CLASS II COMPOSITE RESIN RESTORATION USING ORTHODONTIC BANDS (교정용 밴드를 이용한 구치부 2급 와동의 복합레진 수복)

  • Children and teenagers have a higher frequency of proximal surface caries in the posterior teeth than adults. For proximal restoration, class II amalgam or stainless steel crown has been widely used in the past, however composite resin restoration is getting ore popular due to it's superior cosmetic appearance. When applying composite resin on proximal area, various types of matrix bands can be utilized according to the operator's reference or skill. Such bands have several clinical effects including suitability for proximal margin, reduction of micro-leakage, moisture-control against saliva and ease finishing and polishing. In this case report, orthodontic bands were utilized instead of matrix bands as a remedy for proximal restorations in both primary and permanent teeth and their clinical advantages are as follows. 1. Orthodontic bands showed superior marginal adaptation compared to conventional matrix bands and moisture-control against saliva was excellent. 2. While applying composite resin, deformation of restoration material was estimated to be insignificant due to he rigidity of the orthodontic bands. 3. Natural tooth contour of the orthodontic bands facilitates to reproduce proximal tooth contour of the restoration. 4. In general, pediatric dentists are accustomed to applying orthodontic bands and this may allow pediatric dentists to make proximal composite restorations more efficiently than other dental specialists.

EFFECT OF FLUORIDE VARNISH ON DEVELOPMENT OF ARTIFICIAL CARIES (불소 바니쉬가 인공 우식 병소의 발생에 미치는 영향)

  • The purpose of this study was to evaluate the effect of fluoride varnish on prevention of enamel demineralization. Eighty bovine enamel blocks were divided randomly into 4 groups of 20 specimens: Group I served as the control with no topical application of fluoride. Group II was treated with APF gel for 4 minutes. Group III was treated with Fluor $Protector^{(R)}$. Group IVV was treated with $CavityShield^{TM}$. After 24 hours of treatment, samples were brushed with a soft bristled tooth brush to stimulate normal mechanical wear of varnish. Early caries lesions were produced by placing each specimen into demineralization solution at pH 4.0 for 48 hours. Then the optical density of the lesions was measured by light fluorescence induced by plasma light and the surface microhardness were measured by the vicker's hardness test. The results were as follows: 1. The optical densities of group III, IV were significantly higher than that of group II, but no significant difference was noted between group III and IV. 2. The surface microhardness of group IV was the highest, followed by group III, II, and I(p<0.05). 3. There were no significant differences between group III and IV in optical density. However, the surface microhardness of group IV was significantly higher than that of group III. The results of present study indicate that the fluoride varnish is more effective than APF gel for prevention of dental caries.

DISTRIBUTION OF MUTANS STREPTOCOCCI IN DENTAL PLAQUE OF CHILDREN (소아의 치면세균막에 존재하는 mutans streptococci의 분포)

  • The aim of this study is to survey the frequency of mutans streptococci species and biotypes isolated from dental plaque in Korean children and the relationship between species and biotypes of mutans streptococci and dft index. Dental plaques were collected from the anterior and molar teeth of upper and lower jaws in the subjects, aged below 12 years old. A dental examination was performed for dft (decayed, filled, total) with the WHO caries diagnostic criteria. The mutans streptococci from the sample were cultured selectively on mitis salivarius-bacitracine (MSB) agar plate. For biotyping of mutans streptococci, biochemical test was performed. From the culture, bacterial genomic DNA was prepared for using of PCR template for the identification of mutans streptococci at the species-level. Forty strains of mutans streptococci were isolated from dental plagues of 40 patients. The biotype I (45%) and biotype IV (32.5%) were most frequently detected. The prevalence of S. mutans and S. sobrinus was 69% and 31%, respectively. There was no positive relationship between species and biotypes of mutans streptococci and dft index. Our results revealed that biotype I and S. mutans were frequently detected in Korean children and support that dental caries incidents by many causative factors not only bacterial factor.

COMPARATIVE STUDY ON THE RATE OF DENTAL ENAMEL DEMINERALIZATION USING A QLF (Quantitative Light-induced Fluorescence를 이용한 법랑질 탈회 속도에 관한 비교 연구)

  • The objective of this study was to compare the rate of in vitro demineralization of bovine permanent (BP), human deciduous (HD) and human permanent (HP) enamel. Twenty aye flattened and polished enamel samples for each group (BP, HD, HP) were immersed in a demineralizing solution (0.1 mol/L lactic acid, 0.2% Carbopol 907, and 50% saturated hydroxyapatite) for 1, 2, 4 or 8 days. All 25 samples from each group were subjected to Quantitative light induced fluorescence analysis (QLF) and 5 samples from each group were randomly selected for Transverse Microradiography analysis (TMR). Integrated mineral loss (IML) and lesion depth (LD) were determined by TMR. The fluorescence radiance (FR) of sound enamel $(FR_S)$, demineralized enamel $(FR_D)$ were determined by QLF and FR ratio $(FR_D/FR_S)$ was calculated. Bovine enamel samples showed significant correlation between FR ratio and lesion depth(p<0.05) and deciduous enamel samples does not showed significant correlation between FR ratio and lesion depth(p>0.05). Permanent enamel samples showed significant correlation between FR ratio and lesion depth(p<0.05) The constant of demineralization time between FR ratio from regression analysis were as follows: bovine enamel was -4.643(p<0.05) deciduous enamel was -5.421(p<0.05) and permanent enamel was -4.435(p<0.05).

  • Composite resin restorations are widely used to restore decayed teeth or preventive restoration in children. Children often receive topical fluoride treatments on a semiannual basis. However there is concern that topical acidulated phosphate fluorides (APF) may cause deterioration of composite resins. The aim of this study was to evaluate the surface changes in composite resins due to topical fluoride application. Composite resins(Solitaire $2^{(R)}$, Metafil $CX^{(R)}$, Composan $LCM^{(R)}$, $Charmseal^{(R)}$) in topical fluoride agents were immersed and their surface roughness, weight loss and SEM findings were evaluated. The results were as follows: 1. The 4 minutes-immersion group showed more roughened surface than 1 minute-immersion group and the control group showed the smoothest surface among all the materials, and there was statistically significant difference between the groups except the Composan $LCM^{(R)}$ (P<0.05). 2. There was no significant difference between the 1 minute-immersion group and 4 minutes-immersion group in weight loss (P>0.05) 3. The experimental group treated with topical fluoride gel showed generally more roughened surface than control group in the SEM findings.

  • This study aimed to compare chair-side test results for caries risk assessment and evaluate how well the tests reflect caries experience. The study was conducted on children aged < 6 years in primary dentition. Dental examination of children was conducted to determine the dmft index and subjects were divided into two groups : group I (dmft < 6), group II (dmft > 6). This study used four kinds of test kits (Plaque-check PH kit, Saliva-check buffer kit, Saliva-check mutans kit, Cytoperio analysis system). Saliva buffer capacity was significantly low in the high caries experience group (dmft > 6) and correlated with dmft index. Saliva pH level correlated significantly with saliva buffer capacity. The results showed that plaque pH and saliva pH levels had no correlation with dmft index. The Streptococcus mutans level measured by using the Saliva-check mutans and Cytoperio analysis system did not correlate with dmft index.

The Effect of Silver Diamine Fluoride on Salivary Biofilm (Silver diamine fluoride가 타액 생물막에 미치는 영향)

  • Silver diamine fluoride (SDF) is an effective and efficient agent for arresting dental caries. It can be useful in treating children with behavioral or medical limitations. The purpose of this study was to evaluate the antimicrobial effect of SDF by using salivary biofilm. Pellicle-like saliva coated structure was prepared by using unstimulated saliva. For developing cariogenic biofilm, Streptococcus mutans was added to the mixture of pooled saliva and inoculated into a saliva coated glass or chamber. SDF was applied to cariogenic biofilm to evaluate the antimicrobial effect of SDF. As time passed, total bacteria and S. mutans were reduced after application of SDF (p < 0.000). Confocal laser scanning microscope also showed the increment of the ratio of dead cell. As a result of experiment using enamel and dentin of primary teeth, it was confirmed that the growth of cariogenic biofilm was inhibited when the SDF was treated (p = 0.029 each). This study showed excellent anti-microbial effect of SDF. And anti-caries effect in clinical practice can be expected.

DENTAL TREATMENT OF A PATIENT WITH CATCH22 SYNDROME UNDER GENERAL ANESTHESIA: A CASE REPORT (CATCH22 syndrome 환아의 전신마취 하 치아우식 치료: 증례보고)

  • CATCH22 syndrome or DiGeorge syndrome is a medical acronym of cardiac defects, abnormal facial appearances, thymic hypoplasia, cleft palate, and hypocalcemia. Patients with CATCH22 syndrome are susceptible to infection due to an absent or hypoplastic thymus and often have difficulties in maintaining good oral hygiene, which may require dental treatment. We present a case of dental treatment for the uncooperative child with CATCH22 syndrome under general anesthesia. A 4-year-old, 14.8 kg boy with CATCH22 syndrome visited Seoul National University Dental Hospital for dental check up. The patient had multiple caries requiring dental treatment. He experienced the corrective heart surgery due to Tetralogy of Fallot a few years ago. General anesthesia was planned because his heart rate and vital sign had shown unstable during the previous conscious sedation procedure. Dental restorative treatments were successfully performed and no complications were observed during and after the procedure. Safe and effective dental management of the patients with CATCH22 syndrome could be performed with the help of general anesthesia and careful monitoring.

ORAL HEALTH CARE STATUS IN INSTITUTIONS AND SPECIAL SCHOOLS FOR THE DISABLED PEOPLE IN KOREA (장애인 입소시설과 특수학교의 구강건강관리 실태에 대한 조사 연구)

  • The purpose of study was to investigate the oral health care status in institutions and special schools for the disabled in Korea. Nurses in 123 institutions and nurse-teachers in 102 special schools were asked to fill questionnaires regarding periodic oral examination, preventive dental programs. and dental treatment. The results of this study were as follow. 1. 56.8% of institutions implemented regular oral examinations and all of special schools did it twice a year. 2. Preventive programs for dental caries were done in 69.1% of the institutions and 86.3% of the special schools. Programs included regular toothbrushing and dental health education. 3. Dental treatment was done in 84.2% of the institutions and 39.2% of the special schools. Institutions utilized private dental clinics(60.2%), public health centers(16.8%), volunteers(15.3%), and dental hospitals. 4. 17.7% of the institutions experienced the refusal to treat the disabled by private dental clinics.

THE EFFECT OF GLASS IONOMER ON THE REMINERALIZATION OF ADJACENT INITIAL ENAMEL CARIES LESION BY RELEASING FLUORIDE (Glass Ionomer 수복재의 초기 법랑질 우식증에 대한 효과)

  • There is no adverse opinion on the anticariogenic effect of fluoride, so glass ionomer restoration which release the fluoride is recommended fer child patient. To study the anticariogenic effect of initial carious lesion of fluoride released from adjacent glass ionomer restoration, the in situ model was constructed. A microhardness test, polarized scope investigation and electron probe microanalysis was done for analyzing the distribution of fluoride which was precipitated from glass ionomer restoration to the enamel caries lesion. Fuji IX, the conventional glass ionomer, was used for experimental group and Z-100, composite resin that fluoride was not contained, was used for control group. On the microhardness test, the remineralization was accelerated by fluoride. And on the polarized investigation, the size of caries lesion was reduced in the oral cavity and that phenomenon was accelerated by fluoride, too. Electron probe microanalysis shows that the remineralization was accelerated by fluoride and the fluoride concentration on subsurface area was increased. It maybe that the subsurface area was critical to anticariogenic effect. In summary of these result, initial caries lesion can be remineralized in the oral cavity and that phenomenon can be accelerated by fluoride. The subsurface area of caries lesion was a major part of defense to cariogenic invasion and to conserve the subsurface area, the surface of lesion body have to conserved.
