poorly to current antituberculosis agents. So, authors tried to render a guideline for clinical approach toward cavitary lesions in pulmonary tuberculosis. Methods : Retrospective analysis of 33 pulmonary tuberculosis patients with cavitary lesions was made. All the patients had got treatment at National Masan Tuberculosis Hospital from Jan. 1995 to Aug. 1995. Results: The ratio between male and female was 10:1. Age distribution was 69.7% in 3rd and 4th decades. The locations of cavitary lesion were 60.6% in right upper lung field and 36.4% in left upper lung field. In the extent of disease, there were 12 cases(36.4%) in moderate advanced and 21 cases in far advanced. Cavitary lesions were closed in 5 eases and remained in 28 eases. In the cases of closed cavity, it was happened within $10.6{\pm}4.72$ months after they took treatment, the size of cavity was $35.5{\pm}17.1$ in long diameter, $27.0{\pm}12.2$ in short diameter and $4.6{\pm}2.1\;mm$ in wall thickness. In the cases of remained cavity, the size of cavity was $31.9{\pm}12.3$ in long diameter, $21.0{\pm}9.8$ in short diameter and $5.04{\pm}2.0\;mm$ in wall thickness. In terms of negative conversion, it took $3.8{\pm}2.17$ months in the case of closed cavity but it was happened within 5 months for only 4 patients in the case of remained cavity. In the point of past medication history, there was none in closed cavities but there were none in 1 case, once in 3 cases, two times in 9 cases and more than three times in 13 cases among remained cavitary lesions. Conclusion : In the retreatment cases of pulmonary tuberculosis with cavitary lesions, they tend to respond poorly to current antituberculosis agents. So, if the lesions are localized, operative intervention may be a proper method as adjunctive treatment.
